Factors Influencing the Caring Burden of Families with Dementia in a Community (지역사회 내 치매환자가족의 돌봄부담감에 미치는 영향요인)

  • This study is a descriptive research to analyze the factors affecting the care burden of dementia patients in the community. The data was collected from 223 dementia patients in the community for 10 days from November 30 to December 9, 2018. The results showed that there was a statistically significant difference in caring burden according to demographic characteristics according to the age group of the main caregivers, the relationship with dementia patients, the duration of caring for dementia patients, and the difficulty of caring for dementia patients. Had a negative correlation with dementia knowledge(r=-.145, p=.030). The factors affecting the care burden of dementia patients were as follows: dementia care period(β=.408, p=.006), difficulty in caring for dementia patients(β=-.307, p=.023), relationship with dementia patients(β=-.299, p=.013), and age group of primary caregivers of dementia patients(β=-.265, p=.007). Therefore, in order to alleviate the burden of caring for dementia patients in the community, a practical and continuous care intervention program is needed

  • In this study, a dementia patient care monitoring system is suggested that uses a wearable type bluetooth beacon to prevent them from going missing. This system shows whether the patients stay within the manageable area and sends a warning message to their monitoring managers' or guardians' smart devices when they leave it. The feature of the system is that it provides the service based on indoor location that makes the beacon worn by dementia patients continuously transmit their location information to the managing server through the smart terminal installed indoors or in hospitals and that enables the monitoring managers or the guardians to receive messages sent from the server that tell the patients' whereabouts through their smart devices. The system suggested in this paper is believed to be a system that effectively contributes to the prevention of the dementia patients' going astray from the hospitals and facilities where they are taking care.

  • Recently, the our country has a geriatric dementia patients with poor social welfare policies for the State because it's got a patient exodus and disappearance increases and can cause serious problems socially. The dissemination and the development of communication technologies and smart devices with mobile applications in the diagnosis of dementia through mobile devices and applications, interactions, the Inspector, on behalf of the dementia self-diagnostics, with the possibility of that feature will be able to meet. Therefore, the dissemination of the use of the EEG analysis of patient in the hatch every smart phones use the law hatch proposes a service model for every patient. Proposed dissertation smart service model on the Android platform and identify the symptoms of dementia patients around as a reminder to the patient can be easily managed.

  • 인지능력 손상으로 인해 치매환자의 정상 언어 사용은 점차 감소되고 좌절감과 스트레스는 상승된다. 신체접촉은 비언어적 의사소통의 한 형태로서 치매환자를 지지하고 스트레스 상황에서 겪는 불안을 완화한다. Hollinger와 Buschmann은 1993년 신체 접촉 모형을 개발하였으며, 그 모형을 적용하여 신체접촉 인지도와 우울과의 관계와 신체 접촉의 우울증감소 효과를 정상 노인과 우울증 노인을 대상으로 연구했다. 그러나, 모형을 치매환자 연구에 적용시켜 검토한 연구는 언다. 본 연구의 목적은 신체 접촉 모형을 비평 분석하는 것이다. 또한, 분석을 기초로 하여 모형을 치매 환자 연구에 적용하도록 변형시켰다. 치매 환자의 인지, 언어 능력은 손상되었지만, 환경에 반응하는 감정과 접촉 감각은 유지되고 있다는 것은 신체 접촉 모형을 치매환자에게 적용할 때 반드시 고려해야 할 점이다. 개인의 문화 차이는 신체 접촉에 대한 인지와 반응에 영향을 준다. 따라서, 연구대상자 선택시 대상자가 신체 접촉을 긍정적으로 받아들이고 있는지를 확인해야 할 것이다. 대상자는 인지 능력 손상으로 면담이나 질문지에 적당히 반응할 수 없으므로 연구시 관찰 방법의 이용을 제언한다.

Trends Analysis of Characteristics in Patient with Dementia (치매환자 특성의 추이 분석)

  • South Korea is becoming an aging society, increasing the population with dementia. This study analyzed the characteristics of the trend of dementia patients by utilizing 12 years' worth of data from five V hospitals within metropolitan cities. The results show that there has been an 11.3 times increase in the total number of new dementia patients in these five hospitals from 410 in 2003 to 5,048 in 2014; and among them, the rate of increase was highest in P hospital with a 30.3 times increase. Conversely, there has been a 6-times increase in D hospital, illustrating a noticeable gap between these hospitals. Overall, these hospitals have shown an enormous increasing ratio. In an analysis according to age, it's been shown that those aged between 75 and 84 showed the highest prevalence, and most patients, approximately 93.5%, were 65 years or older, while 20.2% were 85 years or older, showing a high ratio of elderly among dementia patients. In an analysis by type of dementia, it's been shown that Alzheimer's was the most prevalent with 44.4%, followed by unspecified dementia with 33.3%, and vascular dementia with 20.4%. As evident by the outcome of the study, there's been a drastic increase in the number of dementia patients in the past 12 years; since the prevalence of dementia was shown to be higher with older age, it is expected that the number of dementia patients will rise further in these five hospitals, where there's a high ratio of elderly patients. More elaborate and continuous plans are necessary for the management of dementia patients following the growing number of dementia patients.

