Implant-assisted removable partial denture in a maxillary edentulous patient: A case report (상악 무치악 환자에서 전방부 임플란트 지지 고정성 보철물을 이용한 임플란트 보조 국소의치 수복 증례)

  • Implant-assisted removable partial denture (IAPRD) can be considered as a simple and cost-effective treatment approach for an edentulous patient with anatomical or financial limitations. Recently, it was reported that the application of IARPD with implant supported fixed prostheses covered by the National Health Insurance Service (NHIS) were increasing. This case report describes the treatment of maxillary fully edentulous patient with anterior four-implant-supported fixed prosthesis and distal extension IARPD. This treatment approach may be advantageous over maxillary implant overdentures in some circumstances. The patient was satisfied with improved function and esthetics in the anterior area and financial benefit from the NHIS. Further long-term clinical studies are needed to establish clinical validity of the treatment approach described in this case report.

  • Nowadays, dental implant is one of the widely used treatment options for edentulous patients. Recently, a method of improving the accuracy of implant surgery using an implant surgical guide has been introduced and widely used in order to accurately place the implant in a proper position. Full-guided and half-guided implant surgery can be distinguished according to the level of surgical guide application during the implant surgery. It is true that full-guided implant surgery exhibits higher accuracy, but half- guided implant surgery is often performed in a clinical situation due to the factors such as the circumstances of the operation. A partially edentulous patient who lost teeth due to tooth fracture and periodontal disease was treated using implant and fixed prosthesis. Half-guided implant surgery was performed using an implant surgical guide during implant surgery, the accuracy of implant placement was analyzed.

  • This paper is the first paper among two papers which constitute the paper about the structural design of a dental implant. This paper surveyed types and characteristics of dental implants. Merits and shortcomings of clinically used dental implants were analysed through this survey. A new structural model of dental implant was proposed on the basis of survey results. Especially, a comparative structural analysis of the new dental implant and the well known dental implants has been performed using the finite element analysis method. The analysis results confirm that the newly proposed dental implant has a good structural performance. The well known dental implants used for the comparative structural analysis are 3i implant and Sargon implant. These implants are imported dental implants and we could know that most implants clinically used are endosteal implants and these two implants(3i implant and Sargon implant) are also endosteal implants.

  • For fully edentulous patients, implant-supported overdenture can be considered to enhance chewing efficiency and denture stability. Implant planning software can be used to fabricate a surgical guide for a more precise consideration of anatomic factors and prediction of the shape of definitive prosthesis. Though there are many possible attachments for implant overdenture, milled bar can be useful due to its splinting effect of implants and rigid support of overdenture. This report presents a case of implant-supported milled bar overdenture after guided implant surgery performed with two surgical guide that was fabricated before and after bone reduction.

  • Interproximal contact loss (ICL) is the most frequent complication in partially edentulous patients treated with implant-supported prostheses. However, in most cases, ICL occurs between the implant prosthesis and the natural tooth due to the movement of the natural tooth. There are several causes of ICL that occur between an implant prosthesis and a natural tooth. There are only a few reported cases of ICL occurring between implant prostheses, and explaining the causes of ICL between implant prostheses and natural teeth is challenging. This clinical report describes cases of ICL between implant prostheses and discusses its causes and countermeasures.

  • Purpose: There is a growing concern for the cost management of medical institutions. The purpose of this study was to estimate Activity-Based Costing (ABC) for dental implant cost. ABC refers to allocating resources or cost based on the activities of services. Materials and methods: A dental institution located in the metropolitan area was selected in this study. The tax accounting data of the institution were utilized to confirm total cost, and the institution was asked to make out clinical activities to figure out what activities were carried out. The direct cost and indirect cost for dental implant were separately estimated, and cost driver was analyzed to estimate the indirect cost accurately. Results: The rates of the direct and indirect cost respectively stood at 35.8 and 49.5 percent. The cost for a dental implant was found to be approximately 1,579 won, and the cost of prosthetic surgery and treatment that included implant surgery accounted for the largest portion of the cost, which was 470 thousand won (30%). And the weight of training and education on dentistry was relatively higher than that of the other kinds of treatment. Conclusion: In order to ensure accurate and scientific costing for dental implant, not only direct medical procedure but every pre- and post-procedure activity should fully be taken into account. Pre-activities, post-activities, education and training are included in the indirect cost, but all these activities are mandatory and associated with the quality of treatment and the satisfaction level of patients.

  • Dental implant have been in use for a long time to restore at missing tooth. But, To place dental implant at good position very is difficult. Improperly positioned dental implants make some problems to have a good function of dental implant. so, To place dental implant at accurate position is the most important step throughout the whole process. Digital guided system of dental implant is very useful to have a accurate position of dental implant and it makes upper restoration more esthetic and funcional.

  • Implant placement may be restricted by anatomical and/or financial limitations in restoring a completely edentulous arch, or the patients' unwillingness to have extensive surgical procedures. Implant assisted removable partial dentures (IARPD) in combination with anterior fixed implant prostheses can be proposed as an alternative treatment option for the restoration of a completely edentulous arch. In this case, a 56-year-old female patient who has a fully edentulous maxilla opposed by partially edentulous mandible was treated. The treatment option for the maxilla consisted of an implant-assisted removable partial denture supported by four anterior fixed implant prostheses. The mandible was restored with 8 anterior fixed partial dentures and posterior fixed implant-supported prostheses. Long-term follow-up and supportive periodontal treatment were performed for 9 years, and the patient was satisfied with the overall appearance as well as masticatory function.

Full mouth rehabilitation in a patient with peri-implantitis: A case report (Peri-implantitis 환자에서의 전악 재수복 증례)

  • Peri-implantitis appears in almost 20% of patients who received implant treatment, and increase in its number is inevitable as time goes by. Although it can be treated by both non-surgical and surgical procedures, in cases which include severe bone loss, explantation and rehabilitation may be necessary. Careful treatment planning and considerations to prevent recurrent peri-implantitis should be taken into account. In the following case presented, a patient with chronic periodontitis and peri-implantitis was successfully rehabilitated after removal of several implants. Extraction and explantation of multiple teeth and implants were followed by full mouth reconstruction with fixed implant prostheses on the mandible and implant retained overdenture on the maxilla. Surgical and prosthetic measures to prevent recurrent peri-implantitis were taken into consideration.

Rehabilitation of a patient by converting poorly maintained fixed implant prosthesis into implant overdenture: a case report (기존 임플란트 보철물의 불량한 유지관리를 개선하기 위해 임플란트 피개의치로의 상부 보철물 전환 증례)

  • Among many contributing factors of implant failure, excessive loading and inflammation contribute mainly to increasing implant failure rate. Especially, poor oral hygiene around implants is one of the most common reasons for implant failure. When implant prosthesis fails to function, the clinician should re-diagnose and plan treatment using the remaining implants. Additional implantation may be needed to make a new type of prosthesis or either implant-assisted RPD and implant overdenture treatment may be performed using the remaining implants. In this article, a patient whose fixed implant prosthesis in the maxilla had failed of poor oral hygiene maintenance was reported to be recovered with removable implant overdenture by using remaining implant abutments and a milled bar with additional retentive elements. Through this treatment, the clinical results were satisfactory in terms of esthetics, masticatory function recovery and oral hygiene management.
