언어적 의사소통이 어려운 치매환자에서의 통증 사정 (Pain Assessment in Nonverbal Older Adults with Dementia)

  • 목적: 본고는 완화의료에서 언어적 의사소통이 제한되어있어 통증의 자가보고가 어려운 대상자들 중 치매환자를 대상으로 한 통증 사정 방법을 고찰하여 임상실천에서의 지식을 증가하는데 그 목적이 있다. 방법: 본 연구는 문헌고찰연구로, '통증(pain)', '통증 사정/평가(pain assessment)', '인지기능장애/치매(cognitive impairment/dementia)'를 주제어로 Google scholar, 한국교육학술정보원(www.riss4u.net), KoreaMed, Medline, Science-Direct, CINAHL 등의 database에서 2000년 이후에 발간된 국내외 문헌을 검색하였다. 또한 필요시 언어적 의사소통이 어려운 치매환자에서 통증 사정과 관련된 국내외 전문서적을 참고하였다. 결과: 언어적 의사소통이 어려운 치매 환자의 통증 사정 방법에는 자가보고 외에 행동관찰 통증 사정 도구를 활용하여 환자의 행동을 관찰하는 방법과 대리인의 관찰에 의한 측정방법 등이 사용되고 있었다. 이중 행동 관찰 통증 사정에는 여러 가지 표준화된 도구들이 제시되었으나, 표준화되어 일반적으로 흔히 사용되는 도구는 없었다. 이에 통증 행동관찰 사정 도구 중 비교적 타당도 및 신뢰도가 높게 나타난 도구인 CNPI, DOLOPLUS 2, PACSLAC, PAINAD와 국내에서 유일하게 사용된 도구인 DS-DAT를 선택하여 고찰 후 비교분석 하였다. 결론: 완화의료에서 언어적 의사소통이 어려운 치매환자의 통증을 정확하게 사정하는 것은 어려운 일이지만 적절한 통증관리를 위해 매우 중요하다. 본고에서 제시한 치매환자 통증 사정의 일반적 지침들은 임상에서 활용하기에 유용할 것으로 기대된다. 추후 국내에서 언어적 의사소통이 어려운 치매환자들의 통증 사정 시 행동관찰 통증 사정 도구들을 사용하기 위해서 한글판 타당도 및 신뢰도 검증 연구가 선행되어야 할 것이다.

요양보호사의 치매에 대한 지식, 태도 및 교육요구도 분석 (Analysis of Knowledge About, Attitude Toward, and Educational Needs for Dementia in Certified Caregivers)

  • Purpose: The purpose of this study was first to identify knowledge about, attitude toward, and educational needs for dementia in certified caregivers; secondly, to understand the relationships among those variables; and thirdly, to provide base-line data for education program development. Methods: The participants in this study were 670 certified caregivers who were working in nursing homes or care hospitals throughout the whole of the country and engaged in education programs for improvement of their professional abilities. Collected data were analyzed using t-tests, ANOVA, Duncan, and Pearson's correlation coefficients with the SPSS 18.0 for Windows Program. Results: The scores for knowledge about, attitude toward, and educational needs for dementia were $14.12{\pm}2.83$ out of 20, $42.27{\pm}5.54$ out of 50 (item mean 4.22), and $121.51{\pm}22.13$ out of 160 (item mean 3.79) respectively. There were statistically significant positive correlations between knowledge and attitude (r=.238, p<.001), educational needs (r=.083, p=.032), and attitude and educational needs (r=.244, p<.001). Conclusion: Tailored and continuous educational programs for certified caregivers that can improve knowledge about the causes and symptoms of dementia should be developed. Nurses and professors who are experts in dementia should be on the front lines of education for caregivers.

청소년의 치매에 대한 지식과 태도 (A Study on Dementia-related Knowledge and Attitudes in Adolescents)

  • Purpose: The purpose of this study is to examine adolescents' knowledge and attitudes towards dementia and to provide basic data for educating about knowledge of dementia and strengthening positive attitudes towards dementia. Methods: This study used a descriptive research design. The subjects were 502 middle and high school students. A questionnaire organized by 16 questions of knowledge and 10 questions of attitudes towards dementia was used. Results: The subjects' knowledge level of dementia was low, and average score was $8.89{\pm}2.95$. The questions with low rate of correct answer were "Dementia is caused by several dozens of diseases such as internal medicine, neurology, psychiatry, etc.", "In spite of the dementia, patients can enjoy their favorite things." The subjects' average score of attitudes towards dementia was $3.72{\pm}0.58$. Questions of negative attitudes were "I don't want to be closed to patients of dementia.", "I watch information or prevention about dementia in mass communications media." The relationship between dementia-related knowledge and attitudes showed positive correlation. Conclusion: The result means the mediation is necessary for improving dementia-related knowledge and strengthening positive attitudes in adolescents. The education programs should be proceeded to provide adolescents with correct information about dementia.

