구강내에서 발생하는 연조직 질환중 궤양을 수반하여 환자로 하여금 통증을 호소하게 하는 질환으로는 급성 괴사성 궤양성 치간염 (Acute Necrotizing Ulcerative Gingivitis), 급성 수포성 구내염(Acute Herpetic Gingivo Stomatitis), 재발성 구순포진(Recurrent Herpes Labialis), 재발성 아프타성 구내염(Recurrent Aphthous Stomatitis), 대상성 포진(Herpes Zoster) 등이 있으며, 다른 질환보다도 그 발생빈도가 높아 임상에서 접하게 되며 그 치유속도가 늦어 가끔 당혹감을 느끼는 수가 있다. 이들 질환의 임상적 소견과 그 질환에 대한 치료법의 예를 간략하게 생각해 보고져 한다.

치주질환은 초기에는 환자 자신이 전혀 모르는 가운데 진행되어 환자가 증상을 느낄 때 쯤되면 이미 병변이 상당히 진행된 상태가 되어 치료하기 어려운 경우가 많게 되므로 치주질환의 원인과 질병상태를 조기에 파악하여 치료하는 것이 중요하다. 치주질환을 진단하는데 있어서는 객관적인 임상적 기준을 필요로 하게되며 치주질환의 활성도를 알아보기위해서는 여러 임상적 변수가 동시에 필요하므로 우선 환자의 병력을 파악하고 구강의 및 구강내 검사를 완전히 시행한 후 환자의 치아의 치주조직상태를 검사하게 된다. 이 장에서는치주질환의 진단에 필수적인 요인인 치아와 치주조직 검사에 대하여 중점적으로 애기 하고자 한다.

진단기술과 약물치료기법 및 신경학적 과정에 대한 이해가 발전됨으로 인하여 치과의사는 간질환자를 더 잘 이해하고 치료할 수 있게 되었다. 간질환자는 몇 가지 주의사항만 지킨다면 일반치과에서도 쉽게 치료할 수 있다. 즉, 의학적 병력과 함께 매 번 방문 시 마다 치과치료 전에 현재상태에 대한 평가가 필요하다. 기왕력 중 발작은 치료계획수립시 반드시 고려되어야 한다. 치과의사는 간질이나 경련성질환에 대하여 포괄적이며 통합적 이해를 통하여 환자의 구강건강뿐 아니라 나아가 전신건강을 증진하는 데 기여할 수 있다.

보철물이 제기능을 발휘하기 위해서는 건강한 치주조직이 필요할 분 아니라, 보철물 그 자체가 건강한 치주조직을 계속적으로 유지시켜 줄 수 있어야 한다. 보철물이 치주질환을 야기하거나, 치주질환을 가진 지대치에 아무런 처치없이 보철물을 장착한다면 결국 치아의 상실을 초래하게 된다. 이런 일로 인하여 외국에서는 치과이사와 환자가 소송에까지 이르게 되는 불행한 에가 많다. 이런 예는 먼 나라의 이야기가 아니라, 우리 나라에서도 문제가 되어가고 있는 것 같다. 필자의 조사(미 발표)에 의하면, 보철물이 치주조직에 미치는 영향에 관하여 재 조명이 있어야 될것으로 생각한다.

The moderners of industrial society suffer from various stresses, which bring about increase in prevalence of temporomandibular disorders, oral mucosal disease and chronic neuropathic pain, therefore, the number of patients seeking help of those symptoms tend to increase. The purpose of this study was to discuss the importance of oral medicine related disease by investigating questions that appeared in cyber consultation of Kangnung National University Dental Hospital web site. Among the nearly 2000 questions, the rate of questions related to oral medicine was 20.92%, and the rate of questions related to other departments were oral and maxillofacial surgery 16.87%, conservative dentistry 16.67%, orthodontics 14.02%, prosthodontics 12.25%, periodontics 8.36%, pediatric dentistry 4.93%, preventive dentistry 2.08% and otherwise questions 3.90%. Among the 403 questions related to oral medicine, the frequent questions were oral mucosal diseases 44.17%, temporomandibular disorders 41.19%, halitosis 4.47%, xerostomia 3.23%, other orofacial pain 2.23%, forensic dentistry 1.49% and otherwise questions 2.98%. From the higher rate of questions related to oral medicine compared with other fields of dentistry, we would consider that the people are considerably concerned about the oral medicine related disease, such as oral mucosal diseases and temporomandibular disorders.

악관절 질환중 개구장애, 관절통 관절잡음을 주증상으로 하는 악관절증은 급격히 증가하는 추세에 있으며 이에 대한 치료개념과 접근방법도 다양해지고 있다.
