치과용 임플란트(Implant)란 상실된 자연치아를 대신하여 골 내에 매식하는 인공치근을 말한다. 임플란트는 인접 자연치아의 보호, 심미적 안정 등의 장점으로 인해 그 수요가 늘어나고 있으며, 단일치아 임플란트의 경우, 부분 무치악 환자들에게 있어서 우수한 치의학적 해법이 되어왔다. 대부분의 임플란트는 두 개 이상의 구성요소로 이루어져 있으며, 각각의 구성요소는 나사에 의해 결합되어 있다. 많은 연구결과를 통해, 임플란트의 나사 풀림 현상(Screw loosening)은 임플란트와 관련하여 가장 흔한 문제로 나타나고 있다.(중략)

Ameloblastoma is a benign odontogenic epithelial tumor with high recurrence rate and requires extensive resection of the surrounding tissue and reconstruction of defect site. Because of the anatomical limitation of the reconstruction site, prosthetic treatment with implants is the first recommendation. This is a case of prosthetic restoration of the reconstruction site with implant fixed prosthesis in patient who underwent mandibular resection and iliac bone reconstruction due to ameloblastoma. However 14 months after completion of implant prosthesis, adjacent natural tooth erupted unexpectedly, resulting in 1mm infra-occlusion occurred including posterior implant prosthesis and anterior natural teeth. In adults, implant infra-occlusion may occur due to residual growth after placement of the maxillary anterior implant. But this case, hypo-occlusion of molar implant and open bite of anterior natural teeth is occurred due to extrusion of adjacent tooth, is rare. Thus we report the treatment process including orthodontic treatment with intrusion of the posterior tooth, and investigate the causes of sudden, unexpected tooth extrusion.

Adult patients who need implant and prosthodontic treatment often need treatment to improve the existing occlusion through orthodontic treatment for long-term stable treatment results. However, due to non-aesthetic and uncomfortable orthodontic treatment with orthodontic brackets and wires, many adult patients give up treatment even though they know the need for it. Recently, as digital dentistry has affected all areas of dentistry, clear aligner orthodontic systems have begun to be widely used, and their use is increasing in adults and old-aged people due to the esthetic advantage and convenient oral care. SERAFIN clear aligner system developed in Korea has been developed with the aim of implementing a functional occlusion harmony and is used not only for partial orthodontic treatment but also comprehensive orthodontic treatment. This patient presentation is shown the treatment using SERAFIN clear aligner system for the treatment of patient with TMD, severe extrusion of maxillary second molars, and extracted teeth.
This study was performed for the purpose of evaluating the stress distribution around threaded type implants, cylindrical type implants and teeth connected with rigid or non-rigid connector. The stress distribution around the surrounding bone was analyzed by three-dimensional photoelastic method. Twelve mandibular photoelastic epoxy resin models and a circular polariscope were used to record the isochromatic fringes. After the stress distribution around the implant and tooth was observed, the results were as follows ; 1. In threaded type implants, stress concentrated patterns were observed at the neck either vertical or 25 degree lateral force. 2. The stress concentrated patterns were observed at the tooth apical portion and neck portions of the implant and tooth when a threaded implant was connected with the tooth by either a rigid or non-rigid connector. More force was generated at the tooth neck portion by a rigid connector and more force at the implant neck portion by a non-rigid connector. 3. The stress concentrated patterns were observed at the apical portion of the implant and tooth when a cylindrical type ,implant was connected with the tooth either by a rigid or non-rigid connector. More force was generated at the tooth apical portion by a rigid connector and more force at the neck portion of the tooth and implant by a non-rigid connector. 4. The stress around the tooth was more equally distributed in a threaded type implant than in a cylindrical implant when the tooth was connected with either a rigid or non-rigid connector. 5. The stress around a threaded type implant was progressively more equally distributed in the following order : 1) when used a single implant, 2) a non-rigid connection with the implant and tooth, 3) a rigid connection with the implant and tooth, 4) a rigid connection with two implant fixtures.
Purpose: The purpose of this study is to evaluate the accuracy of surgical stent according to the supported type. Materials and methods: 5 sets of dental models which have tooth supported edentulous area and tooth-tissue supported edentulous area were made. Dental model were scanned with model scanner, and CBCT was taken. CT data and model scan data were overlapped using In2Guide software, implant were virtually planned in the software. Surgical stents are fabricated by 3D printing. The implant fixture were installed using the surgical stent, CBCT were retaken. CBCT before surgery and after surgery were overlapped, and the differences (angle difference, coronal difference, apical difference) were evaluated using statistical analysis. Results: In the assessment of the accuracy of surgical guides according to arch type, there are no statistically significant differences between maxilla and mandible. In the case of support type, tooth supported stents showed lower angle difference and length difference than tooth-tissue supported stents, which are statistically significant. Conclusion: Arch type does not affect the accuracy of surgical stents. But tooth support stents are more accurate than tooth-tissue support stents in the case of angle and length difference.
