국내 교통사고 후유증의 침치료에 대한 체계적 문헌고찰 (Acupuncture for Rehabilitation in Patients with Traffic Accident in South Korea: a Systematic Review)

  • Objectives : This study aims to evaluate current clinical evidence of acupuncture treatment for rehabilitation in patients with traffic accident in South Korea. Methods : Seven Korean databases were searched for prospective clinical trials for acupuncture on rehabilitation in patients with traffic accident from their inception to June, 2009. Only studies conducted in Korean language were searched. Risk of bias in included randomized controlled trials were assessed by Cochrane Handbook procedure. Results : Fifteen clinical trials were included among 31 studies searched. Eight were observational studies, five were non-randomizedcontrolled trials, and two were randomized controlled trials. In all of included studies, acupuncture were conducted with other concomitant treatment. Included studies dealt with such conditions as neck pain, low back pain tinnitus after traffic accident, post-traumatic stress, oculomotor nerve palsy, diplopia and insomnia. All of included studies reported favorable effects of acupuncture group compared to baseline or control group. All of included studies lacked the occurrence of adverse events. High risk of bias were observed in two randomized controlled trials. Conclusions : There is no evidence that acupuncture is effective for rehabilitation of traffic accident. All of included studies lacked appropriate methodological qualities and internal validity. Future welldesigned clinical trials that evaluate the effects and safety of acupuncture treatment for rehabilitation in patients with traffic accident is needed.

태음조위탕(太陰調胃湯)과 전기침(電氣鍼) 병행치료의 비만에 대한 효과 (Effect of Taeyeumjowee-Tang and Electroacupuncture Combined-therapy on)

  • We wanted to study the effect of Taeyeumjowee-tang and electroacupunctre combined-therapy on obese patients. From 1 March, 2000 to 30, September, 2000 we studied 33 out-ward patients who clinically diagnosed obesity and classified Tae-Em-In by QSCC(Questionnaire of Sasang Constitution Classification). We treated the patients by electroacupuncture therapy and taeyeumjowee-tang for 6 weeks, We investigated the change of mean body weight, mean body mass index, mean obesity degree, percent body fat and waist-hip circumference ratio. To analyze the results we used repeat measure method by SPSS ver 7.5. The results were as follows: 1. From $70.4{\pm}11.4\;to\;64.9{\pm}8.7$, mean body weight was significantly decreased according to combined-therapy. 2. From $27.2{\pm}3.2\;to\;24.6{\pm}2.7$, mean BMI was significantly decreased according to combined-therapy. 3. From $116.7{\pm}13.9\;to\;104.4{\pm}12.1$, mean obesity degree was significantly decreased according to combined-therapy. 4. From $32.1{\pm}6.8\;to\;27.4{\pm}7.5$, percent body fat was significantly decreased according to combined-therapy. 5. From $0.90{\pm}0.06\;to\;$0.86{\pm}0.06$, waist-hip circumference ratio was significantly decreased according to combined-therapy. In this study we knew the combined-therapy of electroacupuncture and taeyeumjowee-tang is very efficient in obese patient.

대퇴신경지각이상증 환자에 대한 침치료 및 근막이완요법 병용 치험 1례 (Meralgia Paresthetica Treated with Acupuncture Plus Myofascial Release Technique: Case Report)

  • Objectives : The purpose of this case is to report the improvement after treatment about a patient with meralgia paresthetica. Methods : We treated the patient with acupucture therapy, cupping therapy, electroacupuncture therapy, percutaneous radiofrequency thermoablation and myofascial release technique by Turbo SASO from $26^{th}$ June 2015 to $3^{rd}$ July 2015 by evaluating femoral function with VAS score. Results : After 5 times of treatment, this patient achieved excellent outcome following the technique, showing that clinical symptom as able to walked and pain was disappeared, VAS changed from 10 to 3 and the result of patrick test came out negative. Conclusions : The various symptoms appear in the Meralgia paresthetica such as numbness, paresthesia, and pain in the anterolateral thigh, which may result from either an entrapment neuropathy or a neuroma of the lateral femoral cutaneous nerve(LFCN). Oriental medical treatment for meralgia paresthetica resulted in satisfactory results by diminishing the symptoms progressively during the five days of treatment. Differential diagnosis was based on careful physical examination. More research of meralgia paresthetica is needed.