치위생(학)과 학생의 치매에 대한 지식 및 태도와 교육요구도 (Knowledge, attitudes, and the educational needs on dementia for the students of dental hygiene department)

  • Objectives: The purpose of this study was to contribute to develope dental health intervention standard curriculum for dementia patients by identifying the extent of knowledge, attitudes and educational needs toward dementia in the dental hygiene department students. Methods: The study was conducted in the students of the dental hygiene department in colleges and universities, nationwide (from the first year students to the 3rd or 4th year students) for approximately 2 months from March to April, 2018. Among them, 545 students were selected as the subjects for the final analysis. Results: 91.0% of the subjects were not currently receiving education on dementia. The research on the factors to impact the dementia education needs of the subjects showed that the dementia education necessity (p<0.001) and the dementia attitude (p<0.001) had statistically significant effects on the educational needs on dementia. Conclusions: It is considered that the standard education curriculum for the dental health intervention for dementia patients in the department of dental hygiene should be developed and disseminated. This will provide a basis for the dental hygienists to be equipped with the relevant expertise in the intervention in the dental health of dementia patients in the future.

치매교육을 위한 가상현실 프로그램에 대한 체계적 고찰 (A Systematic Review of the Virtual Reality Program for Dementia Education)

  • 본 연구는 치매교육으로 제공한 국내·외 가상현실 프로그램의 특성, 효과, 적용 전략 등을 확인하는 체계적고찰 연구이다. 연구방법은 2020년 7월까지 국내·외에서 발표된 문헌을 대상으로 국외는 CINAHL, Cochrane, EMBASE, Pubmed 전자 데이터베이스를 사용하였으며, 국내는 RISS, KISS, DBpia에 대한 검색을 시행하였다. 선정기준과 배제기준을 근거로 최종 6편의 논문이 분석에 사용되었다. 분석결과 선정된 문헌의 연구대상자는 비전문 간병인 2편(33.3%), 일반인 1편(16.7%), 간호학생과 의학 및 약학대 학생을 포함한 학생들 3편(50.0%)이었다. 가상현실 프로그램은 총 5개(공감능력, 치매 지식과 편안감을 포함한 치매태도, 자신의 능력에 대한 신뢰, 양자관계척도)영역에 효과가 있었으며, 이 중 공감능력에 대한 평가가 가장 많았으며 공감능력을 측정한 연구의 75%에서 가상현실 프로그램이 유의한 효과를 나타내었다. 본 연구 결과를 통해 공감을 기반으로 한 인간중심적 간호에 가상현실 기술을 활용할 수 있음을 확인하였다.

행동증상 아형 전측두엽 치매 표현형모사 증후군 (Behavioral Variant Frontotemporal Dementia Phenocopy Syndrome)

  • 연구목적 소위 '행동증상 아형 전두측두엽 치매 표현형모사 증후군(behavioral variant frontotemporal dementia phenocopy syndrome)' 환자들은 일차성 정신장애와 감별이 힘들고, 기존의 치료 약물에 대한 반응도 회의적이어서, 오진의 위험과 법적 문제의 유발 가능성이 높으므로, 이러한 양상을 보이는 환자군에 대한 지속적인 관심과 연구가 필요함을 환기 시키기 위해서 본 연구를 시행하였다. 방법 2000년 부터 2016년 까지 발행된 영문 학술지에 게재된 논문을 인터넷에서 'frontotemporal', 'phenocopy', 'behavioral'과 같은 단어의 조합으로 검색하여 찾은 참고문헌을 정리, 고찰하였다. 또한 저자가 직접 경험한 두 증례를 간략히 기술하였다. 결과 환자의 행동 증상은 가족과 지역사회에 큰 영향을 끼치므로 행동증상 아형 전두측두엽 치매에서 정확한 진단이 중요하다. 그러나, 행동증상 아형 전두측두엽 치매에서 현재까지 질병 수정 치료법은 개발되지 않았으며, 현재의 약물학적 치료는 특수한 증상에 도움이 될 뿐이고, 적절한 정신의학적 치료에도 불구하고 점진적인 퇴행이 진행된다. 상당수의 '행동증상 아형 전두측두엽 치매 의심(possible bvFTD)' 환자에서 임상적으로 '행동증상 아형 전두측두엽 치매 가능(probable bvFTD)'으로 진행되지 않으며, 인지기능이나 사회적 기능이 저하되지 않고, 활동 기능의 저하를 보이지 않으며, 생존기간이 조금 더 길고, 수년에 걸쳐서 정상적인 뇌영상화 검사 소견을 보인다. 결론 환전두측두엽 치매 및 일차성 정신장애 환자에서 보이는 전형적인 임상적 양상이나 경과, 뇌영상화 검사를 포함한 진단적 평가와는 다른 비전형적인 소견을 보이는 환자군들은 생각보다 많으며, 정신의학과 의사들이 이에 대한 의학적 지식과 판별 능력이 발전되면, 그 발견의 비율은 훨씬 더 높아질 것이다. 그러나, 현재로서는 이러한 비전형적인 환자군의 정체에 대해서는 이견이 많으며, 향후 적극적인 연구가 행해져야할 분야이다.

