Antinociceptive Efficacy of Korean Bee Venom in the Rat Formalin Test (랫드의 포르말린 시험에서 한국산 봉독의 항통각 효과)

  • The study was performed to assess the antinociceptive efficacy of Korean bee venom(BV) in the rat formalin test. Korean BV was collected using BV collector devices in which an electrical impulse is used to stimulate the worker bee(Apis mellifera L.) to sting and release venom. After collection, whole Korean BV was evaporated until dry using BV collector. Experiments were performed on male Sprague-Dawley rats(weighing 260-270, 6 weeks old). Rats divided into 4 groups, each comprising 8 rats. BV was diluted and amounts of 6 mg/kg body weight(BW), 0.6 mg/kg BW and 0.06 mg/kg BW were tested. BV was subcutaneously injected to produce an antinociceptive effect and the antinociceptive efficacy was evaluated using a rat formalin test. BV was subcutaneously injected into an acupoint(Zusanli, ST36) at 15 min prior to 1% formalin($50{\mu}l$) injection. The antinociceptive effect observed during 60 min following formalin administration. BV produced antinociceptive efficacy from 10 to 60 min after formalin injection. The antinociceptive efficacy of Korean BV showed a dose-dependent response. These results suggest that Korean BV may be a suitable and preferred choice for antinociceptive efficacy in pain management.

A Review of Occupational Therapy Approach for Complex Regional Pain Syndrome (복합부위통증증후군의 작업치료적 접근에 관한 고찰)

  • Complex Regional Pain Syndrome(CRPS) is a chronic neruopathic pain syndrome, most frequently encountered after trauma, nerve injury, fracture, cerebral vascular accident(CVA), spinal injury, and surgery to a limb, and affected by diverse factors such as disease process, intervention method, and other biopsychosocial components. Occupational therapy as a multidisciplinary treatment approach may be potentially useful when simultaneously targeting such biological, psychological, and social components. Biological treatment can focus on increasing functional use of extremity for promoting independence. Psychological intervention can include relaxation/biofeedback training and cognitive-behavioral treatments, and social approach can focus on recreational therapy and vocational rehabilitation. Occupational therapy will encourage functional restoration, pain management, and regaining of psychological components with collaboration between different disciplines.

  • Conversion disorder and factitious disorder should be ruled out before making diagnosis of malingering. For this work, inspection of patient's behavior along with complete neurological examinations, psychological tests, and meticulous psychiatric interview are necessary. Facial expression test, thermography, dynamometry were failed differentiating conversion disorders to the malingered pain and motor symptoms, however, controlled diagnostic block showed positive result partly in patients with regional or cervical pain syndrome. Chronic pain patients who are related to the process of litigation encounter stressful life situations which lead them into various neurobehavioral and psychosocial complications. Most of suspected malingered patients would not revealed pure form rather mixed with factitious and/or conversion features. At the time of increasing number of chronic patients associated with traffic accidents or industrial injuries who are involved in litigation, psychiatrist are needed sharp eyes and tenacity for evaluating malingering or medically unexplained symptoms.

A Study on the Status of Oral Health & Oral Health Attitude of Workers in Industries (근로자의 구강건강실태와 구강건강관리행동에 관한 조사)

  • The purpose of this study was to obtain baseline data for establishing oral health policy and developing oral health among industrial workers. A question was used to question paper 226 workers of D heavy industrial company in Sacheon-City. The result obtained were as follows: 1.Generally dental patients asked to not prevention treatment but treatment of dental disease. 2.Most of workers respondent that their oral health is so so.(52.2%). 3.Respondents reported 76.1% of dental calculus, 55.8% of sensitive to cold and hot things. 4.Oral health attitude is tooth brushing experience (39.8%), scaling experience (75.7%), when brushed area all teeth, gingiva and tongue(47.3%). 5.81.8% of respondents received no teeth pain when brushing time is over 3 minutes and 83.7% of smokers had calculus.

