  • 악관절 잡음과 동통,과두 흡수를 동반한 퇴행성 측두하악장애는 교합 불안정과 개구장애 를 동반하기도 한다. 진단을 위해 CT나 MRI를 이용해 과두 형태 및 디스크 위치를 파악하 는 것이 유용한 접근법이다. 퇴행성 측두하악관절 환자는 CT나 MRI를 이용하여 진단하고, 과두-원판 재위치와 근 기능 개선을 위해 장기간 교합 안정장치 사용이 필요하므로 교정치료 기간에 변형된 교합 안정장치의 병용이 필요하다. 이에 본 연구에서 교합 안정장치를 병용하여 교합 재구성 증례를 CT나 MRI로 고찰해 보고자 한다.

  • The coronal approach has been used for over a century by neurosurgeons to access to the anterior cranium. Indications for the coronal approach expanded from use in the correction of congenital skeletal anomalies to applications in acute maxillofacial trauma and secondary deformity correction, oncologic surgery and reconstruction, and esthetic surgery. Complications were such as injury to frontal branch of the facial nerve, motor nerve paralysis, hematoma under flap, trismus, ptosis, epiphora, infection and anterior temporal depression. $Medpor^{(R)}$ is made up of dense polyethylene connected in porous structures. It is easily shapable without collapsing the pores due to it's hardness and tissue growth takes place at the porosities. Based on these advantages, $Medpor^{(R)}$ has been used in augmentation and restoration in craniofacial defect. A temporal depression after the coronal approach for treatment of Le Fort III fracture was successfully reconstruction with $Medpor^{(R)}$ and we report this case with review of literature.

Associations Among Different Types of Quantitative Pain Measures in TMD Patients (측두하악장애환자에서 다양한 종류의 정량적 통각검사들의 연관성에 관한 연구)

  • The aims of this study were to investigate the relationships among several types of thermal pain thresholds, and pressure pain thresholds. This study was designed to examine whether there were associations among different types of pain thresholds, and among different recording sites for each pain threshold measurement. Pain sensitivity thresholds including cold pain threshold (CPT), heat pain threshold (HPT), heat pain tolerance threshold (PTT), and pressure pain threshold (PPT) of 56 subjects with symptoms of temporomandibular disorders were measured on temporal muscle, masseter muscle, TMJ, and tibial areas. Thermal pain thresholds including CPT, HPT, and PTT did not show any gender differences. However, women showed significantly lower PPTs than men on all recording sites. Three thermal pain thresholds including CPT, HPT, and PTT showed weak to high correlations on all the recording sites (r= 0.324 to 0.754, p<0.05). PPTs did not show any significant correlations between each thermal pain threshold. The pain threshold of each recording site showed weak to high correlations in all pain threshold measures (r= 0.284 to 0.878, p<0.05). Our study demonstrated that thermal pain thresholds, and pain tolerance thresholds were significantly correlated, but did not show any correlation between thermal pain thresholds and pressure pain thresholds. There were relatively high correlations among the pain thresholds of different recording sites.

A study on Pre-and Post-surgical Patterns of Mandibular Movement and EMG in Skeletal Class III Prognathic Patients who underwent Intraoral Vertical Ramus Osteotomy (하악 전돌증 환자의 구내 하악골 상행지 골절단술전후의 하악골 운동양상 및 저작근 근전도 변화에 관한 연구)

  • Stomatognathic system is a complex one that is composed of TMJ, neuromuscular system, teeth and connective tissue, and all its components are doing their parts to maintain their physiological relationships. Mandible, in particular, performs various functions such as mastication, speech, and deglutition, the muscular activities that determine such functions are signalled by numerous types of proprioceptors that exist in periodontal membrane, TMJ, and muscles to be controlled by complicated pathways and mechanics of peripheral and central nervous system. Orthodontic treatment, especially when accompanied by orthognathic surgery, brings dramatic changes of stornatognat is system such as intraoral proprioceptors and muscle activities and thus, changes in patterns of mandibular function result The author tried to analyze changes in patterns of mandibular movement and physiologic activities of surrounding muscles in Skeletal Class III ortlrognathic surgery patients who presently show a great increase in numbers. The purpose of this study was to draw some objective guidelines in evaluating funclierual aspects of orthognathic surgery patients. Mandibular functional analysis using Biopak was performed for skeletal Class III prognathic patients who underwent IVRO(lntraoral Vertical Ramus Osteotmy), and the following results were obtained: 1. Resting EMG was greater in pre-surgical group than the control group, and it showed gradual decrease after the surgery. Clenching EMG of masseter and anterior temporalis of pre-surgical group was smaller than those of control group, they also increased post-surgically, and significant difference was found between pre-surgical and post-surgical(6 months) groups. 2. Resting EMG of anterior ternporalis was greater than that of all the other muscles, but there was no significant difference. Clenching EMG of anterior temporalis and masseter were greater than those of the other muscles with statistical difference. In swallowing, digastric muscle showed the highest EMG with statistical significance. 3. Limited range of mandibular movement was shown in pre-surgical group. Significant increase in maximum mouth opening was observed six months post-surgically, and significant increase in protrusive movement was observed three months post-surgically.

