LABIAL APPROACH OF PULP TREATMENT AND RESIN RESTORATION ON DISCOLORED NECROTIC PRIMARY ANTERIOR TOOTH (변색된 유전치의 순측접근에 의한 치수치료 및 레진수복)

  • Traditional method of pulpectomy for a necrotic primary anterior tooth was done on lingual side. But it could not recover the discoloration of crown effectively. For the purpose of treating the discoloration of crown after lingual pulpectomy, additional methods of crown restoration were needed like : celluloid crown, open-faced crown, rasin-faced crown. Neverthless, these kinds of complete coverage methods had some disadvantages such as possibility of tooth fracture by increased tooth preparation. In order to overcome the shortcomings of lingual pulpectomy, labial treatment could be considered as an alternative. It is a method that treats necrotic pulp through the labial access opening. After finishing the pulp treatment, discolored labial tooth structure was removed extending from access opening. Discoloration of deep area could be masked effectively using opaque sealant. Cavity on labial side was restored with composite resin. This labial approach method has several advantages. First, it gives a direct vision for effective pulp treatment which is also very useful for children with poor behavior. Second, most of lingual tooth structure could be saved and occlusal contact of lingual surface remains undisrupted. Only nonfunctional discolored labial surface may removed. Third, complete removal of discolored part of a labial tooth and immediate resin restoration could be done effectively after pulp treatment. Moreover, it also could be used for pulp treatment having serious dental caries on labial surface with sound lingual tooth structure. This report presents cases with discolored upper anteior primary tooth, approaching labial side with successful restoration.

Chronological Trends in Practice Pattern of Department of Pediatric Dentistry : 2006 - 2015 (최근 10년간(2006 - 2015) 서울대학교 치과병원 소아치과의 진료 현황에 대한 연구)

  • The practice pattern of pediatric dentistry has been changing for the last several decades. This change might be influenced by several factors such as development in dental materials and socioeconomic changes. The purpose of this study was to analyze the changing trends of patient distribution and treatment pattern of pediatric dentistry. Patient distribution and practice trends from 2006 to 2015 at the department of pediatric dentistry of Seoul National University Dental Hospital were reviewed. From 2006 to 2015, the proportion of new patient increased from 12.49% to 20.56%. The average age of new patients decreased. In 2006, restorative treatment had highest percentage, followed by preventive treatment, orthodontic treatment, surgical treatment, and pulp treatment. In 2015, preventive treatment had highest percentage, followed by restorative treatment, orthodontic treatment, surgical treatment and pulp treatment Frequency of general anesthesia increased more than 5 times for last 10 years. The proportion of insurance treatment decreased until 2009, and after 2010 it increased steadily.

  • The pulpectomy or root canal treatment on primary teeth is to be done when there is evidence of chronic in flammation or necrosis in the radicular pulp. Due to the tortuous and ribbon-shaped anatomy of the primary teeth, the instrumentation of endodontic hand files and barbed broaches is not an easy procedure. Recently, many automatic Ni-Ti rotary instruments have been developed and has made endodontic treatment easier and faster. This report describes two cases of root canal treatment on primary molars using Ni-Ti rotary files. The cervical constricture was eliminated by the crown-down method, as smaller file proceeded unhindered into the apical third of the canal. In addition, the crown-down technique enhanced the efficacy of the endodontic irrigant. The use of rotary instrumentation for primary teeth seemed to be a more effective way to debride the uneven walls of primary teeth.

A Retrospective Study on the Treatment of Dens Evaginatus for the Last 5 Years (최근 5년간 시행된 치외치 치료에 대한 후향적 연구)

  • The purpose of this study was to analyze treatment methods, results, timings and clinical signs and symptoms in failed cases of each treatment method of dens evaginatus on the premolar areas. In this study, 151 patients and 417 teeth were included. Resin restoration and direct pulp capping as preventive treatment and pulp revascularization, apexification and conventional endodontic treatment as endodontic treatment were included for treatment methods. In the preventive resin restoration, successful results were shown in the intact tubercles and also effective when the tubercles were fractured. In the direct pulp capping, resolved clinical symptoms and growth of the roots were shown when there was only pain during chewing without apical lesion. Apical lesion was the most common pretreatment signs and symptoms of the pulp revascularization, apexification and conventional endodontic treatment. In the pulp revascularization, successful results was obtained in most cases. But in some cases, root length or root wall thickness was not increased. Effective results were shown both of the apexification and conventional endodontic treatment. In order to increase success rate of preventive treatment of dens evaginatus, resin restoration was required to be done when tubercle did not occluded or in the presence of intact tubercles. When tubercle was fractured, root development stage and pulp condition should be considered for successful treatment.

