Interleukin-6 and Interleukin-10 in experimentally induced rat Pulpal inflammation (실험적으로 유도된 백서의 치수염에서의 INTERLEUKIN-6와 INTERLEUKIN-10의 농도와 분포에 관한 연구)

  • 연구목적 : Cytokine은 유해 미생물에 대한 숙주의 방어기전으로서의 염증반응에서 숙주세포 상호간의 작용을 매개해 주는 역할을 하며, 인간의 치수조직에서도 그 존재가 확인된 바 있다. Interleukin-6와 Interleukin-10은 염증의 초기에 작용하는 cytokine으로 알려져 있으나, 치수 및 치근단 질환에서의 역할과 상호작용에 대해서는 잘 알려져 있지 않다. 본 연구에서는 치수염의 원인균으로 알려진 Prevotella nigrescens를 이용하여 백서의 치수염을 유도한 후 시간의 변화에 따른 Interleukin-6, Interleukin-10의 농도의 변화를 측정하여 이들의 치수염에서의 작용을 알아보는 것을 목적으로 한다. 방법 : 실험적으로 치수의 염증반응을 일으키기 위하여 치수염의 원인균으로 알려진 Prevotella nigrescens를 이용하였다. 실험동물의 하악절치의 incisal tip부분을 절단한 후(n=120), 치수강을 개방시켰다. 실험군에서는 Prevotella nigrescens를 멸균된 면구에 묻혀서 개방된 치수강 내에 접종하였으며, 대조군에서는 균을 접종하지 않고 멸균된 면구만을 개방된 치수강 내에 위치시켰다. 그 후 1, 2, 5일이 경과되었을 때 실험에 사용된 치아를 발치하여, 치수조직을 적출하였다. Amersham사의 ELISA kit를 사용하여 적출된 치수조직내의 Interleukin-6와 Interleukin-10의 양을 측정하였으며 그 결과를 Mann-Whitney rank sum test를 사용하여 통계학적 유의성을 검증하였다. 조직학적 검사를 위해서는 발치된 치아를 nitric acid를 사용하여 탈회시킨 후 헤마톡실린-에오신 염색을 시행한 후 관찰하였다. 결과 : 1) Interleukin-6의 농도는 균접종 후 1일, 2일, 5일 모두에서 실험군에서 대조군보다 높게 나타났으며, 균접종 1일째의 결과는 통계적 유의성이 있었다(P<0.05). 2) Interleukin-10의 농도는 균접종 후 1일, 2일, 5일 모두에서 실험군에서 대조군보다 높게 나타났으며, 균접종 1일째의 결과는 통계적 유의성이 있었다(P<0.05). 3) Interleukin-10/1nterleukin-6 ratio는 실험군과 대조군 모두에서 1일보다 2일째의 결과에서 더 높은 값을 보였으며 대조군에서는 통계적 유의성을 보였다(P<0.05). 4)조직학적 관찰결과 균접종 후 2일째의 조직표본에서는 림프구의 침윤과 부분적인 조직의 괴사 등 염증반응의 양상을 관찰할 수 있었으며, 균접종 5일째의 조직표본에서는 염증의 정도가 감소되는 양상을 확인할 수 있었다.

Full mouth rehabilitation with a few remaining teeth and implants for a patient with chronic periodontitis: a case report (만성 치주염 환자에서 소수 잔존치와 임플란트를 이용한 전악 수복: 증례 보고)

  • Chronic periodontitis involves subsequent loss of teeth, and if left untreated, can lead to adjacent teeth drifting and supraeruption of the rest dentition. Careful consideration has to be given when deciding extraction of remaining teeth in treatment of periodontally compromised dentitions. For tooth-supported fixed partial dentures or removable partial dentures, periodontally compromised teeth are extracted due to possible early failure from functional overload, but for implant restoration, the teeth could be used as supports for fixed partial dentures because implants can reduce overload on teeth. The remaining natural teeth can help clinicians restoring vertical dimension and normal occlusal plane in full mouth rehabilitation because it conserves patients' proprioceptive response. This clinical report describes treatment of a patient who has a few remaining teeth and supraeruption of the rest dentition from severe chronic periodontitis. Satisfactory clinical result was achieved with full mouth rehabilitation using a few teeth and implants.

