Calcific Tendinitis Occurred within Conjoined Tendon in a Patient with Rheumatoid Arthritis - A Case Report - (류마티스 관절염 환자에서 발생한 연합건의 석회성 건염 - 증례보고 -)

  • Calcific tendinitis usually occurs within the area where tendon inserts to bone. In the shoulder joints, most cases occur within the supraspinatus tendon just proximal to the greater tuberosity. We report a rare case of calcific tendinitis occurred within the conjoined tendon of coracobrachialis and short head of biceps brachii in a patient with rheumatoid arthritis, diagnosed by means of ultrasonographic evaluation.

  • Late rupture of the extensor pollicis longus (EPL) tendon is an occasional complication following a distal radius fracture. Early rupture of the EPL tendon as a complication of distal radius fracture is rare. We report one case of early rupture of the EPL tendon after distal radius fracture.

Ultrasonographic Findings of the Shoulder in Asymptomatic High School Overhead Athletes (무증상 고교 상지 거상 운동 선수의 견관절 초음파 소견)

  • Purpose: To evaluate the ultrasonographic (US) findings in both shoulders of asymptomatic high school overhead athletes and compare with healthy control. Materials and Methods: 33 individuals (age: $17.5{\pm}1.4$ years) participated in this study. We recruited two groups of high school male athletes, 13 baseball and 10 volleyball players without shoulder pain for recent 6 months as well as one control group of 10 untrained healthy high school students who were age-matched. Ultrasound measurements of thickness of biceps and supraspinatus tendon and acromio-humral distance (AHD) at 0o abduction were taken in dominant and nondominant shoulders. Results: On US examination, subacromial bursa effusion was observed in 5 of the dominant shoulders and in 1 of the nondominant shoulder of 23 overhead athletes and in none of the asymptomatic controls. The thickness of the biceps and supraspinatus tendons and AHD in both shoulders were significantly greater in the athletes than in the controls (P<0.05). We also found that the thickness of the biceps and supraspinatus tendon and AHD of the dominant shoulder were significantly greater than the non-dominant shoulder in overhead athletes (P<0.05), but there were no differences between baseball and volleyball players. Conclusion: Subacromial effusion could be found even in the asymptomatic stage overhead athlete and the thickness of the biceps and supraspinatus tendons and AHD were significantly greater in the overhead athletes than in the healthy controls. Our data is useful reference guide for the ultrasonographic diagnosis of shoulder pathologies occurring in overhead athletes.

  • Simple radiography, CT and MRI were used for the diagnosis of the hip disease. However ultrasonography could be a good diagnostic tool for the lesion of tendons and bursae. Another advantage of ultrasonography is its usefulness for the follow-up of these lesions. Nowadays hip ultrasonography expanded the horizon to the follow-up study of total hip arthroplasty. However, the limitation of ultrasonography is that there is disparity in results according to the physician's experiences. In this article ultrasonographic findings of normal structures and pathologic lesions of the hip joint are introduced and explained briefly.

  • Rupture of biceps brchii is seen mainly in tendinous portion and intrasubstance rupture is rare. Author experienced 2 cases of intrasubstance rupture of biceps brachii diagnosed by sonographic examination which showed satisfactory result by conservative treatment.

최근의 초음파에 의한 비파괴 시험

  • 비파괴검사방법에는 여러가지가 있는데 과거에는 주로 방사건 유도시험이 주역을 이루어 왔다고 볼수 있다. 그런데 최근에는 초음파를 이용한 탐상장치의 성능이 향상하였고 초음파를 이용하지 않으면 검사할수 없는 대상물의 검사가 필요하게 되었다는 점에 의하여 초음파의 이용이 상당히 급속하게 확대되어 그 응용범위도 수도관에서 원자연료까지 이루게 되었다. 그러나 방사선 투과식 시험에서는 검출곤란한 균열을 검출할 수 있는 기술적 특성및 고가인 소모품이 필요치 않고 또한 검사속도도 빠르다는 경제성에 주목하여 방사투과시험이 가능한 대상물에 대해서도 이용하게끔 되었다. 초음파에 의한 비파괴시험을 이용목적에 따라 분류하면 탐상과 계측으로 대별할 수 있다. 이하 이들의 각종방법에 대하여 순차적으로 그 개요를 설명하겠다.

  • Calcific tendinitis is characterized by inflammation around calcium hydroxyapatite crystal deposits. Minimally invasive extracorporeal shock wave therapy (ESWT) has been postulated to be an effective treatment option for treating calcific tendinitis. In clinical practice, shock waves usually are aimed at the painful area after palpation and not focused. It has been known that exact fluoroscopic focusing of ESWT at the calcific deposit for treatment of calcifying tendinopathy is highly effective. Ultrasound is a simple, inexpensive and radiation-free diagnostic tool that has been used to demonstrate tendinopathy including calcific tendinitis. However, focusing of shock wave under ultrasound is less well established. We present a patient in whom large calcific tendinitis of gluteus medius was completely resolved by exact focusing of ESWT by ultrasound with literature review.

Materials and Methods: 42 individuals (age: $7.6{\pm}1.2$ years) participated in this study. We recruited two groups of high school male baseball players, 14 pitchers and 18 fielders without shoulder pain for recent 6 months as well as one control group of 10 untrained healthy high school students who were age-matched. Ultrasound measurements of thickness of the biceps and supraspinatus tendon and acromio-humral distance (AHD) at $0^{\circ}$ abduction were taken in dominant and nondominant shoulders. Results: On US examination, subacromial bursa effusion was observed in 7 of the dominant shoulders and in 2 of the nondominant shoulder of 32 baseball players and in none of the asymptomatic controls. The thickness of the biceps and supraspinatus tendons and AHD in both shoulders were significantly greater in the baseball players than in the controls (p<0.05). We also found that the biceps and supraspinatus tendon thickness and AHD of the dominant shoulder were significantly greater than the non-dominant shoulder in baseball players (p<0.05), but there were no differences between pitchers and fielders and between healthy controls. Conclusion: Subacromial effusion could be found even in the asymptomatic stage and the thickness of the biceps and supraspinatus tendons and AHD were significantly greater in the baseball players than in the healthy controls. Our data is useful reference guide for the ultrasonographic diagnosis of shoulder pathologies occurring in high school baseball players.

  • There are many traumatic foot and ankle problems in orthopaedic fields. Though it is not life-threatening problems, a delay in accurate diagnosis and treatments can danger limb function and therefore correct diagnosis can prevent long-term complications. Achilles tendon rupture is relatively common injury for active sports people. Ultrasonography is cost-effective, irradiation -free, effective for evaluation of soft tissues and dynamic analysis. It has been growing importance in Achilles tendon rupture. Ultrasonography is a diagnostic tool in Achilles tendon rupture. Physical examination and patient history is needed to diagnose Achilles tendon without image, but it is missed up by 20% in private clinic. Discontinuity of normal fibrillar architecture seen on an ultrasonographic image is diagnostic for Achilles tendon rupture, and can be accentuated by the performance of dorsi-flexion and plantar flexion, while observing in real time. And ultrasonography is a reliable method for serial observation after surgical treatment or conservative treatment.
