THE CHANGE OF CANAL CONFIGURATION AFTER INSTRUMENTATION BY SEVERAL NICKEL-TITANIUM FILES IN THE SIMULATED CANAL WITH ABRUPT CURVATURE (수종의 엔진구동형 니켈-타이타늄 파일에 의한 급한 만곡의 근관 성형시 근관형태 변화에 대한 비교연구)

  • The purpose of this study was to evaluate which type of Ni-Ti files are able to maintain canal configuration better in the simulated canal with abrupt curvature near it's apex. Ninety six simulated root canals were made in epoxy resin and $\sharp$15 finger spreader was used as root canal templates. The simulated root canals were made with radius of curvature of 1.5mm, 3.0mm, 4.0mm, 6.0mm respectively and the angle of curvature of all simulated canals were adjusted to 90 degree. The simulated canals were instrumented by ProFile, ProTaper, Hero 642, and $K^3$ at a 300 rpm using crown-down pressureless technique. Pre-instrumented and post-instrumented images were taken by digital camera and were superimposed with Adobe Photos hop 6.0 program. Images were compared by image analysis program. The changes of canal width at the inner and outer side of the canal curvature. canal transportation were measured at 9 measuring point with 1 mm interval. Statistical analysis among the types of Ni-Ti files was performed using Kruskal-Wallis test and Mann-Whitney U-test. The result was that ProFile maintain original canal configuration better than other engine driven Ni-Ti files in the canals above 3.0mm radius of curvature, and in the 1.5mm radius of curvature, most of Ni-Ti flies were deformed or separated during instrumentation.

Retrospective study on three-dimensional characteristics of mesiodens using CBCT in pediatric dentistry (치과용 Cone-beam CT를 이용한 상악 정중과잉치의 3차원적 특성에 관한 후향적 연구)

  • The purpose of this study was to investigate the three-dimensional characteristics of mesiodens using Cone-beam Computed Tomography(CBCT) and analyze the factors affecting complications and anesthetic methods of extraction. This study evaluated 602 mesiodens of 452 patients who underwent extraction of mesiodens at the department of Pediatric Dentistry in Seoul National University Dental Hospital between 2017 and 2019. The ratio of mesiodens patients over total patient per year was gradually increased over the past 20 years. Mesiodens with labio-palatally horizontal direction while root directing labial were the most common among the mesiodens with horizontal direction. Mesiodens were the most common at the cervical side of the adjacent teeth(37.0%) and mesiodens located in the near-palatal side were observed about 3.83 times higher than the far-palatal side. Most of the mesiodens(82.1%) were in contact with adjacent permanent teeth on all three sides of the CBCT and 46.2% of mesiodens had curved roots. The patient's age, vertical position, presence of complications, and proximity showed a significant difference in the selection of general anesthesia among anesthetic methods. The direction and vertical position of mesiodens had a significant effect on complications. These results provide a better understanding of mesiodens for establishing an accurate diagnosis and treatment plan.

A COMPARATIVE STUDY ON THE CANAL CONFIGURATION AFTER SHAPING BY PROFILE, PROTAPERTM AND K-FLEXOFILE IN SIMULATED CANALS WITH DIFFERENT ANGLES OF CURVATURE (ProFile, ProTaperTM K-Flexofile 근관 성형시 근관의 만곡도에 따른 근관 형태 변화 비교연구)

  • The purpose of this study was to evaluate the canal configuration after shaping by ProFile. ProTaper and K-Flexofile in simulated resin canals with different angles of curvature. Three types of instruments were used: ProFile. ProTaper. K-Flexofile. Simulated root canals. which were made of epoxy resin. were prepared by ProFile. ProTaper with rotary instrument using a crown-down pressureless technique. and hand instrumentation was performed by K-Flexofile using a step-back technique. All simulated. canals were prepared up to size 25 file at end-point of preparation. Pre and post instrumentation images were recorded with Scanner. Assessment of canal shape was completed with Image Analysis program. Measurements were made at 1. 2. 3. 4. 5. 6. 7. 8. 9 and 10mm from the apex. At each level. outer canal width. inner canal width. total canal width. and amount of transportation from original axis were recorded. Instrument deformation and fracture were recorded. Data were analyzed by means of one-way ANOVA analysis of variance and the Sheffe's test. The result was that ProFile and ProTaper maintain original canal shape regardless of the increase of angle of curvature than K-Flexofile. ProFile show significantly less canal transportation and maintained original canal shape better than ProTaper.

  • Trauma to the primary incisors may cause developmental or eruptional disturbance of the permanent successors. Two cases illustrate developmental disturbances of arrested root formation, dilaceration, and eruptional disturbances of impaction, ectopic eruption of permanent successors caused by traumatic injury to deciduous incisors. The patient of the first case suffered trauma at the age of 4 years 7 months, causing alveolar bone fracture including the maxillary right primary central and lateral incisors that were immedi-ately extracted. The second patient had trauma episodes at the age of 3 years. Avulsion of the maxillary primary right central and lateral incisors were occurred due to trauma. After such trauma, regular follow-up including radiographs is necessary to detect early any possible interference with normal eruption of permanent successors.

  • Inverted maxillary incisor is that maxillary incisor rotates counterclockwise direction. The cause of this 'Inverted incisor' is the injury of the deciduous predecessor transmitted to the developing permanent tooth germ or displacement of permanent tooth crown portion from unknown origin. Dilaceration, defined as a distorted root from, may result from mechanical injury during eruption period or ectopic development of tooth germ. This article presents a case of an inverted and dilacerated maxillary right central incisor. Through orthodontic traction, the dilacerated and inverted incisor was successfully moved into the proper position.

