THREE-YEAR FOLLOW UP OF FERRIC SULFATE PULPOTOMY IN PRIMARY MOLARS (Ferric sulfate를 이용한 유치치수절단술의 3년 후 임상평가)

  • The objective of this report is to assess clinically and radiographically the state of the primary molars pulpotomized with a 15.5% ferric sulfate solution. The subjects selected were healthy children treated at the pediatric dental clinic of the Seoul National University Hospital in Korea. Thirty teeth were pulpotomized with a ferric sulfate solution(FS). Another twenty-one teeth were pulpotomized with 20% dilute formocresol(FC). Clinical and radiographic data for the fifty-one primary molars were collected with a mean follow-up period of 34 months. The success rate for the FS group was 80.0%. The success rate for the FC group was 81.0%. The differences in the results between the two groups were analyzed statistically utilizing the chi square test. External root resorption was observed in four teeth of FS group and four of the FC group. Periapical bone destruction was observed in three of FS group and two of FC group. There were no significant statistical differences between the success rates for FS group and the FC group.

EVALUATION OF THE GENOTOXICITY OF FERRIC SULFATE BY COMET ASSAY (Comet assay를 이용한 Ferric Sulfate의 유전자 독성에 대한 연구)

  • Although ferric sulfate has been proposed as an alternative to formocresol in pulpotomy treatment in primary teeth, it has been given little concern regarding its cytotoxicity and mutagenicity. In the present study, we assessed the in vitro genotoxic effect of a ferric sulfate on human gingival fibroblast cell line (HGF-1). DNA damage was evaluated using comet assay (single cell alkaline gel electrophoresis) and obtained the results as follows: 1. A dose-response relationship was found between ferric sulfate concentrations (0 to 5mM) and DNA damages. 2. Above the concentration of 0.1mM, DNA damage was significantly increased than those of the control (p<0.05). 2. At the fixed concentration of 0.05mM, no significant difference was found between exposure time and DNA damage. These findings suggest that ferric sulfate as a pulpotomy agent can induce DNA damage in human gingival fibroblasts.

  • 저자는 임상적 진단과 치수내 혈액상과이 관계를 규명하기 위하여 치아 우식증에 의하여 심부로 이환된 이십개의 치아를 선택하여 치수절단술을 시행한 후 기 혈액을 검토한 결과 다음과 같은 결론을 얻었다. 1. 임상적으로 치수충혈로 전단된 일곱 개의 치아중, 세 개의 혈액표본을 정상 혈액상을 나타냈고 두개의 혈액표본은 약간의 급성 염증상을 나타냈으나 나머지 두개의 혈액표본은 심한 만성 염증상을 나타냈다. 2. 임상적으로 급성치수염으로 진단된 열세개의 치아중, 열개의 혈액표본은 심한 만성 염증상을 나타냈고 한 개의 혈액표본은 약간의 만성 염증상을 나타냈으며 나머지 두개의 혈액 표본은 급성 염증상을 나타냈다.

Effects of Mineral Trioxide Aggregate on the Proliferation and Differentiation of Human Dental Pulp Stromal Cells from Permanent and Deciduous Teeth (Mineral trioxide aggregate가 유치 및 영구치의 치수기질세포 증식 및 분화에 미치는 영향)

  • Mineral trioxide aggregate (MTA) has recently been used as a pulpotomy medicament for primary molars. The aim of this study was to evaluate and compare the proliferation and differentiation potential of dental pulp stromal cells of permanent teeth and deciduous teeth cultured on MTA-coated surface. Human dental pulp stromal cells were obtained from human permanent premolars and deciduous teeth and cultured on MTA-coated culture plates. The cells were subjected to proliferation assay and cell cycle analysis. Their differentiation potential was evaluated by analysing changes in the mRNA expressions of runt-related transcriptional factor 2 (Runx2) and alkaline phosphatase (ALP). Morphological changes of cells in direct contact with MTA were observed using scanning electron microscopy (SEM). The proliferation rates, distribution of cell cycles and mRNA expression patterns of Runx2 and ALP were similar in both types of pulpal cells. SEM observations revealed that both types changed into more dendrite-like cells. On the surface of MTA, human dental pulp stromal cells from deciduous and permanent teeth were able to both proliferate and differentiate into cells that induce mineralization. MTA is suitable as a biocompatible pulpotomy medicament for primary teeth.

