Dentoalveolar Compensation according to Skeletal Patterns of Normal Occlusion (정상교합자의 골격형에 따른 치아치조보상)

  • In general, orthodontists make problem lists and treatment plans based on norms of several cephalometric standards. But consideration of dentoalveolar compensation, which tends to maintain normal dental arch relationship in various skeletal jaw relationships, helps orthodontists make more individualized treatment objectives and plans. The purpose of this study was to classify skeletal patterns of normal occlusion samples by cluster analysis and to investigate the dentoalveolar compensation according to skeletal patterns. The subjects were consisted of 125 subjects who were normal occlusion samples at Seoul National University Dental Hospital, Department of Orthodontics. Lateral cephalograms in centric occlusion were traced and digitized. The skeletal patterns of normal occlusion samples were classified into three horizontal groups and three vertical groups by cluster analysis and ANOVA on the skeletal and dentoalveolar measurements among the groups were carried out. The results were as follows ; 1. Anteroposterior and vertical skeletal relationships of normal occlusion samples were very variable. 2. As the mandibular position was anterior to the maxilla, the maxillary incisors inclined more labially, the mandibular incisors more lingually, and the occlusal plane was flattened due to the anteroposterior dentoalveolar compensation. dentoalveolar height was decreased and upper posterior teeth was uprighted to the palatal plane and lower incisors and lower posterior teeth to the mandibular plane. 4. Lower incisors were more strongly associated with the dentoalveolar compensation than upper incisors according to the anteroposterior and vertical skeletal relationship.

Spectral moment analysis of distortion errors in alveolar fricatives in Korean children (치조 마찰음 왜곡 오류 유무에 따른 아동 발화 적률분석 비교)

  • This study investigated acoustic features in spectral moment analysis, comparing accurate articulations with distortions of alveolar fricatives such as dentalization, palatalization, and lateralization. A retrospective analysis was conducted on speech samples from 61 children (mean age: 5.6±1.5 years, 19 females, 42 males) using the Assessment of Phonology & Articulation for Children (APAC) and Urimal-test of Articulation and Phonology I (U-TAP I). Spectral moment analysis was applied to 169 speech samples. The results revealed that the center of gravity of accurate articulations was higher than that of palatalization, while palatalization was lower than dentalization. The variance of dentalization was higher than that of both accurate articulations and palatalization. The skewness of dentalization was higher than that of accurate articulations, and the skewness of palatalization was higher than that of accurate articulations. The kurtosis of palatalization was higher than that of both accurate articulations and dentalization. No significant differences were observed for the position of fricatives (initial, medial) and tense type (plain, tense) across all variables of spectral moment analysis for each distortion type. This study confirmed distinct patterns in center of gravity, variance, skewness, and kurtosis depending on the type of alveolar fricative distortion. The objective values provided in this study will serve as foundational data for diagnosing alveolar fricative distortions in children with speech sound disorders.

  • This paper investigates the durational difference between syllable final segment and syllable initial one within word level. The Korean consonant (m) and (nn) were focused mainly. It could hardly say that there was significant difference between preceding consonant and following one, but it was observed that the preceding consonant tended to be shorter than the following one in the (mm) case. This might be explained by the fact that bilabial sound should appear at the first step of language acquisition. This leads to the conclusion that the articulation of preceding (m) shall be easier than others. In the case of alveolar geminate (nn), there was considerable statistic difference between preceding and following segments. It tends to be that the preceding consonant has longer duration.

Evaluation of alveolar bone density by intraoral periapical radiography (구강 내 치근단 방사선 영상을 이용한 치조골 골밀도 측정의 유용성 평가)

  • Purpose: A method detecting change of jaw or alveolar bone density may be helpful in periodontal care, implant dentistry and evaluation of bone density of whole body. Materials and methods: In this study, bone density of intraoral periapical radiography using phantom-integrated XCP is compared with that of quantitative computed tomography (QCT). Results: Bone density of intraoral periapical radiography and the one measured by QCT showed high correlation (correlation coefficient = 0.92, P<.001) in alveolar bone, and relatively high correlation (0.73, P<.001) in cancellous bone. Conclusion: This study revealed possibility of scoring of bone density by intraoral periapical radiography.

  • Conventional denture impression techniques have limitations for edentulous patients with severe alveolar bone resorption and can cause problems from excessive border extension. Especially when a patient has movable tissue it is difficult to make accurate impression, thus might interrupt stable seating of complete denture. Fabrication of complete denture using closed mouth technique for edentulous patient with severe ridge resorption is thought to provide better stability and retention. In this case, an 86-year-old patient had both edentulous jaws with epulis fissuratum on maxillary anterior ridge and severe mandibular ridge resorption. Thus, tentative vertical dimension was determined by using Centric trayand individual tray attached with gothic arch tracer was fabricated. Complete denture was fabricated using closed mouth technique and the patient was satisfied with better stability and retention of the complete denture.

