외배엽 이형성증(ectodermal dysplasia)는 외배엽에서 분화되는 조직의 일부에 선천적인 이형성이 나타나는 유전적 질환으로 모낭 및 땀샘의 결여로 인한 성긴 모발과 눈썹, 눈과 입주위 피부의 착색 및 각화와 치아형성의 이상 등이 특징적으로 나타나는 질환이다. 구강내 소견으로 무치증 혹은 부분 무치증을 보이며 부분 무치증의 경우 치관이 원추형으로 나타나 결과적으로 전반적인 치아간극 및 수직고경의 감소로 저작 및 발음기능의 저하 뿐 아니라 심미적이지 못한 안모로 사회적 및 정서적 우울감에 빠지기 쉬우므로 조기치료가 요구된다. 바람직한 보철물 제작 및 안모의 개선을 위해서는 교정과 및 타 전문과와의 협진이 요구되며, 본 증례에서는 ectopic dysplasia로 진단된 10세 남아를 교정의사와 보철의사의 긴밀한 협진으로 저작 및 발음기능의 회복과 안면고경의 증가로 심미성이 개선된 좋은 결과를 얻었기에 보고하고자 한다.

다양한 치과 전문분야의 협진 치료가 필요한 환자에서는 심미적인 만족뿐 아니라 기능적이며 장기적 안정을 가져올 수 있는 치료가 전제되어야 한다. 이를 위해서는 분명한 치료목표와 그 목표를 이루기 위한 치료계획을 세우는 것이 선행되어야 한다. 환자와 술자 모두가 만족하는 치료결과를 갖기 위하여 어떠한 치료목표가 필요하며 그 목표를 위하여 치료계획을 세우는 방법과 그 치료과정에 대해 실제 임상증례를 통하여 각 과정을 설명하였다.

외상이나 선천적 결손에 의한 영구치아의 결손은 치료 방법과 적용 시기의 결정이 치료의 예후에 중요한 영향을 미친다. 본 증례는 성장기 환자로서 치아와 두개안면골 상태에 대한 정확한 진단 및 적절한 치료방법을 선택하고 성장 양식을 고려해서 일정기간 임시수복물과 임시 가철성 보철물을 사용하였으며, 성장 완료 후 임플란트 등을 이용한 고정성 보철수복 치료를 시행한 경우로써 교정과와 보철과의 협진 진료, 붕괴된 교합고경의 재확립 과정, 임플란트 보철치료 시 고려해야할 사항들에 대해 발표하고자 한다.

구강방사선스텐트는 구강암의 방사선치료 시 종양조직에 적정 선량을 유도하거나 주변 정상 조직에 대한 불필요한 방사선을 최소화하는 보조 장치이며 환자상황에 따라 개별적으로 적용되므로 효율적인 적용을 위한 관련 외과의, 방사선 치료전문의 그리고 치과전문의들간의 전략적인 협진이 필수적이다. 본 보고에서는 2종의 방사선스텐트(볼러스합입형, 설압형)를 다학제 협진하에 제작하였고 방사선치료 시 선택적으로 적용하여 그 효율성을 경험하였기에 증례와 함께 보고하는 바이다.

전신마취하에서의 치과치료는 자발적인 협조가 불가능하거나 전신적인 문제가 있는 경우, 다수의 치료를 한 번에 하고자 하는 경우 등에 행해진다. 이 연구는 서울대학교치과병원 장애인진료실에서 외래 전신마취하에 행해진 환자와 치과치료를 분석함으로써 보다 나은 장애인 진료 시스템을 위한 방안 제시에 도움이 되고자 하였다. 2002년 6월 11일부터 2006년 12월 31일까지 장애인진료실에서 치료 받은 432명의 환자들을 대상으로 조사한 결과 15세 이하의 환자가 50.46%로, 소아 및 청소년 환자가 높은 비율을 차지하고 있었다. 진료과는 보존과, 소아치과, 구강외과, 치주과, 보철과 순으로 나타났으며 치료내용을 보면 보존치료와 발치가 주를 이루고 있었다. 또 2회 이상 전신마취를 시행한 환자가 14.1%나 되었다. 본 연구는 장애인 환자를 위한 보다 포괄적인 치과 치료 및 진료과들 간의 긴밀한 협진 체계를 구축하고 필요한 인력과 설비를 보충함으로써 장애인 환자들에게 보다 보존적 인 치과치료와 편안한 치료 환경을 제공해야 할 필요성을 제시하고 있다.

