척수손상자의 성재활 교육이 성생활 지식과 성생활 만족도에 미치는 영향 (The Effects of Sexual Rehabilitation Education on the Knowledge of Sexual Activities and Satisfaction of Sexual Activities of Spinal Cord Injury Clients)

  • The purpose of this study is to identify the effects of sexual rehabilitation education on the knowledge of sexual activities and satisfaction of sexual activities of spinal cord injury clients by the method of Nonequivalent control group pretest-posttest group. The study period was between Jan. 6th and Apr 11th, 1999. The experimental group was 21 spinal cord injury clients from one general hospital, and the control group was 21 spinal cord injury clients from three University hospital. Sexual rehabilitation education was done with manuals and video-tapes. Education time was 90 minutes a week for 4 weeks. The study tool was 20 questionnaires about sexual knowledge, designed Song, Chung Sook(1990) and 10 questionnaires from Derogatis's Derogatis Sexual Function Inventory for satisfaction of sexual activities(1979) translated by Jang, Soon Boke(1989). Analysis of data was done by $x^2-test$, t-test, repeated measures ANOVA and simple main effect with SAS/PC. The results are as follows. The 1st. hypothesis. 'The group educated about the sexual rehabilitation has more sexual activities knowledge than those of the uneducated group.' is significant statistically(F=12.06, p=0.001). The 2nd hypotheses, 'The group educated about the sexual rehabilitation has more satisfaction of sexual activities than those of the uneducated group.' is significant statistically(F=5.04, p=0.030). According to the results, sexual rehabilitation education helps spinal cord injury clients to gain knowledge of sexual activities and satisfaction of sexual activities.

백굴채(白屈菜)가 손상된 배양척수감각신경세포에 미치는 영향 (Effects of Herbar Chelidonii on the Cultured Spinal Sensory Neurons Damaged by XO/HX)

  • Objectives and Methods : To evaluate the mechanism of oxidative damage by xanthine oxydase(XO) and hypoxanthine(HX)-induced oxygen radicals, MTT assay and NR assay were carried out after the cultured mouse spinal sensory neurons were preincubated for 4 hours with various concentrations of XO/HX. And the amount of total protein. neurofilament EIA. lipid peroxidation and LDH activity were measured, to evaluate the protective effect of Herbar Chelidonii(HC) water extract on cultured spinal sensory neurons damaged by XO/HX. after the cultured mouse spinal sensory neurons were preincubated with various concentrations of HC water extract for 3 hours prior to exposure of XO/HX. Results : XO/HX decreased significantly the survival rate of the cultured mouse sensory neurons by NR assay and MTT assay In proportion to concentration and exposed time. In proportion to concentration and exposed time on cultured spinal sensory neurons, XO/HX showed the quantitative decrease of neurofilament by EIA. the decrease of total protein amount by SRB assay and the Increase of lipid peroxidation as well as LDH. HC showed the quantitative increase of neurofilament and total protein, but showed the decrease of lipid peroxidation and LDH activity against the neurotoxicity of XO/HX. Conclusions : From the above results, it is concluded that XO/HX have a neurotoxic effect on cultured spinal sensory neurons and that the herbs extract, such as HC, prevent the toxicity of XO/HX effectively in that they decrease lipid peroxidation and LDH activity.

  • Purpose: We tested whether repetitive transcranial magnetic stimulation (rTMS) improved recovery following spinal cord injury (SCI) in rats with transplantation of adipose tissue-derived stromal cells (ATSCs). Methods: Twenty Sprague-Dawley rats (200-250 g, female) were used. Moderate spinal cord injury was induced at the T9 level by a New York University (NYU) impactor. The rat ATSCs (approximately $5{\times}10^5$ cells) were injected into the perilesional area at 9 days after SCI. Starting four days after transplantation, rTMS (25 Hz, 0.1 Tesla, pulse width=$370{\mu}s$, on/off time=3 sec/3 sec) was applied daily for 7 weeks. Functional recovery was assessed using the Basso, Beattie, and Bresnahan (BBB) locomotor rating scale as well as pain responses for thermal and cold stimuli. Results: Both groups showed similar, gradual improvement of locomotor function. rTMS stimulation decreased thermal and cold hyperalgesia after 7 weeks, but sham stimulation did not. Conclusion: rTMS after transplantation of ATSCs in an SCI model may reduce thermal hyperalgesia and cold allodynia, and may be an adjuvant therapeutic tool for pain control after stem cell therapy in SCI.

