Purpose : Prostate specific antigen (PSA) is a useful tumor marker, which is widely used as a diagnostic index and predictor of both treatment and follow-up result in prostate cancer. A prospective analysis was carried out to obtain the period of PSA normalization and the half life of PSA and to analyze the factors influencing the period of PSA normalization. The PSA level was checked before and serially after radical radiotherapy. Materials and Method : Twen쇼 patients with clinically localized prostate cancer who underwent radical external beam radiotherapy were enrolled in this study. Accrual period was from April 1993 to May 1998. Median follow-up period was 20 months. Radiotherapy was given to whole pelvis followed by a boost to prostate. Dose range for the whole pelvis was from 45 Gy to 50 Gy and boost dose to prostate, from 14 Gy to 20 Gy. The post-irradiation PSA normal value was under 3.0 ng/ml. The physical examination and serum PSA level evaluation were performed at 3 month interval in the first one year, and then at every 4 to 6 months. Results : PSA value was normalized in nineteen patients (95%) within 12 months. The mean period of PSA normalization was 5.3 (${\pm}$2.7) months. The half life of PSA Of the nonfailing patients was 2.1 (${\pm}$0.9) month. The nadir PSA level Of the nonfailing Patients waS 0.8 (${\pm}$0.5) ng/ml. The period of PSA normalization had the positive correlation with pretreatment PSA level (R$^{2}$=0.468). The nadir PSA level had no definite positive correlation with the pretreatment PSA level (R$^{2}$=0.075). The half life of serum PSA level also had no definite correlation with pretreatment PSA level (R$^{2}$=0.029). Conclusion :The PSA level was mostly normalized within 8 months (85%). If it has not normalized within 12 months, we should consider the residual disease in prostate or distant metastasis. In 2 patients, the PSA level increased 6 months or 20 months before clinical disease was detected. So the serum PSA level can be used as early diagnostic indicator of treatment failure.

We analyzed retrospectively the patients of granulocytic sarcoma treated with radiotherapy at the Department of Radiation Oncology, Yonsei University College of Medicine from Mar 1987 to Mar.1992 in an attempt to review our experience with irradiation of granulocytic sarcoma and to evaluate the treatment results for the radiation dose response. Fourteen lesions of granulocytic sarcoma in 9 patients were developed in variable clinical settings such as AML, CML and without leukemia. The involved lesions were bone, lymph node, soft tissue and skin in descending order of occurrence. All of the lesions in 9 patients were treated with external beam radiotherapy (Co-60 or electron beam). Both age distribution and clinical settings did not show any correlation with the response to treatment. The response to treatment seemed to be better for lesions in the bone than in other involved lesions. The majority received local irradiation of a total dose of more than 2000 cGy. Radiation dose of more than 2000 cGy showed excellent local control of $100\%$, (11/11), while local control decreased to $33\%$(1/3) with total dose less than 2000 cGy. In conclusion, local radiotherapy seems to be very effective for palliative or curative aim of granulocytic sarcoma, and a radiation dose more than 2000 cGy is highly recommended.

