건강인군과 편마비군에 시행한 침, 전침 및 박동성 전기자장 치료가 상지의 체열 변화에 미치는 영향 (Thermographic Study on Effectiveness of Acupuncture, Electro-Acupuncture and Acupuncture with Pulsed Electromagnetic Therapy on Upper Extremity of Healthy and Hemiplegic Group)

  • Objectives This study was performed to evaluate the effects of acupuncture, electro-acupuncture and acupuncture with pulsed electromagnetic therapy on upper extremity of healthy people and hemiplegic patients by D.I.T.I.. Methods 20 healthy people and 20 hemiplegic patients caused by stroke were selected. They have been treated three different types of treatment; acupuncture, electro-acupuncture and acupuncture with pulsed electromagnetic therapy. And skin temperature of upper extremity has been measured before and after each treatment by D.I.T.I.. The thermographic data was analyzed by 'Independent T-test' and 'Repeated measures ANOVA test'. Results 1. In healthy group, there was very statistical significant difference (p<0.001) of the amount of thermal change by each treatment, and the amount of change was arranged in descending order; acupuncture with pulsed electromagnetic, acupuncture and electro-acupuncture. 2. In hemiplegic group, the skin temperature of hemiplegic side was $0.97^{\circ}C$ cooler than non-hemiplegic side. 3. Skin temperature of all hemiplegic patients was increased after acupuncture treatment. 4. In hemiplegic group, there was statistical significant difference (p<0.05) of the amount of thermal change by each treatment, and the amount of change was arranged in descending order; acupuncture with pulsed electromagnetic, acupuncture and electro-acupuncture. Conclusions The results indicate that acupuncture, electro-acupuncture and acupuncture with pulsed electromagnetic therapy had good effect on the change of skin temperature by stimulating sympathetic nervous system.

교통사고 후 발생한 편타 손상 환자에 대한 신바로약침과 중성어혈약침의 치료효과 비교연구 (A Retrospective Study on the Effects of ShinBaro Pharmacopuncture and Jungsongouhyul Pharmacopuncture on Whiplash Injury by Traffic Accident)

  • Objectives The purpose of this study is to compare the effects of ShinBaro pharmacopuncture and Jungsongouhyul pharmacopuncture on whiplash injury by traffic accident. Methods This study was carried out on 30 patients who received treatment in Jaseng Hospital of Korean Medicine. 30 patients were divided ShinBaro pharmacopuncture group and Jungsongouhyul pharmacopuncture group. Visual analog scale (VAS) and neck disability index (NDI) were compared after treatment. Results 1) Both the ShinBaro group and Jungsongouhyul group showed significant improvement in the visual analog scale (VAS) and neck disability index (NDI) after 3 weeks of treatment. 2) The VAS and NDI of the ShinBaro group decreased gradually with treatment. The VAS scores showed significant improvement up to week 2, but although week 3 showed further improvement compared to week 2, the difference did not reach statistical significance. The NDI scores showed significant improvement consistantly throughout the treatment period. 3) The VAS and NDI of the Jungsongouhyul group decreased with treatment also. The VAS scores significantly improved up to week 2, but though week 3 showed further improvement in comparison to week 2, the difference did not reach statistical significance. The NDI scores showed significant improvement consistantly throughout the treatment period. 4) Although the ShinBaro group showed a swifter decline than the Jungsongouhyul group in both VAS and NDI scores, the difference was not statistically significant. Conclusions We found out that ShinBaro pharmacopuncture group and Jungsongouhyul pharmacopuncture group is effective and useful in whiplash injury by traffic accident. And, further studies will be needed.

요추간판 탈출증에 동반된 편측 족하수 환자 치험 2례 - 장비골근 침치료 중심으로 (A Clinical Study of Two Foot Drop Patients with Herniated Intervertebral Lumbar Disc treated by Acupuncture Treatment on Peroneus Longus Muscle)

  • Objectives : The purpose of this study is to report clinical effects of oriental medical treatments with acupuncture treatment on peroneus longus muscle for two patients with foot drop by Herniated Intervertebral Lumbar Disc(HIVD). Method : The patients were diagnosed as HIVD, and were treated by oriental medical treatments including acupuncture treatment on peroneus longus muscle. And we measured Visual Analogue Scale(VAS), Range of Motion (ROM) of ankle joint, Manual Muscle testing(MMT) and checked the subject power of ankle joint. Result : After acupuncture treatment on peroneus longus muscle, the movement and power of ankle joint were improved in two cases. Conclusion : Acupuncture treatment on peroneus longus muscle showed significant effect on two patients with foot drop patients by HIVD.

