The Effects of Oral Care with Chlorhexidine for Ill Patients to Prevent Ventilator-associated Pneumonia : A Meta-Analysis (중환자의 인공호흡기 관련 폐렴 예방을 위한 클로르헥시딘의 효과 : 메타 분석)

  • This study was to determine the effectiveness of oral decontamination with chlorhexidine for prevention of ventilator associated pneumonia(VAP) using meta-analysis. MEDLINE, Pubmed, Cochrane library CINAHL and RISS, Koreamed, KISS databases were searched. key words used 'Ventilator-associated pneumonia', 'oral care', 'chlorhexidine' Ninety studies met the inclusion criteria for the meta-analysis. data were analyzed by the Revman 5.3 program of cochrane library and assessed for methodological quality using RoB (The Cochrane's Risk of Bias). The main findings of the current study suggest that chlorhexidine oral care have a positive impact on Ventilator associated pneumonia.

THE ANTIMICROBIAL EFFECT OF HORSERADISH(ARMORACIA RUSTICANA) ROOT EXTRACTS AGAINST ORAL PATHOGENS (구강내 미생물에 대한 서양산 고추냉이(Armoracia rusticana) 뿌리 추출물의 항균효과)

  • In this study, the antimicrobial effects of Horseradish (Armoracia rusticana) root extracts against oral pathogens were investigated, and also compared with that of chlorhexidine. The following 7 microorganisms were used in this study, Streptococcus mutans ATCC 25175, Streptococcus sobrinus(d) ATCC 27607, Lactobacillus casei ATCC 393, Staphylococcus aureus ATCC 25923, Enterococcus faecalis ATCC 29212, Actinobacillus actinomycetemcomitans ATCC 29522. Candida albicans ATCC 10261. Horseradish root extracts and chlorhexidine were tested to determine their minimum inhibitory concentration(MIC) and minimum bactericidal concentration(MBC). The results of this study can be summarized as follows : 1. Horseradish root extracts showed antimicrobial effect against the tested oral pathogens. MIC and MBC of this extracts were 30-125, 125-500ppm, respectively. Especially, it was the most effective against C. albicans of other tested microorganisms. 2. Chlorhexidine also showed antimicrobial effect against the tested oral pathogens. MIC of chlorhexidine range between 0.15 and 2.5%, MBC are 0.4-2.5%. In conclusion, it was suggested that AIT had similar antimicrobial effects in the lower concentration, compared with that of chlorhexidine.

  • Bacterial microleakage around restorations is considered the principal cause of pulpal inflammation associated with restorations. The rationale for using cavity disinfectant is based on its antimicrobial properties but the cavity disinfectant may leave debris on dentin surfaces. The purpose of this study was to evaluate the effect of 2% chlorhexidine on shear bond strength and microleakage of adhesive restoration. 45 sound bovine teeth were used for the shear bond strength test and 30 for the micorleakage. For the experimental groups, 2% chlorhexidine was applied before the restorations, and was not for the control groups. The result from the this study can be summarized as follows; 1. No significant difference could be found in shear bond strength of each group. 2. No significant difference could be found in microleakage of each group.

Clinical effect of chewing gum containing chlorhexidine nanoparticles on the dental plaque and gingivitis (클로르헥시딘 nanoparticle을 함유하는 chewing gum이 치태 및 치은염에 미치는 영향)

  • Estimating by clinical index the efficacy of three cases, where chewing gum is mixed with nanoparticle containing chlorhexidine, where chewing gum doesn't contain neither of them, and where with chlorhexidine only, this study has come to the following conclusion. 1. The chewing gum with chlorhexidine and chlorhexidine nanoparticle has shown more reduction of plaque index and gingival index than that with no chlorhexidine. 2. There could be seen a difference between the gum with chlorhexidine and the gum with chlorhexidine nanoparticle. 3. The gum with chlorhexidine nanoparticle has shown less level of plaque index than that chlorhexidine, which difference was only slight. 4. The gum with chlorhexidine nanoparticle wasn't absorved any tooth coloring. To put these results together, it is proposed that mixing chewing gum with chlorhexidine nanoparticle can be an efficient application.

  • 충치와 치석의 원인인 플라그를 줄이기 위해서는 플라그의 원인인 구강내 세균을 줄이기 위해서 클로르헥시딘 제품이 나와 있고, 충치의 원인인 탈회를 줄이기 위해서는 제한급식을 하고 자기 전에 간식을 주면 안된다. 아울러 불소는 재석회화를 촉진시키므로 충치예방에 필수적이다 고양이는 충치가 없기 때문에 불소도포는 필요하지 않다.

Affecting Factors on Hospital Nurses' Practice of Disinfection: Focused on Alcohol, Chlorhexidine Gulconate, and Povidone Iodine (병원 간호사의 주요 피부소독제 인지도와 수행도에 영향을 미치는 요인 - 알코올, 클로르헥시딘, 베타딘을 중심으로 -)

  • Purpose: This study was performed to identify the affecting factors on hospital nurses' practice of disinfection focused on alcohol, chlorhexidine gluconate (CHG), and povidone iodine (PVI). Methods: The participants in this study were 196 nurses of 3 general hospitals and 2 upgrade general hospitals in 5 cities in Korea. Data were collected through self-reported questionnaires in January, 2011. The collected data were analysed by ANOVA, Pearson correlation and stepwise multiple regression using SPSS/WIN 18.0. Results: The subjects used alcohol and PVI daily for intravenous site care and surgical wound dressing respectively. The mean percentage of awareness in practicing main disinfectants were 80.0% and 72.5% respectively. The awareness and practice were highest in alcohol, but lowest in CHG. The mean percentage of practice of disinfectants was 72.5%. There was positive correlation among awareness and practice of main disinfectants. The awareness and nurses' salary explained 34.6% of variance in practice of disinfectants. Conclusion: An educational program focusing on strategy to enhance hospital nurses' awareness would be effective in improving their practice of disinfectants.

