The change of sleep pattern is one of the most often altered normal physiological functions in elderly people. Besides normal change of sleep, insomnia and sleep apnea syndrome (SAS) are (one of) the main complaints. In addition, parasomnia is also frequent in this age group. Several parasomnias frequently found in the elderly are reviewed. Periodic limb movements in sleep (PLMS), restless legs syndrome (RLS), and REM sleep behavior disorder are the most frequent parasomnias in old age. Most parasomnias could be diagnosed by polysomnography, and be treated easily. Therefore, early and precise diagnosis and management for parasomnia in aging people are needed.
Objectives: Periodic leg movements in sleep (PLMS) might be subdivided based upon whether or not they are associated with visible EEG microarousals (MA). MA is considered to be responsible for nonrestorative sleep and daytime fatigue. The American Sleep Disorders Association's (ASDA) scoring rules for MA based on visual analysis of the EEG changes suggest that MA should last more than 3 seconds. However, it has been suggested that visual analysis may not detect some changes in EEG activity. This study is aimed at measuring changes in EEG spectra during PLMS without MA in order to better understand the arousing response of PLMS. Methods: Ten drug-free patients (three men and seven women) diagnosed with PLMS by polysomnography were studied. Spectral analysis of the EEG was performed in each patient on 30 episodes of PLMS without MA, chosen randomly across the night in stage 2 non-REM sleep. We applied stricter criteria for MA compared to ASDA, by defining it as a return to alpha and theta frequency lasting at least 1 second. Results: The mean PLMS index was $16.7{\pm}10.0$. The mean PLMS duration was $1.3{\pm}0.7$ seconds. Comparison of 4-second EEG activity both before and after the onset of PLMS without MA using independent t-test showed that the movements were associated with significant increase of relative activity in the delta band (p=0.000) and significant decrease of activity in the alpha (p=0.01) and sigma (p=0.000) bands. No significant decrease in the theta (p=0.05), beta (p=0.129), or gamma (p=0.062) bands was found. Conclusion: PLMS without MA was found to be associated with EEG change characterized by increase in the delta frequency band. This finding seems to be compatible with the hypothesis of an integrative hierarchy of arousal responses of Sforza's. Considering that the subjects had lower PLMS index and shorter PLMS duration than those of the previous study, it is suggested that an even less severe form of PLMS without MA could induce neurophysiologic change, which may potentially be of clinical significance.

본 연구에서는, 수면환경의 열적 쾌적도의 측정방법으로서 생리적 반응 뿐만 아닌 국소자극의 반응에 대한 평가법 (Allesthesial Response)에 의 한 가능성을 제시 하고자 하였다. 피험자는 19세에서 22세의 건강한 독일 여자 대학생 5명이며, 실험은 12월과 1월 독일의 KASSEL에 있는 Marburg대학 연구소의 인공기후실에서 이루어졌다. 사용의복은 면 $100\%$의 잠옷이며, 침구는 메트리스와 Wool 담요(두께 180 mm)를 사용하였다. 국소자극 반응의 온도는 $20.0^{\circ}C,\;22.5^{\circ}C,\;25.0^{\circ}C,\;27.5^{\circ}C,\;30.0^{\circ}C,\;32.5^{\circ}C$의 set가 사용되었으며, 온도자극은 Pottier Thermode type PKE 36 HO2-1 (독일, Peltroil사)로서, 온도의 도달정 밀도는 60내지 90초 동안에 각 자극온도의 변화조절이 가능하였다. 수면환경 온도는 $15^{\circ}C,\;18^{\circ}C,\;21^{\circ}C,\;24^{\circ}C,\;27^{\circ}C$의 다섯 환경으로 조절하였으며, 습도는 RH $45\%$였다. 수면환경 $18^{\circ}C$에서 $24^{\circ}C$까지에서는, 수면전, 수면후 모두, 피험자는 약간의 Hypothermia의 경향을 보였지만 Neutral Situation과 큰 차이는 나타나지 않았다. 수면전과 수면후의 체온조절 반응의 차이가 Allesthesial Response와 국소의 쾌적한 온도 선택의 두 실험결과 모두에서 현저히 나타났다. 생리적 반응의 결과에서도 $18^{\circ}C$에서 $21^{\circ}C$까지의 수면환경이 가장 쾌적하게 나타났다. 또한, 실험결과에서 행동적 온도 조절 반응이 생리적 반응에 앞서 보다 민감하게 이루어짐을 볼 수 있었다

