The Effects of Auricular Electroacustimulation and Transcutaneous Electrical Nerve Stimulation on Postoperative Pain Control in Total Knee Replacement Patients (외이전기경혈자극과 경피전기신경자극이 슬관절 전 치환슬 환자의 수술 후 통증조절에 미치는 효과)

  • This study was done to determine differences in effect of postoperative pain control in patients receiving auricular electroacustimulation vs transcutaneous electrical nerve stimualtion following total knee replacement surgery. Thirty-one cases referred to physical therapy department after treated by total knee replacement surgery by orthopedic surgery department at the Pohang St. Mary's Hospital from January 1993 through June 1994. Of 31 total knee replacement cases, 13 cases were auricular electroacustimulation group, 11 cases were transcutaneous electrical nerve stimulation group, and 7 cases were control group. The results of the study summerized are as follows: Thirty-one total knee replacement cases(male in 12 cases, female in 19 cases), ranging in age from 34 to 61 years(mean${\pm}$SD=49.90 7.56) with diagnoses of degenerative arthritis(20 cases), rheumatoid arthritis(9 cases), and other(2 cases). In auricular electroacustimulation group, there was a significant change of pain intensity, unpleasantness, and active range of motion after treatment(p<0.01). In transcutaneous electrical nerve stimulation group, there was a significant change of pain intensity, unpleasantness, and active range of motion after treatment(p<0.01). In control group, did not show significant pre-posttreatment differences in pain intensity, unpleasantness, active range of motion(p>0.05). The mean change in pain intensity and unpleasantness, active range of motion from pretreatment baseline for the 3 groups. Auricular electroacustimulation group showed the large magnitude of increase in pain intensity and unpleasantness, active range of motion when compared to its own pretreatment cycle. Transcutaneous electrical nerve stimulation group showed small magnitude of increase in pain intensity and unpleasantness, active range of motion when compared to its own pretreatment cycle. No significant changes were observed in control group. Highly significant differences in pain intensity, unpleasantness, and active rage of motion were found using an ANOVA measures between treatment groups and control group(p<0.01). The squares correlation coefficients of pain and function measures pretreatment-posttreatment differences for each group. In treatment group, there was significant correlation between pain scale and function(p<0.001). In control group, there was no correlation between the pain scale and function (p>0.05). The continuous study is needd for many interesting issues of auricular electroacustimulation in new future.

The Effects of Pulsed Electromagnetic Energy and Microcurrent on Wound Healing in Rabbits. (맥동전자장에너지의 미세전류가 가토의 상처치유에 미치는 영향)

  • The purpose of this study w8s to evaluate the effects of pulsed electromagnetic energy(Diapulse) and microcurrent on the wound healing in rabbits. 15 domestic rabbits were randomly assigned to the PRME(n=5). MC(n=5) and CON(n=91 group. Each rabbits were anesthetized with lidocaine HCL $2\%$. Skin wounds were created laterally on the back of IS domestic rabbits(33cm). From 24 hours after being injured, the rabbits of the PEME group were irradiated with an intensity of 3 at a 300 pulses per second, which were applied for 15 minutes every day during the 12 days. The MC group were stimulated with an intensity of $50{\mu}A$ at frequency of40 pulses per second, which were applied for 15 minutes every day during the 12 days. The CON group were not stimulated. The rabbits were sacrificed and the incised wound pans were processed appropriately for the light microscopic examination on the 3rd day, 6th day and 12th day before the beginning of wound treatment. The areas of wound were measured with metric graph paper. The results were as tallows. 1 The PRME and MC group compared with control group showed that wound closure rate increased on 6th, 12th day. 2. It was found that the CON group did not show a complete maturation and had a chronic inflammatory response. Judging from the irregularity of intercellular space and the loose alignment of connective tissue. these findings showed that wound healing was delayed. 3. It showed that inflammatory cells. fibroblasts and epithelial cells activity rapidly processed in the PEME group compared with the CON group. It was found that the PEMI; group showed a advanced remodeling of epithelial layer and a positive repair of connective tissue. 4. It showed that fibroblasts, epithelial cells and inflammatory cells activity rapidly processed in the MC group compared with the CON group. It was found that the MC group showed a improved remodeling of epithelial layer and a dense connective tissue.

