A Case Study on a Patient with Major Depressive Episode of Bipolar II Disorder Treated with a Combination of Interpersonal and Social Rhythm Therapy and Korean Medicine (제2형 양극성 장애의 주요우울 삽화에 대한 복합 한방 치료 사례보고: 대인관계 및 사회적 리듬 치료와 변증 치료를 중심으로)

  • Objectives: To determine treatment effects of a combination of interpersonal and social rhythm therapy and Korean medicine for a patient with major depressive episode of bipolar II disorder. Methods: A patient was treated with Korean medicine (acupuncture, herbal medicine, etc.) and interpersonal and social rhythm therapy (IPSRT) for four months. Pattern identification for depressive mood and sleep associated symptoms was evaluated using Patient Health Questionnaire-9 (PHQ-9) and Social rhythm metric II-5 (SRM II-5). Results: At the end of the treatment, depression and delayed sleep symptoms were improved and social rhythm was recovered to the regular range. The patient acquired an insight to his interpersonal tensions and conflicts. Conclusions: Korean medicine in combination with interpersonal and social rhythm therapy can be used to treat patients with major depressive episode of bipolar II disorder. More cases are needed to develop guidelines for treating bipolar disorder.

Effect of Chromium Picolinate on Glucose Tolerance and Insulin Sensitivity in the Type I and II Diabetic Rats (1형과 2형 당뇨모델 흰쥐에서 Chromium Picolinate의 당내성과 인슬린 감수성에 대한 영향)

  • Chromium is an essential nutrient and participates in glucose and lipid metabolism in human beings and animals. The present study was conducted to assess the effects of chromium picolinate (Cr-pic) on glucose tolerance and insulin sensitivity in type I and ll diabetic rats. The experimental groups were type I diabetic (streptozotocin-induced: 40 mg/kg, i.p.) and type II diabetic (Goto-Kakizaki rats) models. Each group was subdivided into control. low-dose and high-dose of Cr-pic treated groups. The Cr-pic was orally administered with Cr-pic (100 mg/kg for low dose group and 200 mg/kg for high dose group) for 4 weeks. And then we performed intraperitoneal glucose tolerance test (IPGTT) and insulin sensitivity test (ITT). The glucose tolerance test was carried out by inection of glucose (2 g/kg, i.p.). The peripheral insulin sensitivity test was con- ducted by injection of insulin (5 units/kg, s.c.) and glucose. We performed determining of blood glucose concentration at 0, 10, 30, 60, 90, and 120 min using automated glucose analyzer. The plasma insulin concentration was determined by rat insulin EIA kit. Administration of Cr-pic improved weight gain in all group s with higher significant in the low-dose group. There was no significance between the control and the Cr-pic treated groups in the area under the blood glucose curve and serum insulin concentration plots of IPGTT and peripheral ITT in type I diabetic rats. But Cr-pic treated groups showed significantly lower levels of the area under the blood glucose currie during IPGTT and ITT and the high-dose group showed less effects compared with the low-dose group in the type II diabetic rats. The plasma insulin concentration of both diabetic groups was not influenced by Cr-pic supplementation. We can conclude that chromium picolinate may improve the endogenous and exogenous insulin action and peripheral insulin sensitivity in type II diabetic rats.

Clinical Significances of Hyperamylasemia Following Cardiopulmonary Bypass (체외순환 후 고아밀라제혈증의 임상적 의의)

  • Backgound: This study was performed to evaluate the incidences, the risk factors, and the clinical course of the hyperamylasemia in patients who underwent open heart surgery under cardiopulmonary bypass. Material and Method: Thirty seven patients who underwent cardiopulmonary bypass were studied at Department of Thoracic & Cardiovascular Surgery, Yeungnam University Hospital, from July 1997 to June 1998. The thirty seven patients were divided into two groups, 13 patients in group I had normal serum amylase levels and 24 patients in group II had hyperamylasemia. Result: Mean serum amylase(IU/l) levels and 24 patients in group II had hyperamylasemia. Result: Mean serum amylase(IU/l) levels of gorup II showed 54.3$\pm$4.6, 78.0$\pm$9.2, 372.0$\pm$103.4, 460.5$\pm$80.4, 280.4$\pm$46.6, and 131.0$\pm$15.6, preoperative, immediate postoperative, at postoperative 1, 2, 3, and 7 days, respectively. In group II, serum amylase level of the postoperative day 2 was the highest and was significantly higher than that of the preoperative day(p<0.001). Serum amylase level started to decreased at postoperative day 3 and returned to the normal level at postoperative day 7. Significant clinical symtoms of overt pancreatitis were not shown in patients in group II. The following perioperative variable such as diagnosis, cardiopulmonary bypass time, aortic cross clamping time, mean systemic pressure during bypass, and administration of steroid were compared between groups. There were no significant differences between groups. In all patients, Serum amylase level of postoperative day 2 and aortic cross clamping time were correlated significantly(p=0.047). Conclusion: Serum amylase level after cardiopulmonary bypass could be elevated postoperatively and serum amylase level of POD 2 was considered to have significant correlation with aortic cross clamping time. Shortening of aortic cross clamping time will help in reducing the hyperamylsemia. In this study, although significant clinical symptoms and overt pancreatitis were not seen from hyperamylsemic patients, careful clinical observation of hyperamylasemia would be necessary.

