Reversible Ortner's Syndrome as a Presenting Feature of Thyrotoxicosis in an Adolescent: A Rare Case Report (청소년 갑상선 중독증에서 발현된 가역적 오트너 증후군에 대한 드문 증례 보고)

  • Ortner's or cardiovocal syndrome is hoarseness attributable to left recurrent laryngeal nerve (RLN) palsy associated with mechanical compression of the nerve by pathologically enlarged cardiovascular structures. Ortner's syndrome is a rare condition, and to our knowledge, only a few cases have been reported in Korea. Furthermore, this condition is extremely uncommon in pediatric patients with thyrotoxicosis-related RLN paralysis. We report a case of reversible Ortner's syndrome in an adolescent who presented with secondary pulmonary hypertension related to thyrotoxicosis.

A Physiotherapy Program for Adolescent Baseball Players with Scapular Dyskinesis: Comparison of the effects of posterior shoulder stretching combined with scapular stabilization exercises (PSSE) (어깨뼈 운동 이상증을 가진 청소년기 야구선수들을 위한 물리치료적 프로그램: 어깨뼈 안정화 운동과 동반된 뒤쪽 어깨 스트레칭(PSSE) 중재 후 효과 비교)

  • Purpose : Scapular dyskinesis, it have been shown to be common in overhead athletes and has been associated with RC muscle strength. Posterior shoulder tightness (PST) has been suggested as an important factor causing scapular dyskinesis. Therefore, rehabilitation programs should focus on a posterior shoulder stretch combined with scapular stabilization exercise (PSSE) intervention. Determine the effects of posterior shoulder stretch combined with scapular stabilization exercise on the rotator cuff (RC) muscles strength, functional strength ratio (FSR), range of motion (ROM), and pain. Methods : 30 adolescent baseball players participated and subjects were allocated PSSE group (n=15) or the SSE group (n=15). Both group performed a 6-weeks intervention and measured of isokinetic peak torque/body weight (PT/BW) of concentric external rotator (CER), eccentric external rotator (EER), concentric internal rotator (CIR), eccentric internal rotator (EIR), FSR, ROM, and pain. Results : After 6 weeks PSSE, significant increase CER PT/BW (+6.02±4.76 %), EER PT/BW (+5.39±4.22 %), EER to CIR ratio (+.17±.16), and internal rotation ROM (+15.08±3.57 °). Whereas, significant decrease EIR to CER ratio (-.14±.18), external rotation ROM (-12.00±6.94 °), and GIRD (-17.41±2.84 °) compared with pre-intervention. No significant difference of isokinetic PT/BW of CIR and EER post-intervention. In the SSE group showed no significant difference all measurements for isokinetic PT/BW, FSR, and ROM post-intervention. The pain was significant improve both PSSE group (-3.25±1.60) and SSE group (-2.83±1.85) post-intervention. Conclusion : Both the PSSE and SSE interventions led to more pain relief. However, only the PSSE group showed ROM, CER, EER PT/BW, and FSR improvements. These results might suggest that the PSSE intervention is a more effective program for improving RC muscle strength and balance, in particular, concentric and eccentric ER muscle strength, FSR and can expect to prevent shoulder injuries in adolescent baseball players with scapular dyskinesis.

A Study of Intervention for Adolescent Idiopathic Scoliosis (청소년기 특발성 척추측만증의 중재에 관한 연구)

  • Many methods have been described for the early intervention of adolescent idiopathic scoliosis. Adolescent idiopathic scoliosis is lateral and rotational spinal curvature in absence of associated congenital or neurologic abnormalities, the most common type of scoliosis observed in child and young adults, and refers to curves that develop after the age of $10{\sim}18$. The curves of adolescent idiopathic scoliosis have the potential to progress rapidly during growth. Curves are currently universally measured by the Cobb's method and Ferguson method. Some curves do not remain small, these may be mildly or severely progressive and the ribs on the convex side of the curve separate, and those on the concave side ribs approximate so rib undergoes deformation with rib humping. The latter may make angles that can affect vestibular system, balance, sensory, especially cardipulmonary function. Intervention for adolescent idiopathic scoliosis is based on the patient's age, the angular value of the curve, the maturity of their skeleton, and the topography. The purpose of intervention for adolescent idiopathic scoliosis consists of knowing how to go to the best approach the correction of the lateral curve and rotational deformity holding the achieved for the remainder of spinal growth, preventing significant cosmetic abnormality, pain and cardiopulmonary complication, control the muscle imbalance and proprioceptive postural disturbances, be less need for radical surgery to avoid early surgery.

