  • Kabuki syndrome is characterized by typical facial features (elongated palpebral fissures with eversion of the lateral third of the lower eyelid; arched and broad eyebrows; short columella with depressed nasal tip; large, prominent, or cupped ears), minor skeletal anomalies, persistence of fetal fingertip pads, mild to moderate intellectual disability, and postnatal growth deficiency. A 6-year-old male with kabuki syndrome was referred from the local dental clinic for left facial swelling and dental caries on all primary molars. He was treated for acute periapical abscess with incision and drainage under physical restraint, and left maxillary second primary molar was extracted. Other caries treatment was performed under general anesthesia. As the syndrome involves many different medical problems, special cares should be considered. Dental treatment should be carried out in comprehensive consultation system.

  • Orthodontic treatment is obviously the first choice to get aligned teeth cause of conservative treatment. But it is impossible to get highly esthetic result only orthodontic treatment in case of peg lateralis, discrepancy tooth size, discolored teeth or fractured teeth. As all treatment does, especially in esthetic treatment it is always required interdisciplinary diagnosis and treatment plan; orthodontics, periodontology, prothetics, implant therapy. As also, the treatment should be maintained long-term treatment with stable and harmonious state by esthetic satisfaction and functional occlusion those who needs various dental care as interdisciplinary treatment. This case is the laminate treatment with the orthodontic treatment on 37 ages/ Female patient who has cross bite at anteriors and premolar area and abrasion on centralis at right maxilla. By orthodontic treatment aligned teeth positions, it makes functional occlusion. As laminate treatment with minimum teeth preparation is able to release anterior esthetics. Follow up check for 2 years has been performed since the final prosthetics delivery to the patient. No sign of relapse, fracture of laminate were detected.

  • Protein C deficiency increases the risk of thrombosis due to the lack of anticoagulant factor protein C. Among the numerous congenital protein C deficiencies, homozygous protein C deficiency has an especially low protein C activity level, that it is almost undetectable. It is a rare disease with a probability of 1:250000~500000. The signs and symptoms of homozygous protein C deficiency include purpuric, necrotic dermatosis, ecchymosis, blindness, and thrombosis in central nervous system. A 4-year-old girl was brought to the clinic with a chief complaint of extensive caries. The child was under warfarin medication in order to prevent possible complications during dental treatment. We consulted the pediatric department. Without warfarin intake, serious complications may occur due to thrombosis during dental treatment. Therefore, certain warfarin dosage (INR 3~5) and fresh frozen plasma as a backup for excessive hemorrhage were recommended. This child was a severely disabled child with the loss of vision, and it was difficult to manage her behavior effectively. Thus, dental treatment was carried out under general anesthesia, where bleeding control would be also easier to achieve. This report presents the case of a 4-year-old girl with protein C deficiency, who has received dental treatment for extensive caries under general anesthesia.

  • Purpose: Process management is the activity which manages all procedure of construction by representing visually interrelation of operation or sequence setting. The purpose of this study was for reducing treatment period and higher efficiency of treatment through application of PERT/CPM (Program Evaluation & Review Technique/Critical Path Method) in dental clinic. Materials and methods: The patients were selected for study who needed more than 2 departments' cooperation for prosthodontic treatment in Wonkwang Dental University Hospital. Control group is composed of the patient's whole treatment plan, treatment period, numbers of hospital visit, treatment costs, treatment results. On the other hand, experiment group contains the patient's virtual treatment data based on PERT/CPM technique. We applied PERT/CPM in operation analysis. Results: Treatment period, numbers of hospital visit was decreased as 18.1% and 15.3% when we applied operation analysis based on charts. Also treatment cost in experiment group was 0.9% economized compared with control group's treatment cost. Conclusion: Application of PERT/CPM in dental clinic can achieve reliable treatment and reduced treatment period and establish plan of minimum treatment cost.

  • Mental retardation is defined by the American Psychiatric Association as" subnormal general intellectual functioning which originates during the developmental period and is associated with impairment of either learning and social adjustment or maturation, or both." A patient with mild to moderate mental retardation can be managed adequately using restraints and medications. However, in case of severe or profound mental disability, dental treatment cannot be accomplished even with the use of behavior modification, physical restraints and sedation techniques. When treatment in the dental office has much difficulty, hospitalization for dental treatment under general anesthesia can and should be considered. This case presents the treatment of a patient with mental retardation who was referred to our department for comprehensive dental care. Dental examination revealed widespread dental caries and a severe anterior open bite with crowding problems. Under general anesthesia, generalized caries treatment was performed by our department, and the anterior dental esthetics was achieved in collaboration with the department of prosthodontic dentistry.

  • Lesch-Nyhan syndrome is a rare X-linked recessive genetic disorder. During purine metabolism, the hypoxanthine guanine phosphribosyl transferase (HGPRT) enzyme is deficient causing phosphoribosyl transferase to accumulate and resulting in excessive uric acid. Clinical symptoms include hypercalcemia, choreoathetosis, spasticity, mental retardation, and self-injury to lips, tongue or fingers. This results not only in pain caused by the self-injury but also secondary infection of the wound site and the esthetic damage of the soft tissue defect. Dental treatments include conservative methods using intraoral appliances such as soft mouthguards, fixed lip bumpers, and occlusal guards, and invasive methods such as extraction of all teeth or forming an artificial anterior open bite. We report two cases of Lesch-Nyhan syndrome patients with self-mutilation behavior; one was treated with a preservative method using a soft mouthguard, and the other was treated with extraction of all teeth.

  • In the brain of epilepsy patients, sudden and unusually intense bursts of electrical charges are occurred repeatedly. So, epilepsy patient experiences seizure. Seizure shows various frequency and precipitating factor and has a prodrome. Also, epilepsy patient present various behavior pattern during seizure. If the biting injury occurred during seizure, there would be pain, infection, defect of soft tissue and esthetic problem. The dental management of self-mutilation includes preserve methods of using appliances such as lip bumper or soft mouth guard and radical methods such as extraction of all teeth or orthognathic surgery. A case of epilepsy patient with self-mutilation of mento-labial sulcus is presented. She was treated successfully with soft mouth guard.

  • Fibrous dysplasia(FD) is a benign fibro-ossifying disease in which fibrous tissue replaces normal bone and marrow. Craniofacial bones, including the maxilla and mandible, are commonly involved. A 7 year-old girl visited the clinic with a chief complaint of gingival swelling around the lower left primary molar. Mild bulging of the lower left periodontal tissue was observed. Not only the mandible, but also the maxilla, zygoma, sphenoid, and temporal bones were affected by FD. Permanent tooth germs were involved in the lesions and facial asymmetry was caused by lower left bone expansion. She was scheduled for a follow-up visit at the department of pediatric dentistry and oromaxillofacial surgery.

  • Teeth wear and extrusion of antagonist are commonly observed in deep bite patient having severe vertical and horizontal overlap. These problems cause collapse of occlusal plane and abnormal anterior guidance. Without restoring harmonious occlusion, loss of multiple teeth and decreased masticatory function could not be prevented. To resolve problems associated with deep bite, multidisciplinary treatment including oral surgical, orthodontic and prosthetic treatment should be performed. This clinical report describes the results of increasing occlusal vertical dimension with a full-mouth restoration procedure. The treatment procedures include extraoral and intraoral examination, diagnosis, treatment planning, diagnostic wax-up, segmental osteotomy, orthodontic intrusion and prosthodontic rehabilitation. Full mouth rehabilitation with increasing occlusal vertical dimension can solve esthetic and functional problems.