척수손상 흰쥐에서 수영훈련이 운동기능 회복에 미치는 영향 (The Effect of the Swimming Exercise on Motor Functional Recovery after Experimental Contusive Spinal Cord Injury in the Rats)

  • Purpose: Previous studies have suggested that BDNF has a role in plasticity and survival following spinal cord injury and treadmill exercise increases BDNF levels in the normal brain and spinal cord. We attempted to determine whether swimming exercise improve motor function following experimental contusive spinal cord injury and whether motor outcome is associated with BDNF expression. Methods: Thirty six Sprague-Dawley rats (weight, 250 to 300 g) were divided into control (n=18) and experimental swimming group (n=18). Spinal cord injury was produced using NYU-spinal impactor at the eleven thoracic levels in both groups. Swimming exercise started $7^{th}$ day from SCI operation, lasted 5 min per day, 5 days a week for 4 weeks and then exercise times a day were increased in one number to each week. Motor functional recovery was determined by the Basso-Beattie-Bresnahan (BBB) locomotor rating scale, modified inclined board plane test, histological findings, H&E and BDNF expression observed at $1^{th}$, $3^{rd}$, $7^{th}$, $14^{th}$, $21^{st}$ and $28^{th}$day after injury. Results: 1. The BBB scores were higher in experimental group than control group at $14^{th}$, $21^{st}$ day (left hind limb) and at $21^{th}$ day (right hind limb) (p<0.05) after injury. 2. The inclined board plane test were significantly greater in experimental group than control group at $7^{th}$ day (p<0.05), $14^{th}$ and $28^{th}$ day (p<0.01) after injury. 3. The BDNF expression was severe revealed in experimental group than control group at $7^{th}$, $14^{th}$ and $28^{th}$ day after injury. Conclusion: This study suggests that swimming applied from the early phase after spinal cord injury be beneficial effects in motor functional recovery.

흰쥐 궁둥신경 손상 후 전침에 의한 척수 내 NeuN 발현에 미치는 영향 (The Effect of Electroacupuncture on NeuN Expression in Spinal Cord in Sciatic Nerve Injured Rat)

  • Purpose: The purpose of this study was to investigate the effect of electroacupuncture on NeuN expression in ventral horn motor neurons of spinal cord, changes in pain thresholdchanges in motor function in rats with partially dissected sciatic nerves. Method: A total of 120 male Sprague-Dawley rats were randomly divided into two groups, a control group and a group administered electroacupuncture at ST36, LI11 and SP9 with 120 Hz and 0.5 mA. Animals were sacrificed on days 1, 3, 7, 14 and 28 after nerve injury (the sciatic nerve was partially dissected). The pain threshold was recorded by an Analgesia? meter and a BBB? score was calculated for motor function. After preparing lumbar spinal cord slide sections, they were immunostained with NeuN antisera (1:2,500). Results: The numbers of NeuN immunoreactive neuronsin the electroacupuncture group was increased compared to the control group. The numbers of NeuN immunoreactive neurons on days 14 and 28 day were different (p<0.05), as were the numbers on days 3 and 7 (p<0.01). The pain threshold BBB score for the electroacupuncture group was higher than for controls. Conclusion: The increase in pain threshold, BBB-score and number of NeuN immunoreactive neurons inventral horn motor neurons of spinal cord in rats withnerve dissection showed that electroacupuncture can attenuate pain transduction and increase motor function. Also, NeuN was a good marker for identifying the degree of nerve cell loss after nervous system injury.