본태성 떨림의 침치료 효과에 대한 메타분석 (A Meta-analysis of the Effectiveness of Acupuncture for the Treatment of Essential Tremors)

  • Objectives: This study provides an evaluation of the effectiveness of acupuncture for the treatment of essential tremors (ETs) using randomized controlled trials (RCTs). Methods: Nine databases (PubMed, CINAHL, EMBASE, OASIS, NDSL, CNKI, Wanfang, J-STAGE, and CiNii) and the keywords "Essential tremor", "Benign tremor", "Familial tremor", and "Acupuncture" were used to compile RCTs that used acupuncture to treat ETs. Studies that were not RCTs, such as case studies, reviews, and essays, were excluded. The methodological quality of each RCT was assessed using the Cochrane risk-of-bias tool, and where appropriate, meta-analyses were performed. Results: Initially, 819 studies were found, but 773 were excluded after title and abstract screening. After further screening of the resulting 46 papers, eight RCTs were selected that had been published between 2003 and 2016, and the total effective rate (TER) was used as the primary evaluation method for all eight studies. It was found that the TER was significantly higher in treatment groups (83.33-96.7%) than in control groups (56.5-80.0%), and seven studies included in the meta-analysis showed positive results for acupuncture for ETs (Risk Ratio: 1.43, 95% Confidence Interval: 1.21-1.69, P<0.0001) Confidence Interval. Conclusions: This meta-analysis of clinical trials suggests that acupuncture is effective for treating ET patients; however, further studies are needed or larger populations to prove its effectiveness.

레이노병의 침치료에 대한 연구동향 (A Review of Acupuncture for the Treatment of Raynaud's Disease)

  • Objectives: The purpose of this study was to assess the effectiveness of acupuncture for the treatment of Raynaud's disease. Methods: This review was conducted using nine electronic databases (CINAHL, CiNii, CNKI, EMBASE, J-STAGE, NDSL, OASIS, PubMed, and WANFANG) and the keywords "Raynaud's disease", "Raynaud disease", "acupuncture", and "needling". Only randomized controlled trials (RCTs) using acupuncture for Raynaud's disease were included, and the methodological quality of the trials was assessed using the Cochrane risk-of-bias tool. Results: After screening, six RCTs were selected among 67, and the total effective rate and peripheral blood flow were used as evaluation methods. In five studies, the total effective rate was significantly higher in the intervention group than in the control group. In three studies, peripheral blood flow was higher in the intervention group than in the control group, with two studies being statistically significant and the remaining study not being statistically significant. Conclusions: Acupuncture could be a suitable treatment without adverse effects for Raynaud's disease because it improves peripheral blood flow. However, further studies, including randomized, placebo-controlled, double-blind methods, and clear evaluation criteria are needed.

화병환자의 불면증상에 대한 침치료 유효성 평가연구: 환자-평가자 눈가림, 무작위배정, 일반대조군 임상시험 (The Effects of Acupuncture Treatment on Hwa-byung patient's Insomnia: Patient-assessor blind, Randomized, Placebo-controlled Clinical trial)

  • Objectives: The purpose of this research is to examine the effects of acupuncture treatment on Hwa-byung patient's insomnia. Methods: The study was performed through a patient-assessor blind, randomized, placebo-controlled trial in which the volunteers, data collectors and analysts were unaware of individuals who were receiving the treatment. A total of thirty-seven volunteers were divided into 2 groups. 18 subjects were placed into a trial group and 19 subjects into a control group by a randomization table. The trial group was treated on bilateral Shigu, Ahnmyun, B62 (Shinmaek), and K6 (Chohae), while the control group was not given any other treatment. The ISI(Insomnia Severity Scale) was measured as the 1st evaluative instrument, and then a comparative analysis was conducted by comparing the results with those measured by a Likert scale for major symptoms of Hwa-byung, PSQI, Hwa-byung scale, BDI, STAI, and STAXI. Results: In the ISI, PSQI, and the Likert scale regarding major symptoms of Hwa-byung, Hwa-byung scale, BDI, and STAI, more significant decreases were found in those concerning the trial group compared to those of the control group from a statistical perspective. However, in the STAXI, regarding the trial group, there was no significant decrease compared to the control group. Conclusions: The results suggest that acupuncture treatment is effective in the treatment regarding Hwa-byung patients who suffer from insomnia.

치아 질환의 침 치료 혈위 선택에 대한 고전문헌과 현대 임상연구 비교 (A Comparison Study of Acupuncture Points Selection between Classics of Traditional Medicine and Clinical Trials in Dental Disorders)

  • Objectives : The aim of this study is to summarize and compare acupuncture points used for dental disorder in the classics with those used in recent clinical trials. Methods : We searched the data for acupuncture points used and rationale of acupuncture in dentistry. Following two sources were searched: 1) seven Classics of traditional medicine and 2) clinical trials through Pubmed from January 2000 to March 2013 with no language restriction. Results : Dental pain was the most common disorder in the dentistry section of the Classics of traditional medicine. We found many similarities of acupuncture points used between literatures and clinical trials. From the meridian perspective, large intestine meridian(LI), stomach meridian(ST), triple energizer meridian(TE), and gallbladder meridian(GB) were frequently used in the Classics of traditional medicine and the clinical trials. From the acupuncture point perspective, acupuncture points were selected according to syndrome. The specific points such as five transport points, source points, and connecting points were also used. In the clinical trials, combination of acupuncture points usually consisted adjacent points and distant points. Among them, LI4 was used in all of the studies included in this review. Conclusions : For well-designed clinical trial, appropriate intervention is essential. To establish appropriate acupuncture treatment, we have to suggest reasoning for treatment based on literature and/or expert consensus. Our review only focused on pain relief in dentistry and had many limitations. Further studies based on the literatures such as the Classics of traditional medicine are required to ensure the rationale of acupuncture treatments in various dental disorders.