  • Extracting mandibular incisors for orthodontic treatment may adversely affect the occlusion. However, when properly used, extraction of mandibular inciors is a selection for the correction of the malocclusion. Generally, treatment for crowding needs to select between nonextraction and four premolar extraction. Approaches for crowded mandibular incisors include distal movement of posterior teeth, lateral movement of canines, labial movement of incisors, interproximal enamel reduction, removal of premolars, removal of one or two incisors, and various combinations of the above. Extraction of incisors is used in case of crowding, anterior tooth size discrepancy, absent of maxillary lateral incisors, and ectopic eruption. But severe overjet. overbite, and space are the contraindication of it. A patient had severe crowding on upper anterior teeth, impacted upper left lateral incisor, palatal ectopic eruption of upper right incisor and severe crowding on lower anterior teeth. Lower lateral incisors are extracted for space availability and facial esthetics. We report the case of orthodontic treatment of upper and lower anterior crowding through extraction of lateral incisor.

A study on the preorthodontic prediction values versus the actual postorthodontic values in Class III surgery patients (골격성 III급 부정교합 환자에서 술전 교정전 예측치와 교정 후 실측치의 차이에 관한 연구)

  • The purpose of this study was to find out and evaluate discrepancies between preorthodontic prediction values and actual postorthodontic values and factors contributing to it in 45 patients(17 male, 28 female) who were diagnosed as skeletal Class III ma)occlusion and received presurgical orthodontic treatment and orthognathic surgery at Yonsei university dental hospital. Lateral cephalograms were analysed at pretreatment(T1), orthodontic Prediction(T2), immediately before surgery(T3) and designated the landmark as coordinates or X and Y axes. The samples were divided according to ALD, upper and lower incisor inclination(Ul to SN, IMPA), COS, extraction, the position of extracted teeth and the statistical significance was tested to find out the factors contributing to the prediction. The results were as follows: 1. Differences between preorthodontic prediction values and actual postorthodontic values(T2-T3) were statistically significant(p<0.05) in the x coordinates of U6mbc, L1x and in y coordinates of U1i, U1x, U6me, U6mbc, L6mbc 2. The accuracy of prediction is relatively higher in horizontal changes compared to vortical changes. 3. The statistical significance(p<0.05) between prediction and actual values is observed more in the landmarks of the maxilla than the mandible. 4. Differences between prediction and actual values of incisor and first molar were statistically significant(p<0.05) according to extraction vs non-extraction, extraction type, ALD in the maxilla and according to ALD, IMPA in the mandible. Discrepancies between preorthodontic prediction values and actual postorthodontic values and factors contributing to the prediction must be considered in treatment planning of Cl III surgical patients to increase the accuracy of prediction. Furthermore future investigations are needed on the prediction of vortical changes.

Rapid canine retraction in a Class II bialveolar protrusion case using a lingually extended distraction screw (제II급 치조 전돌 환자에서 설측 견인 장치를 이용한 급속 견치 견인술)

  • Rapid canine retraction, first introduced by Liou, is a distraction osteogenesis applied to the periodontal ligament tissue. Rapid tooth movement was facilitated by establishing minimal bony resistance on the distal surface of the canine by socket preparation and by osteogenesis on the mesial side in response to the periodontal distraction. Since undesired buccal tipping or extrusion of the canine during retraction tends to occur, it is crucial to maintain the firm path of movement and the axis of the canine during retraction. In order to improve the predictability of the canine movement, lingually extended distraction screws with heavy labial guiding wires were designed. Prefabricated plastic canine models for the estimation of socket depth and miniscrew implants for anchorage reinforcement were also devised. Applying these devices to a female patient with Class II anterior protrusion, the whole treatment was effectively finished in 13 months. Loss of vitality or periodontal problems did not occur throughout treatment, and stable occlusion was maintained during 10 months of retention. This case report demonstrates that a predictable rapid canine retraction can be achieved through the use of this modified technique.

A photoelastic study of the stress distribution on canine retraction by segmented TMA T-loop spring (Segmented TMA T-loop spring에 의한 견치 후방이동시의 응력분포에 관한 광탄성법적 분석)

  • The segmented TMA T-loop spring, used for reciprocal space closure and described by Burstone, was used to achievebodily movement of canine. Photoelastic analysis is a technique for the transformation of internal stress into visible light patterns. The two-dimensional photoelastic stress analysis was performed, and stress distribution was recorded by photography. The purpose of this study was to visualize photoelastically the distribution of forces transmitted to the alveolus and surrounding structures using new segmented TMA T-loop spring for canine retraction. The results were as follows: 1. Decreased activation produced decreased stress of upper 1st. premolar extraction site and increased intrusive stress of upper 1st. molar, regardless of T-loop position. 2. At 5mm activation, More posterior positioning of T-loop Produced an increased stress in upper 1st. premolar extraction site. 3. At 3mm activation, More posterior positioning of T-loop produced an increased stress in upper 1st. premolar extraction site and mesial lower half of upper 1st. molar mesio-buccal root. 4. At 1mm activation, More anterior positioning of T-loop produced an increased stress in upper mesial and blew apex area of upper canine root. 5. 0.25 B/L ratio and 3mm activation produced bodily movement of canine. To summarize, desired tooth movement and anchorage requirement is possible by altering the activation and mesio-distal position of the T-loop spring.