성공적인 심미치과치료를 위해서는 종종 교정과, 치주과, 보철과의사의 협진치료가 필요하다. 상실된 치아를 수복하는 경우, 특히 심미가 중요시되는 전치부의 상실치 수복시 임상가는 여러가지 치료방법의 장단점을 고려하여 치료법을 선택해야 한다. 이 글에서는 선천결손된 상악측절치와 발치가 필요한 구치를 가진 환자를 치아교정과 임플란트 수술 그리고 임플란트 보철의 다학제간 진료로 치료한 증례를 보여드리고자 한다.

Many patients with mental retardation need extensive dental treatment because they have much difficulty in maintaining their oral hygiene. However, because they are not cooperative and not manageable, they require physical restraints, drug induced sedation or general anesthesia. General anesthesia is useful in control of the patients who cannot be treated in other ways. Additionally, general anesthesia provides more safe environment for medically compromised patients. And medical treatment can be provided simultaneously under general anesthesia. Furthermore, almost all treatment can be provided without visiting several times. This case reports of periodontal, restorative and ophthalmological treatment of patient with mental retardation under general anesthesia.

This clinical report describes an orthodontic-prosthodontic interdisciplinary treatment for a patient with multiple missing teeth and unilateral scissors bite. A 47-year-old female presented with multiple missing posterior teeth, anterior large overjet, deep bite, and posterior scissors bite on the right premolar area. Periodontal therapy was performed and followed by orthodontic treatment. The maxillary anterior teeth were initially aligned, then two implants were placed for the left mandibular molars to increase occlusal vertical dimension. The scissors bite between the right maxillary and mandibular premolars were corrected using the miniscrews as an anchorage. Other implants were placed for the right maxillary and mandibular molars after the occlusal planes and occlusal relationship were harmonized. The patient adapted well to altered vertical dimension without any specific problems including peri-implant marginal bone loss. Interdisciplinary approach resolve the complex orthodontic-prosthodontic problems and concluded in successful results.

Background: The number of patients with disabilities who require general anesthesia for dental treatment is increasing. Methods: We reviewed the cases of 538 patients with disabilities who underwent outpatient general anesthesia for dental treatment at the clinic for the disabled in Seoul national university dental hospital according to the types of disabilities and comprehensive treatments. Results: The patients mean age was 17.4 years and fifty-eight percent of the patients had epilepsy, autism, mental retardation and cerebral palsy. About twenty-five percent of the patients were medically compromised such as brain disease, heart disease and genetic problem. In 142 cases, more than two dentists from the clinical departments systemically co-worked to provide the patients with comprehensive treatment as accurately and quickly as possible under general anesthesia. Conclusions: The number of medically compromised patients was high and the system of comprehensive dental treatment was well run.

협조능력의 부족으로 치과 외래에서의 치과치료가 불가능한 소아 환자의 경우 약물을 이용한 진정법 또는 전신마취 하 치과치료를 고려하게 된다. 하지만 ASA III급 이상의 환자들을 대상으로 외래에서 약물을 이용한 진정법 하에서 치과치료를 진행하기에는 무리가 있으며, 전신마취 또한 엄격한 사전 선별과 관리 감독을 거쳐 진행해야 한다. 저자들은 지적 장애를 동반한 ASA III급에 속하는 환아의 치과 치료를 타 과와의 협진을 통하여 전신마취 하에서 성공적으로 시행하였기에 이를 보고하는 바이다.