Prevalence of Temporomandibular Joint Dysfunction and Malocclusion in 10 - 12 year Old Children (10-12세 아동의 측두하악장애와 부정교합의 유병율에 관한 연구)

  • The purpose of this study was to evaluate the prevalence of symptoms and signs of temporomandibular joint dysfunction (TMD) and malocclusion in 10-12 year old children and to determine if a relationship exists between symptoms and signs of TMD and malocclusion. The subjects were composed of 465 school children (233 boys and 232 girls). Each subject was evaluated with simplified questionnaire and clinical examination to measure symptoms and signs of TMD and malocclusion. The results showed an elevated prevalence of headache (34.6%), which were more frequent in girls than boys. The most common cardinal sign of TMD was clicking (32.9%), which increased with age. TMJ lateral tenderness was present in 18.1% of the subjects and had a tendency to increase with age. Masseter muscle tenderness was found to be sensitive in 15.1%of the subjects and had a tendency to increase with age. Restriction of mandibular mobilitywas present in 30.3% of the subjects and had a tendency to increase with age. There was no significant difference in the prevalence of TMD signs between sex. The occlusal status showed Class I malocclusion in 73.3%, Class III in 12.9%,Class II division 1 in 11%, Class II division 2 in 2.8%. There was no significant difference in malocclusion traits between sex. There was statistically significant relationship between TMD signs and symptoms and class II division 1 and Class III malocclusion(p<0.05). The results indicate that the prevalence of TMD symptoms and signs in children is high, and the evaluation of TMD in children seems to be important.

Occlusal Analysis in the Policemen with Temporomandibular Disorders Using T-scan II System (경찰 종사자의 측두하악장애환자에서 T-scan II System을 이용한 교합분석)

  • This study suggested correction of excessive mouth opening or maximum occlusal contact to analyse occlusal contact time, occlusal contact number and force through evaluation of occlusal pattern in policemen with temporomandibular disorders. The community of policemen influence on temporomandibular disorder's development and progress due to other condition of mouth opening and maximal occlusal contact. Repeated training or changes of usual life style may cause imbalance of stomatognathic system including the masticatory muscle, then develop or aggravate pain of temporomandibular joints and associated structures. This study uses T-scan II system(Tekscan Co., USA) for evaluation on occlusal pattern may influence temporomandibular disorders, and then the subjects take a sensor at 20 mm opening for maximal occlusal contact force. The policemen with temporomandibualr disorders get more long time on maximum contact timing, more short on end contact timing, and more force on end contact force than general society's. So they get closure of mouth with more short time and more force, then transfer remaining load to temporomandibular joint. There are no statistically significances between affected side and occlusal pattern of occlusal contact time and force. There are Left -right dental arch imbalances seems on Rt. dental arch if affected side is right and Lt. dental arch if affected side is left. In above results, It's worth due consideration that policemen with temporomandibular disorders get more smooth mandibualr movement and less force on maximal occlusal contact position.

The Spiral Taping Treatment on Temporomandibular Disorder in oral Medicine (구강내과영역에서 측두하악장애 환자의 Spiral Taping 치료)

  • The purpose of this study is introduce the spiral taping treatment on Temporomandibular Disorder in oral Medicine. The taping treatment is relatively simple and dosen't have any adverse effect, so it has high stability and superior effect of treatment. In this study, using the spiral taping treatment as one of the effective taping treatments, non-stretched tape was attatched to the muscles which set limit to the range of joint movement and cause pain to temporomandibular joint. With that treatment this study tried to make effective results of treatment of temporomandibular disorder. These results suggest spiral taping treatments contribute to the improvement of tempermandibular disorders. Further this study is needed for the confirmation of this effect of spiral taping treatments on temporomandibular disorders.