A COMPARISON OF STAIN RATIO INDUCED BY CHLORHEXIDINE AND CHLORHEXIDINE VARNISH APPLICATION (클로르헥시딘 바니쉬와 클로르헥시딘 양치액 사용시 치면 착색면적의 비교)

  • The purpose of this study was to compare the staining ratio on the enamel surface following the use of chlorhexidine mouthwash and the chlorhexidine varnish application. Labial and lingual surfaces of maxillary and mandibular incisors of adults were selected to evaluate the staining ratio. The control group was consisted of 8 individuals, the experimental group 1 and 2 were consisted of 50 each. Prophylaxis with pumice was performed to remove the stain already established on the enamel surface of all groups. The group 1 was asked to use chlorhexidine mouthwash(Hexadent, chlorhexidine gluconate 1ml/100ml) for a minute twice a day. The chlorhexidine $varnish^{(R)}$($Chlorzoin^{(R)}$, consisted of solution 1(10% chlorhexidine acetate) and solution 2(polyurethane sealant)) was applied on the enamel surfaces of the group 2. After 4 weeks of experiment, intraoral photogragh of tooth surfaces were taken in order to record the stained area on the enamel of the control and the experimental groups. Outline of teeth and the stained area in the photographs was traced on the OHP film. Scanner and computer processor were used to calculate stained surface ratio.

  • When the symptom of periapical infection is not released by mechanical instrumentation. anti-microbial agents including antibiosis become necessary in order to remove microorganisms from the root canal. Since anti-microbial agents of natural origins are currently popular, more natural remedies are being sought out. As it turns out, it is well known isothiocyanates (ITCs) in horseradish root extract have anti-microbial activity from many studies. In this research, anti-microbial effects of horseradish root extract and chlorhexidine, a typical anti-microbial agent, were investigated and compared against two kinds of obligate anaerobes. Fusobacterium nucleatum and Prevotella nigrescens, that are often discovered in infected root canal, and Clostridium perfringens, which is resistant to antibiotics and frequently used as a control strain for antibacterial studies 1. The MIC and MBC of horseradish root extract were ranged from 87 to 470 ppm and from 156 to 625 ppm against three kinds of obligate anaerobes, respectively. Horseradish root extract showed the strongest anti-bacterial activity (MBC, 156 ppm) against F. nucleatum and also showed anti-bacterial activity against antibiotic resistant obligate anaerobes. C. perfringens. 2. The MIC and MBC of chlorhexidine were ranged from 3.12 to 6.25 ppm and 10.94 ppm against three kinds of obligate anaerobes, respectively. 3. The MIC with 87-470 ppm of horseradish root exact has the same growth inhibiting effect as the one of 3.12-6.25 ppm of chlorhexidine. Likewise, the MBC with 156-625 ppm of horseradish has the similar bactericidal effect as 10.94 ppm of chlorhexidine.

Antimicrobial Activities of Root Surfaces Treated with Tetracycline-Containing Gel and a Mixture of Tetracycline and Citric Acid-Containing Gel : In Vitro study (테트라사이클린 및 테트라사이클린-구연산 혼합젤로 처리한 치근면의 항미생물 활성 변화에 관한 연구)

  • 본 연구의 목적은 비외과적 치주치료시 부가적으로 사용하기 위해 실험적으로 개발한 젤 형태의 테트라사이클린 및 테트라사이클린-구연산 혼합젤의 치근면에 대한 시간에 따른 활성도를 측정하고, 이를 용액 형태의 테트라사이클린 제제 또는 클로르헥시딘 들과 비교하는 것이다. 6명의 환자로 부터 18개의 발치된 치아를 실험대상으로 하였으며, 치아는 발치한 즉시 치석제거술과 치근활택술을 시행한 후 각 각 4개씩 4군으로 나누어 다옴과 같은 처치롤 하였다 ; 1) O.1% 클로르핵시딘 용액에 5분간 침전; 2) 50mg/ml의 테트라사이클린 용액에 5분간 침전; 3) 5% 테트라사이클린 젤에 5 분간 처리 ; 4) 테트라사이클린-구연산 혼합첼로 5 분간 처치; 5) 그리고 2개의 치아는 대조군으로서 멸균된 생리식염수에 5분간 처리하였다. 침전후 치아는 1ml의 tris-buffered saline이 담긴 용기에 옮겨 24시간 간격으로 탈착된 TBS용액올 교체하면서 실온에서 22일간 보관하였다. Porphyromonas gingivalis를 indicator organism으로 하여 microtiter assay를 이용하여 홉광도를 측정 함으로써 제거 된 용액 의 항 미생물 활성올 측정하였다. 1. 50mg/ml 의 테트라사이클린 수용액에 침전되었던 군은 생리식염수로 처리한 군에 비하여 17 일간 클로르헥시딘으로 처리한 군에 비하여는 16일간 항미생물 활성에 있어서 유의성 있는 차이를 보였다. 2. 테트라사이클린 젤과 테트라사이클린-구연산 혼합첼로 처리한 군은 대조군에 비하여 각 각 4일과 3일 까지 활성올 보였다. 3. 0.1% 클로르핵시딘 용액으로 처리한 군은 생리식염수로 처치한 군에 비하여 24시간 밖에 활성을 나타내지 못했다. 4. 전반척으로 테트라사이클린-구연산 혼합첼로 처리한 군에 비하여 테트라사이클린 첼로 처리한 군의 활성이 높았으나 유의성 있는 차이롤 보이지는 않았다.
