Short Segment Pedicle Screw Fixation with Augmented Intra-Operative Vertebroplasty in Unstable Thoraco-Lumbar Fracture - Preliminary Report - (불안정성 흉·요추부 골절에 대한 단 분절 척추경 나사못 고정술 및 추체 보강 성형술 - 예 비 보 고 -)

  • Objectives : Since vertebroplasty has been introduced, we performed short segment pedicle screw fixation with augmented intra-operative vertebroplasty in patients with unstable thoraco-lumbar fracture. Our intentions are to demonstrate the efficacy and indication of this new technique compare to conventional methods. Material and Methods : The surgery comprised of pedicle screw fixations on one level above and below the fracture site, and the fractured level itself, if pedicle is intact, and intra-operative vertebroplasty under the fluoroscopic guide with in-situ postero-lateral bone graft. Also, in cases of bone apposition, we removed those with small impactor through a transfascetal route. During the last 2 years, we performed in seven(7) unstable thoraco-lumbar fracture patients who consisted of two different characteristics, those four(4) with primary or secondary osteoporosis and three(3) of young and very healthy. All patients were followed clinically by A.S.I.A. score and radiography. Results : Mean follow up period was 14 months. We observed well decompressed state via transfascetal route in cases of bone fragments apposition and no hardware pullout in osteoporotic cases, no poly-methyl-methacrylate (PMMA) leakage through the fracture sites into the spinal canal, and no kyphotic deformities in both cases during follow-up periods. All patients demonstrated solid bony fusion except one following osteoporotic compression fracture on other sites. Conclusions : In the management of unstable thoraco-lumbar fracture, we believe that this short segment pedicle screw fixation with augmented intra-operative vertebroplasty reduce the total length or levels of pedicle screw fixation without post-operative kyphotic deformity.

Ultrasonography of the Kidney and Urinary Bladder in Male Korean Native Goat (웅성 한국재래산양에서 신장 및 방광의 초음파검사법)

  • The purpose of this study was to make fundamental ultrasonographic observations of kidney and urinary bladder in Korean native goat. The position, dimensions, and structure of the kidneys and bladders In 7 male Korean native goats were determinded by use of ultrasonography. A 3.5%.MHz linear transducer was used. All examinations were performed on goats in lateral recumbency under sedation. The left kidney was 4.2 to 5.9 cm long, 2.5 to 3.6 cm wide, and 2.4 to 3.2 cm deep. Diameter of the parenchyma and renal sinus of the left kidney ranged between 0.7 and 1.3 cm and 0.7 and 1.4 cm, respectively. Circumferences of the medullary Pyramids varied between 1.3 and 1.9 cm. Similar ultrasonic measurements were obtained for the right kidney. The diameter of the urinary bladder varied between 2.1 and 5.4 cm in the goats. It was concluded that the ultrasonographic findings described in this study can be used as references for diagnosis of morphologic changes in the kidney and urinary bladder of Korean native goat.

Posterior Transvertebral Extension Osteotomy - A Case Report - (후방 추체 경유 신전 절골술 - 증 례 보 고 -)

  • Objective : Flat back syndrome constitutes a syndrome complex characterized by the loss of normal lumbar lordosis. Various techniques of correction for flat back syndrome have been reported. Posterior extension osteotomy has certain drawbacks. Forceful hyperextension of the spine may result in vascular complications such as rupture of the aorta or the inferior vena cava and stretching of superior mesenteric artery, and pseudoarthrosis. We describe a rationale and technique of transvertebral posterior extension osteotomy to avoid complications of posterior extension osteotomy and to achieve an correction of 30 degrees of flat back syndrome. Method : A 63-year-old woman with degenerative lumbar kyphosis presented with low back pain, thigh pain, knee pain and walking difficulty. Transpedicular fixation from L1 vertebra to S1 vertebra was accomplished for lumbar degenerative kyphosis. After 6 months, the patient presented with flat back syndrome. A second operation was performed with transvertebral posterior extension osteotomy. Result : With short segemental fusion, early bone fusion and correction of 30 degrees were achieved. Conclusion : Transvertebral posterior extension osteotomy provide an 30-60 degrees of correction of flat back syndrome. This technique is considered to be good method for the revision of lumbar degenerative kyphosis.

The description of the Flat fish (Pleuronectiformes) Fossils from the Miocene Duho Formation, Pohang Yeonam-dong in Korea and its Implication (포항시 여남동 마이오세 두호층에서 산출된 가자미목 화석의 기재와 의의)

  • Four entities of the Cenozoic fish fossils were discovered in the Miocene Duho Formation, Pohang, Korea. these fossils were identified as the first Pleuronectiformes in Korea based on the following-the presence of postcleithrum, the elongation of the first proximal pterygiophore of the anal fin ray, almost consistent presence of two proximal pterygiophore of the anal fin rays between the two adjacent hemal spines, the fusion of the first and second hypurals, the fusion of the third and fourth hypurals and the first preural centrum, the presence of well-developed anteriormost plate-like neural spine, the presence of the urohyal like fish-hook and its elongated sciatic part, and the division of the parahypural from the first preural centrum. On the other hand, geological studies about the Duho Formation consistently claimed that shallow-sea creatures were washed away by meteorological events such as a great flood and deposited at the bottom of deep-sea by the turbidity current. However, in Duho Formation, only shallow-sea ones have been discovered thus far. This study reported that Flat fish, deep-sea creature, was discovered in Duho Formation for the first time in Korea.

