ITI - Octa system을 이용한 fixed removable bridge의 제작

  • 일반적으로 악골의 흡수가 심한 환자에게 총의치 보철물은 적은 유지력 때문에 매우 불리하다. 특히, 하악골의 흡수가 심한 환자의 경우 2$\sim$4 정도의 임플란트를 식립후에 제작되는 stud type의 over-denture나 bar-over denture 또는 magnetic over-denture 등이 있고, 5$\sim$6개 정도의 임플란트 식립으로 제작되는 fixed removable bridge가 환자에게 높은 만족감을 줄 수 있어 총의치 보철 치료시 고려해야 할 보철 치료 방법중의 하나이다. fixed removable bridge를 제작하는데 있어 먼저 placement guide를 이용하여 모델의 임플란트의 위치와 구강내의 임플란트의 위치를 반드시 확인해야 하고, 구조물을 만들기 전에 wax denture를 제작하여 환자의 simile line 이나 악간 관계를 확인 후에 구조물 제작을 해야 한다.

Surface Characteristics of Metallic 3D Printed Dental Framework (금속 3D printing으로 제작한 치과보철물의 표면특성)

  • 다양한 소재(금속, 세라믹, 고분자 소재 등)들이 3차원 형상기반 적층제조법에 적용되고 있는데, 금속 소재를 이용하여 3D 프린팅 법으로 치과용 수복물을 제조하는 연구가 많이 보고되고 있다. 하지만, 티타늄 또는 티타늄 합금 분말을 이용하여 3D 프린팅 법으로 제작한 치과용 보철물에 관한연구 보고는 많지 않다. Kanazawa 등 (2014)은 Ti-6Al-4V 합금분말을 이용하여 SLM법으로 총의치 용 framework를 제작하여 주조법으로 제작한 것과 비교 평가하였고, Mangano 등(2013)은 Ti-6Al-4V 합금분말로 지름이 작은 일체형 (1-piece narrow-diameter) 임플란트를 SLS법으로 제작하여 16명의 환자에게 식립한 다음, 2년간 관찰하였고, Mangano 등 (2014)은 cone-beam computed tomography (CBCT) data를 3D이미지로 변환시켜 DLMS법으로 치근 형상의 임플란트를 제작하여 15명의 환자에게 식립한 다음, 1년간 관찰하였다. 또한 서울대학교 및 연세대학교 치과생체재료과학교실 (2016)에서는 3D 프린팅 법으로 제작한 티타늄 시편과 기계 가공한 티타늄 시편의 물성을 비교하였다. 그러나 티타늄 합금 분말을 이용하여 3D 프린팅 법으로 제작한 치과용 보철물을 실제 임상에 적용하는 단계에서 기존 기계가공 방식으로 제작한 티타늄 보철물과 3D 프린팅 법으로 제작한 티타늄 보철물의 물성과 표면특성을 다양하게 비교 평가하는 것이 필요하여 본 연구에서는 3D 프린팅 법으로 제작한 티타늄 시편과 기계 가공한 티타늄 시편의 물성특성과 표면특성을 비교하여 조사하였다.

Rehabilitation of posterior support and vertical dimension in a class 3 malocclusion patient: A case report (III급 부정교합 환자에서 상악 가철성 국소 의치와 하악 임플란트 지지 고정성 보철물을 이용한 구강 회복 증례)

  • A crossed occlusion resulting from the presence of posterior teeth in one arch but no opposing teeth in the opposite arch results in collapse of the vertical dimension. In this case, the patient has a class III malocclusion with crossed occlusion and anterior crossbite. In order to evaluate the proper vertical dimension, provisional denture was used to stabilize the vertical occlusal dimension for 3 months. After, provisional fixed restoration was used for the stabilizing occlusal relationship and aesthetic improvement for lip support. Definitive prosthesis in implants in the mandible and abutments in the maxillary were using Porcelain-fused-to-metal crown (PFM) crown and the maxillary unilateral edentulous area was treated with removable partial dentures. Through this, proper support of the posterior region and normal anterior occlusal relationship were formed, and the patient was able to obtain aesthetically and functionally satisfactory treatment results.

Mini-implant with additional retentive structure by using digital method (부가적인 유지구조를 가진 미니 임플란트의 디지털 수복 증례)

  • Mini-implant has been reported as an alternative method to conventional diameter implants in the treatment of mandibular anterior region with narrow ridge. However, one-piece type mini-implant showed biological and technical complications, such as peri-implantitis by residual cement and prosthetic detachment. Recently, one-piece type mini-implant with additional retentive structure has been introduced to increase the retention and stability of the mini-implant. The mini-implant is based on spreading the upper retentive structure with a spreader and connected it with the inner surface of the zirconia prosthesis. In two cases, we tried to reduce these complications through the mini-implant on the narrow mandibular anterior region, and it was possible to reduce the overall treatment period through a digital system. In this case report, restoration using the digital mini-implant system showed improvement of patient satisfaction on the aspect of function and esthetic in the narrow mandibular anterior region.