Effects of Auricle Electric Stimulation on Pain, Gait and Balance in the Old Aged with Knee Joint Disease (외이전기자극이 슬관절질환 노인의 통증과 보행 및 균형에 미치는 영향)

  • Purpose: This study examined the application of electric stimulation to the auricle acupoint (frequency 2 Hz, stimulation level: noxious) with 30 elderly people over 65 years that received treatment in a rural hospital to determine the influence of pain, gait and balance in the aged with knee joint disease. Methods: The subjects were divided into three groups; the degenerative joint diseases (DJD) group (1 male, 9 females), the total knee replacement (TKR) group (1 male, 9 females) and the control group (1 male, 9 females). Auricualr electrical stimulation (AES) was applied with low frequency, high intensity transcutaneous electrical nerve stimulation for 10 seconds per each point. Results: 1. For the change of pain according to AES, there was interaction in the resting period (p<0.001) and gait (p<0.001) and pain of the DJD group and TKR group was decreased. 2. The range of motion (ROM) of the knee joint showed a significant difference in interaction for each group of elderly people (p<0.001) and the ROM for the DJD group and TKR group of elderly people was increased. 3. In the analysis of gait speed changes, there was a significant difference in interaction for each group of elderly people (p<0.001) and for gait speed in the DJD group and TKR group of elderly people. 4. It was found in the change of static balance that there was a significant difference in interaction for each group of elderly people (p<0.01) and balance capacity in the DJD group and TKR group of elderly people was increased. Conclusion: The aged with knee joint disease have pain, and a decreased function of gait and balance. AES was an excellent treatment for control of pain, and an excellent treatment to enhanced joint functions. AES was useful for improving gait and balance due to decreased pain.

Quantitative Assessment of Climate Regulating Ecosystem Services Using Carbon Storage in Major Korean Ecosystems (탄소 저장량을 이용한 국내 주요 생태계 기후 조절 서비스 지표 산정)

  • Ecosystems have functions of providing, supporting, regulating and cultural services. In particular, there is an increasing attention to the importance of regulating ecosystem services in carbon sequestration function, since it is closely related to the issue of climate change. In this study, to quantify benefits of climate regulating ecosystem services, the carbon storage was defined as an indicator. Nine major Korean ecosystems were classified and research papers on carbon storage were analyzed. The collected carbon storage data were categorized according to classified ecosystems, methodologies, and carbon storage components. For each category, the mean, standard error and coefficient of variation were calculated. The carbon storage indicator was highest in vegetation biomass of deciduous forest ecosystems. The uncertainty was also estimated by the IPCC 2006 guidelines. The estimations of the uncertainty differed by methodologies and carbon storage components. With exception of forest ecosystems, the limited number of studies were available which might have hindered to conduct accurate estimations. These findings indicate that there are needs for further clarification in the measurement standards by different ecosystems.

Assessment Using Practices and Quality of Food Safety Information on the Internet (인터넷 위생 정보 이용 실태 조사 및 질 평가)

  • This study was conducted to suggest potential future directions in the effective operations of food safety information on the internet. This study consisted of two sections. The first section was a survey that was performed by 186 dieticians in Daegu and Gyeongbuk area. The second section was the assessment of the quality of food safety information on the internet. 163 web sites that provided food safety information on three major searching engines using the search words 'food safety' and 'HACCP' were investigated. The trained panels evaluated the contents of the web sites using a 3-point scale assessment tool. The data collected data from the questionnaires were statistically analyzed using the SPSS package program (version 14.0). The results of this analysis were as follow. The primary purpose of internet usage by detitians was to search for information (87.2%). In addition, the detitians' satisfaction degree of food safety information on the internet was 3.17 out of 5 points. The operators of web sites that were evaluated by the researchers were business and industries (76.1%), public organizations (15.3%), and individuals (8.8%). The following four items, update of content, offer scientific basis or explanation of content, offer useful information and new informations, should be improved immediately. Among evaluation items, six items were shown to be significantly different according to the operators of web sites. The evaluation scores of web sites that were operated by public organizations were significantly higher than those of web sites operated by individuals or business and industries. In conclusion, regular and consistent qualitative evaluation of web sites is needed to improve the quality of information that is provided via the internet. In addition, web sites that provided information on food safety should be actively operated by public organizations.

