ANTI-INFLAMMATORY EFFECTS OF PPARγ ON HUMAN DENTAL PULP CELLS (치수세포에서 PPARγ의 항 염증작용에 관한 연구)

  • Dental pulp is a loose, mesenchymal tissue almost entirely enclosed in the dentin. It consists of cells, ground substance, and neural and vascular supplies. Damage to the dental pulp by mechanical, chemical, thermal, and microbial irritants can provoke various types of inflammatory response. Pulpal inflammation leads to the tissue degradation, which is mediated in part by Matrix metalloproteinase leads to accelerate extracellular matrix degradation with pathological pathway We have now investigated the induction of MMPs and inflammatory cytokines by Lipopolysaccharide (LPS) control of inflammatory mediators by peroxisome proliferator-activated receptors (PPARs). Human dental pulp cells exposed to various concentrations of LPS ($1-10{\mu}g/ml$) revealed elevated levels of MMP-2 and MMP-9 at 24 hrs of culture. LPS also stimulated the production of ICAM-1, VCAM-1, $IL-1{\beta},\;and\;TNF-{\alpha}$. Adenovirus $PPAR{\gamma}\;(Ad/PPAR{\gamma})\;and\;PPAR{\gamma}$ agonist rosiglitazone reduced the synthesis of MMPs, adhesion molecules and pro-inflammatory cytokines. The inhibitory effect of $Ad/PPAR{\gamma}$ was higher than that of $PPAR{\gamma}$ agonist. These result offer new insights in regard to the anti-inflammatory potential of $PPAR{\gamma}$ in human dental pulp cell.

A HISTOLOGICAL CHANGES AND CGRP EXPRESSION AFTER EXPOSURE INJURY BY ER:YAG LASER IN DENTAL PULP OF RAT (흰쥐 치수에서 Er:YAG laser에 의한 노출손상에 따른 조직학적 변화 및 CGRP 단백질의 발현)

  • The purpose of this study was to investigate early histological changes and calcitonin gene-related peptide (CGRP) expression in the dental pulp of the rat after Er:YAG laser preparation. Occlusal cavities were prepared in the upper first molars using either Er:YAG laser and conventional bur. At 48 hours after cavity preparation, the teeth were processed for hematoxylin-eosin stain and CGRP immunohistochemistry. The results were as follows : 1. The cavity floor by Er:YAG laser preparation was more irregular shape compared with those by bur preparation and there are some cracks in the directions of dentinal tubules. 2. There were more inflammatory cell infiltration and disruption of odontoblast in the dental pulp by Er:YAG laser preparation in comparison with the dental pulp by bur preparation. 3. CGRP expression in the pulp tissue by both Er:YAG laser and bur preparations were increased and higher than in the normal pulp. The expression pattern of CGRP was more strong in the pulp by Er:YAG laser preparation. These results indicate that Er:YAG laser is useful in the operative dentistry such as caries removal and cavity preparation if properly applied.

  • The author investigated on the responses of pulp and dentine following tooth movements. The material consisted of fifty-four intact teeth from twenty-seven adult white rats. The half of the teeth were employed as controls and the other half served as experimental group. These teeth were moved with forces ranging from 30 grams to 120 grams for from 3 to 7 days. All these were extracted immediately after the force was relieved. The main pulp changes in the experiment were vacuolization of the pulp tissue and circulatory disturbances. The magnitude of the force had an important role. In addition to these changes, the resorption in dentine and cementum was observed, which was related to the magnitude of the force and the duration of experiment.

Histological evaluation of direct pulp capping with DSP-derived synthetic peptide in beagle dog (비글견에서 DSP 유도 합성 펩타이드를 이용한 직접 치수 복조술에 대한 조직학적 연구)

  • The purpose of this study was to investigate the pulpal response to direct pulp capping with dentin sialoprotein (DSP) -derived synthetic peptide in teeth of dogs, and to compare its efficacy to capping substances $Ca(OH)_2$ and white mineral trioxide aggregate (WMTA). A total of 72 teeth of 6 healthy male beagle dogs were used. The mechanically exposed pulps were capped with one of the following: (1) DSP-derived synthetic peptide (PEP group): (2) $Ca(OH)_2$ (CH group): (3) a mixture paste of peptide and $Ca(OH)_2$ (PEP+CH group): or (4) white MTA (WMTA group). The access cavity was restored with a reinforced glass ionomer cement. Two dogs were sacrificed at each pre-determined intervals (2 weeks, 1 month, and 3 months). After the specimens were prepared for standard histological processing, sections were stained with hematoxylin and eosin. Under a light microscope, inflammatory response and hard tissue formation were evaluated in a blind manner by 2 observers. In the PEP group, only 3 of 17 specimens showed hard tissue formation, indication that the DSP-derived synthetic peptide did not induce proper healing of the pulp. Compared with the CH group, the PEP group demonstrated an increased inflammatory response and poor hard tissue formation. The CH and WMTA groups showed similar results for direct pulp capping in mechanically exposed teeth of dogs.

