Modeling of compensator fabrication for missing tissue using RFID Tag in U-Health (U-Health 환경에서 RFID Tag를 이용한 결손조직보상체 모델링)

  • U-Health에 대한 연구는 환자, 의료장비에 대한 위치 추적을 통한 의료업무 지원관리 분야로 집중되고 있다. 본 논문에서는 RFID Tag를 의료 측정에 적용한다. 즉, RFID Tag를 이용하여 방사선치료에서 사용되는 결손조직 보상체의 체표 윤곽을 모델링하는 방법을 제안한다. 기존의 모델링 방법은 환자의 체표 윤곽을 컴퓨터단층촬영이나 자기공명촬영을 사용한 의료영상을 이용해왔다. 이러한 방법은 고가의 비용이 소요되고 방사선치료에 따른 체표윤곽의 변화에 대응하지 못한다. 본 연구에서는 U-Health에서 기본적으로 사용하는 RFID Tag를 환자의 체표윤각에 고정하여 3차원 위치정보를 획득한다. 체표윤곽에 위치한 RFID의 상대적 위치를 통해 결손조직 보상체를 제작하고 이의 유용성 평가를 위해 기하학적, 선량학적 평가를 수행하였다.

  • The aim of this work is to develop 3-D stereotactic localization system in order to determine the precise shape, size and location of the lesion in the brain in the field of Stereotactic Radiosurgery(SRS) and neurosurgery using CT/MRI/angiography and multi-purpose QA phantom. The algorithms to obtain a 3-D stereotactic coordinates of the target have been developed, and targets on each CT image were superimposed each other on MR/angiography images without distortion corretion. This system was implented in Visual C++ as a PC-based application program.

Three Dimensional Computed Tomography in the Assessment of Subtle Fracture in Dogs (컴퓨터단층촬영에서 3차원 재구성 영상을 통한 개의 골절 진단)

  • Three dimensional computed tomographic images were obtained in two cases with trauma. The first case of a 3 year-old male Maltese, with ataxia and head tilting due to head trauma was referred to veterinary medical teaching hospital, Seoul National University. Remarkable findings were not found on survey radiographs. With the help of three-dimensional reconstruction computed tomographic imaging, parietal and occipital bone fracture was identified. The second case of 4 month-old female Yorkshire terrier with left forelimb lameness was referred right after trauma. Survey radiography showed obvious incongruity of the elbow joint. Lateral and medial condyle of the left humerus fracture and lateral displacement of the left ulna were apparent in three-dimensional computed tomographic image. It was considered that three-dimensional computed tomography could be used as an aid modality for the exact evaluation of extends and degree of fracture as well as planning of orthopedic surgery.

  • Paragonimiasis was one of the most common causes of cavitary lung lesions until the 1960s, but now it has become a very rare disease in Korea. A 16-year-old boy presented with hemoptysis and several days of cough. His plain chest radiograph showed nodular opacity with a cavity in the left upper lung region. Although his symptoms disappeared after taking antituberculous drugs, his plain chest radiograph and computed tomography images after completion of therapy showed a new nodule with a cavity in the left lung field. Through video-assisted thoracoscopic wedge resection, Paragonimus eggs were found in the lung tissues.

  • 컴퓨터 단층촬영(CT)을 활용한 골격근 단면적은 근감소증과 관련된 기능을 평가하는 데 사용된다. 일반적인 근감소증 연구는 요추 3번의 골격근량을 주로 보지만 암 또는 폐절제술과의 상관관계를 예측하기 위한 다양한 연구에서는 흉추 4번, 7번, 8번, 10번, 12번 다양한 수준의 골격근량으로 연구를 진행하고 있음을 알 수 있다. 본 논문에서는 흉부와 복부 CT 영상에서 근감소증 진단을 위해서 흉추와 요추의 영역별 슬라이스를 검출하기 위해서 CNN 구조의 EfficientNetV2를 전이학습하여 인공지능 모듈을 개발하였다. 인공지능 모듈은 전체 흉부 및 복부 CT 영상에서 Cervical, T1, T2, T3, T4, T5, T6, T7, T8, T9, T10, T11, T12, L1, L2, L3, L4, L5, Sacral 총 19 클래스를 검출하도록 하였다. Test 데이터셋을 사용하여 Confusion Matrix와 Grad-CAM으로 모델의 정확도를 시각화하여 보였으며 검증으로 인공지능 모듈의 정확성을 측정하였다. 끝으로 우리가 개발한 다기관 공동연구 지원플랫폼에 적용하여 시각화된 결과를 보였다.

