본 연구는 요양병원 간호사의 치매태도, 치매행동심리증상 부담감, 간호전문직관의 관계를 파악하고, 치매간호 수행에 영향을 미치는 요인을 파악하기 위해 실시되었다. 자료수집은 구조화된 설문지를 이용하여 요양병원 간호사 161명을 대상으로 2019년 11월 4일부터 12월 13일까지 시행하였으며, 자료분석은 SPSS/WIN 21.0프로그램을 이용하여 t-test, ANOVA, Pearson's correlation coefficient, stepwise multiple regression으로 하였다. 치매간호수행에 영향을 미치는 주요 요인은 치매태도(β=.280, p=.001), 간호전문직관(β=.237, p=.006), 치매행동심리증상 부담감(β=-.175, p=.012), 치매교육경험(β=.166, p=.014)이었다. 따라서 요양병원간호사의 치매간호수행도 향상을 위해서는 긍정적인 치매태도와 간호전문직관의 확립, 치매행동심리증상 부담감 감소를 위해 다양한 중재프로그램 개발과 치매교육의 적용이 필요하다.

1990년대 후반부터 공립요양병원이 설립되었고, 2018년 기준, 79개소로 전체 요양병원의 5.1%를 차지하고 있다. 문재인 정부에서 치매 국가책임제를 발표하면서 공립요양병원을 치매안심병원으로 지정하여 중증치매환자에 대한 전문치료를 수행하는 기관으로 육성하고자 다양한 정책을 펼치고 있다. 이와 관련하여 2018년에 치매관리법을 개정하여 공립요양병원 운영평가를 위한 법적 근거가 마련되었으며, 2020년부터 본 평가가 시작되었다. 본 연구는 2020년도 공립요양병원 운영평가를 시행하기 위한 연구의 일환으로 공립요양병원을 치매환자의 전문치료기관 및 회복기 재활환자를 치료할 수 있는 역량 있는 기관으로 육성할 수 있도록 기능을 정립하였으며, 양질의 진료서비스, 공공성, 책무성 등 공공기관으로서의 행태를 평가할 수 있도록 평가체계를 개발하였다. 또한 개발된 평가지표가 적용가능한지 검증하기 위해 공립요양병원 79개소 중 20개소를 대상으로 시범 평가를 시행하였다. 본 평가의 결과를 바탕으로 교육 및 피드백이 진행되고 지속적으로 관리 할 수 있는 모니터링시스템을 구축함으로써 전체 요양병원의 기준(standard)으로서의 기능과 역할을 수행할 수 있을 것으로 기대한다.

최근 노인관련 요양 문제는 이미 사회적 이슈로 떠오르면서 각 가정에서의 돌봄이 곤란한 어르신들이 요양원, 재가복지센터, 주간보호센터, 노인전문병원 등에서 요양보호사의 도움이 절대적으로 필요한 일상이 되었다. 무엇보다도 장기요양서비스를 이용하는 어르신 대부분은 노인성 질환 또는 치매를 앓고 있다. 이는 각 가정에서의 돌봄이 어려운 어르신들에게 직접서비스를 제공 하는 종사자인 요양보호사들은 다양한 치매어르신들의 문제행동을 안고 현장에서의 돌봄을 제공한다. 따라서, 요양보호사의 직무 관련 스트레스에 미치는 영향력 파악과 이와 더불어 직무스트레스가 치매어르신의 문제행동과 종사자의 소진 간의 매개 효과가 있는 지 규명하는 연구들이 활발하게 이루어 짐으로 이를 규명한 내용을 토대로 대안 마련을 위한 유익한 제언들이 필요하다.

The purpose of this study is to suggest the direction of the specialized dementia hospital which can confront in the old age society of Korea changing quickly. We need to make the architectural planning guide including the form determining factors which is useful for the domestic dementia hospital. The factors can be extracted from a database by analysing and investigating the documents of the developed nations equips the excellent dementia hospital. The result of this study is like this : at first, it is very important that the patient has to have the feeling of comfort and familiarity through the external space leads a direct participation. Second, the interior space must have the composition for the patients primarily. So, this research can be used the basic document of the dementia hospital planning.