It is a short transition period from wakeful consciousness to sleep. The lack of prominent sleep events characterizing stage 1 sleep is a major obstacle in automatic sleep stage scoring. In this study, utilization of simultaneous EEG and EOG processing and analyses to detect stage 1 sleep automatically were attempted. Methods: Relative powers of the alpha waves and the theta waves were calculated from spectral estimation. A relative power of alpha waves less than 50% or relative power of theta waves more than 23% was regarded as stage 1 sleep. SEM(slow eye movement) was defined as the duration of both-eye movement ranging from 1.5 to 4 seconds, and was also regarded as stage 1 sleep. If one of these three criteria was met, the epoch was regarded as stage 1 sleep. Results were compared to the manual rating results done by two polysomnography experts. Results: A total of 169 epochs were analyzed. The agreement rate for stage 1 sleep between automatic detection and manual scoring was 79.3% and Cohen’s Kappa was 0.586 (p<0.01). A significant portion (32%) of automatically detected stage 1 sleep included SEM. Conclusion: Generally, digitally-scored sleep staging shows accuracy up to 70%. Considering potential difficulty in stage 1 sleep scoring, accuracy of 79.3% in this study seems to be strong enough. Simultaneous analysis of EOG differentiates this study from previous ones which mainly depended on EEG analysis. The issue of close relationship between SEM and stage 1 sleep raised by Kinnari remains a valid one in this study.

Objectives: The gold standard for diagnosing obstructive sleep apnea syndrome (OSAS) is nocturnal polysomnography (NPSG). This is rather expensive and somewhat inconvenient, however, and consequently simpler and cheaper alternatives to NPSG have been proposed. Oximetry is appealing because of its widespread availability and ease of application. In this study, we have evaluated whether oximetry alone can be used to diagnose or screen OSAS. The diagnostic performance of an analysis algorithm using arterial oxygen saturation ($SaO_2$) base on 'dip index', mean of $SaO_2$, and CT90 (the percentage of time spent at $SaO_2$<90%) was compared with that of NPSG. Methods: Fifty-six patients referred for NPSG to the Division of Sleep Studies at Seoul National University Hospital, were randomly selected. For each patient, NPSG with oximetry was carried out. We obtained three variables from the oximetry data such as the dip index most linearly correlated with respiratory disturbance index (RDI) from NPSG, mean $SaO_2$, and CT90 with diagnosis from NPSG. In each case, sensitivity, specificity and positive and negative predictive values of oximetry data were calculated. Results: Thirty-nine patients out of fifty-six patients were diagnosed as OSAS with NPSG. Mean RDI was 17.5, mean $SaO_2$ was 94.9%, and mean CT90 was 5.1%. The dip index [4%-4sec] was most linearly correlated with RDI (r=0.861). With dip index [4%-4sec]${\geq}2$ as diagnostic criteria, we obtained sensitivity of 0.95, specificity of 0.71, positive predictive value of 0.88, and negative predictive value of 0.86. Using mean $SaO_2{\leq}97%$, we obtained sensitivity of 0.95, specificity of 0.41, positive predictive value of 0.79, and negative predictive value of 0.78. Using $CT90{\geq}5%$, we obtained sensitivity of 0.28, specificity of 1.00, positive predictive value of 1.00, and negative predictive value of 0.38. Conclusions: The dip index [4%-4sec] and mean $SaO_2{\leq}97%$ obtained from nocturnal oximetry data are helpful in diagnosis of OSAS. CT90${\leq}$5% can be also used in excluding OSAS.
Objectives Summary: A 20-year-old man was presented with a history of difficult waking for 10 years. He suffered from morning headache, chronic fatigue and mild daytime sleepiness but had no history of irresistible sleep attack, cataplexy, hypnagogic hallucination or sleep paralysis. Methods: Night polysomnography (PSG), multiple sleep latency test (MSLT) and HLA-typing were carried out. Results: The PSG showed short sleep latency (4.0 min) and REM latency (2.5 min), increased arousal index (15.7/hour), periodic limb movements during sleep (PLMS index=8.1/hr) with movement arousal index 2.1/hr and normal sleep efficiency (97.5%). The MSLT revealed normal sleep latency (15 min 21 sec) and 4 times sleep-onset REM (SOREM). HLA-typing showed DQ6- positive, that corresponded at the genomic level to the subregion DQB1*0601, which was different from the usual locus in narcolepsy patients (DQB1*0602 and DQA1*0102). Conclusion: Differential diagnosis should be made with circadian rhythm disorder and other causes of primary waking disorder. The possibility of a variant type of narcolepsy could be suggested with an unusual clinical manifestation and a new genetic marker.

