건강관리코너 - 여름철화상관리

  • 여름철에는 덥고 습한 날씨가 이어져 사람들은 최대한 신체를 노출시켜 시원함을 유지하고자 한다. 따라서 뜨거운 수증기나 국물에 데거나 다리미 등의 뜨거운 쇠붙이에 의해 접촉화상을 입는 경우가 흔하다. 이런 경우 심한 통증과 상처가 생기게 되는데, 대부분의 사람들은 당황하여 어떻게 대처해야 할지 몰라 우왕좌왕하기 마련이다. 일부의 사람들은 간장이나 된장, 알로에 등을 상처에 바르는 등 전근대적인 방법으로 상처를 치료하다가 상처를 키워서 병원에 가기도 하고, 심지어는 창상을 그대로 노출시켜 세균에 의한 감염이 발생되어 얕은 화상이 더욱 깊어져 치료만으로 나을 수 있는 상처를 피부이식술 등의 수술을 필요로 하게 되는 결과를 낳기도 한다. 또한 수술을 하는 정도는 아니더라도 화상흉터가 많이 남게 될 수도 있어 초기의 화상관리가 중요하다 하겠다. 과연 독자들은 화상상처에 어떻게 대처하는지? 초기에 상처를 어떻게 돌보는가에 따라 평생 화상흉터의 멍에를 지고 갈수 있다는 것을 알고 있는지? 이러한 많은 문제점들에 대한 관리적인 차원에서 일반인들이 어렵고 복잡하다고 생각할 수 있는 화상관리에 대해 이야기해 보고자 한다. 화상이 생겼을 때의 처치는 일반적인 열창 때의 처치와는 다른 점이 있다. 우선 열에 의해 피부가 손상을 받아 이를 식히는 과정이 필요하며 이 과정(찬물로의 창상세척)이 간과될 경우 이차적 점은 화상에 의한 상처는 손상된 피부로 인해 노출된 생살을 어떻게 세균감염으로부터 보호하느냐 하는 점이 문제가 되며 이를 예방하는 과정이 필요하다는 것이다.

Treatment of Malignant Sacral Pain by Radiofrequency Lesion Generator (전기적(電氣的) 열응고술(熱凝固術)을 이용(利用)한 암성천골통증(癌性薦骨通症)의 관리(管理))

  • Efforts from many different approaches have been made to manage malignant sacral pain that commonly occurs in cancers from pelvic origin. Radiofrequency thermocoagulation provides a safe method of achieving long standing relief of intractable pain without some of the objectable side effects associated with other forms of chemical or surgical therapy. Radiofrequency sacral rhizotomy is a new method for the relief of the sacrococcygeal pain. We report a case in which we managed a patient with malignant sacrococcygeal pain with radiofrequency thermocoagulation.

Central Anticholinergic Syndrome by the Use of Scopolamine Patch -A case report- (수술후 통증관리 환자에서 Scopolamine Patch에 의해 발생한 중추항콜린성증후군 -증례 보고-)

  • A 77-year-old male patient was admitted to the hospital with acute appendicitis and underwent emergency appendectomy under inhalation anesthesia without any complications. After the operation, the patient was placed on epidural analgesia with the mixture of fentanyl-bupivacaine and scopolamine patch for nausea and vomiting. The patient experienced central anticholinergic syndrome (CAS) with hallucination, confusion, somnolence, dysarthria and delirium at 20 hours after patch placement and recovered from all symptoms of CAS in 24 hours after the removal of the patch. The CAS occurs when central cholinergic sites are occupied by specific drugs and also as a result of an insufficient release of acetylcholine. The diagnosis of CAS is often determined by a process of exclusion and not actually made until a positive therapeutic response to physostigmine, a centrally active anticholiesterase agent, has taken place. Treatment of CAS includes prompt removal of the patch, cleansing of the area, and physostigmine administration. The scopolamine patch should be used with extreme caution in elderly patient.

Superior Hypogastric Plexus Blocks for Malignant Pelvic Pain (상 하복신경총 차단을 이용한 골반내 통증관리)

  • Pain from pelvic cancer is very difficult to manage because it's vague ness and bilateral nature. Furthermore, nerve blocks in this area are dangerous because sensory afferent nerves from pelvic viscera are adjacent to nerves that regulate bowel and bladder control, and motor nerve of lower extremities'. Bilateral lumbar sympathectomy has been used for malignant pelvic pain with little risk of neurologic complication. However it is not a specific block for pelvic visceral pain, because the lumbar sympathetic chain does not innervate pelvic viscera in a direct manner. Therefore the potentials of lumbar sympathectomy for pelvic visceral pain are attributed to caudad diffusion of neurolytic agents to the smperior hypogastric plexus. I have experienced 3 cases of superior hypogastric plexus neurolysis per se without any significant complications.

  • 본 연구는 근골격계질환의 예방을 위해 전북권 지역에 해당하는 근로자들을 대상으로 설문조사를 하여 업종별 근골격계질환의 실태를 파악하고 이를 분석하였다. 조사분석 결과, 업종에 따른 근로자들의 신체 부위별 통증 호소 부위와 정도, 신체 부위별 통증 부위수, 각 신체 부위별 부적절한 작업 자세 등의 실태를 파악 할 수 있었다. 이를 통하여 근골격계질환에 대한 근로자들의 인식 개선과 근골격계질환의 유해요인을 감소시킬 수 있는 환경변화의 제도적 장치가 필요하며, 이를 인간공학적 시스템으로 재설계되어 예방 관리의 기틀을 마련해나가야 한다.

