DECORONATION ON ANKYLOSED PERMANENT INCISOR AFTER DENTAL TRAUMA (외상으로 유착된 영구 전치에서의 치관 절제술)

  • Trauma commonly leads to ankylosis for the tooth whose periodontal ligaments have been injured. In growing patients, an ankylosed tooth can disrupt alveolar development, resulting in infraocclusion of the affected tooth. Consequently, this causes aesthetic problems during adolescence, interferes with prosthetic treatment due to the inclination of adjacent teeth, and complicates orthodontic dental movement. When the infraoccluded ankylosed tooth is extracted, a considerable amount of alveolar bone is lost, especially in the maxillary anterior region. Moreover, depression of the thin buccal alveolar bone compromises aesthetic restoration. In order to prevent alveolar bone loss, the ankylosed tooth should be treated by decoronation. In the present study, the traumatically injured maxillary incisors of 9-year-old and 10-year-old patients that present infraocclusion accompanied by ankylosis were treated with decoronation procedures. Decoronation procedures were performed when the affected teeth were 2-3 mm below relatively to the adjacent teeth. Moreover, the patients were treated before they get to 16 years old considering the maximum growth peak of the korean adolescents. As the results, there are favorable clinical results associated not only with preservation of horizontal alveolar volume but also with increase in vertical alveolar height after decoronation in growing individuals.

The Effect of Resin Base Surface Treatment on Shear Bond Strength in Indirect Bracket Bonding Technique (브라켓 간접부착술식시 레진베이스의 표면처리가 전단결합강도에 미치는 영향)

  • The purpose of this study was to evaluate the effects of the surface treatments of resin bases in indirect bracket bonding technique by study of shear bond strengths and failure patterns. Ninety metal brackets were bonded to the stone models of specimens involving bovine lower incisor with light-cured adhesive(Light-Bond). After removal of brackets with the resin base from the stone models, the surfaces of resin bases in thirty brackets were treated with Plastic Conditioner and the surfaces of resin bases in another thirty brackets were treated with sandblaster and the remaining thirty brackets were served as controls. All brackets were transferred to the specimens and bonded using sealant. The shear bond strength was tested on universal testing machine, and failure pattern was assessed with the adhesive remnant index(ARI). The results were as follows: 1. Surface treatments of resin bases with Plastic Conditioner or sandblasting showed statistically higher shear bond strengths than no treatment group. 2. No significant difference in shear bond strength was found between Plastic Conditioner treatment and sandblasting treatment groups. 3. No significant difference in ARI scores was found among the three groups. 4. As the result of correlation analysis between shear bond strengths and hnl scores, failure at adhesive/bracket base interface tends to increase when the shear bond strength was high, but it was not significant statistically. The above results suggest that improvement of bond strength can be obtained by surface treatment of resin base in the indirect bonding technique.

The Effect of A Static Magnetic Field on The Bone Nodule Formation of MC3T3-E1 Cells (정자기장이 MC3T3-E1 세포의 골결절형성에 미치는 영향)

  • To evaluate the effect of a static magnetic field on the bone producing potential of MC3T3-E1 cells, the alkaline phosphatase activity was measured after the cells having been cultured under 76.4mT static magnetic field using a $SmCo_5$ magnets for 5days, 7days, 11days, 15days and 21days for each cell culture group. Also, the amount of bone nodule stained with Alizarin red S was observed. The results were as follows . The alkaline phosphatase activity of the 7, 11, and 15 days group among the experimental groups was decreased as compared with the control groups, and the decrease of alkaline phosphatase activity in the 11 days group was the most evident among them. . Any stained bone nodules of both groups had not been observed until the 11th day. The stained bone nodules in the control groups were found on the 15th day, but not in the experimental groups. The stained bone nodules were observed in both groups on the 21st day, but the control groups have more bone nodules than the experimental groups.

Analysis of the hereditary factor in craniofacial morphology and fingerprints in Class III malocclusion (III급 부정교합에서 두개안면 형태와 지문의 유전성향 분석)

  • In growing patients with Class III malocclusion, the various patterns of maxillofacial growth are a key element that affects the success or failure of treatment. Therefore it is important to correctly predict maxillofacial growth before initiating treatment. The purpose of this study was to find out the correlation between the maxillofacial morphology of parents and their Class III children by analyzing lateral cephalograms and hereditary factors. Among Class III preadolescent children, 50 families were obtained. To find out the specific hereditary factors involved, fingerprints were obtained and genetic correlation with the maxillofacial morphology was analyzed. The following conclusions were made. 1. A significant correlation (P<0.05-0.00l) was found in many of the cephalometric measurements between the offspring and their parents. The correlation in the skeleton measurements was higher than in the denture measurements. The father-offspring correlation was higher than the mother-offspring correlation 2. A significant correlation (P<0.05-0.00l) was found in fingerprint units between the offspring and their parents. The mother-offspring correlation was higher than the father-offspring correlation. 3. Between the maxillofacial morphology and fingerprint units, there was significant genetic correlation (P<0.05-0.01). Based on the analysis of genetic correlation, higher correlation was found in the parent-son pairing than the parent-daughter pairing.