만성 요통 환자의 침 치료 경험 : 질적 연구 (Acupuncture Experience in Patients with Chronic Low Back Pain : A Qualitative Study)

  • Objective : To explore and describe chronic low back pain (cLBP) patients' experience of acupuncture treatment Methods : Eleven patients with cLBP who agreed and completed the informed consent to take part in this qualitative study were interviewed thoroughly and tape-recorded. Transcribed data were read independently by 4 researchers and analysed thematically with ground theory. Results : All participants chose acupuncture treatment for their cLBP because either they preferred, other people recommended, or felt conventional treatment being more comfortable. Most of the participants experienced improvement during acupuncture treatment and continued treatment motivated by mere expectations. Participants also expressed the specific physical responses/sensations to the effect of acupuncture; various adverse effects during the course of acupuncture treatment. Economic burden was another concern when combined with other therapies in Korean Medicine hospitals. It is considered as a specifically important to those who give acupuncture treatment to obtain a full treatment effect. Conclusions : The emotional facts that Korean patients with cLBP receiving acupuncture treatment benefited by the treatment and considering specific sensations from doctor-patient relationship were turned out to be important to have the maximum treatment effect. Findings from this study should be considered in both acupuncture research and practice.

임신 시 침 치료에 관한 인식 조사 - 한의사, 환자 대상으로 (A Survey on Practitioners' and Patients' Understanding of Acupuncture Treatment during Pregnancy)

  • Objectives : To investigate how Korean Medicine Doctors (KMDs) and pregnant women understand acupuncture treatment during pregnancy with a focus on safety. Methods : A survey was conducted in 163 KMDs and 157 women who were hospitalised in postnatal care clinics after childbirth. KMDs were asked regarding experience in acupuncture treatment for pregnant women and condition/disease for acupuncture treatment, intention to give acupuncture treatment to pregnant women and reason for yes/no, and consideration for acupuncture treatment for pregnant women. Patients were asked regarding their experience in acupuncture treatment before and during pregnancy and willingness to receive acupuncture treatment if necessary during future pregnancy. Results : 78% of KMDs have given acupuncture treatment to pregnant women if necessary, and it was mainly for pain. 63% of KMDs were willing to give acupuncture treatment for pregnant women with consideration of patient’s condition, specific acupuncture points, and stimulation methods. Pregnant women, however, were reluctant to receive acupuncture treatment during pregnancy (58%) in fear of miscarriage or needling itself. KMDs’ clinical experience, patients’ income level and level of education were not associated with greater use of acupuncture treatment during pregnancy (P>0.05). Conclusion : While many KMDs are willing to give acupuncture treatment to pregnant women if necessary, little evidence on its safety and effectiveness prevents patients from seeking acupuncture treatment during pregnancy. Further research on this issue is urgently needed.

침을 이용한 국소 피부주름 치료에 대한 연구 (Study on the Local Wrinkle Cure Using Acupuncture Needle)

  • The wrinkle is a typical sign showing aging. These days lots of people want to have a young and healthy appearance and many medical doctors are studying about wrinkle cure. In fact a young face without wrinkle looks more healthy and energetic. Therefore the wrinkle cure is important at the point of medical view. This research is an investigation of several studies about wrinkle cure using acupuncture needle. We have to consider the problem of the whole body which is related 12 meridians and meridian muscles as well as topical skin and tissues. There are three parts affecting wrinkle formation and cure. The first is a meridian and meridian muscle, the second is a local muscle and fascia on the head and neck, the third is a local skin structure. There are three kinds of method to cure the wrinkle locally. The first one is the perpendicular treatment of acupuncture, called the microneedle therapy, the process of this method is that lots of microneedles stab in the skin to dermis. The second one is the transverse treatment of acupuncture. The process of this method is that an acupuncture needle inserts just under the wrinkle skin. The third one is the incision treatment of acupuncture, called subcision, the process of this method is to cut the fibrous band which connect from the facial muscle to SMS(superficial musculoaponeurotic system) using cuttable needle. The hematoma after treatment let a collagen increase.