DECISION OF PROPER PROJECTION ANGLE AND EVALUATION OF REPRODUCIBILITY IN TRANSCRANIAL RADIOGRAPH (악관절규격촬영시 적절한 입사각의 결정 및 재현성의 평가)

  • The purpose of this study was to decide the proper projection angle that could avoid the superimposition of the petrous portion of temporal bone and posterior clinoid process in cephalometric lateral and basal view of the 10 dry skulls and to analyze the reproducibility of a modified Accurad-100 head holder device from Denar Co. and a cephalostat head holder in 30 transcranial radiographs of the dry skull. The results were obtained as follows: The vertical angle was 24.5±4.1° and the horizontal angle was 3.0 ±5.7°. The distances between arbitrary 3 points which were on the TMJ area were as follows: AB 16.76±0.27㎜, BC 8.79±0.18㎜ in the transcranial radiographs using modified Accurad-100 head holder, AB 29.03±0.14㎜, AC 19.95±0.24㎜, BC 10.08±0.07㎜ in the transcranial radiographs using cephalostat.

  • The characteristics of cranium, vertebral column and fin supports of Acanthorhodeus gracilis were examined. The fish did not have a fontanell in cranium nor process of supraethmoid. It had double orbitosphenoids and a basioccipital process developed from anterior portion of basioccipital. It had a high and triangular dorsal process on supraoccipital. In the urohyal, hypohyal attachment was bifurcated and horizontal and vertical plate were in an elongated rhombus shape with posterior edge pointed. It had teeth of 1-rowed, 5/5 and 4 free teeth. The fish did not have a coracoid foramen on shoulder girdle. It does not have a uroneural on ural centrum. It had a long and slender posterior process of pelvic bone. It had a large and flat supraneural. The fish had 14 or 15 interneural spines, of which the 1st, 2nd, 3rd and 4th had single basiosts and the rest had double basiosts. It had 11 or 12 interhemal spines, of which the 1st, 2nd, 3rd and 4th had single basiosts and the rest had double basiosts.

A Case Report of a Schizophreniform Disorder Treated by Oriental Medical Treatment and Emotional Freedom Techniques. (양약 복용 후 추체외로증후군을 나타낸 정신분열형 장애 환자에게 EFT 요법과 한방치료를 병행하여 치료한 치험 1례)

  • Objectives : Schizophreniform Disorder can be put as pre-stage of Schizophrenia, which is known as one of the most common mental disorder. Many studies have shown that Antipsychotic Treatment for Schizophrenia has many side effects such as EPS(Extrapyramidal Symptoms), and recently it has been found that even Non-Antipsychotic Treatment has side effects such as weight gain. This clinical study was aimed to search the therapeutic effects of Oriental medicine in Schizophreniform Disorder, and in reducing the side effects of Western medicine. Methods : We treated the patient diagnosed as Schizophreniform Disorder, whose chief complaint was auditory hallucination, with herbal medicine and acupuncture. Improvement in her clinical symptoms were recorded daily. We also used Emotional Freedom Techniques to control her anxiety effectively. Results : Auditory hallucination and extrapyramidal symptoms such as tremor disappeared. The patient's anxiety was controlled by Emotional Freedom Techniques effectively. Conclusions : From the above results, we conclude that Oriental medical treatment is effective in treating clinical symptoms of Schizophreniform Disorder, as well as in reducing the side effects of Western medicine.

Diagnostic Radiology and Conservative Management of L1 Lumbar Spine with Compression Fracture (L1 요추 압박골절에 대한 진단방사선학 및 보존적 치료)

  • Diagnostic radiology and conservative management for S75 patient with L1 lumbar fracture by traffic accidents were discussed with references, and then the obtained results were as follows ; 1. Wedging compression fractures with 10% deformity was confirmed at anterior vertebral body of L1 lumbar spine through lateral plain X-ray film. 2. Irregular bony fractures were observed at anterior vertebral body of L1 lumbar spine by CT scans, anatomically T12-L1 sites showed highly frequency of injuries, Denis's fracture type was classified as multiple compression fracture at anterior column without abnormal middle and posterior column, also no Cobb's angle, and then Frankel's neurological classification was E grade. 3. Orthopaedic treatments were performed with conservative methods. With rest on the bed, anti-in-flammatory medication, electrolyte and nutritional solution, the pain diminished. 4. After 3 weeks, rehabilitation was worked with putting on polyethylene back corset, although pains remained slightly until after 8 weeks, thereafter the spine showed gradually stability.

Development of SW convergence Education program for the patriotism applying Novel Engineering (Novel Engineering을 적용한 SW융합형 호국보훈 수업 방안)

  • The purpose of this study is to describe the SW Convergence education class applying the Novel Engineering teaching method during the creative experiential activity time. We developed a SW convergence education program for a patriotism class applying Novel Engineering, a new educational method instead of writing and drawing. Through this lesson program, students can improve their problem-solving ability based on their computational thinking and enhance their love spirit about country. Students read a book that tells the story of patriot, writes the historical scene presented in the book through scenarios through Re-enactment, and implements the simulation situation using Lego WeDo 2.0. The results of this study based on the analysis of the data on the questionnaire showed that there is a significant students' change through the SW convergence education program for patriotism class. In the future, we will expand the scope to more various grades and subjects so that we can generalize the effectiveness of SW convergence education applying Novel Engineering.

  • The involvement in the spine of eosinophilic granuloma is not common. Especially, involvement of cervical spine and posterior neural arch is quite rare. In addition, radiographic findings including magnetic resonance images of eosinophilic granuloma are manifested as vertebral body collapse, loss of pedicle and paravertebral soft mass formation; it must be differentiated with other conditions, such as malignant bone tumor, metastatic cancer or tuberculous spondylitis. We experienced three cases of eosinophilic granuloma involving posterior neural arch of C4, C7, and body of C4 respectively, which were performed surgical treatment and achieved complete healing. We report these cases with review of literatures.