주관절 외측상과염의 침치료에 대한 국내 연구 동향 분석 (The Domestic Trends of Acupuncture Treatment on Lateral Epicondylitis : A literature review)

  • Objectives : Lateral epicondylitis on elbow is one of the most common causes of elbow pain. Lateral epicondylitis can be managed with acupuncture, but the evidence for its effectiveness is uncertain. The purpose of this review is to investigate the trends of acupuncture treatments on lateral epicondylitis. Methods : We investigated the studies about acupuncture treatments for lateral epicondylitis via searching 5 Korean web databases(KTKP, RISS, OASIS, KCI, Kisti) The key search terms were 'Lateral epicondylitis', 'Tennis elbow', 'External epicondylitis'. 12 research papers(10 case reports, 1 RCT, 1NRCT) were found to be analyzed according to their published year, the titles of journals, published institution, the types of study, the number of cases, the types of treatments, the instruments for assessment and ethical approvals. The effectiveness of acupuncture treatment was classified. Results : 12 papers were published since 2003. The studies on acupuncture treatments about lateral epicondylitis were mainly published in The journal of pharmacopuncture. 10 case reports, 1 non-randomized controlled trials, 1 randomized controlled trials had been under research. In most of the research, the number of the cases were not enough. In 9 cases of the studies, various korean medicine treatments including acupuncture were used to treat the symptoms. In 3 cases of the studies single method was used to treat the symptoms. Visual analogue scale(VAS) and range of motion(ROM), grip strength were used as primary assessments. Among 12 clinical studies, 1 of them were accepted by institutional review board(IRB). Conclusions : In this study, we analyzed the trends of acupuncture treatments on lateral epicondylitis. Reviewing the domestic trends of studies on acupuncture treatments for lateral epicondylitis and examining the strong and weak points of those treatments are essential for the future studies.

  • Objective : The purpose of this study was to investigate the effects of acupuncture treatment on frequently adopted acupuncture point and self exercise therapy for lumbar sprain patient Methods : For this study, a retrospective chart review was conducted. Data are derived from lumbar sprain patients who have received hospital treatment in Daejeon Univ. Cheon-Ahn Korean Hospital. All patients were treated with acupuncture treatment on high frequency acupuncture point (BL25, BL23, BL24, GB30, BL40, BL26, BL60, GB34), physical therapy and herbal medicine and self exercise therapy(Swiss ball exercise, Lumbar stabilization exercise.) We measured visual analogue scale(VAS), Oswestry disability index(ODI), Pain threshold twice(pre-treatment and post-treatment) on 1st day and 5 days later. Results : After being treated, lumbar sprain patients were meaningful improved in VAS, ODI, pain threshold The VAS score, ODI score were statistically decreased after treatment and the pain threshold was statistically increased after treatment. Conclusion : In this study, treatment on frequently adopted acupuncture point and self exercise therapy were effective on lumbar sprain patient.

경도인지장애의 침 치료에 대한 무작위배정 대조 임상시험(RCT) 연구 논문에 대한 질 평가 : STRICTA와 CONSORT 지침을 바탕으로 (Quality Assessment of Studies on Randomized Controlled Trials (RCTs) of Acupuncture Treatment for Mild Cognitive Impairment: Based on the STRICTA and CONSORT Guidelines)

  • Objective: The purpose of this study was to investigate quality of reporting acupuncture intervention for mild cognitive impairment (MCI) based on the STRICTA and CONSORT guidelines Methods: We searched for randomized controlled trial studies of acupuncture treatment for MCI in the MEDLINE (PubMed) database. Once the online search was completed, studies were selected manually by the inclusion criteria, and the selected studies were evaluated by STRICTA and CONSORT guidelines. Results: Five studies were included. The most frequently selected points for acupuncture treatment of MCI were on the head, such as GV20, EX-HN1, GV24, and GB20. By STRICTA guidelines, all items from STRICTA were reported except three items on the checklist. By CONSORT guidelines, apart from six items which were not available, quality assessment was performed with 31 items. Eighteen items were reported by 80% of the studies. Four items were reported by 40% of the studies and another four items were reported by 20% of the studies. One item was not reported in all studies. Conclusion: Most items were reported by STRICTA guidelines, whereas many items were insufficiently reported based on CONSORT guidelines. This needs to be considered in future RCTs of acupuncture treatment for MCI.