Neurolytic Splanchnic Nerve Block for the Treatment of Upper Abdominal Pain (상복부 암성통증 환자관리에 있어서 내장신경통 차단술의 임상적 고찰)

  • Neurolytic splanchnic nerve block is an effective method for the relief of pain of upper abdominal cancer. Nine cases of intractable upper abdominal cancer pain were treated by splanchnic nerve block with absolute alcohol (25 ml) at the pain clinic of Kangdong Sacred Heart Hospital, Hallym University, during a 19 month period from March, 1988 to September, 1989. The group included six patients with stomach cancer, two patients with pancreatic cancer, and one patient with hepatobiliary cancer. We used fluoroscopy in all cases of alcohol spianchnic nerve block to determine both, the position of the needle tip, and the spread of the neurolytic solution. Of the 9 patients, 6 patients had excellent pain relief, and 3 patients who had combined upper abdominal and lower back pain had relieved upper abdominal pain only, but remaining lower back pain. Of the 9 patients', 5 patients had excellent pain relief through the patients remaining life (1-2 months) in which follow up was possible.

Pain Management of Post-herpetic Neuralgia (대상포진 후 신경통의 통증관리)

  • Postherpetic neuralgia, the most feared complication of acute herpes zoster, may agonize the pain clinician because the appropriate management of intractable pain can fail inspite of various and prolonged therapeutic techniques. Of all patients with herpes zoster, approximately 5~10% will develop postherpetic neuralgia. Postherpetic neuralgia is very rare in young patient but very common in patients over 60. In other words, the older, the higher incidence. In our pain clinics, 13 postherpetic neuralgic patients were treated with sympathetic blocks, local infiltration with local anesthetics and steroids, TENS, Laser and various drugs including antiderpressant. The results of management of pain in 13 patents were as follows. 1. Excellent pain control :4(30.8%) 2. Good pain control :3(23.0%) 3. Fair pain control :2(15.4%) 4. No effect :4(30.8%).

Pediatric Nurses' Knowledge and Attitude towards Management of Children's Postoperative Pain (아동병동 간호사의 수술 후 통증 관리에 대한 지식과 태도)

  • Purpose: This study aimed to describe pediatric nurses' knowledge and attitude towards management of children's postoperative pain. Methods: In this cross-sectional descriptive study, the participants were 220 pediatric nurses who worked at a general hospital in Seoul. The survey questionnaires used to assess the nurses' knowledge and attitude regarding children's postoperative pain were developed for this study. Results: The average score for knowledge was 67.7 out of 100. The mean score for attitude was 72.5 out of 100. The factor related to the knowledge level was education for pain management. Moreover, age, working department, position, working experience, education level, and number of children were associated with the attitude. Conclusion: The findings of this study suggested the need for a systematic education program for pain management of children in the postoperative condition.

Development and Effect of Pain Management Protocol for Nursing Home Patients with Dementia (노인 간호 요양시설에서의 치매환자 통증관리 프로토콜 개발 및 효과)

  • Purpose: This study was done to develop a pain management protocol for nursing home patients with dementia and to examine effects of the protocol on pain assessments and interventions by the nurses and on pain relief signs in the patients. Method: The six steps in the protocol development and the examination of effect are outlined. Three rounds using the Delphi technique and one group pretest-posttest design experiment were developed. Design issues, such as sample selection and sample size, are addressed in relation to the study protocol. Results: After implementation of the pain management protocol, there were significant changes nursing actions including frequency of number of physical examinations, utilization of pain assessment tools, and request to doctors for discomfort management and there were significant changes in frequency in the number of verbal and physical expressions of pain, and emotional patterns. Conclusion: This is the first pain management protocol for patients with dementia in Korea. However, more study will be needed to determine the methodological strength and necessary revisions for the protocol.

Analysis of Correlation Coefficient between Musculoskeletal Symptoms and Job Stress of Physical Therapists in Twenties: Focusing on Therapeutic Exercise Task (20대 물리치료사의 근육뼈대계 증상과 직무스트레스와의 상관관계 분석: 운동치료 업무를 중심으로)

  • The purpose of this study is to investigate musculoskeletal symptoms and job stress of physical therapists in their 20s who are responsible for therapeutic exercise and to understand the correlation between the two. This study conducted a research on 144 subjects based on the measurement tools provided by national institution. The results are as follows. Many subjects were experiencing musculoskeletal symptoms and disorder in their hands/wrists/fingers, shoulders, and waist with high proportionate of subjects experiencing the symptoms and disorder in their right hand/wrist/finger. Among job stress, job environment, job requirement, and compensation had significantly positive correlation with physical burden(r=.297~.392, p=.000). In particular, shoulder pain intensity has significantly positive correlation with job requirement, duration of pain in hand, wrist, and fingers as well as workplace culture(r=.331~.478, p=.003~.009) which indicates that musculoskeletal symptoms are some what correlated with job stress. Based on this study, it is necessary to show more concern and make an effort to manage musculoskeletal symptoms and job stress.