Development of a Smart Application for Protecting Dementia Patients (치매환자의 보호를 위한 스마트 앱 개발)

  • The applications for considering their position and daily patterns of dementia patients have been developed in an early stage even if the older patients who have weaker or serious symptoms has increased in various forms. In this paper, we develop an android-based application which displays positions and pathways of patients on maps and provide messages in the cases of dangerous situations. Guardians need to register schedules including safe areas and personal information of their patients. This system registers behavior status categorized as normal or abnormal each position which is sent to a database. In particular, the deviation status is assigned in case the patients are not within the safe areas that their guardians registered on their schedule. The wandering status is assigned in case the patients are repeatedly passed by their pathways. This smart application contains the modules such as patient position sending, guardian and patient information, patient schedule and safe zone registration, position and behavior status registration, pathway display and message sending, and rescue request. This system sends the notification and alarm service providing normal and abnormal behavior with deviation and wandering status of patients respectively.

  • In this paper, we introduce the trends in age-related health care services for dementia research due to the rapid aging society. In Korea, the current increase in dementia patients and the disappearance of runaway because social welfare policy is insufficient state and requires a lot of effort in order to protect the patient's guardian. Therefore, In this paper, we propose the need for a monitoring service for dementia patients. The attachment by the wearable sensor and monitor the mobility of the patient causes the protector as possible to trace the location of the patient to monitor the safety of the patient standing with the system provides.

The Study on the Balance Reaction and Physical Activity of Dementia Patients (치매환자의 균형반응과 신체활동에 관한 연구)

  • The purpose of this study was to investigate the balance reaction characteristics of dementia patients. Under the object of 30 dementia patients, they were divided into moderate group and severe group and examined the correlation among Clinical test of sensory interaction and balance(CTSIB), Berg balance scale(BBS) and Bathel index(BI) according to perceptional function. As a result, CTSIB was no significant difference between severe group and very severe group and BBS was significant difference between severe group ($47.32{\pm}4.80$) and very severe group ($43.09{\pm}4.18$)(p<0.05) and BI was significant difference between severe group ($92.89{\pm}9.33$) and very severe group ($76.82{\pm}6.81$)(p<0.05) and significant correlation among perceptional function, BBS and BI(p<0.05, p<0.01). Therefore, these results should be used the base data of developing rehabilitation program or protection management for dementia patients.

Factor Constructs Exploration and Model Goodness-of-fit Verification of Empathy for Dementia Patient among Nurses (간호사의 치매환자에 대한 공감도 요인구조 탐색 및 모형적합성 검증)

  • The purpose of this study is to analyze what constitutes the sympathetic component of nurses care of dementia patients. For this study, 219 nurses in nursing dementia patients were treated in nursing hospitals. Using the Empathy Consensus Rating Scale (ECRS), basic model extraction, model modification, and model suitability verification were performed through factor structure search and confirmation factor analysis (CFA). The results of this study confirmed the validity and reliability of the constituents with 49 questions, including 3 factors including the acceptance of other emotions, the suppression of one's own emotions, and the exchange and interaction of emotions. According to these results, it is expected that the ECRS that can secure validity and reliability can be used for the study to confirm the nurse's Empathy in the dementia patients' nursing field.

Perception about Shared Decision Making of Family Caregivers of Early Dementia Patients: A Qualitative Content Analysis Study (초기 치매환자 가족 돌봄제공자의 공유 의사결정에 대한 인식: 질적 내용분석 연구)

  • The purpose of this study was to explore perception about shared decision making of family caregivers of patients with early dementia (PWED). This study was conducted with a sample of 12 family caregivers (mean age = $71.4{\pm}10.4$) of PWED from three dementia safety centers in Seoul. In-depth interviews were done for each participant about shared decision making and data were analyzed using qualitative content analysis. Six categories and 17 sub-categories identified for participants' perception about shared decision making: means to facilitate communication with patients with dementia, means to secure autonomy of patients, opportunity to facilitate treatment, cause of increasing family caregivers' burden, cause of worsening relationship with patients, and option for choices depending on priority change. The findings of this study can provide a knowledge basis for health care professionals and policy makers to understand how family caregivers of PWED think about shared decision making. It would be of great value to develop educational programs and practical guidelines about shared decision making for PWED and their family, which may contribute to respecting PWED's self-determination right as well as reducing burden of their family.
