배경: 2015년 한국의 중동호흡기증후군 유행에서 지역간 치명률의 차이는 극명하였다. 이 연구는 대전 클러스터와 다른 지역 간의 치명률의 관련된 일반적 특성 및 역학적 요인을 밝히고저 하였다. 방법: 입원병원 소재지를 기준으로 대전과 타 지역으로 구분하여 관련변수에 따른 카이제곱검정 및 피셔정확검정 등으로 분석하였다. 대전과 다른 지역의 치명률(CFR)의 차이와 관련된 요인을 분석하기 위하여 단변량 및 다변량 로지스틱 회귀분석를 실시하였다. 결과: 모형 I에서는 65세 이상 연령군일수록 7.12배(95% CI 2.33-21.8)(p=0.001), 동반질환이 있는 경우 10.29배(95% CI 2.94-36.06)(p<0.001), 잠복기가 7일 이하인 경우가 8.55배(95% CI 2.54-26.7), 입원기간이 17일 이하인 경우 10.08배(95% CI 2.99-31.9)(p<0.001) 등이었으며, 모형 II에서는 65세 이상 연령군일수록 5.34배(95% CI 1.65-17.2)(p=0.005), 잠복기가 7일 이하인 경우가 6.70배(95% CI 1.96-22.89), 입원기간이 17일 이하인 경우 8.90배(95% CI 2.59-30.6)(p=0.001), 동반질환에서 암의 경우에서 7.15배(95% CI 1.64-31.14)(p=0.009) 등이었다. 결론: 2015년 한국 중동호흡기증후군 유행에서 대전 클러스터의 높은 치명율은 연령(≥65세), 동반질환(특히 암), 잠복기(≤7일), 입원기간(≤17일) 등이 유의한 변수로 도출되었다.
Equine colic is a major concern in equine industry due to high morbidity and mortality rates. The purpose of this study was to establish mortality rates during medically and surgically treated colic and to identify potential prognostic indicators for mortality of equine colic in Korea. The overall mortality rate was 18/119 (15%). The 93 cases (78%) were treated medically and 26 cases (22%) surgically out of the 119 records reviewed. The mortality rate in small intestinal lesion was 7/9 (78%) and strangulating lesion was 10/11 (91%). The significance between individual factors and prognosis of equine colic was not valid but high frequency rate was found in ${\leq}3$ years age (59%) and male (50%). In dead group showed higher, rectal temperature and heart rate than those of survived group. The mortality rate was significantly high in the pale mucous membrane color and severe pain and decreased intestinal motility. In laboratory factors, dead group showed lower level of platelet than survived group, and had higher values of RBC, hemoglobin, PCV with significance (P < 0.05). Group with increased enzyme activity of CK, LDH, glucose showed poor prognosis.
Infection is the most important cause of death in children with nephrotic syndrome. Influenza viral infections can be fatal for these children, given the annual outbreak of this virus, with the mortality rate being similar to that of respiratory syncytial virus in healthy children. Pneumonia is recognized as the most important complication of influenza infections, as it is associated with high death rates. However, the influenza vaccine, as well as antiviral agents, can be used for prevention and treatment. Therefore, aggressive management with influenza vaccination and antiviral agents will lower the overall mortality rate in children with nephrotic syndrome. Here we report the case of a 7-year-old boy with nephrotic syndrome and influenza A virus (H1N1) pneumonia.

In this study, semiquantitative failure mode, effects, and criticality analysis (FMECA) for the reliability analysis of a solid rocket propulsion system is performed. The semiquantitative FMECA is composed of failure mode and effects analysis (FMEA) and criticality analysis (CA). To perform FMECA, the structure of the solid rocket propulsion system is divided into 43 parts down to the component level, and FMEA is conducted at the design stage considering 137 potential failure modes. CA is then conducted for each failure mode, during which the criticality number is estimated using the failure rate databases. The results demonstrate the relationship between potential failure modes, causes, and effects, and their risk priorities are evaluated qualitatively. Additionally, several failure modes with higher criticality and severity values are selected for high-priority improvement.