Marine Algal Assemblages on Artifical Reefs in Jeju-do Before and After Rocky Cleaning and the Growth Pattern of Ecklonia cava with Water Depth (갯닦기 전후 제주도 인공어초의 해조상 및 수심별 감태의 생장양상)

  • Marine algal assemblages on the artificial reefs at three stations (Haengwon, Geumneung, Pyoseon in Jeju-do) and the growth pattern of Ecklonia cava with water depth were studied before and after rocky cleaning. Nine algal species occurred at three artificial reefs before rocky cleaning in July 2012, however, 19 algal species were found at three artificial reefs after rocky cleaning in July 2013. In particular, 13 of 19 species in 2013 were replaced by different species which were not found in July 2012. Algal biomass rapidly increased in July 2013 after rocky cleaning. The nMDS plot based on the presence and absence data of macro-algal assemblages on the artificial reefs showed that the species compositions between artificial reefs were similar to each other before rocky cleaning operations in July of 2012. However, after rocky cleaning, the species composition of macro-algal assemblages in Haengwon region was similar to that in Pyoseon region while that in Geumneung region was different to those in Haengwon and Pyseon regions in July 2013. It is needed to clarify the suitable water depth for transplantation of perennial Phaeophyta E. cava. According to the data on seasonal changes in total length, total weight, blade length, blade width, blade weight of E. cava, it began to grow rapidly from April and reached to the maximum value in June, and then degeneration of the blade occurred in July and continued to early August. Although the total length of E. cava at 10m water depth was larger than those at 5 m and 15 m water depths, there was no significant difference statistically among water depths by ANOVA test. However, in the values of total weight, blade length, blade width, and blade weight, the growth patterns at 5 m water depth were larger than those at 10 m and 15 m. Thus, it is assumed that the most suitable water depth for transplantation and its maximum growth and effective release of zoospores of E. cava will be 7~10 m water depth.

Anatomical Measurement of the Masseter Muscle and Surface Mapping of the Maximal Thickness Point Using Computed Tomography Analysis (전산화단층촬영 영상분석을 이용한 교근의 해부학적 계측 및 최대 두께점 피부 표지화)

  • Purpose: Masseter muscle is an important muscle of mastication. Because it has a great influence on the shape of low facial contour, patients who have masseteric hypertrophy show square-shaped jaw appearance. As aesthetic procedures for the reduction of the masseter muscle volume, radiofrequency ablation or botulinum toxin injection is at the center of attention. Authors studied the anatomical measurement of the thickness and width of masseter muscle and the surface mapping of the maximal thickness point using computed tomography (CT) scan to identify the useful guide for the injection of botulinum toxin in masseteric hypertrophy patients. Methods: We analyzed 2 mm-thickness OMU (ostiomeatal unit) CT of 112 normal people (224 masseter muscles) taken from June 2009 to May 2010. First, we measured the thickness, width and depth of the masseter muscle from the skin surface and analysed each by side, sex and age, respectively. The distribution of the thickness of the muscle and the correlation of thickness and width of the muscle were studied also. Second, we underwent surface mapping of the maximal thickness point using CT analysis by means of checking the vertical and horizontal distance from the angle of the mandible. Results: The average thickness and width of the masseter muscle was 17.73 mm and 40.78 mm in the male patients and were 14.33 mm and 37.42 mm in the female patients. Statistically, both figures of the male patients were larger than those of the female patients. However, the depth of the muscle from the skin surface in female patients (7.37 mm) was larger than that of the male patients (6.15 mm). There were no statistical difference in side or age. The width and thickness of the masseter muscle were in the positive correlation. The location of maximal thickness point of the masseter muscle was 27.77 mm vertically and 27.68 mm horizontally in the male patients, and 25.19 mm vertically and 25.42 mm horizontally in the female patients from the angle of mandible. Conclusion: We were able to present statistical evidence of the diagnosis and treatment of the masseteric hypertrophy regarding the anatomical measurements such as the thickness and width. And the maximal thickness point of the masseter muscle may be a useful guide for the clinical procedures of botulinum toxin injection.

Improvement for the Degree of Client Satisfaction in the Sample Collection Room (검체채취실의 고객만족도 향상)

