The Effects of Systemic Morphine to Analgesic Level in Spinal Anesthesia (Morphine 정주가 척추마취의 레벨에 미치는 영향)

  • 척추마취는 국소마취제를 지주막하강에 주입하여 척수신경 전근과 후근을 차단하는 방법으로 하복부나 하지 수술 뿐 아니라 만성 통증과 암성 통증의 치료에도 이용되고 있는데 마취시간이나 제통시간의 연장 및 적절한 피부분절의 마취나 진통의 달성은 척추마취에서 중요한 사항이다. 본 연구에서는 morphine정주가 척추마취에 어떤 영향을 주는지 알아보기 위해 척추마취하에서 하지 수술을 받은 40명의 환자를 대상으로 척추마취를 시행한 80분에 척추마취 레벨, 수축기 및 이완기 혈압, 맥박 그리고 호흡수를 조사한후 morphine 10 mg을 정맥내로 주사후 20분후에 척추마취 레벨과 혈압, 맥박, 호흡수를 조사하여 다음과 같은 결과를 얻었다. 1) 척추마취 레벨은 morphine 투여진 $T_{7.5{\pm}0.32}$에 비해 morphine 투여 20분후에 $T_{6.0{\pm}0.31}$로 의의있게 상승하였다 (p<0.005). 2) 수축기 및 이완기 혈압과 맥박수는 morphine투여전과 투여후에 의의있는 변화가 없었다. 3) 호흡수는 morphine 투여전에 비해 투여후 감소가 있었다(p<0.005). 이상의 결과로 척추마취하에서 수술을 시행할 때나 통증치료시 전신적으로 morphne을 투여하여 마취와 진통부위를 넓일 수 있을 것으로 사료된다.

척수에서의 통각전달체계 연구

  • 척수에서의 통각전달은 말초에서 전달되는 통각정보가 척수내의 척수세포에 전달되면서 시작된다. 척수에 전달된 유해자극 정보는 척수내의 이차감각세포를 통하여 시상으로 전달되고 이로 인해 통증을 느끼게 된다. 이러한 척수내의 세포는 연수등의 여러 부위에 존재하는 신경 세포에 의하여 억제를 받으며 이와 같은 하향성 억제는 뇌에 존재하는 내재진 통계를 설명하는 중요한 인자가 되고 있다. 본 실험은 하부연수에 위치하는 신경핵인 Lateral reticular nucleus가 이러한 하향성 억제를 가졌는가를 알아 보고자 하였다. 이를 위하여 고양이를 마취시키고 척수궁을 절제하여 척수를 노출시키고 미세전극을 꽂아 척수세포의 활성을 기록하였다. 여덟마리의 고양이에서 31개의 척수세포를 기록하였다. 이 세포들 중 WDR세포가 14 (45%), HT가 9 (29%), LT 및 Deep세포가 각각 4 (13%)가 되었다. 이 척수세포에 북외측하부 연수인 lateral reticualr nucleus 주위를 건기자극하면 21개 (68%)의 세포가활성의 억제를 받았고 9개의 (29%) 세포는 아무런 변화가 없었고 1개의 (3%) 세포는 흥분되었다. 전기의 자극은 강도 100$\mu$A이며 자극길이는 100$\mu$S 그리고 100Hz의 주파수를 가진 펄스파였다. 이와같은 북외측하부연수의 전기자국은 신경세포의 자발활성뿐 아니라 수용장 자극에 의한 반응도 억제하였다.

Effectiveness of acupuncture-type interventions to prevent nausea and vomiting during and after cesarean delivery under spinal anesthesia : A systematic review

  • 목 적: 이 논문은 제왕절개 분만을 위한 척수마취로 인해 수술 중 또는 그 후에 발생할 수 있는 오심 및 구토에 대한 침관련 치료의 항구토 효과를 규명하기 위한 계통적 리뷰 논문이다. 방 법: 제왕절개 분만을 위한 척수마취로 인해 발생할 수 있는 오심 및 구토를 예방하기 위한 침관련 치료에 대한 randomized clinical trials(RCT's)를 검색하기 위해 2008년 3월까지 수록된 6개의 데이터베이스(MEDLINE, EMBASE, The Cochrane Library, CINAHL, AMED and PsycINFO)를 검색하였다. 논문의 선정 기준은 원저이며, 플라시보 대조군을 설정한 무작위 임상시험이고, 오심 또는 구토에 대한 결과가 실려 있는 것으로 하였다. 결 과: 총 5개의 논문을 선정하였으며 각 논문들의 연구방법은 일반적으로 좋은 편이었다. 5개의 논문들을 종합한 결과는 수술중 오심 발생률(OR=0.468, 95%CI:0.250 -0.876, P=0.018), 수술 후 오심 발생률(OR=0.616, 95%CI:0.042-0.942, P=0.026), 수술 후 구토 발생률(OR=0.640, 95%CI:0.432-0.948, P=0.026)이 침관련 치료군이 대조군에 비해서 통계적으로 의미있게 적었다. 결 론: 본 문헌 고찰을 통해 제왕절개 수술을 위해 시행한 척수 마취로 인해 수술중 또는 수술후 발생한 오심 및 구토에 대해 침관련 치료가 예방효과가 있음을 밝힐 수 있었다.