A HISTOPATHOLOGIC STUDY ON THE PULPAL RESPONSE TO DEMINERALIZED FREEZE-DRIED BONE IN DOGS (탈회냉동건조골에 대한 성견의 치수조직반응에 관한 연구)

  • The purpose of this study was to investigate the effects of demineralized freeze-dried bone (DFDB) on mechanically exposed pulp of dog by evaluating the pulpal inflammation and healing process, formation of dental hard tissue, and structural changes of fibroblasts of the remaining pulp tissue. Teeth of 4 dogs, weighing 10kg, were used in this study. Class V cavities were prepared followed by exposed the pulp tissue mechanically by sterilized round bur. In control group, exposed pulps were capped with calcium hydroxide paste followed by sealed with IRM. In experimental groups, the exposed pulps of one group were capped with the collagen and those of the other group were capped with DFDB. All cavities were sealed with same manor as control group. The animals were sacrificed at the intervals of 3, 7, 14, and 28 days for histopathlogic evaluation. The specimens were observed by the light microscope and trans-electron microscope. The results were as follows: 1. Pulp necrosis was not observed in all groups. Inflammatory response was disappeared from 1 week in control group and group 2. But it was not disappeared until 2 weeks and also irregular arrangement of odontoblasts was showed at the lateral walls of root canal just beneath the amputated site of the pulp in group 1. 2. Dentinal bridge was formed incompletely at 2 weeks but it was formed completely at 4 weeks in control group. Odontoid tissue was also found in control group at 4 weeks from treatment. Amputated site of pulp was encapsulated with fibrous tissue and odontoblast and dentinal bridge was not found in group 1. Preodontoid tissue and reparative dentin which were formed by odontoblast differentiated around DFDB were found, but dentinal bridge was not found in group 2. 3. Cell with large basophillic-stained nuclei infiltrated to amputated site and DFDB at 1 week from treatment in control group and group 2. They were found more in group 2 than in control group. Odontoblasts arranged more regularly and reparative dentin was found more as time elapsed. 4. Dentin-formative odontoblasts which showed ultramicrostructure of cytoplasm with polarized nucleus, rEM, Golgi complex, secretory granules, secretion of organic matrix in control of group and group 2. In regards to above results, the demineralized freeze-dried bone(DFDB) induce odontoblastic differentiation and further come up to the dentin formation in amputated pulp.

  • Dens evaginatus is a dental anomaly most commonly seen in premolar teeth in which a tubercle or protuberance projects from either the center of the occlusal surface or the buccal triangular ridge. These tubercles are easily fractured from mastication as the tooth erupts and frequently leads to pulp necrosis as a common complication. To prevent these sequelae, prophylactic treatment soon after the tooth starts erupting is essential. These preventive treatments include, selective grinding and protection of the tubercle by pit and fissure sealant. When the tooth does shows signs of pulp necrosis and apical periodontitis, endodontic procedures are needed. Apexification and apexogenesis are usually the treatment of choice for the affected teeth which have immature apices. Apexogenesis is a vital pulp therapy procedure performed to encourage continued physiological development and formation of the root end. It involves removal of the inflamed pulp and the placement of calcium hydroxide on the remaining healthy pulp tissue. This case report describes an atypical apexogenesis of a mandibular premolar which showed to be a dens evaginatus. The tooth which was treated with calcium hydroxide shows good results and is planned for permanent root canal filling.

  • When a tooth is fracture with the pulp exposure, and if a fragment is large enough to save, reattachment of the fragment would be a choice of treatment and reattachment of the fragment provides several advantages over other forms of dental restoration following crown fracture. For the purpose of tooth reattachment, it is important to preserve the sound enamel around fracture area. For young patients, tooth reattachment has more advantages in the sense that they recover faster and that it enables other treatments at the same time. Through the continuous advancement in adhesion technology and the effort for aestheticism, various reattachment methods have been practiced and more efforts are considered necessary. The presented cases are that we had practiced in our department in Kyung Hee Medical center, I may address that fragmentation has been successfully done with aesthetically fine results, and no pathologic changes were found in short term follow-ups.

Pulp Treatment of Triple Tooth in Primary Dentition: Two Case Reports (유치열에 나타난 삼중치에서 치수 치료 : 증례 보고)

  • Triple tooth is rare in primary dentition; it is the abnormal fusion of three teeth. There are a few reports of double teeth, but triple teeth are rare. These multiple teeth create several clinical problems, not only esthetic problems but also a high caries susceptibility, congenital missing permanent tooth germ(s), orthodontic problems, and periodontal problems. They also make it difficult to perform pulp treatment because of the complex tooth structure. A 1-year 7-month-old male underwent a partial pulpotomy with mineral trioxide aggregate (MTA) when pulp exposure caused by deep caries occurred at a maxillary anterior triple tooth in the first case. The second case was a 1-year 9-month-old male presenting with a fracture line at a mandibular anterior triple tooth. After removing the fractured fragment, a pulpectomy was performed at the remaining primary lateral incisors. Specific complications were not observed during 24 and 6 months of follow-up, respectively.