치매 노인을 돌보는 요양보호사의 서비스 질에 미치는 영향요인: 치매에 대한 지식, 일 가치감과 업무수행 자신감을 중심으로 (Influencing Factors Affecting Caregivers' Service Quality for the Elderly with Dementia: Focusing on Dementia Knowledge, Work Value, and Job Performance Confidence)

  • Purpose: The purpose of this study was to verify influencing factors affecting service quality provided by caregivers working for the elderly with dementia. Methods: Data were collected using a self-reported questionnaire from 214 caregivers in a long-term care facility in D city. The data were analyzed with service quality, dementia knowledge, work value, and job performance confidence. For data analysis, the descriptive statistics, t-test, one-way ANOVA, Pearson correlation coefficients, and multiple regression were performed using SPSS/WIN 21.0 program. Results: There were significant differences in service quality depending on the health status. Factors influencing service quality were work value, and job performance confidence with R2 value of 38%. The highly influencing factors were work value, and job performance confidence. Conclusion: The results of this study indicate that the effort to improve the service quality of caregivers should focus on work value and job performance confidence.

저소득층 중년여성의 치매 지식, 태도, 자기효능과 예방행위와의 관계연구 (Relationships between Dementia Knowledge, Attitude, Self-Efficacy, and Preventive Behavior among Low Income Middle-Aged Women)

  • Purpose: This correlational study was to examine the relationships between dementia knowledge, attitude, self-efficacy and preventive behavior of low income middled-aged women. Methods: The subjects for this study were 125 low income middle aged women living in I city. The data were collected using the questionnaires for dementia knowledge, attitude, self-efficacy and preventive behavior. The data analysis was done by descriptive statistics, t-test, ANOVA, Pearson product moment correlation and stepwise multiple regression analysis. Results: The mean of dementia knowledge was 13.96 out of 20, attitude was 43.98 out of 60, self-efficacy was 54.07 out of 75 and preventive behavior was 25.98 out of 36. The positive correlations were revealed between dementia knowledge (r=.458, p=.000), attitude (r=.498, p=.000), self-efficacy (r=.573, p=.000) and preventive behavior. The influencing factors for dementia preventive behavior were self-efficacy, belief in Buddhism and attitude which accounted for 42.5% of the total variance. Conclusion: Dementia knowledge, attitude and self-efficacy were identified as variables that correlate dementia preventive behavior. Also, self-efficacy is the most influential factor affecting dementia preventive behavior. On the basis of these results, it is necessary for nurses to consider using dementia knowledge and mode of efficacy expectation in order to improve dementia preventive behavior.

치과위생사와 치위생(학)과 교수의 치매에 대한 지식 및 태도와 교육요구도 (Knowledge, attitudes, and educational needs on dementia among dental hygienists and dental hygiene professors)

  • Objectives: The purpose of this study was to contribute to the development of standard curriculum on oral health intervention on dementia patients for dental hygienists and dental hygiene professors by identifying the extent of knowledge, attitudes and educational needs on dementia among dental hygienists and dental hygiene professors. Methods: We performed survey to dental hygienists and dental hygiene professors for about 2 months from April to May, 2018. Among them, 325 copies were used for final analysis. Frequencies and percentages were calculated to identify general characteristics of respondents and their dementia-related characteristics, and means and standard deviations were calculated to find out the extent of knowledge, attitudes, and educational needs on dementia among subjects. Multiple regression analysis was performed to investigate the effects on the educational needs on dementia. Results: The analysis on the factors that affect the dementia education needs of the subjects showed that the dementia education necessity (p<0.001) and the dementia attitude (p<0.001) had statistically significant effects on the educational needs on dementia. Conclusions: As a result, dental hygienists who are responsible for oral health intervention of dementia patients need to have proper knowledge about dementia and positive attitude toward dementia patients, so professional education is needed to improve knowledge and positive attitude. This will provide a basis for the dental hygienists to be equipped with the relevant expertise in the intervention in the oral health of dementia patients in the future.

간호학생을 위한 치매교육프로그램의 효과 (The Effect of Dementia Education Program on Nursing Students)

  • Purpose: This study was done to evaluate the effects of dementia education program on knowledge and attitude toward dementia among nursing students. Methods: A non-equivalent control group pre-post test design with an experimental group (n=35) and a control group (n=32) was used. The experimental group participated in the program three times a week for two weeks. The program is composed of lectures, preparation and practice for running the program, The effect of program was evaluated using measures of knowledge and attitude toward dementia. Data were analyzed using the PASW 18.0 program. Results: The knowledge score toward dementia was significantly higher in the experimental group than in the control group. The attitude toward dementia was significantly more positive in the experimental group than in the control group. Conclusions: The study findings indicate that dementia education program had beneficial effects on knowledge and attitude toward dementia in nursing students.
