The aesthetic appearance of the incisors is essential for a natural and healthy smile, and it is also an important factor for confidence in interpersonal relationships. If there is crowding or protrusion of the anterior teeth, root canal treatment and excessive tooth removal are often required to resolve it in a short period of time. However, it is difficult to achieve sustainable aesthetic results. In this case, orthodontic treatment will be necessary to preserve the remaining tooth structure as much as possible. Middle-aged and elderly patients are often aware of the need for orthodontic treatment, but they are hesitant to use the traditional orthodontic treatment method using brackets because of the social gaze. In this case, clear aligners can be a good option. So, in this article, a case in which aesthetic needs were resolved with clear aligners alone and another case in which satisfactory results were obtained through restorative treatment following clear aligners application are reported.

The introduction of smart technology provides accuracy, safety, and efficiency to both physicians and patients. Although interest in a clear aligner is increasing among users worldwide, the current clear aligner requires a visit to the hospital every one or two weeks for replacement, which is a very cumbersome process. There is also confusion among dentists and patients because about 40 to 80 devices are made, and calibration is done based on the order and duration of the clear aligner. Therefore, this study designed and developed a clear aligner management system so that communication between the patient and dentist can be smoothly performed by inserting the QR code into the transparent correction device. As a result, the size of the QR code was recognized as $6{\ast}6mm^2$ which can be used in the oral and the recognition distance was 100% within 12 cm. Since the dentist can remotely manage the patient with the proposed system and improve the correction effect, it is possible to manage patients abroad, as well as domestically.

최근 의료 분야 소프트웨어가 발전함에 따라 3차원 가상 모델을 이용한 의료 소프트웨어의 개발도 꾸준히 증가하는 추세이다. 치열 교정 분야는 이러한 3차원 가상 모델을 적용할 수 있는 분야 중 하나이다. 치열교정 분야에서는 3차원 가상 모델을 이용하여 부정 교합 환자의 치료 초기부터 끝까지 여러 단계의 치아이동 과정을 제공한다. 치아 이동의 각 단계별로 Stereolithographic Model이 만들어지는데, 이는 투명하고 얇은 Overlay 장치를 제작하는 기초가 된다. 투명한 재질의 Overlay 장치는 심미적이고 사용이 간편하여 많은 환자들이 선호하고 있다. Overlay 장치를 제작하는 시스템을 구축한 회사로는 Invisalign System이 가장 대표적이다. Invisalign System은 미국에 본사를 둔 회사로 한국에 지사를 두고 적극적인 마케팅을 통해 3D 치열 교정 분야의 국내시장을 거의 독점하고 있다. 독점적이다 보니 비용이 다른 교정 치료보다 2배 가까이 들고 있지만, 현재 한국에서는 개발이 적극적으로 이루어지지 않는 실정이다. 이에 본 논문에서는 환자 치아의 정확하고 과학적인 진단자료를 얻어 높은 치료비용에 대한 개선과 치료 전 과정에 대한 시뮬레이션 및 Stereolithographic Model을 미리 환자에게 제공하여 만족도를 높일 수 있는 3D치열 교정 시스템을 제안한다.

There are interferences between the teeth to be moved and the clear aligner made from setup. These interferences generate forces which move teeth to the desired position. However, these can cause incomplete tooth capture of clear aligner. When these interferences exceed the acceptable deformation of aligner, unwanted intrusion of teeth to be moved occurs. When correction of rotation or tipping of teeth is attempted, intrusion prevails before rotation or tipping. However, clear aligner can induce labiolingual tipping or intrusion easily. To achieve preliminary alignment including control of rotation and tipping, NiTi archwires with fixed appliance can be used first, and then clear aligner can be used for finishing in mild Class I maloccusion cases. For this purpose, clear aligner can be made using a digital setup and 3D printing. To move teeth using a clear aligner, tooth capture is critical. To improve tooth capture of clear aligners, slingshot or vertical elastics can be used. These were discussed with illustrations.

Objectives : The purpose of the study is to investigate the knowledge and self care behaviors of orthodontic patients using clear aligner. Methods : A total of 181 orthodontic patients using clear aligner in Daegu filled out the self-reported questionnaire from September 6 to October 18, 2010. and 162 subjects completed orthodontic treatment. Results : There existed a significant difference in knowledge and self care behaviors between gender and wearing duration of clear aligner(p<0.01). Treatment duration influenced significantly on the score of self care behaviors(p<0.01) and the high score of self care behavior showed in short duration of treatment. Conclusions : Self care behaviors were closely correlated with treatment duration. An information for the clear aligner will be helpful to improve the knowledge and self care behaviors.

Adult patients who need implant and prosthodontic treatment often need treatment to improve the existing occlusion through orthodontic treatment for long-term stable treatment results. However, due to non-aesthetic and uncomfortable orthodontic treatment with orthodontic brackets and wires, many adult patients give up treatment even though they know the need for it. Recently, as digital dentistry has affected all areas of dentistry, clear aligner orthodontic systems have begun to be widely used, and their use is increasing in adults and old-aged people due to the esthetic advantage and convenient oral care. SERAFIN clear aligner system developed in Korea has been developed with the aim of implementing a functional occlusion harmony and is used not only for partial orthodontic treatment but also comprehensive orthodontic treatment. This patient presentation is shown the treatment using SERAFIN clear aligner system for the treatment of patient with TMD, severe extrusion of maxillary second molars, and extracted teeth.