THE PULP TREATMENT OF IMMATURE PERMANENT TEETH USING PARTIAL PULPOTOMY (Partial pulpotomy를 이용한 미성숙 영구치의 치수 치료 : 증례 보고)

  • The primary objective of pulp treatment is to maintain the integrity and health of the oral tissues. The most important and difficult aspect of pulp therapy is determining the health of the pulp, or its stage of inflammation, so that a decision can be made regarding the best form of treatment. Immature permanent teeth are good candidates for many pulp healing procedures, due to their rich blood supply, which is believed to enhance the pulp's ability to react successfully to various insults. Healing was considered to have taken place when the tooth fulfilled the following criteria: 1. Abscence of clinical symtoms 2. Radiographic evidence of dentin bridge formation 3. No intrapulpal or periapical pathosis was evident radiographically 4. Continued root development and closure of the apex in immature teeth 5. Normal dentin apposition in mature teeth 6. Positive response to electrical pulp test This presents a report of cases in which immature permanent teeth, pulp-exposed by caries or trauma, was treated successfully by partial pulpotomy.

A HISTOPATHOLOGIC STUDY ON THE PULPAL RESPONSE TO DEMINERALIZED FREEZE-DRIED BONE IN DOGS (탈회냉동건조골에 대한 성견의 치수조직반응에 관한 연구)

  • The purpose of this study was to investigate the effects of demineralized freeze-dried bone (DFDB) on mechanically exposed pulp of dog by evaluating the pulpal inflammation and healing process, formation of dental hard tissue, and structural changes of fibroblasts of the remaining pulp tissue. Teeth of 4 dogs, weighing 10kg, were used in this study. Class V cavities were prepared followed by exposed the pulp tissue mechanically by sterilized round bur. In control group, exposed pulps were capped with calcium hydroxide paste followed by sealed with IRM. In experimental groups, the exposed pulps of one group were capped with the collagen and those of the other group were capped with DFDB. All cavities were sealed with same manor as control group. The animals were sacrificed at the intervals of 3, 7, 14, and 28 days for histopathlogic evaluation. The specimens were observed by the light microscope and trans-electron microscope. The results were as follows: 1. Pulp necrosis was not observed in all groups. Inflammatory response was disappeared from 1 week in control group and group 2. But it was not disappeared until 2 weeks and also irregular arrangement of odontoblasts was showed at the lateral walls of root canal just beneath the amputated site of the pulp in group 1. 2. Dentinal bridge was formed incompletely at 2 weeks but it was formed completely at 4 weeks in control group. Odontoid tissue was also found in control group at 4 weeks from treatment. Amputated site of pulp was encapsulated with fibrous tissue and odontoblast and dentinal bridge was not found in group 1. Preodontoid tissue and reparative dentin which were formed by odontoblast differentiated around DFDB were found, but dentinal bridge was not found in group 2. 3. Cell with large basophillic-stained nuclei infiltrated to amputated site and DFDB at 1 week from treatment in control group and group 2. They were found more in group 2 than in control group. Odontoblasts arranged more regularly and reparative dentin was found more as time elapsed. 4. Dentin-formative odontoblasts which showed ultramicrostructure of cytoplasm with polarized nucleus, rEM, Golgi complex, secretory granules, secretion of organic matrix in control of group and group 2. In regards to above results, the demineralized freeze-dried bone(DFDB) induce odontoblastic differentiation and further come up to the dentin formation in amputated pulp.

  • This study was carried out in order to study early histologic changes and repair reaction appling to extrusive force for 3rd premolar of adult dogs. After 1 week of extrusive force with elastic chain, one of dogs was sacrified and after 3 weeks retention period, another dog was sacrified. The paraffin sections of samples were stained with Hematoxylin - Eosin and Masson's Trichrome and were examed by light microscopy . The obtained results as follows 1. In Hematoxylin - Eosin and Masson Trichrome stain of control group , the periodontal ligament width was constant from apical third to cervical third of the root and periodontal fiber arrangement was horizontal or oblique in cervical third. oblique in middle third, oblique in apical third of root. in alveolar bone, smooth appearance was shown 2. In Group 1, all periodontal fiber arrangement was oblique toward tooth, and the periodontal ligament width increased Partially PDL was ruptured in apex. In MT stain, immature bone formation was seen at alveolar crest area. Active bone formation was observed along the one side of alveolus, and apical portion of pulp was involved with blood vessel rupture , vacuolization of pulp tissue and hyperemia 3. In Group 2, most periodontal ligament arrangement and PDL width was repaired and fiber density increased. In MT stain, mineralization of immature bone on the alveolar crest was progressed. In pulp, vacuole and hyperemia was diminished and fibrotic change was diminished 4. After 3 week periodontal ligament has more repair ability than pulp tissue. pulp was involved with vacuolization and fibrosis, so it takes more time for repair.
