복합레진을 이용한 전치부의 수복

  • 치과용 복합레진은 bis GMA형의 monomer가 개발되고, 무기 filler가 첨가되어 물리적인 성질이 더욱 향상되었으며, 또한 법랑질에 대한 산부식법을 통하여 치아에 대한 결합력을 높을 수 있는 방법이 도입되면서 치과용 수복물로서 각광을 받아 왔다. 특히 복합레진은 이전까지 이용되었던 silicate cement 이나 acrylic resin에 비하여 변색이 적고, 원래의 형태를 비교적 잘 유지하는 장점을 가져서 전치부의 수복에 유용한 재료로 인식되었다. 복합레진을 이용한 전치부의 수복은 비교적 적은 치질의 삭제만으로도 가능하고, 심미적으로도 우수한 결과를 보이고 있어서 올바른 술식을 통하여 적절히 이용한다면 좋은 임상적인 결과를 얻을 수 있는 것이 사실이지만 재료학적인 한계가 아직 까지는 엄연히 존재한다. 복합레진을 이용한 전치부의 수복에 있어서, 복합레진의 문제점과 이를 줄이기 위한 방법, 임상시의 주의점 등에 관해 살펴보도록 한다.

Behaviors-Based Safety Management of Dental Professionals on Dental Amalgam and Resin (치과용 아말감과 레진에 대한 치과종사자의 안전성에 대한 관리행동 실태)

  • This study investigated the safety management behavior of dental professionals to minimize exposure to harmful elements of amalgam and resin. As for the use of amalgam and resin, 85.8% of them used amalgam. 100% of used composite resin 100%. 97.8% used acryl resin, and 95.7% used tray resin. 95.2% and 36.5% of them were aware of the harm of amalgam and resin respectively. When using resin, their safety management behavior score was 1.99 on average, and the lowest score was 2.50 on average for 'ventilate the indoor air when handling resin'. Their average score of the safety management behavior was 1.83 when using amalgam. The lowest score was 2.89 on average for 'use the rubber dam for patients when handling amalgam'. As a result of the safety management behavior based on general characteristics, all characteristics were significant with regard to amalgam, and my position at work and work experience were significant with regard to resin (p<0.05). The correlation between the safety management behavior and general characteristics was analyzed when dental amalgam and resin are used. Amalgam was statistically correlated with academic background (p<0.05) and position at work (p<0.05), whereas resin was statistically significantly correlated with age (p<0.05) and position at work (p<0.05). Accordingly, education about the harm of dental materials must be continuously provided for dental professionals when they handle dental materials so that safety management behavior will be conducted correctly, and active efforts to protect the health of dental professionals from harmful chemicals must be made.

Polymerization Shrinkage Distribution of a Dental Composite during Dental Restoration Observed by Digital Image Correlation Method (디지털 이미지 상관법을 이용한 치과용 복합레진의 수복 시 중합수축분포 관찰)

  • The shrinkage distribution of a dental composite (Clearfil AP-X, Kuraray, Japan) used for dental restoration was observed using a digital image correlation method. In order to analyze the shrinkage distribution formed during and after light irradiation, digital images were taken with different photographing conditions for each period. Optimal photographing conditions during LED irradiation were obtained through a preliminary experiment in which the exposure time was applied from 0.15 ms to 0.55 ms in 0.05 ms intervals. The DIC analysis results showed that the strain was non-uniform. For the initial 20 s of light irradiation the composite resin shrank to the level of 50~60% of the final curing shrinkage. Such large shrinkage amount of the composite resin lump affected the tensile stress concentration near the adhesive region between the composite resin and the substrate.

  • 치과용 복합레진으로 사용하기 위해서 새로운 다관능성 메타크릴레이트 유도체를 합성하였으며, 이 유도체와 기존의 Bis-GMA를 혼합하여 새로운 복합레진을 만들었다. 제조된 복합레진의 물리적 물성 및 생체친화성은 기존의 Bis-GMA control보다도 더 우수하였다.

In Vitro Study on the Bond Strength Between 3D-Printed Resin and Resin Cement for Pediatric Crown Restoration (소아용 크라운 수복을 위한 3D 프린팅 레진과 레진 시멘트 간의 접착 강도 평가)

  • A 3D-printed resin crown is a novel option for esthetic crown restoration for primary teeth, which are typically bonded with resin cement. The purpose of this study was to evaluate the bonding ability of a 3D printing resin and compare it with other indirect resin materials for crown fabrication. The shear bond strengths of two 3D printing resin materials, Graphy (GP) and NextDent (NXT), and two indirect resin materials, VIPI Block (VIPI) and MAZIC Duro (MZ), were compared in the study. For all materials, the shear bond strength at the interface between the surface of the resin material and resin cement was measured. The mean shear bond strength values of GP, NXT, MZ, and VIPI were 23.29 ± 3.88, 26.14 ± 4.67, 25.41 ± 4.03, and 18.79 ± 4.26 MPa, respectively. There was no significant difference among the SBSs of GP, NXT and MZ except for VIPI. The result of this study indicates that the 3D printing resin meets the essential requirement for clinical use by showing clinically adequate bond strength.

Evaluation of Surface Condition and Food Solution on the Color Stability of Dental Restoration Materials (표면 상태와 용액에 따른 치과용 수복재료의 색안정도 평가)

  • For this study, specimens were prepared using five types of composite resin and resin-reinforced glass ionomer Fuji II LC, and 29 days precipitation was done in four kinds of solution, which were soy sauce, gochujang, wine, and distilled water. Ten specimens for each coloring sample were divided into two groups, one those surfaces was polyester film and the other with 1200-grit abrasive paper, and 20 specimens per materials. Color change (${\Delta}{\ast}ab$) in the colored specimens is measured in 2days, 7days, 14days and 29days using spectrophotometer (CM-2600d, Konica, Minolta, Ramsey, NJ). All values were considered significant when P<0.05. 1. Each material is discolored over time under the influence of different storage solutions. 2. In soy source, among the specimens polished with polyester film, color change was observed in resin-modified glass ionomer Fuji II LC, prodigy and compomer F-2000 on day 2. Meanwhile, Fuji II LC showed noticeable color change in specimens prepared with 1200-grit sandpaper on day 2, which was followed by flowable resin on day 7. 3. On day 2, there was a color change with the specimens of flowable resin, resin-modified glass ionomer Fuji II LC and Prodigy among the hybrid specimen in the groups of polyester film in gochujang.

Surface roughness and surface free energy components of various orthodontic adhesives (다양한 교정용 접착제의 표면거칠기와 표면에너지 요소 분석)

  • Objective: Surface characteristics of dental materials play an important role in bacterial adhesion. The purpose of this study was to investigate surface characteristics of 5 different light-cured orthodontic adhesives (1 fluoride-releasing composite, 3 non-fluoride-releasing composites, and f resin-modified glass ionomer). Methods: Surface roughness was measured using a confocal laser scanning microscope. Contact angle and surface free energy components were analyzed using the sessile drop method. Results: Surface roughness was significantly different between adhesives despite a relatively small variation (less than $0.05\;{\mu}m$). Lightbond and Monolok2 were rougher than Enlight and Transbond XT. There were also significant differences in contact angles and surface free energy components between adhesives. In particular, considerable differences in contact angles and surface free energy components were found between resin modified glass ionomer and the composites. Resin modified glass ionomer showed significantly smaller contact angles in 3 different probe liquids and had higher total surface free energy and stronger polarity, with notably stronger basic property than the composites. Conclusion: Resin modified glass ionomer may provide a more favourable environment for bacterial adhesion than composite adhesives.