Determination of Energy Release Rate of Penny-shaped Interface Crack on Bimaterial Cylinder (동전모양 균열이 존재하는 이상복합체의 에너지해방율 산정)

  • The mixed mode problem (I and II) of a peny-shaped interface cracks in remote tension loading on a bi-material cylinder is studied using finite element method. The energy release rates for the tip of the crack in the interface were calibrated for several different moduli combinations and crack ratios using the modified crack closure integral technique and J-integral method, with numerical results obtained from a commercial finite element program. Numerical results show that non-dimensional value of$\sqrt{G_{II}E^*}/\sqrt[p]{\pi a}$ increases as the crack size or moduli ratio increases. Meanwhile, non-dimensional value of$\sqrt{G_{I}E^*}/\sqrt[p]{\pi a}$ decreases as the moduli ratio increases, but above the moduli ratio of 3 its value decreases then increases again as the crack size increases. Reliability of the numerical analysis in this study was acquired with comparison to an analytical solution for the peny-shaped interface crack in an infinite medium.

The Effect of Joint Taping and Muscle Taping on Dynamic Balance and Gait in Patents with Chronic Stroke (관절 테이핑과 근육 테이핑이 만성 뇌졸중 환자의 동적 균형과 보행에 미치는 영향)

  • Purpose : Elastic taping is a therapeutic method, used for treatment of various musculoskeletal and neuromuscular deficits. However, there is limited evidence, of the effects of ankle elastic taping in neurologic patients. The purpose of this study, was to investigate the effect of elastic taping on gait, in the affected ankle area of chronic stroke patients. Methods : Subjects were randomized to receive 30 chronic stroke patients, who were 6 months old from the date of onset according to screening criteria. Group I showed ankle joint taping, and Group II had ankle muscle taping. Dynamic balance and temporal and spatial gait, were measured before taping application, and after 30 minutes of taping application. Results : Dynamic balance was measured using the Time up & Go test (TUG). There was statistically significant difference, between Group I and Group II (p<.05). There was no statistically significant difference, between Group I and Group II. Temporal and spatial gait were measured using GaitRite. In Group I, there was significant difference, before and after taping (p<.05). In Group II, there was no significant difference, before and after taping (p>.05). There was significant difference in Group I, between Group I and Group II (p<.05). Conclusion : Results suggest that intervention using elastic taping, may have a positive effect, on rehabilitation diversity and function in stroke patients. Based on this, it can be used for rehabilitation of stroke patients. Various studies on the application method, and effect of the application site as well as application time, should be continued with stroke patients.

Reductive Degradation Kinetics and Pathways of Chlorophenolic Organic Pollutants by Nickel-Coated Zero Valent Iron (니켈로 코팅된 영가금속을 이용한 염소계 페놀화합물의 반응경로 및 반응율 평가)

  • Reductive dechlorination of chlorophenols by nickel coated iron was investigated to understand the feasibility of using Ni/Fe for the in situ remediation of contaminated groundwater. Zero Valent Iron(ZVI) was amended with Ni(II) ions to form bimetal(Ni/Fe). Dechlorination of five chlorophenol compounds and formation of intermediates were examined using Ni/Fe. Rate constant for each reaction pathway was quantified by the numerical integration of a series of differential rate equation. Experimental results showed that the sequence of hydrodechlorination rate constant was in the order of 2-CP>4-CP>2,4-DCP>2,4,6-TCP>2,6-DCP. The hydrodechlorination pathways for the conversion of each chlorophenol compound involves a full dechlorination to phenol via both concerted and stepwise mechanisms. Reaction pathways and corresponding kinetic rate constants were suggested based on the experiments and numerical simulations.

  • 본 논문은 포항가속기에서 건설 중인 진공 chamger의 용접에 관한 것이다. 저장링에 전자빔을 5시간 이상 저장하기 위하여 10-10torr 이하로 진공도를 유지하도록 설계되어 있기 때문에 초고진공에 적합한 TIG 용접(tungsten inert gas welding)을 하였다. 진공 chamber는 Al 5083-H321이며 flange의 재질은 Al 2219-T852이다. Sector chamber I, II의 총 용접 길이는 각각 27m, 37m이며 진공에 유해한 균열과 virtual leak가 생기지 않도록 하는 용접조건 등을 선정하였다.

  • 우리나라 종합병원의 의료서비스의 질을 향상시킬 수 있는 방안을 모색하기 위해 K대학병원의 소아과와 처방전 서비스를 대상으로 연구를 수행하였다. 연구방법은 시뮬레이션을 수행하여 분석였다. 시뮬레이션을 수행하기 위해서는 네트워크 모델링 방식인 SLAM II를 사용하였다. 분석결과를 요약하면 소아과보다는 처방전의 서비스를 향상시키기 위한 대안적 방법을 고려할 필요가 있는 것으로 분석되었다.

  • 세계보건기구(WHO)에서는 동남아시아를 중심으로 발생하기 시작했던 급성호흡기감염을 '중증급성호흡지증후군(SARS)'으로 공식 명명하고 전 세계에 경보령을 발동, 각국에서는 경계령을 발동하여 환자 발생감시체제를 가동하였다. 또한 올 겨울 SARS가 다시 유행할 것에 대비해 철저한 대비책을 세워나가야 한다는 전문가의 의견이 제시되기도 하였다. 특히 우리나라는 WHO의 SARS 위험평가 분류에 의거 저위험지역에 해당되나 상반기 사스유행지역인 중국 등과 밀접한 교류가 많은 점을 감안하여 교점지역에 준하여 의료기관 중심 SARS 감시체계를 운영하고 단계별로 감시체계를 전국 병원으로 확대하기로 하였다. 이에 편집실에서는 한동안 전 세계 초미의 관심사였던 SARS에 대해 정리해보는 기회를 갖고자 원고를 청탁하여 특별기고로 다루고자 한다.