  • The sample collection room(SCR) will have much more influence than all the other departments for the improvement of hospital image, if anyone coming to the SCR in the hospital goes back with the perfect complacency and because most clients who have much stresses and fatigues pay a final visit to the SCR via receipt-diagnosis- acceptance process. SCR has improved its image for the purpose of gratifying clients, in order for clients to visit the hospital again, the quality improvement(QI) team in the Diagnosis Inspection Medical Department has come to a conclusion as follows. The degree of client gratification before improvement marks 65.9 point, but the degree after improvement was 74.2 point. Therefore, satisfaction has increased by 8.3 points. The degree of client gratification in groups before improvement marks (1) service parts-89.2 points (2) facilities and environments-49.1 point (3) toilet facilities-46.3 point. But its gratification after improvement marks (1) 92.5 point (2) 60.1 point (3) 61.0 point. Therefore the degree of satisfaction has increased by (1) 3.3 point, (2) 11.0 point, (3) 14.7 point. The progress of facility improvement plans and the exclusion of improvement on the facility contents in the hospital have made facilities and environments of SCR and toilet facilities to be poorly improved. Although service parts have a good mark, and the facilities and environments are not scoring well, the whole degree clients' gratification of SCR couldn't be helped by the low grade. Therefore the bottom line for the clients' gratification of SCR in the future is to ameliorate the facilities and environments. SCR will take the clients' gratification survey every year and if any items get low marks, that is, below 90 point throughout the survey, SCR will immediately starts the improvement work for the clients' gratification with operating the programs of controlling quality continually, and SCR should induce the operation of services, participating in the kind campaign drive for clients. So SCR will adopt the incentive system for the best staff members who perform these kinds of services.

  • Due to global warming and increased energy costs around the world, interests of energy saving and efficiency have been increased. In particular, forging factories need methods to save energy and increase productivity because of needing amounts of energy uses. To solve the problem, we propose a system, which includes collection, monitoring, and analysis process, to monitor energy uses each facility in realtime based on the IoT devices. This system insists of worksheets management, facility/energy management, realtime monitoring, history search, data analysis through connecting with existed ERP/MES Systems in manufacturing factories. The energy monitoring process is to present used energy collected from IoT devices connected with installed gasmeter and wattmeter each facility. This system provide the change of energy uses, usage fee, energy conversion, and green gas information in realtime on Web and mobile devices. This system will be enhanced with energy saving technology by analyzing constructed big data of energy uses. We can also propose a method to increase productivity by integrating this system with functions of digitalized worksheets and optimized models for production process.

A Study on the Aesthetic Values of Medieval Look Focus on Asceticism, Naturalism, Mysticism and Secularism (중세풍 복식의 미적가치에 관한 연구 -금욕성, 자연성, 신비성, 세속성을 중심으로-)

  • The purpose of this study is to present a theoretical framework for analysis and interpretation of medieval look in fashion by investigating its aesthetic values. Accordingly, it would provide a better understanding of the medieval culture with its composite aspects. Modernism sought to make the Middle Ages in its own image.'New Medievalism' has on the whole tried to avoid reading the Middle Ages onto the modern world. Instead it designates a predisposition about the discipline of medieval studies broadly conceived. Consequently, it has changed the viewpoint of medieval culture from the one-sided angle into the diversified one. As recent studies based on New Medievalism have argued, a close look at the medieval culture revealed the dualistic nature of it, with contradictory aspects such as piety and secularity, ideality and formality coexisting. The characteristics of the medieval culture are categorized to asceticism, naturalism, mysticism and secularism. Asceticism, which is responsible for the melancholic and heavy atmosphere of the medieval culture, is related to christianity and despair of life. It is expressed in medieval dress in forms of body-concealing semi-fitted silhouette, coarse texture and dark color. As a reaction to the extreme splendor and exposure of recent fashion, the ascetic medieval look is attracting attention. The monastic look is characterized by body-concealing silhouettes and minimalized usage of colors and details. Naturalism is the feature seeking for purity of the nature and the human itself. It appears in terms of line as a smooth curve flowing along the body contours, or as a revival style of the Greco-Roman drapery in the medieval dress. Naturalism in the medieval look of the late 20th century fashion is expressed as a pure image with the use of sheer and soft fabric to form a smooth curve flowing along the body contours. Mysticism symbolizes the authority of the Christ and the glory of heaven. It is presented in the medieval dress of the body-concealing straight silhouette made of gorgeous fabrics, brilliant colors and decorations by gems and gold. The insecurity caused by the fin-de-siecle mood is creating a strong interest in religion and it is reflected in fashion as a medieval look reviving the solemn and magnificent style of the medieval dress. Secularism is a reflection of the medieval mind of indulging in the beauty of the mundane world. Heraldry and excessive decorations of the medieval dress are forms of this secularism. These styles of gorgeousness are being refined into a new medieval look of the 1990's. The medieval look of the modern fashion is featured in various styles, reflecting the pluralism of the postmodern society.
