Feasibility and Efficacy of Adaptive Intensity Modulated Radiotherapy Planning according to Tumor Volume Change in Early Stage Non-small Cell Lung Cancer with Stereotactic Body Radiotherapy (폐암의 정위적체부방사선치료에서 육안적종양체적 변화에 따른 적응방사선치료의 효용성 및 가능성 연구)

  • The purpose of this study is to evaluate efficacy and feasibility of adaptive radiotherapy according to tumor volume change (TVC) in early stage non-small cell lung cancer (NSCLC) using stereotactic body radiotherapy (SBRT). Twenty-two lesions previously treated with SBRT were selected. SBRT was usually performed with a total dose of 48 Gy or 60 Gy in four fractions with an interval of three to four days between treatments. For evaluation of TVC, gross tumor volume (GTV) was contoured on each cone-beam computed tomography (CBCT) image used for image guidance. Intensity modulated radiotherapy (IMRT) planning was performed in the first CBCT (CBCT1) using a baseline plan. For ART planning (ART), re-optimization was performed at $2^{nd}$, $3^{rd}$, and $4^{th}$ CBCTs (CBCT2, CBCT3, and CBCT4) using the same angle and constraint used for the baseline plan. The ART plan was compared with the non-ART plan, which generated copying of the baseline plan to other CBCTs. Average GTV volume was 10.7 cc. Average TVC was -1.5%, 7.3%, and -25.1% in CBCT2, CBCT3, and CBCT4 and the TVC after CBCT3 was significant (p<0.05). However, the nine lesions were increased GTV in CBCT2. In the ART plan, $V_{20\;Gy}$, $D_{1500\;cc}$, and $D_{1000\;cc}$ of lung were significantly decreased (p<0.05), and $V_{30\;Gy}$ and $V_{32\;Gy}$ of the chest wall were also decreased (p<0.05). While D min of planning target volume (PTV) decreased by 8.3% in the non-ART plan of CBCT2 compared with the baseline plan in lesions with increased tumor size (p=0.021), PTV coverage was not compromised in the ART plan. Based on this result, use of the ART plan may improve target coverage and OAR saving. Thus ART using CBCT should be considered in early stage NSCLC with SBRT.

Evaluation of Effective Dose for Asian Adult Men on Dental Cone-Beam Computed Tomography using Personal Computer Based Monte Carlo Simulation (개인용 컴퓨터 기반 몬테카를로 시뮬레이션을 이용한 동양인 성인 남성의 치과용 콘빔시티 유효선량 평가)

  • Cone-Beam Computed Tomography (CBCT) for dental diagnosis has been used for maxillofacial imaging in dental radiology for many years, and increasing in popularity. The number of such inspections increased by approximately 32.4% from 850,000 in 2020 to 1.12 million in 2022. With the increase in dental radiology examinations, the collective effective dose from dental radiology increased by approximately 19.4% from 766 man·Sv in 2020 to approximately 914 man·Sv in 2022. In this study, the effective dose of Cone Beam CT examination was evaluated using Personal Computer(PC)-based computer simulation. The simulation were set identically to the test conditions of the imaging equipment (RCT-820 Ray Co., Ltd.) and compared two methods with Field Of View(FOV) values of 18×16.5 cm and 16×10 cm and an asian adult men phantom was used. The average result at 18×16.5 cm FOV was 223.1 µSv, and the effective dose value at 16×10 cm FOV was averaged at 122.8 µSv. As the irradiation area became smaller, the effective dose value decreased, and the absorbed dose to organ was 1.6 mSv in the 18×16.5 cm FOV, indicating a considerably high tissue equivalent dose. As a result, this study finds the importance of appropriate FOV setting and protective equipment such as a thyroid protector when Cone Beam CT test is performed at a dental site. Also, It is believed that it can be expected that dental medical workers will be able to actively and carefully control irradiation dose settings using the easy-to-use PCXMC program.

Digital technique in diagnosis and restoration of maxillary anterior implant: a case report (디지털 기법을 활용한 상악 전치부의 진단 및 수복 증례)

  • The implant prosthesis of anterior maxilla requires careful consideration in planning. In order to satisfy both esthetic and functional needs of a patient, fusion of intra-oral scan in Cone-beam computed tomography (CBCT) and facial scan can be considered. Bony structures and soft tissues captured in CBCT and occlusal surfaces of intra oral scan were incorporated into personal characteristics from facial scan. The patient had insufficient buccal bone on maxillary anterior area. The maxillary implants could not be placed on the most ideal position. However, the "top down" approach completed by computer-generated arranging of teeth in implant planning and surgery with surgical guide resulted in esthetically and functionally satisfying result regardless of the limitation. Careful diagnosis with digital technique and the usage of surgical guide resulted in successful surgery and esthetic restoration. The temporary fixed prostheses were designed, restored and evaluated. The patient was not satisfied with the first design of temporary prosthesis, which showed uneven space distribution between teeth due to the position of maxillary implant. The design was modified by changing proximal emergence contours and line angle to alter the perceived since of incisors. The patient was satisfied with the new design of provisional restoration. A digital occlusion analyzer (Arcus Digma II, KaVo, Leutkirch, Germany) was used to measure inherent condylar guidance and anterior guidance of a patient to provide a definitive prosthesis.

ERUPTION PATTERN OF THE MANDIBULAR FIRST MOLAR USING THE CONE BEAM CT (Cone Beam CT를 이용한 하악 제 1대구치 맹출 양상에 관한 연구)

  • The purpose of this study was to investigate the eruption pattern of the mandibular first molar in sagittal, frontal and horizontal views using the cone beam CT scanning. CT images were obtained from healthy 83 children (42 boys, 41 girls) between 3 to 10 years of age with a normal dentition according to Nolla stage. 1. In the frontal and horizontal view, the intermolar width decreased continuously with stage and slightly increased at the last stage. 2. In the sagittal and frontal view, eruption distances from occlusal plane were observed the largest change between stage 5 and 7. 3. In the horizontal and sagittal view, mandibular first molar from distal surface of primary second molar moved distally between stage 4 and 6. 4. In the sagittal view, angle from occlusal plane to mesio-distal axis increased between stage 4 and 8. 5. In the frontal view, angle from occlusal plane to bucco-lingual axis increased continuously during all stage. 6. In the horizontal view, angle from midsagittal plane to long axis increased between stage 5 and 8.