The Effects of 12 Weeks Aquatic Exercise Training on Shoulder Joint Isokinetic Muscle Function in Adolescent Spastic Cerebral Palsied (12주간의 수중운동이 경직성 뇌성마비 청소년의 어깨 관절 등속성 근기능에 미치는 영향)

  • Purpose: The present study investigated the effects of 12-week aquatic exercise training on isokinetic muscle function of the shoulder in adolescents with cerebral palsy. Methods: The study included four male and four female adolescents with cerebral palsy. Isokinetic muscle function was measured at an angular velocity of $60^{\circ}/s$, using Biodex System VI Pro. The peak torques of internal rotation and external rotation were measured before and after training. Aquatic exercise training was performed once a day for 120 min, 4 times a week for 12 weeks. Results: The peak torque of external rotation according to body weight and mean power of internal rotation were significantly higher after training (p < 0.05). Conclusion: Our findings suggest that 12-week aquatic training for adolescents with cerebral palsy can improve isokinetic muscle function of the shoulder. Future studies should analyze the changes in isokinetic muscle function of the shoulder in more detail using various aquatic exercise programs to investigate their effects on individuals with cerebral palsy.

The Effect of Exercise Therapy on Pain, Muscle Function and Radiological Evaluation in a Female Youth Golf Player with Low Back Pain: Case Report (치료적 운동이 허리통증을 가진 여자 청소년 골프선수의 통증, 근기능 및 방사선학적 평가에 미치는 영향: 증례보고)

  • PURPOSE: The purpose of this study was to determine the effect of exercise therapy on low back pain (LBP), the function of paraspinal and abdominis muscles, and the sacrohorizontal angle as seen on the radiographs of the lumbar spine in a young female golf player with LBP. METHODS: This case report describes an 11-year-old female golfer who presented with LBP. The exercise therapy program comprised lumbar joint mobilization, lumbar spine flexion distraction, abdominal bridge, plank, side plank, and single-leg extensions from a 4-point kneeling position for 40 min/day; this was done twice a week for 8-weeks. LBP [visual analog scale (VAS) and Oswestry disability index (ODI)] and function of paraspinal and abdominis muscles [Ito test, curl-up test, $90^{\circ}$ stop test, squat test, opened eye one leg stance test (OEOL), and closed eye one leg stance test (CEOL)] were measured before and after 4 and 8 weeks of exercise therapy. The radiographs were analyzed for the lumbar Cobb's angle and sacrohorizontal angle before and after 8 weeks of exercise therapy. RESULTS: After 4 and/or 8 weeks of exercise therapy, VAS and ODI scores decreased; results for the Ito test, curl-up test, $90^{\circ}$ stop test, squat test, and OEOL and CEOL of muscle function improved; and the lumbar Cobb's angle and sacrohorizontal angle improved. CONCLUSION: These results suggest that exercise therapy improves LBP, muscle function, and radiographic parameters associated with LBP in young golf players. These findings have clinical implications for exercise therapy in young female golf players who have LBP.

  • An eruption failure can be observed for child and adolescent periods when the primary dentition is changed to the permanent dentition through the mixed dentition frequently. The eruption failure can lead to miss erupting times of the tooth, then it will cause a lot of problems including root resorption, esthetic problem, transposition of adjacent tooth, malocclusoin and etc. Especially, the maxillary first molar is importantly concerned with occlusion and growth and is an essential tooth for development and maintenance of occlusion. So, it is a momentous part of more proper occlusal management to find these abnormal cases at the early stage and solve the problems. The sorts of eruption failures of the maxillary first molars can be divided into delayed eruption, impaction and the primary retention and the secondary retention. When physical obstacles cause impaction, first of all they must be removed then we can treat the impaction with observation after removal, surgical exposure or orthodontic traction. If the source of impaction is an ectopic eruption, the treatment can be a brasswire, a pendulum appliance, a space maintainer or space regainer after the extraction of the second deciduous tooth and etc. These cases are made a diagnosis of eruption failures of the maxillary first molars in mixed dentition period and have good prognosises after my treatments. So I reported them.

An ICF-Core Sets for Children and Youth With Cerebral Palsy Based Approach From a Physical Therapist Perspective: A Single Case Study (물리치료사의 관점에서 뇌성마비 아동과 청소년을 위한 ICF-Core Set을 기반으로 한 접근법의 효과: 단일 사례 연구)

  • Background: The International Classification of Functioning, Disability, and Health-core set (ICF-core set) for children and youth (CY) with cerebral palsy (CP) provides a useful conceptual framework and a guide for health care planning and measuring the changes brought by interventions across a multitude of dimensions from body functions to personal activities, social participation, and environmental factors for them. Objects: This single case study was reported to illustrate the use of a goal directed approach in applying the ICF-core set for CY with CP from a physical therapist perspective. Methods: An eleven year old boy with spastic CP, Gross Motor Function Classification System (GMFCS) level V, and his mother participated in an evaluation of his functioning state. The intervention goal was set through an interview using the ICF-core set, Canadian Occupational Performance Measure (COPM) and Goal Attainment Scale (GAS). Physical therapy was carried out on an outpatient basis using a goal directed approach for 30 min, 1 time/week during 12 weeks and the boy's gross motor function was assessed using the Gross Motor Function Measure (GMFM)-66 version (item set 2) before and after the intervention. Results: As measured by the boy's mother, the COPM score showed a meaningful clinical change (performance=mean 3.5, satisfaction=mean 2.5) and the T-score of GAS changed 34.4 after the goal directed approach. The GMFM-66 (item set 2) score changed from 31.8 to 38.7 and evaluation using the ICF-core set displayed improvement in 6 items of activity level between before and after the intervention. Conclusion: The ICF-core set for CY with CP is useful for understanding the overall functioning of CY with this condition and provides an opportunity to share and integrate information and opinions from different disciplines. We consider it as a useful tool in the universal language for the therapy and education of CY with CP.