3D Shape Reconstruction using the Focus Estimator Value from Multi-Focus Cell Images (다초점 세포 영상으로부터 추정된 초점 값을 이용한 3차원 형태 복원)

  • As 3D cell culture has recently become possible, it has been able to observe a 3D shape of cell and volume. Generally, 3D information of a cell should be observed with a special microscope such as a confocal microscope or an electron microscope. However, a confocal microscope is more expensive than a conventional microscope and takes longer time to capture images. Therefore, there is a need for a method that can reconstruct the 3D shape of cells using a common microscope. In this paper, we propose a method of reconstructing 3D cells using the focus estimator value from multi-focal fluorescence images of cells. Initially, 3D cultured cells are captured with an optical microscope by changing the focus. Then the approximate position of the cells is assigned as ROI (Region Of Interest) using the circular Hough transform in the images. The MSBF (Modified Sliding Band Filter) is applied to the obtained ROI to extract the outlines of the cell clusters, and the focus estimator values are computed based on the extracted outlines. Using the computed focus estimator values and the numerical aperture (NA) of the microscope, we extract the outline of the cell cluster considering the depth and reconstruct the cells into 3D based on the extracted outline. The reconstruction results are examined by comparing with the combined in-focus portions of the cell images.

A Study on Radiation Dose and Image Quality according to CT Table Height in Brain CT (두부 CT 검사 시 테이블 높이에 따른 선량과 화질에 관한 연구)

  • The height of the table should be considered important during computed tomography (CT) examination, but according to previous studies, not all radiology technologists set the table at the patient's center at the examination, which affects the exposure dose and image quality received by the patient. Therefore, this study intends to study the image quality exposure dose according to the height of the table to realize the optimal image quality and dose during the brain CT scan. The head phantom images were acquired using Philips Brilliance iCT 256. When the image was acquired, the table height was adjusted to 815, 865, 915, 965, 1015, and 1030 mm, respectively, and each scan was performed 3 times for each height. For the exposure dose measurement, optically stimulated luminescence dosimeter (OSLD) was attached to the front, side, eye, and thyroid gland of the head phantom. In the signal to noise ratio (SNR) measurement result, The SNR values for each table height were all lower than 915 mm. As a result of exposure dose, the exposure dose on each area increased as the table height decreased. The height of the table has a close relationship with the patient's radiation exposure dose in the CT scan.

  • Persistent left superior vena cava (PLSVC) is a rare congenital, thoracic, and vascular anomaly. Although PLSVCs generally do not have a hemodynamic effect, several types of PLSVC and some cardiac anomalies may manifest with clinical symptoms. The presence of PLSVC can render catheterization via left subclavian access difficult when placing a pacemaker or central venous catheter. As such, recognizing a PLSVC that is typically incidentally discovered can prevent complications such as vascular injury. Differentiating vessels found in a similar location as PLSVC is necessary when performing thoracic vascular procedures. This pictorial essay explains the multi-detector CT findings of a PLSVC, and provides a summary of other blood vessels that require differentiation during thoracic vascular procedures.

  • Spinal epidural arteriovenous fistulas (SEDAVFs) are rare spinal vascular malformations that are difficult to diagnose and treat. SEDAVFs can be asymptomatic; however, symptoms can arise from the compression of adjacent nerve roots by dilated vein and perimedullary venous reflux, caused by shunting into the epidural venous plexus. A 31-year-old male presented to our institution with a 2-year history of progressively worsening low-back pain, radiating thigh pain, and sensory changes in his lower extremities. MRI and CT angiography demonstrated dilated epidural vascular lesion compressing the nerve root. The SEDAVF was embolized with multiple coils, which alleviated the nerve root compression from the engorged venous varix and improved the patient's radiculopathy. Our experience from this case shows that endovascular coil embolization using the transarterial approach can be an effective treatment for SEDAVF and an alternative to surgical ligations.

Bilateral Persistent Sciatic Arteries Complicated with Aneurysmal Dilatation and Thrombotic Occlusion: A Case Report (동맥류와 혈전성 폐색이 동반된 양측 잔류좌골동맥: 증례 보고)

  • Persistent sciatic artery (PSA), a rare congenital vascular anomaly, increases susceptibility to aneurysms and accounts for 40-61% of the cases. Here, we describe a case of PSA in a 70-yearold man with a history of alcoholic liver cirrhosis. Bilateral complete PSAs were detected incidentally on computed tomography angiography during evaluation for spontaneous intramuscular bleeding in the thigh due to cirrhosis-related coagulopathy. Selective angiography of the left PSA revealed aneurysmal dilatation and thrombotic occlusion of the PSA, which was partially removed with aspiration thrombectomy. Intramuscular bleeding was succesfully managed with empirical embolization of the deep femoral artery.