The Effect of Listening to Music on Anxiety, Sedation, and Vital Signs of Patients Undergoing Spinal Anesthesia (수술 중 음악청취가 척수마취 환자의 불안, 진정 및 활력징후에 미치는 효과)

  • Purpose: The aim of this study was to examine the effect of listening to music on the level of anxiety, sedation, and vital signs of patients undergoing surgery with spinal anesthesia. Methods: A convenience sample of 70 adult patients, ASA physical status I-II, scheduled for elective urologic or orthopedic surgery with spinal anesthesia, was included in this study. They were assigned to either an experimental group (n=35), listening to music during surgery, or a control group (n=35), not listening to music. Their anxiety was measured with the Spielberg's Trait and State Anxiety Inventory (STAI-KYZ). Sedation level was monitored with the Bispectral Index, and the vital signs at beginning, 10, 20, 30, and 45 min after operation. Results: The music group reported significantly lower state anxiety level during surgery as compared with the control group (t=3.91, p<.001). Repeated measures of ANOVA indicated a significant by group interaction on BIS index scores of sedation (F=4.23, p=.006). Among the vital signs, only heart rate was a significant by group interaction (F=5.529, p=.004). Conclusion: These findings indicate that listening to music during surgery with spinal anesthesia is a useful and effective nursing intervention to reduce anxiety and to maintain proper sedation.

General Anesthesia in a Child with Possible Spinal Muscular Atrophy Combined with Gingival Hyperplasia -A Case Report - (치은비대를 동반한 척수성 근위축증 환자의 마취관리 -증례보고-)

  • Spinal muscular atrophies are inherited neurodegenerative disorders affecting anterior hem cells. There are various problems, especially weakness of respiratory muscle and abnormal reaction to muscle relaxants during the general anesthesia. And gingival hyperplasia can make the proper airway management difficult. Experience with anesthetic management in a patient with spinal muscular atrophy combined with gingival hyperplasia has been very rare. We report the anesthetic experience of a wheel-chair-bound child, who underwent gingivectomy under general anesthesia. The child was safely managed with fibroscopic nasotracheal intubation under sevoflurane without muscle relaxants. Also, there was no deterioration of her underlying neurologic conditions.

Acute postoperative myelopathy caused by spontaneous developed cervical disc herniation: Case report & literature review (수술후 자연발생 경추간판탈출에 의한 척수병증: 증례보고 및 문헌고찰)

  • Non-traumatic acute myelopathy caused by cervical disc herniation is rare. To date, no case has been reported to be caused by extrusion cervical disc herniation, unrelated to patient posture during surgery. Here, we report the case of a 65-year-old male patient with cervical myelopathy who underwent subsequent arthroscopic rotator cuff surgery under general anesthesia; non-cervical spine surgery. Ed. Patient showed acute postoperative tetraplegia in spite of optimal anesthetic management. He showed no limitation of neck movement at pre-operative airway evaluation, and had no history of trauma to the cervical spine. During surgery, there had been no overextension or twisting of the neck, including at the time of anesthetic induction by tracheal intubation. However, cervical disc herniation causing spinal canal cord compression was detected in the postoperative magnetic resonance imaging, which probably resulted in tetraplegia of the patient. Motor and sensory functions were recovered after 21 days of conservative treatment, including steroid pulse intravenous therapy without any surgical intervention. In this report, the disease is described after reviewing other reported cases; furthermore, we also discuss the pathophysiology of the disease. Based on our report, we propose that under general anesthesia, clinicians should pay attention to the possibility of pre-existing cervical disease, even in non-cervical spine surgeries of geriatric patients.

Protective Effects of Trimetazidine in a Rabbit Model of Transient Spinal Cord Ischemia (허혈성 척수 손상의 동물실험모델에서 Trimetazidine의 척수 보호효과)