As the population of the elderly increases drastically year by year in Korea, this phenomenon is being a serious problem because of the increasing speed not having been showed ever all over the world. This thesis would propose the space organizing features for well-organized dementia hospital focused on the public living space. The way arranging each function is very important because values of the physical and functional accessibility have a wide range of value as each type of facility. Moreover, if the arranged function unit is the same as the territory of the space, it could be a good guideline on architectural planning.

In Korea, it is urgently required to develop the health care facilities and services for People with dementia. The progression of dementia is associated with a gradual decline in functional, cognitive, emotional, and social abilities. Therefore, architectural design for People elderly with dementia should be focused to maintain and preserve this abilities. This study aims to derive the design guidelines of the ward and patient roon unit planning to maintain this abilities in the health care facilities for People with dementia. For this, investigation and analysis through the case studies of domestic and foreign facilities and P.O.E of domestic facilities were performed.

According to the rapid increase of the elderly population, especially frail older population, many kinds of elderly care facilities have been supplied within a relatively short period. Among them, elderly hospitals and elderly welfare facilities have occupied a major portion. The elderly hospital, which had emerged from 1994, has recorded sharp increase in facility numbers and bed numbers by the support of Korean government together with the increase of care demand. However, the concept and fundamental planning criteria of elderly hospitals have not yet been set up. This paper has derived the concept of the elderly hospital from the Medical Law and Elderly Welfare Law, and prospected the supply of it from domestic and international statistics. Also this paper has explored the fundamental design issues of elderly hospitals by analysing precedent studies and designs, and by surveying some facilities.

본 논문은 요양원·요양병원에 입원한 노인들을 위한 실행능력중심 기능성 스마트 게임기기 및 데이터를 관리할 수 있는 시스템을 개발하고자 한다. 시스템은 증강현실 기반의 스마트맵, 동작 감응형 스마트 스틱, 스마트 명령과 연동하는 로봇, 이러한 게임 콘텐츠로 동작되는 모든 기기에서 나오는 데이터를 수집·관리·분석하는 정보 측정 및 분석 시스템으로 동작되는 시스템을 설계하였다. 이 시스템을 통해 한국이나 일본의 노인병원에서 간절히 필요로 하는 요양보호사 없이 노인들끼리 셀프 진행 가능하고 재미있는 기능성 콘텐츠의 수요를 만족시켜 줄 수 있을 것이다. 또한 치매와 같이 완벽한 치료가 불가능한 질병에도 증상악화를 늦추는 효과가 있으리라 예상한다. 향후 시스템을 개발하여 직접 실험을 하면서 다양한 데이터 수집을 통해 빅데이터로 활용하여 보다 전문적이고 효율적인 재활을 진행할 것이다.

본 연구는 요양시설 치매노인들의 인지기능과 구강건강상태 및 구강 내 병원성 미생물의 관계를 파악하기 위해 시도되었다. 대구소재 요양기관에 거주하는 70세 이상 치매노인이라는 특수성으로 비확률적 표본방식을 통해 30명을 대상으로 2019년 2월 한 달간 자료를 수집하였다. SPSS window ver. 21.0 프로그램을 이용하여 기술통계, ANOVA, Pearson's 상관계수로 분석하였다. 연구결과 요양판정등급이 낮을수록 고위험 병원성 세균이 많았고, 현존 치아수가 많을수록 구강 내 미생물의 수가 높게 나타났다. 하지만 인지기능과는 무의미한 차이를 보였다. 따라서, 요양시설의 치매노인 구강건강 관리를 위해서는 구강 내 환경에 따라 관리 방법을 달리하여야 하고 실제적인 건강관리를 위해서는 전문적인 관리자가 구강건강관리를 할 수 있어야 한다고 제언한다.

The purpose of this study is to clarify the actual condition of the daily living activities of the elderly with dementia in recuperating facilities. The thorough investigation and observation works were made to them from the view points of daily living behaviors and behavioral places of the recuperating elderly. This article discusses about the basic characteristics of the elderly and the actual condition of the daily living activities of the elderly with dementia in recuperating facilities for the aged with dementia. The results of this study are summarized as follows; There is no wide difference between geriatric hospital and nursing home for the aged with dementia on the characteristic of living behaviors of the elderly with dementia. The usage of behavioral places in recuperating facilities are influenced by the physical environments and the basic characteristics of the elderly with dementia.