An analysis of the dental arch and skeletal characteristics in adult patients exhibiting open bite (Openbite을 나타내는 성인의 치열 특성 및 그에 따른 골격적 특성 분석)

  • It is the purpose of this study to characterize oral symptoms and to comprehend the cause and the relapse possibility of patients with open bite. This case study examines the orthodontic treatment of a group of female patients with open bite and Angle's Class I malocclusion. A cephalograph of the patient was taken and tracing of the radiograph was completed. In addition to Bjork and Ricketts analysis, additional measurements of specific areas were taken. The occlusal plane was determined by drawing a line connecting the mesiobuccal cusp tip of the maxillary first molar and the incisal edge of the maxillary central incisors. Patients were divided into two groups depending on the relationship between the marginal ridge of the maxillayy first premolar and the drawn line. Those patients with marginal ridges above the occlusal plane were placed into Group 1, while Group 2 subjects exhibited marginal ridges lower than the occlusal plane. The common characteristics within each group and the characteristic differences between each group both prior to and after orthodontic treatment were examined, and finally, the functional oral volume of each patient was analyzed. The results of the case study were as follows: 1. An examination of the skeletal relationship and anatomical form for both Group 1 and 2 showed that all subjects exhibited hyperdivergent skeletal forms, but Group 2 subjects generally demonstrated underdevelopment of the mandible and a smaller articular angle, resulting in an anterior positioning tendency of the mandible. 2. An analysis of the maxillary arches of Group 1 subjects prior to and after orthodontic treatment showed that the antero-inferior direction had changed to an antero-superior directional tendency, while the maxillary arches of the Group 2 patients showed a trend from an antero-superior direction to an antero-inferior relationship. The mandibular arches in both groups showed a change to an antero-superior direction. 3. Functional space analysis showed that Group 2 patients exhibited a greater tendency of haying palatal planes that drop in a postero-inferior direction, resulting in a more severe open bite than their Group 1 counterparts. The results of this case study show that although patients belonging to either Group 1 or 2 exhibited few external differences in the appearance of open bite, an examination of the dental and skeletal relationships by analyzing patient cephalographs showed that patients presenting with flat maxillary occlusal planes exhibited more severe open bite relationships than patients with curved occlusal planes.

Histologic changes of pulpal tissue after laser-aided ceramic bracket debonding (레이저를 이용한 도재 브라켓 제거 술식 후 치수의 조직학적 변화)

  • Laser-aided debonding has advantages in that the heat produced is localized and controlled, the debonding tool is not heated, and it can be used for the removal of various types of ceramic brackets, regardless of their design. However, the range of safe power usage for laser-aided debonding has not vet been confirmed. The Purpose of this study was to evaluate the histologic changes of pulpal tissue in a rabbit's incisor after Nd-YAG laser-aided ceramic bracket debonding at different levels of power. The result were as follows: 1. At 3-5W Nd-YAG laser power level and 3 seconds of exposure time, the ceramic bracket debonding procedure was not easy. At 5W of power a tie-wing fracture occurred on one bracket during debonding using Weingart plier. The histologic section of pulp represented no adverse changes. 2. At 7-13 W power level and less than 5 seconds of exposure time, the debracketing procedure was done easily and bracket facture did not occur. The histologic section of pulp represented mild and reversible changes. All the results were reversible and no pulpal degeneration or necrosis occurred. Considering the results, it appears that the laser-aided debonding technique is a safe method that does not result in irreversible pulpal changes, softens bracket bonding resin within a saie range of power and exposure time, and is useful for ceramic bracket recycling by lowering the tie- wing fracture rate.