Clinical Effect of Low Level Laser Therapy on the Trigger Points of Orofacial Pain Patient (구강안면동통 환자의 발통점에 대한 저출력 레이저치료의 임상적 효과에 대한 연구)

  • 구강안면동통환자의 발통점에 대한 보존적 치료방법중 저출력레이저의 효과를 평가하기 위해 교근, 측두근과 승모근에 발통점을 가진 치과대학생 69명을 무작위로 분류하여 37명에게는 GaAlAs 반도체 레이저를 조사하였고 나머지 32명은 레이저를 실제로 조사하지 않고 대조군으로 삼았다. 50mW, 820nm의 GaAlAs 반도체 레이저를 이용하여, 4주 동안 첫 주는 2회, 이후 3주 동안 1회씩 총 5회 조사하였고 전자통각계를 이용하여 압력통각역치를 측정한 후 이를 대조군과 비교한 바 다음과 같은 결과를 얻었다. 1. 남녀 및 조사군과 비조사군의 치료 전 압력통각역치는 차이가 없었다. 2. 조사군의 각 근육에서 측정한 압력통각역치는 레이저 치료 2주 후부터 유의하게 높아졌으나 비조사군에서는 차이가 없었다, 3. 비조사군의 치료 전, 후 압력통각역치에는 성 차가 없었다. 반면 종사군의 압력통각역치는 치료 전에는 성 차가 없었으나 치료 후에는 남성이 여성보다 유의하게 높았다.

EPITHELIAL-MYOEPITHELIAL CARCINOMA ARISING IN PLEOMORPHIC ADENOMA OF PALATE (다형성 선종에서 발생한 구개부의 상피성-근상피암종)

  • A case of epithelial-myoepithelial carcinoma transformed in pleomorphic adenoma occurring in palate of a 61 years old woman is reported. The tum or was composed of 2 different components, pleomorphic adenoma and epithelial myoepithelial carcinoma, accounting for approximately 40% and 60% of whole tumor, respectively. As the results of the immunohistopathologic study, epithelial-myoepithelial carcinoma showed multiple tubular or solid nest, which were separated by a basement membrane and considered of variable proportion of 2 cell types, cuboidal epithelial cells positive for cytokeratin and clear myoepithelial cells positive for glial fibrillary acid protein, wheres the myoepithelial nest of pleomorphic adenoma intermingled with hyaline and myxoid stroma. The malignancy was demonstrated by convincing evidence of invasion into the submucosa, although the epithelial-myoepithelial carcinoma component was mostly surrounded by the pleomorphic adenoma componemts. An increased immunoreactivity of proliferating cell nuclear antign in the epithelial myoepithelial carcinoma area in comparison to the pleomorphic adenoma also suggested epithelial-myoepithelial carcinoma arising in a pleomorphic adenoma.

Short-term Effect of Botulinum Toxin Injection on the Surface EMG of Masticatory Muscles and Jaw Function (교근 부위의 보툴리눔 독소 주사가 저작근의 근전도 및 악기능에 미치는 영향)

  • Botulinum toxin injection has been used in the masticatory muscle area as an effective treatment method of various movement disorders and facial contouring, but its effects on jaw function have not been evaluated. The aims of this study were to evaluate the effects of botulinum toxin type A injection into the masseter muscle on the EMG activities of masseter and anterior temporal muscles, and the limitation of jaw function. Fourteen healthy subjects were recruited. Five subjects were injected with 80 units of botulinum toxin type A(Dysport, Ipsen, Wrexham, UK) into each side of masseter muscle, and nine subjects were injected with saline into the same site as the botulinum toxin group. The surface EMG activities at maximum voluntary contraction of masseter and anterior temporal muscles were recorded before, 1 week, 2 weeks, and 3 weeks after injection. Presence of jaw functional limitations in each subject was investigated using Korean version of Jaw Functional Limitation Scale(JFLS) questionnaire. The masseter muscle EMG was gradually decreased in the botulinum toxin group comparing with that of the control group(p<0.001), but the anterior temporal muscle EMG did not show significant changes. There was significant increases in the mastication (p<0.01), and global jaw limitation(p<0.05) subscales of JFLS at 1 week after injection, but no significant changes in the other subscales including opening, and verbal and emotional expression during the recording periods. Our results suggest that botulinum toxin injection into masseter muscle can affect modest limitation in mastication function at 1 week after injection but recovered to the baseline until 3 weeks after injection. The EMG activity of masseter muscle had been gradually decreased until 3 weeks after botulinum toxin injection but the anterior temporal muscle did not show any significant changes.