악성종양을 치료하는 방법중 방사선과 온열요법은 가장 강력한 치료방법으로 연구되어왔으며 이를 병용함으로 서 상승효과를 얻을 수 있다. 인체조직에 41$^{\circ}C$ 이상의 열을 가하면 세포질의 단백질변성으로 세포에 손상을 주어 세포가 사멸하게 되며 세포의 생존율은 가열시간 즉 열량에 따라 지수적으로 감소한다. 온열은 세포주기중 방사선 저항성이 매우 큰 DNA 합성시기와 산도가 높을 때 감수성이 매우 크기 때문에 방사선과 병용요법은 상호 상승효과를 가져온다. 이와 같이 온열을 이용한 악성종양의 치료가능성은 생물학적 기초연구와 임상시험에서 경이적인 효과를 얻을 수 있었으나 아직 까지 가열방법과 온도분포측정이 큰 과제로 남아있으며 주위건강조직의 가열을 피하면서 인체 깊은 곳에 존재하는 종양에만 집중 가열하는 방법인 삽입형 온열치료방법에 대한 연구가 집중되었다. 한편 방사선 치료방법은 주위 건강조직의 피폭을 최소로 줄이고 종양에만 집중 조사가 요구되며 자궁암, 유방암, 뇌암등 부피가 작고 집중적 치료를 요하는 종양은 방사성동위원소를 이용한 근접 삽입치료 (Brachyradiotherapy)가 큰 효과를 나타내고 있다 방사선과 온열의 병행 치료를 위하여 방사선 삽입 치료에 사용한 선원 삽입관을 그대로 두고 삽입관 속에 방사성 동위원소 대신 온열 전극을 넣어 열을 가하는 방사선 온열 병용치료방법을 고안하였으며 방사선과 온열병용에 사용할 최적 삽입관의 제작과 이에 따른 온도분포의 측정과 최적삽입방법을 결정하였다. 방사선 삽입치료용 폴리에찌렌 삽입관의 외부에 금박을 입혀 라디오파 첨극을 삽입할 때 서로 연결되도록 고안 제작함으로서 방사선 삽입치료와 자입식 온열치료를 동시에 만족하게 수행할 수 있는 병용삽입관 (Flexible thermoradiotherapy probes)을 제작하였다. 전도율이 큰 금박부위가 직접 조직에 접촉됨으로 라디오파의 전달이 용이하며 금박의 길이를 2 cm 에서 5 cm 로 구분제작 함으로서 종양의 크기와 모양에 따라 선택할 수 있도록 하였다. 라디오파를 이용한 온열분포의 측정은 인체조직과 전기적 특성이 비슷한 물질인 한천 팬텀 제작하여 사용하였으며 온도분포 측정은 열전대와 서머그람으로 시행하였다. 생체조직 내에서의 온도분포와 온열효과를 관찰하기 위하여 직접 개의 뇌를 이용하여 시행하였으며 4 개의 전극을 이용하여 43$^{\circ}C$로 50분간 가열하고 일주일후 개를 회생시켜 개 뇌에 대한 조직학적 검사를 시행하였다. 한편 팬텀 표면에서 중앙부로 안테나 길이가 2 cm 인 4 개의 전극을 1 cm 간격으로 정사각형이 되도록 삽입하여 가열하였을 때 90% 등온곡선이 반경 1.25의 원형으로 균일하게 분포되었고 종단면상 삽입관의 길이에 따라 균일한 온도분포가 이루어졌다. 전극을 2 cm 간격으로 삽일 하였을 때 90% 등온곡선이 1.75 반경으로 거의 4 각형의 균일한 분포를 얻었으나 전극의 간격이 증가하면 전도율이 떨어져서 전극 중심부에 불균일한 온도분포를 형성하였다. 동물실험에서 정상 개의 뇌 실질에 자입하여 직접 정방형의 중심을 43$^{\circ}C$로 유지하며 50분간 온열 요법을 시행한 후 관찰한 조직병리학적 소견은 liquefactive necrosis, pyknosis of neuronal element 및 polymorphonuclear leukocytes들의 회백질에서 급성기에 관찰되었고 liquefactive necrosis 주위에 lipid-laden macrophage들이 관찰됨이 공통적인 특정이었으며 후기변화로 괴사조직 주위로 신경교세포의 증식이 관찰되었다.

To generate on-board digital tomosynthesis (DTS) for three-dimensionalimage-guided radiation therapy (IGRT) as an alternative to conventional portal imaging or on-board cone-beam computed tomography (CBCT), two clinical cases (liver and bladder) were selected to illustrate the capabilities of on-board DTS for IGRT. DTS images were generated from subsets of CBCT projection data (45, 162 projections) using half-fan mode scanning with a Feldkamp-type reconstruction algorithm. Digital tomosynthesis slices appeared similar to coincident CBCT planes and yielded substantially more anatomic information. Improved bony and soft-tissue visibility in DTS images is likely to improve target localization compared with radiographic verification techniques and might allow for daily localization of a soft-tissue target. Digital tomosynthesis might allow targeting of the treatment volume on the basis of daily localization.