In this study, design of a small hybrid rocket is carried out using Failure Mode and Effects Analysis (FMEA) and Criticality Analysis(CA), which is a method for qualitative and semi-quantitative reliability analysis. In order to carry out FMEA, the structure of the hybrid rocket is divided into 31 parts and 72 potential failure modes. As a result of the FMEA, the relationship between potential failure modes, causes and effects, and their severity are evaluated qualitatively. Criticality analysis is followed for the failure modes, in which the criticality number is estimated using the failure rate information available from the handbook. Moreover, the failure modes with higher criticality and severity are chosen for improvement, and a series of design or material changes are made for the improvement of the hybrid rocket reliability.
Bronchopleural fistula (BPF) is relatively rare, but it has high morbidity and mortality rates and it is associated with a prolonged hospital stay and high costs. Surgical treatment is the treatment of choice, but other minimal invasive forms of conservative management, and particularly bronchoscopy, have recently been investigated. We report here on the bronchoscopic treatment of a bronchopleural fistula accompanied necrotizing pneumonia, and we used coils and fibrin glue to treat the fistula.
Background: The natural history of bacillary tuberculosis was studied in India and results showed that at the end of the 5-year period, 49% of the patients were dead, 33% were cured and 18% remained sputum-positive. The aim of this survey is to observe the natural course of the patients with intractable tuberculosis disease who were incurable with all drug regimens of the national tuberculosis programme(NTP). Method: Of the patients who have been found as intractable cases in Kang-Weon Province by the supervisory medical officer during the period from January 1,1987 to December 31,1992, 179 were eligible for this study. Sputum examination was done for those who were survived until October in 1993 at the Kang-Weon provincial laboratory of KNTA. 49 out of 179 patients were transferred to the private sectors and retreated with the combination of prothionamide, cycloserine, ofloxacin, enviomycin, etc. They seemed to have been bacteriologically cured, and so they were excluded from the study. Finally 130 patients were analyzed by modified life table method to calculate the fatality rate and the survival rate during the period of 7 years. Results: 1) 80.8% of intractable cases were male and 19.2%, female. 2) More than 94% of intractable cases showed moderately or far advanced Tb findings on their X-rays at the time of registration at health centres. 3) The cumulative case-fatality rate was 19.74% at the end of 1-year period and has risen to 34.55% by the end of 4-year period(increasing by 4.9% a year on an average). The case-fatality rate has shown no appreciable rise since then until the end of 7-year period. 4) The case-survival rate was 80.26% at the end of 1-year period and has decreased to 65.45% by the end of 4-year period. And then there was no appreciable change in the survival rate until the end of 7-year observation. Conclusion: The case-survival rate of intractable cases was higher than that of untreated pulmonary tuberculosis patients and they may have risk of spreading multidrug resistant organisms. It is time we made an effort to improve case-management qualitatively.
Traumatic pelvic injuries usually include high-energy crush injuries and are associated with significant morbidity and mortality. Mortality rates range from 6% to 15% and increase to 36%-54% in cases of fractures that result in increased pelvic volume. Therefore, retroperitoneal hemorrhage can spiral and progress to hemorrhagic shock. Pelvic hemorrhage most commonly occurs secondary to disrupted pelvic veins or fractured bones, and 10%-20% of cases involve arterial injuries. Owing to extensive bleeding and limitations of surgery for pelvic hemorrhage, interventional treatment is at the forefront of pelvic hemorrhage management. CT is an accurate indicator of active hemorrhage in patients with pelvic trauma that affects the diagnosis and management, including interventions. Identification of the site of hemorrhage is necessary for focused interventional treatment. The current trend toward a more conservative approach for treatment of pelvic trauma and advances in interventional radiology in the field of pelvic trauma may favor widespread use of interventional treatment for patients with pelvic injuries. In this review, we discuss therapeutic modalities available to the interventional radiologist and common angiographic treatment strategies and techniques.
COVID-19 is an emerging infectious disease that is hard to predict in terms of fatality rate, treatments, and the timing of its end. World is developing treatments and vaccines for COVID-19. Several treatments and vaccines currently have emergency use authorization, but the treatments are only allowed for critically ill patients with COVID-19. Therefore, the aim of this study is to analyze the confirmed cases of COVID-19, including mortality and testing, in OECD countries and to assess the effect of vaccination on mortality. Looking at the confirmed cases, mortality, and vaccination rates of COVID-19, the number of confirmed cases was lower than previously reported cases after full vaccination. In early 2022, with Omicron, the confirmed cases increased sharply, while mortality dropped, and the mortality showed a gentle curve as the cumulative fully vaccinated exceeded 50%. This indicates that COVID-19 vaccines have an effect on reducing mortality. However, the duration of effectiveness of vaccines was considerably short, which decreased the initial inoculation effect and increased the monthly mortality. As this study was carried out during the COVID-19 pandemic, there was not enough data to analyze comprehensively. However, it is meaningful to compare and analyze the impact of COVID-19 by country.

A bronchopleural fistula after pulmonary resection is still showing high mortality and morbidity despite of advancing of treatment. Several treatment options have been developed including surgical treatment. In 1990, endobronchial closure using vascular occluding coils was introduced. These coils can occlude a bronchial air-leakage by mechanical obstruction as well as inducing fibrosis. We report, herein, the experience using a vascular occluding coils in treating postoperative bronchopleural fistula.