  • Paraplegia remains unresolved as the most dreaded operative complication with surgical treatment of descending thoracic and thoracoabdominal aortic diseases. In this study, the neuroprotective effect of trimetazidine that has been used clinically for ischemic heart disease was investigated in a rabbit spinal cord ischemia model. Material and Method: Thirty-three New Zealand white rabbits were randomized as follows: control group undergoing abdominal aortic occlusion but receiving no pharmacologic intervention(Group 1, n= 17); TMZ group(Group 2, n= 16) receiving 3 mg/kg trimetazidine intravenously before the occlusion of the aorta. Ischemia was induced by clamping the abdominal aorta just distal to the left renal artery for 30 minutes. Neurologic status was assessed at 2, 24, and 48 hours after the operation according to the modified Tarlov scale, then the lumbosacral spinal cord was processed for histopathologic examinations 48 hours after the final assessment. Result: The average motor function score was significantly higher in the TMZ group(3.20 $\pm$ 0.77 vs 1.13 $\pm$ 1.25 at 2 hours, 3.50 $\pm$ 0.76 vs 1.45 $\pm$ 1.57 at 24 hours, and 3.91 $\pm$ 0.30 vs 1.86 $\pm$ 1.86 at 48 hours after operation; p value$\leq$0.05). Histologic observations were correlated with the motor scores. Conclusion: The results suggested that trimetazidine reduced spinal cord injury during aortic clamping and that it may have clinical utility for the thoracoabdominal aortic surgery:

Airway Management Using the I-gel Supraglottic Airway Device in Patients with Grisel's Syndrome -Case Report- (그리셀증후군 환자에서 I-gel 성문상기도유지기를 사용한 기도관리 -증례보고-)

  • Grisel's syndrome is a non-traumatic subluxation of the atlantoaxial joint with an inflammatory condition in the adjacent soft tissues. Due to the instability of the cervical spine, careful airway management is crucial to prevent potential cervical spinal cord injury following airway manipulation. We successfully secured the patient airway using a supraglottic airway device (I-gel) in a patient who had previously diagnosed with Grisel's syndrome. The operation was successfully completed, and the patient recovered without any neurological complications. I-gel can be a good option for airway management during general anesthesia in a patient diagnosed with Grisel's syndrome.

Capsaicin Induces Acute Spinal Analgesia and Changes in the Spinal Norepinephrine Level (Capsaicin에 의한 척수 수준에서의 급성 진통효과와 Norepinephrine의 변화)

  • Central analgesic effect of capsaicin was assessed by the tail flick reflex (TFR) test, using male Sprague-Dawley rats under anesthesia with pentobarbital sodium (induction with 40 mg/kg and maintenance with $4{\sim}8\;mg/kg/hr$). Level of norepinephrine in the spinal cord was also measured. Capsaicin, $35{\sim}150\;{\mu}g$, was injected intrathecally, and the TFR latency was measured before, 10, 30, and 60 minutes after the drug administration. TFR latency was increased 100% or more immediately by intrathecal capsaicin, from 2.9 seconds to the maximum of 7.0 seconds at 10 minute after the drug; P<0.01. The increase in TFR latency was maintained during the course of experiment of 2 hours. Concomitant reduction of NE content in the spinal cord was observed; from 16 ng/mg protein to 7 ng/mg protein. On the other hand, subcutaneous injection of capsaicin of 50 mg/kg did not change the TFR latency although the NE content reduced similarly to the case of intrathecal injection. Pretreatment of the animal with 0.5 mg/kg of MK-801 reversed the increase of TFR latency and NE reduction induced by intrathecal capsaicin. These results suggest that capsaicin causes analgesia at the spinal cord level by activating the excitatory amino acid-NE-dorsal horn interneurons axis of the descending inhibitory pain modulation pathway.

The Effect of the Swimming Exercise by Load on Concentration of BDNF in Serum and Behavioral Change of CNS Injury in the Rats (부하유무에 따른 수영운동이 중추신경계 손상 흰쥐의 혈청 BDNF 농도 및 행동변화에 미치는 영향)

  • The purposes of the present investigation was to evaluate the effects of loaded and unloaded swimming stimulation after central nerve system injury in the rats. SCI model rats were damaged in L1-L2 injected with 6-OHDA. The twenty one Sprague-Dawley adult male rats weights($200\pm10g$) were randomly divided into control group and 2 swimming groups and then swimming groups divided into 15 minute unloaded swimming group and 15 minute loaded swimming group by swimming intensity. Behavioral Change was evaluated by the BBB(Basso, Brestti, Brenahan) scales test and the maximal angles of the inclined board on which the rat could maintain its intial position for the progressive locomotor recovery. Using enzyme-linked immunosolbent assays(ELISA), we measured concentrations of brain-delived growth factor(BDNF) in serum after swimming. There was significant change of BBB scores in control group as compared to unloaded swimming group and loaded swimming group(p<.05), and unloaded swimming group were significantly higher than loaded swimming group(p<.05). The maximal angles of the inclined plane test were higher in the unloaded swimming group and loaded swimming group than the control group(p<.05), and loaded swimming group were significantly lower than unloaded swimming group(p<.05). There were singnificant difference of concentration of BDNF in serum change in each group(p<.05). The results suggest that swimming applied from the early phase after spinal cord injury may be beneficial in the early recovery of motor function.