Pain Management Knowledge, Attitudes, and Performance of Nurses in Long-Term Care Hospitals (요양병원 간호사의 통증관리에 대한 지식, 태도, 수행정도)

  • Purpose: This study investigated long-term care hospital nurses' knowledge and practice of pain management and their attitudes towards the job with an ultimate aim to provide fundamental information for development of a pain education program. Methods: A cross-sectional survey was carried out with 120 nurses from four long-term care hospitals. Nurses' knowledge of and attitudes towards pain management was measured using a tool developed by Watt-Watson. To examine their pain management practice, an instrument was developed based on the pain management guidelines used by the long-term care settings. Results: For pain management knowledge, the participants gave an average of 26.2 (${\pm}13.10$) correct answers out of 40 questions. The most frequently missed question was one about subjectivity of pain, "Patients' physiological and behavioral reactions to pain hint at the presence and intensity of pain" (89.2%), and 56.7% of the nurses believed that increasing tolerance for a narcotic analgesic means addiction. Regarding attitudes towards and practice of pain management, 80.2% of the nurses used placebos to patients who complain about pain often. Conclusion: This study revealed poor level of pain management knowledge of and attitude among long-term care hospital nurses. This might negatively affect their pain management practice. From the educational perspective, nurses should be provided with education on pain management education with emphasis on the nature of pain and misuse of placebo drugs.

The effect of oral breast milk on pain response of the neonates during heel lancing (모유경구투여가 발뒤꿈치 천자 시 신생아의 통증반응에 미치는 효과)

  • This study was performed in order to investigate the effect of breast milk on pain relief in newborn during heel lancing. Nonequivalent control group pretest-posttest design by double blinded experiment was used. 50 neonates were randomized to receive breast milk (experimental group, n=25) or no treatment (control group, n=25) before undergoing heel lancing. Informed consent was obtained from parents of 50 neonates. Neonatal infant pain scale (NIPS), respiration rate, heart rate, oxygen saturation, and crying duration were used to assess subjects' pain reaction. Data were analyzed using SPSS 18.0. The experimental and control group showed a statistically significant differences in NIPS score (F=28.89, p<.001), heart rate (F=14.03, p<.001), respiration rate (F=4.79, p=.001), oxygen saturation (F=2.69, p=.027), and crying duration (t=-8.78, p <.001) at each time points (during heel lancing, right after heel lancing, 1, 2, 3 minutes after heel lancing). The result suggest that oral administration of expressed breast milk is safe and natural agents for reducing procedural pain of neonates. Another clinical trial study with more samples is recommended.

A Study on the Preliminary Validation of a Postoperative Pain Measure for Parents for Children's Pain Assessment after Surgery (아동의 수술 후 통증사정을 위한 부모용 통증행동관찰척도의 타당성에 대한 연구)

  • Parents are primary care taker for the children and have an important role for the assessment and managent of children's pain following surgery. The purpose of the present study was to examine the validity and clinical utilization of the Postoperative Pain Measure for Parents (PPMP) developed by Chambers et al. Subjects were 52 children aged 4-12 years admitted for tonsillectomy and other minor surgery and their mothers. Faces Pain Scale, State Anxiety, and Postoperative Pain Measure for Parents were used. The data were collected by two research assistant on the operation day and 1st day after surgery at hospital during the period of July 20 to August 28, 1998. The results are as follows: 1. Eta correlation coefficient between 15 items of PPMP and child rated pain were calculated. Correlation coefficients were more than .2 for both day. 2. Internal consistency for PPMP were .82 and .83. 3. The scores of the PPMP were 10.73 (SD=3.71) and 9.27(SD=4.07) on the operation day and 1st day after surgery and there was no significant difference between two days(p=.056) On the other hand, there was a significant difference on the child rated pain by Faces Pain Scale between operation day and 1st day after surgery(p=.001). 4. The correlation(Spearman Rho) between PPMP and child rated pain were .40(p=.003) and .56(p=.000). The score of the PPMP and the children's state anxiety were highly correlated on the operation day and 1st day after surgery (.60, .52, p=.000). 5. Partial correlation between PPMP and child rated pain except state anxiety were .18(p=.23) and .48(p=.001) on the opration day and 1st day after surgery. 6. Using a cut-off score 10 out of 15, the measure showed excellent sensitivity (>80%) and moderate specificity (46.15%, 60% ). This study provides preliminary evidence for the use of the PPMP as a valid pain assessment tool with children between the ages of 4-12 years following surgery. It is suggested to explore the validity with a different subjects with other surgery and to examine the validity for infant and younger children.