교통사고 후 발생한 경항통에 대한 황련해독탕약침, 봉독약침, 중성어혈약침 치료의 효과 비교 연구: 후향적 관찰 연구 (Comparative Effectiveness of Hwangryunhaedok-tang Pharmacopuncture, Essential Bee Venom Pharmacopuncture and Jungsongouhyul Pharmacopuncture for Cervical Pain caused by Traffic Accidents: A Retrospective Observational Study)

  • Objectives The purpose of this study is to compare the effectiveness of Hwangryunhaedok-tang Pharmacopuncture, Bee Venom Pharmacopuncture and Jungsongouhyul Pharmacopuncture on cervical pain caused by traffic accident retrospectively. Methods The retrospective observational chart review was conducted to 52 cases of traffic accident patients who had admitted to Daegu haany University Pohang Korean Hospital from December 1st, 2015 to May 31th, 2017. The patients were classified into three groups by which pharmacopuncture was used as Hwangryunhaedoktang pharmacopuncture treatment group, bee venom pharmacopuncture treatment group and Jungsongouhyul pharmacopuncture treatment group. Additionally, outcomes should include Neck disability index(NDI) and patient's global assessment(PGA) for evaluate the treatment effects. Results Each pharmacopuncture treatment group showed significant decrease of NDI and PGA. But there was no significant difference in NDI and PGA between three groups. Conclusions According to the results, we found out that 3 kinds of pharmacopunctures are considered to be effective and useful for cervical pain patients caused by traffic injury. Further well designed prospective studies are needed.

당귀작약산(當歸芍藥散)과 침 치료로 호전된 상기도 감염 후에 발생한 후각장애 1례 (A Case of Postviral Olfactory Disorder Treated with Dangguijakyak-san and Acupuncture)

  • Objectives : The aim of this study was to report the effect of Dangguijakyak-san and acupuncture on postviral olfactory disorder. Methods : Dangguijakyak-san extract granule was administered three times a day. Acupuncture was applied to 11 acupoints(EX-HN3, GV20, GV23 and both side of LI04. LR03, LI20, EX-HN8) for 15 minutes once a week. Results : The score of olfactory threshold test and TDI(Threshold-Discrimination-Identification) score increased by 3 points. The subjective sense of smell was improved for the substances that could not previoulsy be smelled. Conclusions : This study shows the possibility of using Dangguijakyak-san and acupuncture to treat postviral olfactory disorder.

돌발성 난청의 침치료에 대한 최근 임상 연구 동향 (Recent Clinical Research on Effect of Acupuncture on Sudden Hearing Loss)

  • Objectives : The purpose of this study is to investigate recent clinical studies on the effect of acupuncture on hearing loss. Methods : Based on the PubMed search with the key search terms of 'hearing loss, acupuncture', dated from 2004 to 2017, 1 controlled trials, 3 case reports and 3 case series was found, and were analyzed for this study. Results : 1. The most commonly used acupoints were Shuaigu(GB8) and Fengchi(GB20). 2. The most commonly used meridians were the du channel, the gall bladder meridian of foot-shaoyang, and the triple bunner meridian of hand shaoyang. 3. Acupuncture treatment was effective for conductive and sensorineural hearing loss, especially sudden sensory neural hearing loss. 4. More clinical studies are needed to prove the effectiveness of the acupuncture on hearing loss. To be more objective on the study results, we can measure auditory brainstem response for hearing loss after acupuncture treatment.

팔미지황탕가미 및 침, 뜸 치료를 이용한 당뇨병성 신경병증 치험 1례 (Case Report of Diabetic Neuropathy Treated with Palmijihwang-tang, Acupuncture, and Moxibustion)

  • Objective: Diabetic neuropathy is a common complication of diabetes mellitus. Patients with this disease suffer from various peripheral sensory symptoms that are not easily alleviated. We report on a case of a patient with diabetic neuropathy whose symptoms were alleviated after traditional Korean medical treatment.Methods: A patient with hand and foot numbness and coldness diagnosed with diabetic neuropathy was treated with Palmijihwang-tang (Bai Wei Di Huang Wa, 八味地黃湯) three times a day. The progress was evaluated with the numerical rating scale (NRS), neuropathy disability score (NDS), and neuropathy symptom score (NSS).Results: The improvement of symptoms appeared to be faster and more effective than that of conventional treatment. The improvement was observed from the second day of the treatment.Conclusions: This case study shows that Palmijihwang-tang may be an effective treatment for diabetic neuropathy.