The effect of indomethacin on the matrix metalloproteinases in canine permanent tooth eruption (인도메타신의 투여가 치아 맹출 시 기질금속단백분해 효소의 분포에 미치는 영향)

  • Tooth eruption requires remodeling of surrounding tissues. This study was aimed to investigate the effect of indomethacin on the dental follicle and paradental tissues during tooth eruption by observing the distribution and expression of MMP by the immunohistochemical method. Ten mongrel dogs of ten to twelve weeks old were divided into 5 groups; four experimental groups administered indomethacin 2 mg/Kg/day and 8 mg/Kg/day orally 2 times a day for 14 days and 7 days respectively, and the control group was administered a placebo. Permanent teeth before eruption and their surrounding tissues were selected and excised. H&E staining and immunohistochemical stainings of MMP-3 and -9 were performed and examined under the light microscope. Osteoclasts, osteoblasts, periodontal ligament cells, ameloblasts and odontoblasts of the control group all expressed MMP-3 and -9. In the experimental group, osteoclasts, osteoblasts and periodontal ligament cells showed reduced expression of MMP-3 and -9. Magnitude of MMP reduction In the experimental group showed a time and dose of indomethacin administration dependent manner. These results show that indomethacin inhibited MMP-3 and -9 expression in the dental follicle and surrounding tissues and suggest that when indomethacin is administered for long periods, tooth eruption could be delayed.

The validity of transcranial radiography in diagnosis of internal derangement (악관절 내장증 평가 시 경두개 방사선사진의 임상적 유용성: MRI와의 비교연구)

  • The purpose of this study was to determine whether association exists between temporomandibular joint (TMJ) characteristics in transcranial radiographs and TMJ internal derangement and to evaluate the validity of transcranial radiographs in diagnosis of internal derangement. Transcranial radiographs and magnetic resonance imaging (MRI) of 113 TMJs from 76 subjects were used for this study and all TMJs were classified into 3 groups according to the results of MRI: normal disk position, disk displacement with reduction, and disk displacement without reduction. Transcranial analysis included linear measurement of joint spaces and condylar head angle measurement. To determine any relationship between transcranial measurements according to disk displacement, one-way ANOVA was used. The results showed that condyle-fossa relationship in standard transcranial radiographs had no relationships with disk displacement. And, as disk displacement progressed, condylar angle between head and neck increased significantly. This result can be interpreted that condylar head angle reflects structural hard tissue change according to internal derangement progress. But this is insufficient in the determination of internal derangement. Therefore, although still clinically helpful, the validity of standard transcranial radiographs to diagnose TMJ internal derangement was questioned.

A cephalometric study on the velopharyngeal changes after maxillary protraction (상악골 전방견인치료후 구개범인두 변화에 대한 단기간의 측모두부방사선 계측학적 연구)

  • The purpose of this study was to investigate cephalometrically the short term static velopharyngeal changes in 25 patients (10 boys and 15 girls, aged from 5 years 9 months to 12 years 10 months in the beginning of treatment) with skeletal Class III malocclusions who underwent nonsurgical maxillary protraction therapy with a facemask. The linear, angular and ratio measurements were made on lateral cephalograms. Only the change in hard palatal plane angle was negatively correlated with the change in maxillary depth or N-perp to A (p<0.01). The change in velar angle showed a statistically significant increase (p<0.001). This change was influenced more by the soft palatal plane angle than by the hard palatal plane angle (p<0.001). The changes in soft tissue nasopharyngeal depth and hard tissue nasopharyngeal depth showed statistically significant increases (p<0.001). Correlations between the changes in soft tissue (or hard tissue) nasopharyngeal depth and the change in soft palatal plane angle were significant (p<0.05). The increase in hard palate length was statistically significant (p<0.001). The change in hard palate length was negatively correlated with the change in soft tissue nasopharyngeal depth (p<0.05). The change in need ratio S (C) showed a statistically significant increase (p<0.001). But this difference was within the normal range reported by previous studies. These findings indicate that the velopharyngeal competence was maintained even if the anatomical condition of the static velopharyngeal area were changed after maxillary protraction.

The Effect of Surface Treat`ment on Bond Strength of Polycarbonate Bracket (폴리카보네이트 브라켓 부착 시 결합강도 증진을 위한 표면처리 효과)

  • Objective: The purpose of this study was to evaluate whether the bond strength of polycarbonate brackets can be increased through surface treatment. Methods: One hundred polycarbonate brackets (Alice) were bonded to bovine incisors with light-cured adhesive. The bracket bases were treated with one of three methods; sandblasting, plastic conditioner application, and combined treatment with sandblasting and plastic conditioner. The brackets without any suraace treatment served as the control. The shear bond strength was tested with a universal testing machine, and failure pattern was assessed with the adhesive remnant index. Results: The shear bond strength in all experimental groups was higher than that of the control group (p < 0.001). The group treated with plastic conditioner after sandblasting showed statistically higher shear bond strength than the sandblasting only group to (p < 0.05). The group treated with plastic conditioner after sandblasting showed higher shear bond strength than plastic conditioner only group, but the difference was not statistically significant. Conclusion: The above results suggest that the surface treatments of polycarbonate bracket is mandatory to improve bond strength, and the most effective method is an application of plastic conditioner after sandblasting.