Systematic Review of the Effects of Blood Flow Exercise for Health-care Promotion: A Focus on Korean Domestic Research (헬스케어 증진을 위한 혈류조절 가압 운동의 효과에 대한 체계적 문헌고찰 ; 국내 연구 중심으로)

  • The purpose of this study was to determine the clinical effects of blood flow regulation exercise for improving patients' health care and its usefulness as a rehabilitation model for various diseases by analyzing and examining the existing literature. A literature review of Korean academic journals published over a 10-year period, from 2010 to 2019, was conducted using words such as "blood flow regulation," "blood flow restriction," "low-intensity exercise," and "Kaatsu." Kaatsu is a blood flow regulation exercise developed in 1966 by Dr. Yoshiaki Sato of Japan. It is an efficient and effective exercise method that uses blood flow regulation bands that increase the secretion of growth hormones to develop muscles within a short time, improves blood circulation and metabolism to prevent and improve adult diseases, shortens the rehabilitation period, and improves cardiovascular function. The study participants consisted of 10 patients, of whom four were elderly, four had obesity, one was a stroke patient, and one was a trauma patient. The results of this study show that the blood flow regulation exercise, which is a low-intensity exercise, has the same effect as high-intensity exercise, which supports the evidence that it is a highly efficient exercise method for muscle development and rehabilitation of the elderly, adolescents, and patients with injuries who have difficulty in general exercising. For future studies, further reviews are necessary to verify the effectiveness of the exercise method according to blood flow regulation site and type of disease.

  • The grip strength is orthopedics department, the place where from rehabilitation medical science and industrial medical science and it evaluates the function of the hand certainly is the ability which is necessary. There is specially by an occupation rehabilitation of mentally retarded child and the evaluation of grip strength is one which is meaning. The samples for this study were 39 normal males and 59 normal females, 62 mentally retarded males, 33 mentally retarded females ranging in age from Prepubeal period to Adolescence. This study was used to measure grip strength BASELINE Hydraulic Hand Dynamometer manufactured by EEI(Fabrication Enterprises Incorporated)in USA. The objective of research from the Prepubeal period to Adolescence the normal and mentally retarded students comparison evaluates the grip strength and effective fine motor program of one's it makes with the fundamental data for a development and it does. The results are as follow : (1) The hand which the normal student and the schoolgirl of rises from grip strength measurement result of the Prepubeal period research object people 10.7Kg with the same left hand was each measured 10Kg with 9.8Kg with in. The grip strength of the mentally retarded schoolboys was the hand which rises 6Kg with the left hand 5.4Kg, the grip strength of the schoolgirls 6.5Kg with was identical. (2) The hand which the normal schoolboys and the schoolgirls rises from grip strength measurement result of the Adolescence research object people 29.6Kg, 20Kg the left hand to be was each measured 27.8Kg with 18.4Kg with. The hand which the mentally retarded schoolboys and the schoolgirls rises 13.1Kg, 11.3Kg the left hand to be was each measured 12.3Kg with 10.9Kg with. (3) Among the normal students $10.2\%$ (Prepubeal period $0.94\%$, Adolescence $15.2\%$) in Mentally Retarded students $17.9\%$ (Prepubeal period $0.77\%$, Adolescence $19.1\%$) left hand it was higher. (4) The grip strength of the students who attend school from the general family dwells were appeared higher than students residence at institution.

The Influence of Adolescent's Scoliosis on Stress and Study-attitude (청소년의 척추측만증이 스트레스 및 학습태도에 미치는 영향)

  • Purpose: The purpose of this study was to investigate the influence of adolescent's scoliosis on the stress level and study-attitude, to inform the public that there exist an important correlation between adolescent's scoliosis and stress, as well as, study-attitude, using a questionnaire. Methods: For this study, questionnaires were distributed to elementary school in G city from May 10 through May 31, 2011. This study measured a primary scoliosis-test by a scoliometer with 300 elementary school students and a secondary test by Cobb's angle using an X-ray. The subjects were divided into 2 groups (normal group (n=20), scoliosis group (n=20)). The collected data were analyzed by a t-test and Pearson analysis. Results: As a result of this study, the Cobb's angle showed significant difference between the scoliosis group and the normal group (p<0.001). Also, it tended to have a greater stress level in the scoliosis group than the relationship between scoliosis and the stress or study-attitude compared to the normal group (p<0.05). The correlation of stress according to Cobb's angle showed a positive relationship (r=0.36), and the correlation of study-attitude to Cobb's angle showed a negative relationship (r=-0.04). Conclusion: In conclusion, this study showed that scoliosis affects stress and study-attitude. We hope that this result would help diagnose adolescent's scoliosis in anearly stage and prevent any phychologic and social problems relating to it.