The incidence of the abutment screw loosening and its affecting factors in posterior implant restorations (구치부 임플란트 고정성 수복물에서의 지대주 나사 풀림 현상과 이에 영향을 미치는 요인)

  • Purpose: This study was to assess clinically the incidence of abutment screw loosening of posterior implant-supported fixed prosthesis and its affecting factors. Materials and methods: 391 implant-supported crowns restored from January 2013 to January 2016 were included in this study. All restorations were fabricated with either a single crowns or a splinted crown, and cemented with temporary cement. The incidence of abutment screw loosening is investigated and gender, restoration position, opposing teeth, restoration type, abutment connection type were assessed as possible factors affecting abutment screw loosening. Results: During the observation period (2 - 5 years), abutment screw loosening was found in 29 restorations (7.4%). It took 3 to 48 months (means 19.5 months) to loose the screw, and three of these implants were fractured. Among the factors considered, there were statistically significant differences at abutment screw loosening rate between molar group (9.4%) and premolar group (2.6%) (P<.019). According to the type of opposing teeth, there were statistically significant differences between nature teeth (74.7%) and implant (25.0%), removable denture (3%) (P<.019). The other possible factors did not have a significant effect on loosening of the abutment. Conclusion: The incidence of abutment screw loosening in posterior restoration was 7.4%. Abutment screw loosening were more likely to occur in molars group than premolar group, and according to the opposing teeth, there were the greatest frequency in nature teeth than implant and removal denture. There was a statistically significant difference.

Influence of varying cement types and abutment heights on pull-off force of zirconia restorations (시멘트의 종류 및 임플란트 지대주 높이가 지르코니아 수복물의 제거력에 미치는 영향)

  • Purpose: The purpose of this study is to evaluate Ti-base abutment's three different heights and three different cement types on the pull-off force of zirconia-based restorations. Materials and Methods: A total of 90 fixture lab analogs were embedded in auto polymerizing resin bloack. 90 Ti-base abutments heights of 3 mm, 5 mm, 7 mm were scanned and zirconia restoration were prepared from scanned files. Zirconia restoration were cemented with three different types of cements (temporary, semi-permanent, permanent) following manufacturer's instructions. All 90 specimens were placed and tested in a universal testing machine for pull-out testing. Retention was measured by recording the force at load drop. Statistical analysis was performed using Kruskal-Wallis test for detecting whether there are any statistical significance along cement types or abutment heights. After that, Mann-Whitney test was used for figuring out differences regarding abutment height and the comparison between 3 cements. Results: Temp bond showed significantly lower pull-off force compared to Fujicem regardless of any abutment height. However, there were significant differences between Cem-implant and Fujicem in abutment height of 3 mm and 7 mm, but there was no significant difference in 5 mm. Temp bond and Cem-implant had significant differences only in abutment height of 5 mm. Conclusion: Although Ti-base abutment height did not influenced zirconia restorations' retentiveness, cement types showed significant differences.

Oral rehabilitation of a patient with collapsed occlusal plane resulting from loss of posterior teeth (구치부 상실로 인해 교합 평면이 기울어진 환자에서 임플란트를 이용한 구강 회복 증례)

  • Restoring lost teeth is very important in terms of both function and aesthetics. If tooth loss occurs in the posterior region and the loss of support is persistent, it may cause a gradual shift in the position of the mandible and a change in occlusion. This clinical case attempted to restore support for the posterior teeth with a fixed prosthesis using implants in a patient whose opposing teeth were erupted and the occlusal plane collapsed due to long-term loss of the maxillary left posterior teeth and mandibular right first molars. To correct the occlusal plane of remaining dentition, wax-up of maxillary left posterior teeth was duplicated with acrylic resin and placed on maxilla. Surgical template for implant placement were fabricated using digital technology. After the support of the posterior teeth was restored with the placement of the implant, stable occlusion with temporary restorations was observed for a sufficient time. Afterwards, monolithic zirconia prosthesis was placed on the patient to ensure functional and aesthetic improvement.

  • The labio-palatal location of the implant in the maxillary anterior region is one of the important factors affecting the aesthetics of the implant prosthesis. However, the thin labial bone of maxilla could be absorbed in significant amounts after extraction of the teeth, which makes the implant be placed on the palatal side rather than the ideal location. In fact, in the cases of maxillary central incisor loss, UCLA was used for prosthetic restoration of palatally placed implant. In addition, with multidisciplinary treatment, GBR (Guided Bone Regeneration) was performed for compensating the absorbed alveolar bone and adjacent anterior tooth were aligned. Definitive restoration was performed after confirming aesthetic recovery of the gingiva with sufficient provisional restoration period. There were satisfactory results of functional and esthetic recovery of tooth loss through implant prosthesis.

  • Edentulous patients with a severely resorbed mandible or maxilla often experience problems with conventional dentures, such as insufficient stability and retention, together with a decrease in chewing ability. Because of the good prognosis of dental implants, these patients can be successfully treated with implant-retained or implant-supported prosthesis. Ideally, a maximum number of implants of maximum length are placed in appropriate surgically prepared sites that are surrounded by a maximum amount of bone of favorable quality. The implants are favorably aligned faciolingually and mesiodistally to enhance optimal prosthodontic design. This article describes the clinical problems and complications encountered when treating a consecutive number of edentulous patients with osseointegrated implant-supported prostheses.