Evaluation of Electric Pulp Test (EPT) as a Tool for Measurement of Dentinal Hypersensitivity (상아질 지각과민 측정 도구로서의 전기치수검사에 대한 평가)

  • In this study, 40 hypersensitive teeth of 19 patients were investigated. The procedures performed were as follows: Before desensitization, EPT at occlusal third of buccal surface was done for the evaluation of pulp vitality and the EPT value was recorded for the reference value. And mechanical and thermal test was executed for the test of hypersensitivity. If the tooth responded to the above tests, we did EPT at the exposed surface, using toothpaste as a electrolite medium and recorded the EPT value at patient's response. After the tests had been done, desensitization procedures with Gluma(R) Desensitizer were performed according to the manufacturer's instructions. After desensitization, the same tests except EPT at occlusal third were repeated. All the 40 teeth responded positive before desensitization and negative after desensitization procedures. The EPT value at occlusal third ranged from 31 to 65 (48.9${\pm}$7.2). Before desensitization 34 teeth responded at EPT value of 2 and the remaining 6 teeth was in the range of 17 to 25. After desensitization all 40 teeth responded from 12 to 27 (19.6${\pm}$3.5). The 6 teeth responded at greater number than 2 before desensitization was in the range of 18 to 23. Within the limitations of this study we can conclude that: When a tooth with dentinal hypersensitivity responds to mechanical and thermal stimulation, the tooth shows very low resistance to electricity at the exposed surface while when a tooth is desensitized and doesn't show respond to mechanical and thermal stimuli, the tooth shows increased level of resistance to electric stimulation at the exposed surface. EPT can be used for the diagnosis of dentinal hypersensitivity. Furthermore EPT will be useful to evaluate the outcome of desensitization procedures. However, EPT is not a valid tool for measuring dentinal hypersensitivity.

VITAL PULP THERAPY USING PLATELET-RICH FIBRIN IN AN IMMATURE PERMANENT TOOTH : CASE REPORTS (미성숙 영구치에서 혈소판 농축 섬유소를 적용한 생활 치수 치료 : 증례보고)

  • Preserving the pulp is important in the treatment of carious pulp exposure in young permanent teeth. Pulpotomy is a vital pulp therapy in which a portion of the coronal pulp tissue is surgically removed, and the remaining radicular tissue is covered with suitable material that protects the pulp from further injury and permits and promotes healing. It is important to develop biocompatible treatment directed at maintaining pulp vitality and increasing tooth longevity. Platelet-rich fibrin (PRF) has been referred to as a second-generation platelet concentrate. Two clinical cases in which PRF was applied as a medicament after pulpotomy of an immature permanent tooth are presented. After isolation, caries removal and pulpotomy with PRF was performed. A layer of mineral trioxide aggregate (MTA) was placed over the PRF, and the final restoration was performed. Postoperatively, the patient had no pain or discomfort, and follow-up radiographs revealed normal periodontal ligament space and trabecular bone pattern.

PULP TISSUE REGENERATION AND ROOT FORMATION OF PERMANENT TEETH WITH PULPAL/PERIAPICAL DISEASES (치수/치근단 질환에 이환된 영구치의 치수 조직 재생과 치근 형성)

  • Numerous cases about additional growth of roots or pulp tissue regeneration by using various intracanal medicaments in immature permanent teeth with periapical or pulpal disease have been reported. The underlying mechanism has not been clearly delineated, but it has been widely accepted that undifferentiated mesenchymal cells and stem cells are involved. Moreover, the growth and deposition of osteoid or cementoid tissues have been observed in regenerated pulp and roots. This new and non-invasive treatment has brightened the future of endodontics, and enlarged the vision of regenerative root canal treatment with multi-potent stem cells and various tissue engineering techniques.

Changes in pulpal blood flow during orthodontic tooth movement studied by Doppler ultrasound (Doppler ultrasound를 이용한 교정적 치아 이동 시 치수 혈류량의 변화의 측정- 예비실험)

  • Objective: This study was to change of pulp blood flow among maxillary and mandibular anterior tooth with mild crowding and adjacent teeth using Ultrasound Doppler graphy. Methods: The change of pulp blood flow was measured three times using Ultrasound Doppler graphy; before the attachment of brackets, after 3 week, and after 6 week. The sample consists of 15 year old eighteen patients. Results: Before the attachment of brackets, after 3 weeks, and after 6 weeks, there were no significant differences in the change of pulp blood flow in each part (maxilla and mandible) and each tooth according to period. In addition, to compare internal dangerousness of loss of the pulp vitality, when pulp blood flow is compared in each tooth before orthodontic treatment, there were no statistically significant differences in maxillary lateral incisor and mandibular canine but it showed low values in all measurement items (p > 0.05). Conclusions: Results of this study can be not only methodological preliminary data in further study such as tooth movement type of Ultrasound Doppler graphy and particular study considered the patient age, but also reference materials for the loss of pulp vitality in orthodontic treatment.