Central sleep apnea (CSA) is characterized by respiratory failure of at least 10 seconds without any effort of the chest and abdomen in the absence of upper airway resistance during sleep. In this case, the patient experiences respiratory failure that does not meet the CSA diagnostic criteria and CSA symptoms. Magnetic resonance imaging diffusion-weighted imaging (MRI DWI) scans revealed a lateral medullary infarction. Continuous positive airway pressure (CPAP) was applied as a primary treatment for CSA and respiratory failure. During the titration of CPAP, the apnea-hypopnea index (AHI) and arousal index (AI) were worse than the results before its use (AHI: 42.5/hr→82.8/hr, AI: 21.7/hr→40.8h). As a result, adaptive servo-ventilation (ASV) was chosen as the secondary treatment. Compared to the night-polysomnography results before the ASV treatment, the AHI improved (42.5/hr→8.6/hr). Therefore, ASV is a potential treatment for CSA and respiratory failure in these patients.
The goal of the study is to provide a new type of audio-visualization method through case analysis and work production based on Polysomnography(PSG) data that is difficult to interpret or not familiar to the public. Most art works are produced with conscious actions during waking hours. On the other hand, during sleep, we get into the world of unconsciousness. Therefore, through the experiment, want to discover if could get something new when we were in the subconscious state, and if so, wondered what kind of art could be made through it. The study method is to consider definition of sleep and sleep data first. The sleep data were classified into normal group and Narcolepsy, Insomnia, and sleep apnea by focusing on sleep disorder graphs that is measured by sleep polygraph. After that, I refined and converted the acquired biometric data into a text-based script. The degree of sleep in the text form of the script was rendered as a 3D animated image using Maya. In addition, the heart rate data script was transformed into a midi format, and the audition was implemented in the garage band. After Effects combines the image and sound to create four single channel images of 3 minutes and 20 seconds each. As a result of the research, I made an opportunity for anyone easy to understand the results, having difference with the normal data, through art instead of using difficult medical term. It also showed the possibility of artistic expression even when conscious actions did not occur. Through the results of this research, I expect the expansion and diversity of artistic audiovisual expression of biometric data.

최근 이상홍수 및 하천정비 등으로 인해 갈수시 또는 홍수시 하천 모니터링을 위한 기초자료로 유량측정의 필요성이 부각되고 있다. 기존 유량측정 방법은 고비용과 다수의 측정인력이 필요하기 때문에 이를 개선하기 위하여 최근 전자장비를 이용한 비접촉식 유량측정에 대한 연구가 다수 진행 중에 있다. 이 중 간편하고 측정 정확도를 유지할 수 있는 표면영상유속계(SIV, Surface Image Velocimetry)에 대한 연구가 국내외에서 다양하게 이루어지고 있다. 표면영상유속계는 일정 시간 간격을 갖는 두 영상을 분석하여 유속을 계산하기 때문에 수표면이 움직이는 시간보다 두 영상 사이의 시간 간격을 길게 할 경우 잘못된 상관영역을 찾게 되어 유속 측정의 정확도를 떨어뜨릴 수 있다. 이러한 문제점을 개선하기 위해서 영상간의 시간간격 변화에 따른 표면영상유속계의 정확도 분석을 수행하였다. 본 연구에서는 영상간의 시간간격에 따른 표면영상유속계의 정확도 분석을 위해 현장에서 수면 요철이 발생하는 근경 영상을 이용하여 영상내의 수면 움직임의 최소 지속시간을 측정하였다. 측정 결과 대부분의 수면 요철은 0.1 ~ 0.2 초 내에서 발생하였다. 측정 결과 일반 캠코더를 이용하여 초당 최대 30장의 영상을 이용하여 하천 표면유속을 측정한다면 문제가 없을 것으로 판단된다.
