Pain Management in Cancer Patients Who Are Registered in Public Health Centers (보건소 등록 재가암환자가 지각하는 통증관리와 통증관리 방해 정도)

  • Purpose: This study was designed to describe outcomes of pain management, to identify pain intensity, pain management and barriers to pain management, and to test correlation among the variables in cancer patients who are registered in public health centers. Methods: By using a descriptive survey design, 3 instruments were used to collect data: the Numeric Rating Scale for pain, the Barriers Questionnaire-Korean version, and a one-item self-report tool about patient satisfaction. A sample of 190 patients with cancer was recruited from a public health center. Results: The mean rating for pain during the past 24 hours was mild and the mean score of barriers to pain management was 3.20. Patients were satisfied with pain management but they also had concerns it. A negative correlation was found among pain severity, pain relief and satisfaction of pain management. However, there was not significant correlation between the patient-related barriers to pain management and other variables. Conclusion: These results suggest that the intervention for cancer patients should focus not only on patient-related barriers to pain management, but also address health-care system related barriers.

Factors Influencing Pain Intensity in Patients with Advanced Cancer (진행암 환자의 통증강도에 미치는 영향요인)

  • This study was conducted to identify pain intensity and factors affecting pain intensity in patients with advanced cancer. Data were collected between June 1 and September 30, 2016 using a questionnaire. The sample size was 221 patients with advanced cancer who were admitted to the oncology department or who visited the outpatient of the general hospital. Data were evaluated by descriptive and Pearson's correlation analyses, one way ANOVA, t-tests and stepwise multiple regression analysis. The mean scores of pain intensity of cancer patients were 4.23 (${\pm}1.68$) based on the average daily pain intensity. Factors influencing pain intensity were illness perception (${\beta}=.27$, p<.001), pain opioid analgesics beliefs (${\beta}=.24$, p<.001), education (middle school, ${\beta}=.24$, p=.001), economic status (${\geq_-}400$, ${\beta}=.20$, p=.001), gender (female, ${\beta}=.14$, p=.017), pain management education (${\beta}=-.14$ p=.020) and diagnosis (Pancreatic Ca, ${\beta}=.14$, p=.020). It explained 28%. Overall, the results of this study revealed that illness perception and pain opioid analgesics beliefs were important factors influencing pain intensity, but that the most important influencing factor was illness perception. Accordingly, it is necessary to develop pain management strategies that include not only pain management knowledge and pain opioid analgesics beliefs, but also illness perception.

A Study for Assessing Pain Intensity and Investigating Factors of Satisfaction with Postoperative Pain Management (복부 수술 환자의 통증 정도 및 통증관리 만족도 영향 요인 - 통증 자가조절기 사용 환자를 중심으로 -)

  • Purpose: The purpose of this study was to identify pain intensity according to activities over postoperative days and to identify factors of patient satisfaction with pain management after abdominal surgery. Methods: The subjects were 123 patients who were admitted at a general surgical ward and had abdominal surgery from Mar. 2005 to June 2005. The collected data were analyzed using the SPSS 12.0 program. Results: The average postoperative pain scores were 6.13 at POD 1, 5.46 at POD 2, and 4.22 at POD 3. In stepwise multiple regression analysis a total of 40% of variance in satisfaction with pain management was accounted for by pain intensity on POD 3 at rest (29.4%) and attitude toward using pain medicine (6.6%), and side effects of pain medicine (4.0%). Conclusion: According to the findings of this study, patients had inadequate pain management after abdominal surgery. Therefore, nurses need to provide educational programs focused on changing attitudes toward using pain medicine, to reduce side effects of pain medicine, and to develop nursing intervention for relieving pain.

Cost Analysis of Post Operative Pain Management for Surgical Patients using PCA (자가 통증조절장치를 이용하는 수술환자의 통증관리 실태 및 통증관리 비용분석)

  • Purpose: The purposes of this study were to identify and compare various types of post operative pain management and the costs for pain management following 4 different types of surgery. Methods: Data were collected from 325 medical charts which were extracted from the billing databases of a tertiary hospital and analyzed using numbers, percentages, one way ANOVA, and Scheff$\acute{e}$ test. Results: For pain management, 10.5% of patients used PCA only, but the other patients combined other methods with PCA. The average length of PCA use was significantly different by operation. Almost one third (32.9%) of patients experienced at least one of side effects due to analgesics used for pain management, with highest incidence being for nausea and vomiting. For patients who underwent a total abdominal hysterectomy, 34.7% used PCA less than 2 days due to side effects of the analgesics and the ratio of analgesia cost to total hospital cost and total pharmacy cost were highest compared to other operations. Conclusion: The results of this study indicate a need to develop new strategies to more effectively manage postoperative pain to decrease incidences of side effects without increasing medical costs.

The effect of oral breast milk on pain response of the neonates during heel lancing (모유경구투여가 발뒤꿈치 천자 시 신생아의 통증반응에 미치는 효과)

  • This study was performed in order to investigate the effect of breast milk on pain relief in newborn during heel lancing. Nonequivalent control group pretest-posttest design by double blinded experiment was used. 50 neonates were randomized to receive breast milk (experimental group, n=25) or no treatment (control group, n=25) before undergoing heel lancing. Informed consent was obtained from parents of 50 neonates. Neonatal infant pain scale (NIPS), respiration rate, heart rate, oxygen saturation, and crying duration were used to assess subjects' pain reaction. Data were analyzed using SPSS 18.0. The experimental and control group showed a statistically significant differences in NIPS score (F=28.89, p<.001), heart rate (F=14.03, p<.001), respiration rate (F=4.79, p=.001), oxygen saturation (F=2.69, p=.027), and crying duration (t=-8.78, p <.001) at each time points (during heel lancing, right after heel lancing, 1, 2, 3 minutes after heel lancing). The result suggest that oral administration of expressed breast milk is safe and natural agents for reducing procedural pain of neonates. Another clinical trial study with more samples is recommended.

  • 척추의 가장 윗부분인 경추(목뼈)는 7개의 뼈로 이루어져 있으며, 위로는 머리, 아래로는 역시 척추의 일부인 흉추와 연결되어있다. 경추는 움직임이많고 큰 만큼 인체에서 아주 중요한 역할을 한다. 그러나 잘못된 자세와 오랜 시간 고정된 자세의 일과로 현대인들의 목은 뻐근하다. 더욱이 컴퓨터작업이 늘어나면서‘컴퓨터 모니터 증후근’, ‘거북목 증후군’등이 생겨날정도이다. 목 부위의 통증은 크게 척추 이상과 근육의 이상으로 인해생기는 경우가 대부분인데 이들의 초기증상은 매우 비슷해서 스스로 판단하기 어렵기 때문에 목 부위 통증이 간헐적으로 생기는 경우라면, 어떤 원인에 의해서인지 정확한 진단이 필요하다. 이번 호에서는 인체의 신비,‘목’에 대해 관절척추전문 세란병원 오명수 척추센터장과 자생한방병원목디스크센터 송우섭 원장의 도움말을 들어본다.