The Effect of Calcium Sulfate on the Periodontal Healing of 2-Wall Intrabony Defects in Dogs (성견의 2면 골내낭에 Calcium Sulfate 이식이 치주조직 치유에 미치는 영향)

  • 성견의 2면 골내낭에서 calcium sulfate 이식이 치조골 및 백악질 재생 능력과 접합상피의 근단이동에 미치는 영향에 대해 알아보고자 3마리 성견의 상하악 제3대구치 근심면을 연결하는 깊이 4mm, 너비 4mm의 2면 골내낭을 외과적으로 형성하고 calcium sulfate를 이식한 군을 시험군으로, 치은박리술만을 실시한 군을 대조군으로 설정하였다. 술후 8주후에 치유결과를 접합상피의 길이, 백악질 형성, 치조골 형성, 결합조직 유착으로 나누어 조직학적으로 비교 관찰하여 다음과 같은 결과를 얻었다. 1. 접합상피의 길이는 대조군에서 2.28mm, 실험군에서 0.51mm로 나타났으며, 대조군과 실험군간에 유의성 있는 차이가 있었다(p<0.05). 2. 백악질 형성은 대조군에서 1.33mm, 실험군에서 2.59mm로 나타났으며, 대조군과 실험군간에 유의성 있는 차이가 있었다(p<0.05). 3.치조골 형성은 대조군에서 1.02mm, 실험군에서 2.30mm로 나타났으며, 대조군과 실험군간에 유의성 있는 차이가 있었다(p<0.05). 4. 결합조직 유착은 대조군에서 0.50mm, 실험군에서 1.14mm로 나타났으며, 대조군과 실험군간에 유의성 있는 차이가 있었다(p<0.05). 이상의 결과를 본 실험상의 한계내에서 고려하여 볼 때, calcium sulfate는 2면 골내낭의 치주치료에 치조골 및 백악질 재생 효과가 있으며 안전하고 경제적인 골 이식재 재료로 사용될 수 있으리라 사료된다.

  • In maxilary anteriors, aesthetic aspect are of critical importance. but it is difficult to achieve esthetic results because of the narrow buccal-lingual alveolar bone width compared to the posterior teeth and alveolar bone resorption during tooth extraction. This case report describes how to minimize alveolar bone resorption and soft tissue collapse when immediate implant placement is done after extraction of the maxillary anterior teeth due to trauma.

Finite-element analysis of the shift in center of resistance of the maxillary dentition in relation to alveolar bone loss (치조골 상실에 따른 상악 치아군 저항중심의 변화에 관한 유한요소해석)

  • Objective: The aim of this study was to investigate the changes in the center of resistance of the maxillary teeth in relation to alveolar bone loss. Methods: A finite element model, which included the upper dentition and periodontal ligament, was designed according to the amount of bone loss (0 mm, 2 mm, 4 mm). The teeth in each group were fixed with buccal and lingual arch wires and splint wires. Retraction and intrusion forces of 200 g for 4 and 6 anterior teeth groups and 400 g for the full dentition group were applied. Results: The centers of resistance were at 13.5 mm, 14.5 mm, 15 mm apical and 12 mm, 12 mm, 12.5 mm posterior in the 4 incisor group; 13.5 mm, 14.5 mm, 15 mm apical and 14 mm, 14 mm, 14.5 mm posterior in the 6 anterior teeth group; and 11 mm, 13 mm, 14.5 mm apical and 26.5 mm, 27 mm, 25.5 mm posterior in the full dentition group respectively according to 0 mm, 2 mm, 4 mm bone loss. Conclusions: The center of resistance shifted apically and posteriorly as alveolar bone loss increased in 4 and 6 anterior teeth groups. However, in the full dentition group, the center of resistance shifted apically and anteriorly in the 4 mm bone loss model.

A study on the dental arch characteristics of bialveolar protrusion patients using a three-dimensional digital model (3차원 디지털 모형을 이용한 양악 치조골 전돌자의 치열궁 특성 연구)

  • Recent beauty trends have caused an increase in the number of bialveolar protrusion patients seeking treatment of lip protrusion. But studies of these patients are not common. Studies using their dental models are especially rare. Dental models have been measured manually or through a computer by digitizing two dimensional images of models. Nowadays, we are able to study dental models more easily and accurately by utilizing the three-dimensional (3-D) laser scanner in dentistry. An investigation was carried out to evaluate the characteristics of the dental arch in a bialveolar protrusion group in comparison with a normal group using 3-D digital models. The normal group was composed of 20 subjects who were selected from students of the School of Dentistry, Kyungpook National University. The bialveolar protrusion group was composed of 20 subjects who visited for treatment at the Department of Orthodontics, Kyungpook National University Hospital. Tooth size, arch width and arch length were measured digitally, and arch shape and the shape of the palate were drawn. Based on the results of this study, the differences of the arch characteristics in the bialveolar protrusion group were that the size of the teeth was larger, mandibular canine width and 1st premolar width were wider, and arch length was longer than in the normal group. And there were differences in the arch shape and the shape of palate between the bialveolar protrusion and normal groups.

Analysis of dentoalveolar compensation and discrimination of skeletal types (골격형에 따른 치아치조성 보상기전의 분석 및 골격형 판별)

  • The purpose of this study is to analyze dentoalveolar compensation in normal occlusion samples previously classified into 9 skeletal types, and to provide clinically applicable diagnostic criteria for individual malocclusion patients. Cephalometric measurements of the 294 normal occlusion samples previously divided into 9 types were analyzed. The descriptive features of dentoalveolar variables were compared for the 9 types using analysis of variance, followed by post hoc multiple comparisons. In addition, the correlation between skeletal and dentoalveolar variables were analyzed. Discriminant analysis with a stepwise entry of variables was designed to find out several potential variables for use in skeletal typing. The dentoalveolar compensation pattern of the skeletal types varied, especially with regards to the variables that indicated the inclination of incisors and the occlusal plane. Stepwise variable selection identified four variables: AB-MP, SN-AB, PMA and ANB. Discriminant analysis assigned a classification accuracy of $87.8\%$ to the predictive model. On the basis of these results, this study could provide rudimentary information for the development of diagnostic criteria and treatment guidelines for individual skeletal types.
