A case of true thymic hyperplasia in the mediastinum with ectopic thymus in the neck (종격동 진성 흉선 증식증에 동반된 경부 이소 흉선 1례)

  • True thymic hyperplasia and ectopic thymus are very rare in children. In embryologic aspect, thymus is distributed around cervical area and ends up in mediastinum. This case is simultaneous thymic hyperplasia of neck and mediastinum. Ectopic thymus in the neck and thymic hyperplasia in the mediastinum in children were reported 2 and 7 cases respectively in Korea. In Clinical aspects, these thymic hyperplasia were presented by mass. So we should suspect these benign condition to avoid unnecessary operation or biopsy. We report a case of true thymic hyperplasia in the mediastinum with ectopic thymus in the neck in a 4-month-old male infant and review the relevant literature. We believe this is the first reported case in the world of true thymus hyperplasia in the mediastinum with cervical ectopic thymus in the neck.

  • Radial artery aneurysm is an extremely rare disease and this is usually caused by iatrogenic trauma such as arterial cannulation. Most traumatic aneurysms in the extremities are false aneurysms and most cases have occurred at the level of the wrist. Very few true aneurysms of the radial artery have been reported, with most of them being iatrogenic. A right handed 38-year-old female had a true aneurysm of the distal radial artery in the thenar groove of the palm. The patient had a history of excessive handclapping and the injury was suggestive of repetitive occupational injury due to scissoring. She underwent surgical treatment for the aneurysm of the radial artery in the thenar groove of the palm of her hand.

A Case of True Precocious Puberty Complicating Congenital Adrenal Hyperplasia (진성 성조숙증으로 전환된 선천성 부신 과형성증 1례)

  • Congenital adrenal hyperplasia(CAH) is a recognized cause of precocious pseudopuberty. Some children with CAH also develop true precocious puberty with early maturation of the hypothalamic-pituitary-gonadal axis. We review a case of CAH who eventually developed central precocious puberty nine months after initial treatment with corticosteroid. A 3-year-old boy visited complaining of rapid growth, a large penis and frequent penile erections. This patient was diagnosed with CAH with elevated 17-OH progesterone and cortical hypertrophy of adrenal gland on CT scan. His gonadotropin levels were within the normal prepubertal range. Even on treatment with corticosteroid he grew rapidly and had testicular enlargement, pubic hair development and rapid bone maturation. At second admission, his gonadotropin levels were elevated both basally and in response to LHRH stimulation, suggesting that the CAH led to early activation of pubertal gonadotropin secretion(true precocious puberty). He was treated with monthly depot injections of a LHRH analog in addition to the hydrocortisone. His second sexual characteristics regressed gradually and rate of linear growth and bone maturation decreased.

Optimization of traveling-wave electroabsorption modulator using FDTD method (FDTD를 이용한 진행파형 전계 흡수 광 변조기 최적화)

  • In this paper, the microwave characteristics of traveling-wave electroabsorption coplanar waveguide modulator have been analyzed and optimized precisely by using the 3-dimensional finite-difference time-domain method (FDTD). Microwave characteristics are affected by the thickness of intrinsic layer, the width of meas, and the distance between signal electrode and ground electrode on traveling-wave type structure. In case that intrinsic layers are composed of InAsP/InGaP (1.3Q), the optimized distance between signal electrode and ground electrode, the optimized intrinsic region thickness and the width of waveguide are founded to be $3{\mu}m,\;039{\mu}m\;and\;2{\mu}m$, respectively, to minimize microwave loss and to obtain velocity and impedance matched structure. By using the FDTD, we could design the traveling-wave electroabsorption modulator more precisely.

  • An aneurysm is a focal, localized dilatation of a blood vessel. This term is most commonly applied to dilatation of arteries. However, dilatation can occur in any part of the vascular system. Primary true aneurysm of the superficial venous system that contains all the vascular layers is known to be very rare. We report here on surgically treating a case of primary true aneurysm on the dorsalis pedis vein and we briefly review the related literature.