Tetramethylpyrazine이 흰쥐 척수압박손상의 신경세포 자연사에 미치는 영향 (Effect of Tetramethylpyrazine on Neuronal Apoptosis in Spinal Cord Compression Injury of Rats)

  • Objectives : The pathophysiology of acute spinal cord injury(SCI) may be divided into primary and secondary mechanisms of injury. The secondary mechanism involves free radical formation, excitotoxicity, inflammation and apoptotic cell death, and sets in minutes after injury and lasts for weeks or months. During this phase the spinal tissue damages are aggravated. Therefore, secondary mechanisms of injury serve as a target for the development of neuroprotective drug against SCI. The present study investigated the effect of tetramethylpyrazine(TMP), an active ingredient purified from the rhizome of Ligusticum wallichii(川芎, chuanxiong), on neuronal apoptosis in spinal cord compression injury in rats. Methods : SCI was subjected to rats by a static compression method(35 g weight, 5 mins) and TMP was treated 3 times(30 mg/kg, i.p.) during 48 hours after the SCI. Results : TMP ameliorated the tissue damage in peri-lesion of SCI and reduced TUNEL-labeled cells both in gray matter and in white matter significantly. TMP also attenuated Bax-expressed motor neurons in the ventral horn and preserved Bcl-2-expressed motor neurons. Conclusions : These results indicate that TMP plays a protective role in apoptotic cell death of neurons and oligodendrocytes in spinal cord injury. Moreover, it is suggested that TMP and TMP-containing chuanxiong may potentially delay or protect the secondary spinal injury.

Ginsenoside Rg3이 흰쥐 척수압박손상의 초기 염증반응에 미치는 영향 (Effects of Ginsenoside Rg3 on Early-stage Inflammatory Response in Spinal Cord Compression of Rodents)

  • Objectives : In present study, we investigated the effects of ginsenoside Rg3 on early-stage inflammatory response in spinal cord compression of rodents. Methods : Spinal cord injury(SCI) was induced by a vascular clip method(30 g, 5 min) on the spinal cord of mice. Rg3 was treated orally at 1 hour prior to the SCI induction. Messenger ribonucleic acid(mRNA) expression of tumor necrosis factor-${\alpha}$(TNF-${\alpha}$), interleukin-1${\beta}$(IL-1${\beta}$), interleukin-6(IL-6) and cyclooxygenase-2(COX-2) was measured by the real-time polymerase chain reaction(RT-PCR). Microglia in the spinal cord tissue, neurophils and COX-2 in the peri-lesion and inducible nitric oxide synthase(iNOS) expression in the ventral horn of SCI induced rats were measured by immunohistochemical stain. Results : 1. Rg3 significantly reduced the mRNA expression of TNF-${\alpha}$, IL-1${\beta}$, and COX-2 in the spinal cord tissue compared with SCI group(p<0.05, p<0.01). 2. Rg3 significantly reduced the total number of activated microglia and proportion of phagocytic form in the total activated microglia compared with SCI group(p<0.05, p<0.01). 3. Rg3 significantly reduced myeloperoxidase(MPO) positive neurophil in the peri-lesion compared with SCI group(p<0.05). 4. Rg3 reduced the COX-2 expression in the tissue and motor neurons compared with SCI group. 5. Rg3 significantly reduced iNOS positive motor neurons in the ventral horn compared with SCI group(p<0.01). Conclusions : In conclusion, we demonstrated at first that treatment of ginsenoside Rg3 could reduce significantly the levels of inflammatory mediators in a spinal cord compression model of rodents. Therefore, these results suggested that ginsenoside Rg3 may be a useful antimiflamatory therapeutic candidate for SCI.

재활 후 척수손상환자 보행능력의 양상과 보조 장구 사용 실태 (Effects of Using Aid in Enhancing Walking Ability After Rehabilitative Care in Patients With Spinal Cord Injury)

