  • 성대의 마비는 그 원인과 정도에 따라 애성, 연하장애, 호흡장애 등 다양한 기능 결손을 초래하게 되므로, 마비된 성대에 의한 불편을 외과적으로 처치하기 이전에 반드시 마비의 원인에 대한 이해와, 환자의 불편에 대한 정확한 이해가 선행되어야만 적합한 술식을 선택할 수 있다. 예를 들어 두개저수술의 합병증으로 갑자기 발생된 성대마비와 갑상선암에 의하여 서서히 진행된 성대마비의 경우 그 예후나 경과가 전혀 다른 질환이라 할 수 있기 때문이다. (중략)

  • 우리나라의 손 마비환자의 수는 매년 증가하고 있다. 병원에서 진행하는 일반적인 재활치료는 비용이 많이 들기 때문에 손 마비환자가 가정에서 재활운동을 하는 경우가 많아지고 있다. 본 논문에서는 손 마비환자들이 가정에서 재활운동을 할 시, 개인별 맞춤 케어를 제공해주기 위하여 스마트 글러브를 이용한 개인별 움직임 패턴을 분석하는 방법에 대해 기술한다. 손 마비환자는 움직임 패턴 분석을 통해 자신의 각 손가락 마비 정도를 정확히 인지할 수 있게 될 것으로 기대한다.

Hemiplegic Migraine Presenting with Unilateral Facial Palsy: A case report (편측안면마비로 발현한 편마비편두통 1예)

  • Hemiplegic migraine (HM) is a rare subtype of migraine with aura and is accompanied by a fully reversible motor aura. HM can occur in two forms: familial or sporadic. Currently, three genes are related to familial HM. Typically, HM occurs in the first or second decade of life and involves gradually progressing aura symptoms in succession, accompanied by headaches. The aura includes visual, sensory, motor, aphasic and often basilar-type symptoms. Motor aura (weakness) is related to the regions where the sensory aura is involved, and it usually starts at the hand before spreading to the arm and face. Aphasia is a common form of speech aura, but does not typically present as a difficulty in understanding. In this case report, the sensory-motor aura started at the right face and then gradually progressed to the right leg without any symptoms in the ipsilateral upper extremity. To the best of my knowledge, there has been no previous case report for the presentation of a hemiplegic migraine, as in this case report. As there is a possibility of misdiagnosis of Bell's palsy at the early stage of this case, this case report suggests that a physician should consider the rare possibility of stroke or HM when a patient presents with unilateral facial palsy.

Effect on Myopia Progression Wearing Eye-glasses for School Children (학령기 아동의 안경착용이 근시진행에 미치는 영향)

  • Purpose: We analyzed the influence of myopic's eye-glasses wearing on myopia progress after cycloplegic refraction. Methods: The 33 people (66 eyes) were school children from 8 years to 12 years having no experience eye-glasses wearing, they were taken cycloplegic refraction at the 100th, the 200th and 300th days in order to evaluate myopia progression. We investigated the eye-glasses wearing group (experimental group, 32 eyes) and the non-eye-glasses wearing group (control group, 34 eyes). The eye-glasses power of the experimental group were -0.50 D, -0.75 D, -1.00 D, -1.25 D and -1.50 D. We compared experimental group with control group for myopia progress according to period, age, and refraction error and investigated the myopia progress according to the eye-glasses power of experimental group. Results: At the 300th day from the first cycloplegic refaction, spherical equivalent for the experimental group increased as -1.03${\pm}$0.43 D (t=13.36, p<0.001) and for the control group increase as -0.61${\pm}$0.35 D (t=10.05, p<0.001) and two groups were statistical difference. Myopia power for experimental group increased 60.75%, for control group increased 56.66% at the 300 days. According to eye-glasses power increased 41.19${\pm}$15.25% at -1.50 D, 36.74${\pm}$19.29% at -1.25 D, 56.57${\pm}$20.21% at -1.00 D, 87.26${\pm}$49.38% at -0.75 D and 106.69${\pm}$59.60% at -0.50 D. Conclusions: The myopia power for the eye-glasses wearing group was 0.46 D faster than the non-eye-glasses wearing group at the 300th day from the first cycloplegic refraction. We will consider the effect of non-eye-glasses wearing to protect the progressing myopia and prescribe the under correction for school children having no experience eye-glasses wearing.

