ELISA FOR MEASURING SERUM IgG AND IgM LEVELS IN PATIENTS OF ACUTE PULPITIS AND ACUTE APICAL ABSCESS (ELISA를 이용한 급성 치근단 농양 및 급성 치수염 환자에서의 혈청 항체 수준에 관한 실험적 연구)

  • Numerous studies have been focused on the immunologic aspects of inflamed pulp and periapical tissues. The purpose of this study was to evaluate levels of serum IgG and IgM in patients of acute pulpitis and acute apical abscess using Enzyme-Linked Immunosorbent Assay. Streptococcus mutans, Streptococcus sanguis, Bacteroides intermedius and Bacteroides gingivalis were grown for use as antigen and they were harvested by centrifugation. The patients were divided into 3 groups; patients of acute apical abecess, acute pulpitis and normal control 5 patients of each group were selected and their blood was obtained via intravenous puncture. Sera were prepared by centrifugation of each blood samples. Then serum antibodies were measured by modified ELISA. The following results were obtained; 1. Serum IgM levels of patients with acute pulpitis and acute apical abscess seemed to be slightly higher than those of normal control 2. Serum IgG levels of patients with acute apical abscess were slightly higher than those of normal control 3. Serum IgG and IgM levels of acute apical abscess patients and serum IgM levels of acute pulpitis were highest to Bacteroides gingivalis.

SEPSIS FROM ODONTOGENIC INFECTION : A CASE REPORT (패혈증으로 진행된 치성 감염 : 증례보고)

  • Incidence and mortality rate of maxillofacial infection is relatively low in the era of antibiotics. Despite the use of antibiotics, delayed treatment, underlying systemic diseases, drug-resistant microorganisms may result in life-threatening situations. The deep neck infection developed from odontogenic infection may result in sepsis, mediastinitis, aspiration pneumonia, asphyxia. Sepsis is the most dangerous complication which can quickly result in a number of lethal situations. The treatment of sepsis includes awareness of such complication, use of sensitive antibiotics, removal of infection source, and hemodynamic, respiratory and metabolic support. We experienced a patient who died of sepsis, which developed from odontogenic infection. The initial diagnosis was a buccal space cellulitis. However, in spite of medical and surgical treatment, this progressed to Ludwig's angina and then deep neck infection and finally sepsis. On the 10th hospital day, the patient died of multiorgan failure caused by sepsis.

Acute Severe Periocular Swelling Caused by Periapical Abscess in a Dog (치근단 농양에 의해 발생된 급성 중증 안구주위부종 1례)

  • A 10-year-old, intact female Yorkshire terrier was presented with right side facial swelling for 3 days and periocular swelling had progressed over the last 24 hours. On physical examination, periocular and infraorbital swelling were observed around right maxillary region. Severe ocular discharge and protrusion of the 3rd eyelid were observed. The patient was diagnosed as periapical abscess around the root of right upper first molar based on the skull radiography. Affected tooth was extracted using closed technique. Systemic corticosteroid and antibiotic were administrated and topical corticosteroid/antibiotic combination was applied to the right eye six times a day for 10 days. Ten days after tooth extraction, clinical signs were disappeared completely.

  • As the dental pulp is encased with a rigid, noncompliant shell, changes in pulpal blood flow or vascular tissue pressure can have serious implication for the health of pulp. Numerous studies have demonstrated that orthodontic force application may influence both blood flow and cellular metabolism, leading degenerative and/or inflammatory responses in the dental pulp. The aim of this case report is to present a case about tooth with chronic periapical abscess which showed normal vital responses. Excessive orthodontic force is thought to be the prime cause of partial pulp necrosis. Owing to remaining vital tissue, wrong dianosis can be made, and tooth falsely diagnosed as vital may be left untreated, causing the necrotic tissue to destroy the supporting tissuses. Clinician should be able to utilize various diagnostic tools for the precise diagnosis, and be aware of the endodontic-orthodontic inter-relationship.

  • Early childhood caries which mainly affect maxillary anterior area, defined as 'the presence of 1 or more decayed, missing, or filled tooth surfaces in any primary tooth in a child 71 months of age or younger'. Extraction of teeth when early childhood caries affected in maxillary primary incisors often develops progressively, result in severe destruction of crowns, acute or chronic pulpitis, and periapical abscess formation. Maxillary primary incisors are need to preserve as possible, because the early loss of maxillary primary incisors may lead to various functional, esthetical, and psychological problems. It is necessary to the availability of an easy to perform technique capable of providing efficient, durable, functional, and esthetic restorative methods. Polyethylene fiber-reinforced post can be used in strengthen of composite resins that is esthetic and good physical and mechanical properties. $Ribbond^{(R)}$ is made from an polyethylene fiber, has numerous usages, its surface is treated to enhance adhesion to resins, ease of manipulation, relatively cost effective. We report this case, had restored of maxillary primary incisors with severe coronal destruction due to affecting severe early childhood caries, using polyethylene fiber-reinforced posts, composite resin cores, and celluloid crowns. We could obtain good result of treatment.

GARRE'S OSTEOMYELITIS IN CHILDREN (소아에서의 Garre 골수염)

  • Garre's osteomyelitis is associated with bacterial infection and bone necrosis resulting from obstruction of blood supply. The most common cause for Garre's osteomyelitis is odontogenic infection that originates from periodontal tissue or dental pulp. Subperiosteal abscess may also cause Garre's osteomyelitis in the progress of the infection. Mandible is more often affected than maxilla, most commonly in the permanent first molar region of mandible. Clinically, it results in a hard swelling over the jaw, producing facial asymmetry. Meanwhile, radiograph shows a characteristic feature of irregular pulpal cavity, showing new periosteal proliferation located in successive layers to the condensed cortical bone on stimulated site. The treatment method for Garre's osteomyelitis are removal of the infection source, root canal treatment, antibiotic medication, and incision and drainage. This report presents a case of Garre's osteomyelitis under 15 years old. The patient was successfully treated by antibiotic medication accompanied with root canal treatment. Since the symptom of pediatric patients is less severe than adult, careful diagnosis with history taking and clinical examination is necessary. Furthermore long-term follow-up examination is needed to prevent recurrence even after the symptom disapears.