  • The purpose of this study was to find the effects of using aid in enhancing walking ability inpatients with spinal cord injury who have received rehabilitative care. The study population consisted of 24 spinal cord injury patients referred to the Department of Rehabilitation Medicine in the National Rehabilitation Center (NRC). All subjects were ambulatory with or without an assistive devices. All of the participants were assessed on SCIM II, WISCI II, FIM, MBI, gait speed (m/s), and walking endurance (120 min/m). The data were analyzed using a paired t-test, a one-way ANOVA, and a Duncan test. The results revealed that TSCIM II and all of the items of SCIM II of the cervical ASIA D group patients were higher than those of the Thoracic ASIA A and C group patients (p<.05). The FIM, MBI, and WISCI II of the cervical ASIA D group patients were higher than those of the Thoracic ASIA C group patients (p>.05). The walking velocities of the lumbar ASIA C group patients were higher than those of Thoracic ASIA A group patients (p<.05). The walking endurance of the lumbar ASIA C group patients was higher than that of the thoracic ASIA C group patients (p<.05). The ASIA D group patients used bilateral standard canes or crutches, but none used AFO. The ASIA A and C group patients used bilateral standard walkers with KAFO for standing and walking. The findings suggest that injury level as well as the functionality of walking aids should be considered when formulating a rehabilitative plan for patients with spinal cord injury.

PNF 패턴을 결합한 협응적 이동 훈련이 척수손상환자의 보행에 미치는 효과 (The Effects of Coordinative Locomotion Training Using the PNF Pattern on Walking in Patients with Spinal Cord Injury)

  • Purpose: The purpose of this study was to prove the effects of coordinative locomotion training (CLT) on walking speed, walking endurance, and balance for incomplete spinal cord injury patients. Methods: Ten subjects were randomly assigned to the CLT group (n = 5) and the treadmill (TM) group (n = 5). The CLT group performed PNF pattern exercise using the motions of the sprinter and skater for 30 minutes, while the TM group performed using a treadmill for 30 minutes. Both groups performed these therapeutic interventions for five days per week, for a period of four weeks. A 10 meter walking test, Berg Balance Scale (BBS), and 6 meter walking test were used for the assessment of gait speed, balance, and gait endurance. The SPSS Ver. 18.0 statistical program was used for data processing. A Wilcoxon signed rank test was used for the comparison of pre- and post-intervention performance and a Mann-Whitney test was used for comparison between the groups. The significance level for the statistical inspection was set at 0.05. Results: Both groups showed significant improvements in the 10 meter walking test, Berg Balance Scale, and 6 meter walking test (P < 0.05). Conclusion: CLT had an effect on the improvement of walking speed, walking endurance, and the balance of incomplete spinal cord injury patients. Thus, we suggest that CLT is a therapeutic intervention for incomplete spinal cord injury patients.

승모근과 전거근 강화운동이 기능적 어깨충돌증후군을 가진 척수손상 환자의 통증과 근 활성도에 미치는 영향 (The Effects of Trapezius and Serratus Anterior Strengthening Exercise on Pain and Muscle Activation in Spinal Cord Injury Patients with Functional Shoulder Impingement Syndrome)

  • Purpose: The aim of this study was to investigate the effects of trapezius and serratus anterior strengthening exercise on the shoulder pain and muscle activation of patients with spinal cord injury and functional shoulder impingement syndrome. Methods: The study consisted of 10 patients with spinal cord injury who were hospitalized in Rehabilitation Hospital U, Uijeongbu, South Korea. The exercise was implemented three times a week for 10 weeks. In each session, the subjects performed one of a total of five types of exercise at mid-level intensity. The shoulder pain and disability index (SPADI) was used to evaluate the patients before and after the intervention. The muscle activation of the upper trapezius, middle trapezius, lower trapezius, and anterior serratus muscle was assessed by surface electromyography (EMG) at the beginning of the experiment and 10 weeks later. Wilcoxon's singed-rank test was conducted to determine differences in the pain index and muscle activation before and after the exercise. The level of statistical significance was set at ${\alpha}=0.05$. Results: SPADI scores significantly decreased after the exercise (p<0.05). In comparisons of muscle activation, there was a significant improvement in the upper trapezius at $60^{\circ}$ shoulder joint flexion (p<0.05). There was no significant improvement at $90^{\circ}$ shoulder joint flexion. The middle trapezius showed a significant improvement at $120^{\circ}$ shoulder joint flexion (p<0.05). Conclusion: Trapezius and serratus anterior strengthening exercise reduced pain in spinal cord injury patients with functional shoulder impingement syndrome. The decreased muscle activation of upper trapezius and increased muscle activation of the anterior serratus muscle at $60^{\circ}$ shoulder joint flexion point to positive effects of the exercise on supraduction of the scapula.