In this study, plans to apply 3D conformal radiotherapy, intensity modulated radiotherapy, and volumetric intensity modulated arc radiotherapy to esophageal cancer radiotherapy were compared. In particular, arc therapy was applied to reduce irradiated volume and spread of low-dose during intensity modulated radiation therapy and volumetric intensity modulated arc radiotherapy by limiting part of irradiated angle, in order to compare target doses and dose for surrounding normal tissues of the two methods and those of 3D conformal radiotherapy. No significant difference in target dose was found among the three methods. The 5 Gy volume(V5) of the lung showed 56.53% of conformal radiotherapy, 52.03% of intensity modulated radiotherapy, and 47.84% of volumetric modulated arc therapy(CRT-IMRT p=0.035, CRT-VMAT p<0.001, IMRT-VMAT p<0.001). The 10 Gy volume(V10) showed a significant difference in conformal radiotherapy 35.12%, intensity modulated radiotherapy 34.04%, and volumetric modulated arc radiotherapy 33.28%, showing significant difference in intensity modulated radiotherapy(p=0.018), volumetric modulated arc therapy(p=0.035), no significant difference in dose was found at 20 Gy volume. The mean dose and 20 Gy volume of the heart were not significantly different according to the treatment plan, but the 30 and 40 Gy volumes were 37.16% and 22.46% in the volumetric modulated arc radiotherapy, showing significant differences(p=0.028) in comparison with conformal radiotherapy. It is believed that, by limiting part of the irradiated angle during intensity modulated radiotherapy and volumetric intensity modulated arc radiotherapy, the irradiated volume and, thereby, the 5-10 Gy area and toxicity of the lung can be reduced while maintaining dose distribution of the target dose.
The aim of this study was to compare the dose distribution of intensity modulated radiation therapy (IMRT) with 3 dimensional conformal radiation therapy (3DCRT) in prostate cancer. The IMRT plan and the 3DCRT plan used the 9 fields technique, respectively. In IMRT, tumor dose was a total dose of 66 Gy at 2.0 Gy per day, 5 days a week for 5 weeks. All cases were following the dose volume histogram (DVH) constraints. The maximum and minimum tumor dose constraints were 6,700 cGy and 6,500 cGy, respectively. The rectum dose constraints were <35% over 50 Gy. The bladder dose constraints were <35% over 40 Gy. The femur head dose constraints were <15% over 20 Gy. Tumor dose in the 3DCRT were 66 Gy. In IMRT, the maximum dose of PTV was 104.4% and minimum dose was 89.5% for given dose. In 3DCRT, the maximum dose of PTV was 105.3% and minimum dose was 85.5% for given dose. The rectum dose was 34.0% over 50 Gy in IMRT compared with 63.3% in 3DCRT. The bladder dose was 30.1% over 40 Gy in IMRT compared with 30.6% in 3DCRT. The right femur head dose was 9.5% over 20 Gy in IMRT compared with 17.5% in 3DCRT. The left femur head dose was 10.6% over 20 Gy in IMRT compared with 18.3% in 3 DCRT. The dose of critical organs (rectum, bladder, and femur head) in IMRT showed to reduce than dose of 3DCRT. The rectum dose over 50 Gy in IMRT was reduced 29.3% than 3DCRT. The bladder dose over 40 Gy in IMRT was similar to 3DCRT. The femur head dose over 20 Gy in IMRT was reduced about 7~8% than 3DCRT.
Purpose: The purpose of this study was to investigate the positional change of the uterus during radiotherapy. Materials and Methods: Between 1997 and 2001, 47 patients received definitive radiotherapy for cervical cancer at the Samsung Medical Center. For each patient, two MRI scans were taken; one before and the other 3$\~$4 weeks after the radiotherapy treatment. In T2 weighted MRI images, the positional change of the uterine was quantified by measuring six quantities; the distance from the cervix os to the isthmus of the uterus (Dcx), the maximum length from the isthmus of the uterus to the uterine fundus (Dco), the maximum vertical distance of the uterine body (Dco-per), the angle between the vertical line and the cervical canal in the sagittal images (Acx), the angle of the uterine corpus from the vertical line in the sagittal plan (Aco-ap), and the relative angle of the uterine corpus from a fixed anatomical landmark in the axial images (Aco-axi). Results: The mean Dcx values, before and during the treatment, were 36.7 and 27.8 mm, respectively. The Dco deviated by more than 10 mm in 14 cases (29.8$\%$). The change in the Acx ranged from 0.1 to 67.8$^{\circ}$ (mean 13.2$^{\circ}$). The Aco-ap changed by a maximum of 84.8$^{\circ}$ (mean 16.9$^{\circ}$). The differences in the Dcx plus the Dco in the smaller (<4 cm) and larger ($geq$4 cm) tumors were 5.3 and 19.4 mm, respectively. With patients less than 60 years old, or with a tumor size larger than 4 cm, the difference in the Acx was statistically significant. Conclusion: The positional changes of the uterus, during radiation treatment, should be considered in the 3DCRT or IMRT treatment planning, particularly in patients under 60 years of age or in those with a tumor size greater than 4 cm in maximum diameter.