Arch Forms & Dimensions after Orthodontic Treatment by Premolar Extraction (소구치 발치에 의한 교정치료후의 치열궁 형태 및 크기에 관한 연구)

  • This study was done to estimate arch forms and dimensions at the bracket level where archwire was placed in Angle's Class I first premolars extraction cases. 60 post-treatment dental casts which had attained good orthodontic treatment results were used in this study Many landmarks and linear measurement items to describe arch forms and dimensions were determined and measured. With a computer system and digitizer, arch forms were described and linear measurement items were statistically analysed. The following results were obtained. 1. The average labial and lingual arch forms at the bracket level were obtained. 2. Arch forms were expressed by parabolic equations and coefficients of determination. 3. Arch widths were larger in male than in female. 4. There were statistical significances in upper intercanine width, upper interfirst molar width, upper intersecond molar height, lower intercanine width and lower interfirst molar width between both sexes (p<0.05, p<0.01). 5. Interfirst molar width differences between maxilla and mandible were 6.43mm in male and 6.05mm in female.

Retrospective Study of Clinical Characteristics and Treatments in Natal and Neonatal Teeth (선천치와 신생치의 임상적 특성과 치료에 관한 후향적 연구)

  • Natal teeth that are already present at birth and neonatal teeth that erupt shortly after birth may cause various complications. The purpose of this study is to evaluate the characteristics of natal/neonatal teeth in Korean infants who visited to Kyung Hee University Dental Hospital from 2006 to 2015. A retrospective review of clinical data, including the sex of the patients, chief complaints of the mothers, clinical appearances and locations of the natal/neonatal teeth, and associated complications and treatments, was collected. Overall, a total of 75 teeth were found in 48 patients and 69% of the infants had natal teeth, while 31% had neonatal teeth, all of which were mandibular incisors. Females showed more natal/neonatal teeth than males. Major reasons for visiting the dental clinic were parental anxiety, referrals from other clinics, Riga-Fede disease, and pain during breastfeeding. Extraction was the most common treatment choice; observation and grinding were also used.

TREATMENT OF MAXILLARY FIRST MOLARS WITH ERUPTION FAILURES (맹출장애를 가진 상악 제1대구치의 치료)

  • An eruption failure can be observed for child and adolescent periods when the primary dentition is changed to the permanent dentition through the mixed dentition frequently. The eruption failure can lead to miss erupting times of the tooth, then it will cause a lot of problems including root resorption, esthetic problem, transposition of adjacent tooth, malocclusoin and etc. Especially, the maxillary first molar is importantly concerned with occlusion and growth and is an essential tooth for development and maintenance of occlusion. So, it is a momentous part of more proper occlusal management to find these abnormal cases at the early stage and solve the problems. The sorts of eruption failures of the maxillary first molars can be divided into delayed eruption, impaction and the primary retention and the secondary retention. When physical obstacles cause impaction, first of all they must be removed then we can treat the impaction with observation after removal, surgical exposure or orthodontic traction. If the source of impaction is an ectopic eruption, the treatment can be a brasswire, a pendulum appliance, a space maintainer or space regainer after the extraction of the second deciduous tooth and etc. These cases are made a diagnosis of eruption failures of the maxillary first molars in mixed dentition period and have good prognosises after my treatments. So I reported them.

Severe bidentoalveolar protrusion treated with lingual Biocreative therapy using palatal miniplate (구개측 미니플래이트를 이용한 양악 치아치조성 전돌환자에서의 설측 Biocreative therapy 적용)

  • This case report describes the treatment of a 23-year, 8-month-old female patient with a Class II malocclusion who showed severe bidentoalveolar protrusion and anterior crowding. The treatment plan consisted of extracting all the first premolars, decrowding and en masse retraction of the upper six anterior teeth and lower anteriors. The upper C-plate placed in the midpalatal area combined with lingual sheath fixtures were used as substitutes for posterior anchorage teeth during upper anterior retraction. Preadjusted brackets (0.022-inch) were used for upper anterior decrowding. A 0.9 mm diameter stainless steel lever-arm soldered to the main arch wire facilitated controlled retraction of upper anteriors. The upper and lower dentition was detailed using a tooth positioner during the finishing stage. Correct overbite and overjet were obtained by decrowding and retraction of the upper six anterior teeth into their proper positions. Use of the C-plate and lingual appliances provided ideal anchorage to enhance the improvement in facial balance. The active treatment period was 19 months. The treatment result was stable 13 months after debonding.
