외과적 치주낭 제거 술식에 관한 소고

  • 외과적 치주처치의 근본 목적은 치주낭을 제거함으로써 질환의 진행을 정지시키고 치주조직을 건강한 상태로 회복시켜 주는데 있다. 임상에서 적용할 수 있는 외과적 처치술식은 치주낭의 깊이, 부착치은의 폭경, 치조골의 상태, 염증의 진행정도 등의 사항을 고려하여 선택되어져야 하며, 대표적인 술식으로는 치은 절제술(gingivectomy), 치은 판막술(modified Widman flap, open flap curettage)등을 들 수 있다. 치은 절제술은 치은조직을 제거해야 한다는 의미에서 볼 때 부착치은의 넓이와 골내낭의 유무를 판단하여야 하며 치은판막술은 깊은 치주낭을 성성하고 있거나 골내낭이 있거나 부가적인 치조골의 처치가 필요한 경우 시행될 수 있다. 그러나 외과적 치주처치의 어떤 방법을 택하던지간에 계속적인 치태관리가 더 중요한 요인으로 강조되어야 한다.

The effects of nasal breathing due to pediatric allergic rhinitis on dentofacial growth and its dental considerations (소아 알레르기 비염으로 인한 구호흡이 악안면 성장에 미치는 영향과 이에 대한 치과적 고려사항)

  • Allergic rhinitis is a specific IgE mediated inflammatory disease of the nasal mucosa, characterized by symptoms such as rhinorrhea, nasal congestion, nasal obstruction, nasal and eye itching, and sneezing. The prevalence of allergic rhinitis varies according to country, age, and surveying methods, but it seems to increase worldwide, also in Korea. Prolonged mouth breathing caused by allergic rhinitis can produce muscular and postural alterations, causing alterations on the morphology, position, growth direction of the jaws, and malocclusion. Also, mouth breathing leads to dryness of the mouth, causing various oral diseases; gingivitis, halitosis, inflammation of tonsil, increased risk of dental caries and dental erosion. In dental clinic, using rapid maxillary expansion to persistent allergic rhinitis patients with narrow maxilla can enlarge maxillary dental arch and nasal cavity anatomy, improving nasal breathing and reducing nasal cavity resistance. However, it is desirable to use along with otolaryngologic treatment. Dentists should be aware of the characteristics of allergic rhinitis and its effects on patients, and consider when planning dental treatment.

  • The fundamental principles and the role of surgeons and orthodontists to produce successful results in orthodontic treatment combined with orthognathic surgery is not different from those of conventional procedures and FOS: surgery-first-orthodontic-treatment-later approach. The communication and cooperation between surgeon and orthodontist is of crucial importance. In FOS, the pre-surgical orthodontic preparation is not carried out in the patient's mouth, but in the mounted stone model and in addition to the simulation of tooth movement, to get a precise surgical occlusion, the entire steps of treatment should be simulated on the articulator as well. Right after the surgery, due to the instability of the occlusion, appropriate post operational care should be given according to the surgical technique applied to the mandible by use of final surgical wafer about 8 weeks.

  • The proper occlusal design is one of the most important factors for the long-term success. The purpose of this research is to investigate and define occlusal considerations to reduce failure of implant-supported fixed prostheses. The physiological movement of implants is markedly lower than that of natural teeth and they also lack in occlusal sensitivity. Proper occulsal pattern may be assigned to compensate for the biological disadvantages and occlusal contacts must be formed where the cantilever effect is minimized. Moreover, the long-term success can be assured by reducing early occulsal loading to avoid implant overloading and selecting appropriate occlusion material.

  • Occasionally, complete dentures constructed in accord with the best clinical and laboratory procedures fail to serve successfully. These failures probably result from present ignorance of the occlusion and functions of living tissues. Nonetheless, in each case, the whole treatment procedure, both clinical and laboratory, must be reexamined in order to ascertain the point of failure. One can employ a technique more intelligently, and often improve on it, if the reasons for its use are known. When one is guided only by a recipe, the outcome will be doubtful whenever conditions are not the same as in the recipe. In this manuscript, occlusal consideration of edentulous patients will be discussed thoroughly.