  • 구음마비환자의 특징적 문제로는 조음 명료성의 저하오 kdns율의 장애등이 논의되어왔다. 지금까지 조음 명료성을 진단하고 치료하기 위한 연구는 활발히 진행되어 왔으나 운율 장애를 측정, 치료하기 위한 연구는 극히 소수이며 특히, 우리나라의 경우는 전무한 상태이다. 이에 본 연구자는 정상군과 이완성 구음마비환자군의 운율적 차이를 음폭, 음도차, 지속시간, 강도의 측면에서 기술하여 진단과 치료를 위한 기초 자료를 제시하고자 한다. 실험 대상은 이완성 구음마비환자군 6명과 정상군 6명이다. 정상군은 환자군과 성별, 나이, 학력, 방언을 맞추어 선정한다. 본 연구는 억양 대비가 분명히 나타나는 의문형 어미와 서술형 어미를 선택한다. 예문은 억양 곡선을 잘 나타내기 위해 유성 자음과 모음으로 구성된 것으로 한다. 종결형 어미가 분석 목표이나 예문의 길이가 너무 짧은 경우 경도환자의 특징이 잘 나타나지 않을 수 있으므로, 주어+목적어+서술어로 구성된 3어절로 구성된 예문을 사용한다.

  • Spinal hematomas are a rare but serious complication that is typically observed in the epidural space. Spinal subdural hematomas are a dangerous clinical situation because of their potential to cause significant compression of the neural elements and can be mistaken easily for spinal epidural hematomas. This paper reports a case of a severe whole lumbar subdural hematoma after a simple epidural injection that was treated with surgical decompression with excellent clinical results.

The Effect of Teratainment Task Oriented Activity on the Hemiplegia Weight Shifting of Stroke Patients (테라테인먼트 과제 지향적 활동이 뇌졸중 환자의 마비측 체중이동 능력에 미치는 영향)

  • The purpose of this study was to investigate the effect of hemiplegia on weight shift when mediating task-oriented activities in stroke patients. To this end, from September to December 2019, 33 patients with stroke were randomly assigned and divided into 17 experimental groups and 16 control groups. The experimental group provided task-oriented activities, and the control group general occupational therapy was conducted 3 times a week, 35 minutes per session for a total of 4 weeks. The weight shift of the left and right body was evaluated by the MTD balance system, and an initial evaluation before intervention and a re-evaluation was performed after 4 weeks of intervention. The result were as follows: 1) When evaluating the pre- and post-intervention within the group, the weight shift to the hemiplegia side showed an improvement in the weight shift to the hemiplegia side in both the experimental group(p<.001), and control groups(p<.001, p<.05). 2) In the pre- and post-intervention evaluation between groups, the experimental group showed a statistically significant improvement in weight shift to the control group(p<.01, p<.05). Through future supplementary studies, it is thought that task-oriented activities in the treatment of stroke patients could be usefully applied to promote weight transfer to the paralysis side.

  • 현재 많은 곳에서 생체신호를 이용하여 보다 쾌적한 삶의 환경을 구축하려는 연구가 활발하게 진행되고 있으며 뇌-컴퓨터 인터페이스(Brain-Computer Interface, BCI)기술은 미래 손꼽히는 기술 중 하나로 보고 있다. 본 논문에서는 기존에 웹사이트나 애플리케이션으로 나와 있는 윷놀이 게임을 뇌전도(Electroencephologram, EEG)를 이용한 상호작용을 바탕으로 한 윷놀이 게임의 개발에 대해 기술하고 있다. 이 게임을 통해 마비 환자나 부득이하게 손을 사용하지 못하는 경우에도 게임을 진행할 수 있으며, 뇌신경 운동에도 도움이 될 것으로 기대하고 있다.