The labio-palatal location of the implant in the maxillary anterior region is one of the important factors affecting the aesthetics of the implant prosthesis. However, the thin labial bone of maxilla could be absorbed in significant amounts after extraction of the teeth, which makes the implant be placed on the palatal side rather than the ideal location. In fact, in the cases of maxillary central incisor loss, UCLA was used for prosthetic restoration of palatally placed implant. In addition, with multidisciplinary treatment, GBR (Guided Bone Regeneration) was performed for compensating the absorbed alveolar bone and adjacent anterior tooth were aligned. Definitive restoration was performed after confirming aesthetic recovery of the gingiva with sufficient provisional restoration period. There were satisfactory results of functional and esthetic recovery of tooth loss through implant prosthesis.
Purpose: The purpose of this study was to investigate the stress distribution in mandibular implant-supported overdentures and tooth-supported overdentures with telescopic crowns. Materials and methods: The assumption of this study was that there were 2, 3, 4 natural teeth and implants which are located in the second premolar and canine regions in various distributed conditions. The mandible, teeth (or implants and abutments), and connectors are modeled, and analyzed with the commercial software, ANSYS Version 10.1. Stress distribution was evaluated under 150 N vertical load bilaterally on 3 experimental conditions - between canine areas, canine and $2^{nd}$ premolars, 10 mm posterior to $2^{nd}$ premolars. Results: Overall, the case of the implant group showed more stress than the case of the teeth group in stress distribution to bone. In stress distribution to superstructures of tooth and implants, there was no significant difference between TH group and IM group and the highest stress appeared in TH-IV and IM-IV. The stress caused from bar was much higher than those of implant and tooth. TH group showed less stress than IM group in stress distribution to abutment teeth and implant. Conclusion: The results shows that it is crucial to make sure that distance between impact loading point and abutment tooth does not get too far apart, and if it does, it is at best to set abutment tooth on premolar tooth region. It will be necessary to conduct more experiments on effects on implants, natural teeth and bone, in order to apply these results to a clinical treatment.
목적: 노인 환자에서 식립된 임플란트의 생존율을 조사하고 다양한 요인에 따라 생존율에 미치는 영향을 후향적으로 평가하고자 하였다. 재료 및 방법: 2001년 1월부터 2007년 12월까지 조선대학교 치과병원 치주과에 내원한 65세 이상 노인 환자 56명에게 식립된 138개의 임플란트를 대상으로 하였다.(남자 38명, 여자 18명, 평균 연령$69.38{\pm}3.91$세) 임플란트 생존율은 환자 성별 및 연령, 전신질환 유무, 치아 상실 원인, 식립 위치, 임플란트 직경 및 길이, 골질, 골이식 여부, 상부 보철물 유형에 따라 조사되었다. Kaplan-Meier survival analysis을 통해 각 요인에 따른 누적 생존율을 조사하였고, chi-square test를 통해 누적 생존율과 각각의 요인 간의 통계학적 유의성을 평가하였다. 결과: 보철물 시적 후 5 ~ 140개월 간 추적 관찰을 시행하였다. 65세 이상 노인 환자 56명에게 식립된 총 138개의 임플란트 중 추적 관찰 기간 동안 5개의 임플란트가 실패하였다. 그 중 63개의 임플란트는 환자가 정기적인 내원 약속에 응하지 않아 도중에 조사대상에서 제외되었으며, 결과적으로 5년간 누적 생존율은 94.9%였다. 임플란트 생존율에 영향을 미치는 다양한 요인 중 골질 (P=0.037) 및 상부 보철물 유형 (P=0.015) 간에 누적 생존율이 통계적으로 유의한 차이가 있었다. 그외 연령, 성별, 전신질환 유무, 치아상실 원인, 식립 위치, 임플란트 직경 및 길이, 골이식 유무 관련 요인과 누적 생존율 사이에는 통계적으로 유의한 차이가 없었다. 결론: 여러 한계가 있었지만 노인 환자의 무치악 부위에 있어 임플란트는 장기간에 걸쳐 예지성 있고 받아들여질 만한 치료이면서 적은 합병증과 실패율을 가진다.