  • Many of the problems which are faced to the edentulous patients are related to a minimal amount of available mandibular bone volume and height. Most of the patients with mild atrophy of the mandible are treated using endosseous implant prosthodontics. TMI(Trans-mandibular Implant) can be used in case of severe mandibular atrophy, poor bone quality, atrophy of the mandible with exposed of inferior alveolar nerve, osteoporosis and the fracture of the atrophic mandible. Also it can eliminate the need for bone grafting and vestibuloplasty. The TMI is a rigid box frame structure which controls and distribute the masticatory force over the severely resorbed mandible. The box frame structure consist of a superstructure, baseplate, 4 transosseous posts, and 5 cortical screws. This is a case report that also describes about the transmandibular implant reconstruction system.

  • As the increased certainty of osseointegration, new parameters are now being used to assess implant success. Accordingly, patients' and clinicians' high demands and expectation for esthetics have expanded and implant-supported restorations show better esthetic outcomes. The pre-implant treatment planning process, the implant surgical steps and the post-surgery prosthetic process can affect all esthetic outcomes. Prevention of esthetic implant failures can be achieved by appropriate treatment at each stage, considering the 3 factors of alveolar bone, soft tissue, and implants. It is necessary to achieve the esthetic implant prostheses followings: minimal invasive surgery, bone augmentation, ideal 3-dimensional implant position, peri-implant soft tissue management, and provisional restorations to optimize peri-implant soft tissue architecture.

  • Anchorage in orthodontics is very important factor for orthodontist to treat malocclusion from diagnosis and treatment planning to end of treatment. Skeletal anchorage like miniscrew is supposed to be more effective method in anchorage control than conventional anchorage which needs patient's good cooperation. So this article will be mentioned about various clinical application of miniscrew through the general investigation and case reports about orthodontic use of miniscrew, specially about screwing area and clinical consideration of miniscrew's screwing on midpalate. The changes of treatment philosophy and methods by using skeletal anchorage were summarized and following results were obtained. 1. The orthodontic anchorage changed from relative concept to absolute one. 2. Bodily movement of teeth gets easier and determinate force system is possible on biomechanical consideration. 3. Some part of treatment that needs surgical intervention is possible by just orthodontic treatment.

Use of Ketamine Hydrochloride for Pediatric Dental Patient at General Hospital (응급실에서 소아외상환자의 치과적 처치를 위한 케타민 진정법의 사용 현황)

  • Children commonly experience orofacial injuries and often need emergency treatment. Due to fear and anxiety, children tend to be uncooperative in emergency rooms. Ketamine hydrochloride is a well-known sedative agent at medical-based emergency rooms which has been used for procedural sedation. In this paper, we will discuss the sedation of uncooperative young patients, who needed dental treatments in the emergency room at Wonju Severance Christian Hospital, using ketamine. We collected the records of patients under 18-years-old who visited the emergency room for dental treatment from January 2010 to May 2014. The data was categorized by age, sex, required dental treatments and application of ketamine sedation. Among 659 pediatric patients who visited for emergency dental treatments, 118 patients were treated under sedation using ketamine. Majority of patients were under the age of 6 (110 patients), and the most frequent cause of sedation was suture of oral laceration (105 patients). Though ketamine should not be used by dentists alone, dentists in emergency rooms can easily meet the patients under deep sedation using ketamine. Hence, dentists in emergency rooms need to be aware of the clinical effects, considerations, and potential adverse effects of ketamine.

Properties of translucent zirconia and lithium disilicate glass-ceramics: a literature review (반투명 지르코니아와 리튬디실리케이트 결정화유리의 물성에 관한 문헌고찰)

  • Although low translucency 3 mol% yttria stabilized tetragonal zirconia polycrystal has excellent mechanical properties, it has limited application as a monolithic prosthesis. To improve these optical limitations, translucent zirconia has improved esthetics due to an increase in the cubic phase; however, it is accompanied by a decrease in mechanical properties simultaneously. Lithium disilicate has improved its mechanical properties through crystal size reduction and various heat treatment methods; therefore, its clinical application range is continuously increasing. Translucent zirconia shows a wide distribution of physical properties depending on the yttria content and lithium disilicate according to the size and density of crystal grains. As a result, the indications for translucent zirconia and lithium disilicate are increasing. Therefore, in this literature review, we intend to examine the rationale behind the material selection criteria in clinical situations and considerations for designing fixed dental prostheses including pontic, in particular, by summarizing recent studies.