A COMPARISON OF SHAPING ABILITY OF THE THREE ProTaper® INSTRUMENTATION TECHNIQUES IN SIMULATED CANALS (ProTaper®의 세 가지 사용방식에 따른 성형능력 비교)

  • The purpose of this study was to compare the shaping ability of the three $ProTaper^{(R)}$ instrumentation techniques in simulated canals. Thirty resin blocks were divided into 3 groups with 10 canals each. Each group was instrumented with manual $ProTaper^{(R)}$ (Group M), rotary $ProTaper^{(R)}$ (Group R), and hybrid technique (Group H). Canal preparation time was recorded. The images of pre- and post-instrumented root canals were scanned and superimposed. The amounts of canal deviation, total canal width, inner canal width, outer canal width and centering ratio were measured at apical 1, 2, 3, 4, 5 and 6 mm levels 1. Canal preparation time was the shortest in R group (p < 0.05). 2. The amounts of total canal width in R group was generally larger than the other groups, but no significant differences were observed except at the 1, 3 mm levels (p > 0.05) .3. The amounts of inner canal width in R group was larger than M group at the 1 mm level and H group was larger than R group at the 6 mm level (p < 0.05). The amounts of outer canal width in R group was larger than H group only at the 1 mm level (p < 0.05). 4. The direction of canal deviation in H, R group at the 1, 2, 3 mm levels was outward and that in M group at the 1, 2 mm levels was inward. The amounts of canal deviation in H group was larger than R group at the 6 mm level (p < 0.05). 5. The amounts of centering ratio in H group was larger than R group at the 6 mm level (p < 0.05).

EFFECTS OF ANTICURVATURE FILING ON DANGER ZONE WIDTH IN CURVED ROOT CANALS (만곡된 근관 성형시 스테인레스 강 파일과 니켈 티타늄 파일의 anticurvature filing 영향)

  • The aim of this study was to compare the effects of anti curvature filing with stainless steel k-file versus nickel-titanium ProFile in the shaping of mesial root canals of extracted mandibular molars. A total of 60 canals from 30 mesial roots of mandibular molar teeth were randomly assigned to three groups with n=20 each. They were prepared with different instruments and methods: The first group with stainless steel k-file and circumferential filing. the second with precurved stainless steel k-file and anticurvature filing and the third with ProFile C06 taper) and anticurvature filing. Using a micro-computed tomography system (skyscan-1076, SKYSCAN, Antwerpen, Belgium), pre-and post-operative specimens were scanned. Subsequently, canal images were superimposed and changes in root dentin thickness were measured at distal side (danger zone) of the canal. The data was analyzed using a one-way ANOVA and the comparison of means was conducted using a post hoc multiple comparison Tukey test. There were significant differences in the change of root dentin thickness at the $7.5{\sim}8.5\;mm$ level between group 1 and 2, $3.5{\sim}6mm$ level between group 1 and 3 and $3.5{\sim}6mm$ level between group 2 and 3(n=20, P<0.05).

  • The main objective of nonsurgical retreatment is to disinfect the root canal space for periradicular healing. Thus, efficient removal of the filling material from the root canal system is essential to ensure a favorable outcome. Traditionally, the removal of root canal filling material was performed by stainless steel hand instrument and this procedure is time and effort-consuming. Recently, rotary NiTi retreatment instruments are developed to effective removal of root canal filling material. The aim of this study was to evaluate the effectiveness of stainless-steel hand instrument and rotary NiTi retreatment instrument when removing gutta-percha in curved root canals. For the remaining materials, there were no significant differences between groups. But the rotary NiTi system proved to be faster than hand instruments in removing root filling materials.

  • The etiology of mesiodens is unknown but the most widely accepted theory is the hyperactivity of the dental lamina. Complications of mesiodens are delayed or prevented eruption of maxillary central incisors, displacement or rotation of permanent teeth, crowding of affected region, abnormal diastema or permanent space closure, dilaceration or abnormal root development of permanent teeth, primordial or follicular cyst formation, root resorption of adjacent teeth, eruption into nasal cavity. If mesiodens rotate the maxillary central incisors, space deficiency is not common and relapse is very common. So overcorrection is needed. To prevent the rotational relapse, early treatment, overcorrection, long retention period, properly formed proximal surface, use of coupled force, and surgical techniques have been suggested. The authors present two cases, whose chief complain were severely rotated maxillary incisors by mesiodens, treated by orthodontic and surgical technique and showed good results.

  • Preterm infants have higher possibility of undergoing endotracheal intubation after birth than normal children due to medical conditions. Developmental disturbances of primary incisors following intubation can occur as crown malformation, enamel defects, delayed eruption, displacement of dental follicle in crypt, groove formation of palate or alveolar ridge, acquired cleft palate, and dental arch distortion. This clinical report presents the effect of intubation on primary dentition of preterm infants. A 2-year-old girl with cerebral palsy and premature birth history visited our clinic with chief complaint of unerupted primary upper incisor. A 1-year-old boy with cerebral palsy, status epilepticus and premature birth history visited our clinic due to crown malformation. Developmental disturbances of primary incisors in these cases were not related to the patients' systemic disease, and there were no history of dental trauma. A long term endotracheal intubation prior to tooth eruption might have caused local trauma on alveolar ridge. It is very important to monitor dental problems of preterm infants who had experienced endotracheal intubation during neonatal period. Due to influences on both primary and permanent teeth, periodic re-evaluation of affected areas and establishment of comprehensive treatment plans are necessary.