Health Risk Behavior and Oral Symptoms in Adolescents in Multicultural Families (다문화 가정 청소년의 건강 위험 행동과 구강 증상에 관한 연구)

  • This study used the raw data of the 15th (2019) Korean Youth Health Risk Behavior Survey to investigate the risk factors of the health-risk behaviors of Korean multicultural adolescents on the subjective oral symptoms. The scope of the multicultural family was limited to children of immigrant women's families married to Korean men and immigrant male families married to Korean women, and 572 were selected as the final study subjects. The sociodemographic variables were surveyed as gender, grade, economic level, and school performance. The health risk behaviors included drinking, smoking, medication, stress, sleep satisfaction, suicidal thoughts, depression, and brushing before bedtime. The oral symptoms examined were toothache, gum pain, and halitosis. As a result, toothache was related to drinking, toothbrushing before sleep, stress, sleep satisfaction, suicidal thoughts, and depression. Gum pain was associated with drug use, sleep satisfaction, suicidal thoughts, and depression. Halitosis was associated with medication, brushing before sleep, stress, suicidal thoughts, and depression. In terms of health risk factors, drinking, stress, and sleep satisfaction were risk factors for toothache. Smoking, sleep satisfaction, and suicidal thoughts were risk factors for gum pain. The presence of toothbrushing and stress were risk factors for halitosis. In conclusion, a policy system is needed to support oral health education programs nationally at times, such as after-school activities or club activities, to promote oral health for teenagers.

The Characteristics of Illness Behavior in Patients with Somatization (신체화에 따른 질병행동의 특성에 관한 연구)

  • Abnormal illness behavior in patients with somatoform disorders were known formed by their traditional disease concepts and somatization-prone socio-cultural factors. The authors evaluated the characteristics of abnormal illness behavior in patients with somatoform disorders(who had somatization) by using abnormal illness behavior questionnaire. Methods : 29 somatoform disorders(SD) and 57 disease controls were compared by clinical characteristics, severity of pain, state anxiety(by Spielberger's State & Trait Anxiety Inventory), depression(by Beck's Depression Inventory) and level of psychosocial stess(by DSM-III-R). The illness behavior was measured by illness Behavior Questionnaire(IBQ). Results SD group had longer period of somatic symptoms with less severity in pain. The degree of anxiety and depression were higher in SB compared with controls. However, the degree of psychosocial stress was almost same between both groups. In IBQ, SD showed higher scores in general hypochondriasis, disease conviction, and affective disturbance subscales compared to control group. Conclusion: High disease conviction and hypochondriacal nature revealed by IBQ seemed to be a role in making somatization by way of somatic focusing and hypervigilance. And those tended to lead patients visit hospital frequently and report various somatic complaints. Evaluating abnormal illness behavior in somatoform disorders would be not only helpful in understanding the natures of somatoform disorders but also useful differentiating SD with other psychiatric conditions.

Effects on pain behavior in non-medicinal treatment applied to chronic headache patients (만성두통환자에게 적용한 비약물적 치료가 통증행동에 미치는 영향)

  • Objectives : This study is a random-controlled post-design for confirmation of headache degree, quality of life, and satisfaction to therapeutical effect on chronic headache adults after management of non-medicinal treatment (acupuncture therapy and stellate ganglion block therapy). Methods : 51 clinical experiment participants were gathered and through a questioutaire patients who experienced headache for more than 4 hours a day and more than 15 days per month were qualified as chronic headache patients. The qualified patients were classified in to two groups, acupuncture group (AT group, n=28) and stellate ganglion block group (SGB group, n=23). Treatment was applied 2 times a weeks for 4 weeks. The effects of both groups were analyzed using VAS scores, BPI (Brief Pain Inventory) and the satisfaction degree to the therapy. Results : 1. The recognized score of the headache of AT group and SGB group was reduced indicating the degree of the headache was released. 2. The recognition at damage to the quality of life was reduced post therapy of AT group and SGB group, in which general activity, mood, enjoyment of life, personal relationship, and sleeping showed significant improvement of life quality, 3. The satisfaction degree to the therapy showed lower score than expectation to it in AT group and SGB group, however, it was not significant. Conclusions : The results showed that the four-week non-medicinal treatment (AT and SGB) in chronic headache patients was effective for reducing headache and releasing its damage in daily life, however, no difference in superiority was found. Therefore, non-medicinal treatment (AT therapy and SGB) could be utilized in chronic headache patients.
