The effect of operating the referral center on the Length of stay of transferred inpatients in a tertiary medical center (진료의뢰센터 운영이 전원환자의 재원일수에 미치는 영향 - 일개 3차 의료기관 진료의뢰센터 경유 환자를 중심으로 -)

  • To compare differences of LOS between the group of inpatients via referral centers(the referral group) and the group of inpatients via different routes (the general group) and analyze factors which determine the difference. The 9,484 cases of newly inpatients of Mar., Jun., Sep., Dec. of year 2003 were used for the data. We calculate the Case-mix index of length of stay($CMI_{LOS}$) to compare LOS adjusted with the disease as the ratio of a patient' LOS to all patients' average LOS by 21 classification diagnosis group. We conducted t-test to compare differences of $CMI_{LOS}$ between two groups and multiple regression to analyze factors determining the difference. $CMI_{LOS}$ was 0.94 in the referral group, which was smaller than 1.01 in the general group. LOS was 7.5days in the referral group, which was 0.6day shorter than that in the general group. Multiple regression also showed that $CMI_{LOS}$ was smaller in the referral group than in the general group after controlling patients' general characteristics, diagnostic information variables. The referral centers can be said to playa role in decreasing LOS. We suggest that variety of depth studies on referral centers should be continued at the strategic level of hospital management using additional medical informations.

Analysis of Arrival Information and Status of the Patients in Emergency Department (응급의료센터 환자의 내원 정보 및 실태 분석)

  • Background: For effective and systematic management of patients in the emergency department(ED), the data on patient arrival and status in ED of Yeungnam University Hospital were evaluated. Materials and Methods: During the seven days from Apr. 1 to Apr. 7, 1998, the general patient information such as onset time and place, factors associated with transportation, causes of admission, cared department and patient disposition were recorded. Results: Total of 464 patients visited the ED during the seven days, and the mean number of patients per day was 66.3. Male to female ratio was 1:0.71. Daily staying patients were 17.3, and 83.6 patients were cared totally each day. The methods of transportation and distribution of patients according to region and event were as follows: visit by walk(57.3%). transportation by car(58.0%), place of event in residence(85.3%), regional distribution in Taegu(81.5%), and direct visit(97.4%). Cause of admission due to diseases was 74.6%. The percentages of departments which cared the patients were internal medicine 26.6%, pediatrics 16.8%, orthopedics 8.6%, neurology 8.2%, neurosurgery 7.8% and other department including emergency medicine 8.2%, respectively. Patient dispositions were admission 38.4%, discharge 61.0% and death on arrival(DOA) 0.6%, but referred patient-to-another-hospital was zero. Conclusion: Improvements in several aspects of ED's caring system such as "fast tracking" system and reinforcement of disease and trauma caring system, would be helpful for effective management of emergency patients.

Consultations to Department of Dentistry for Child and Adolescent Inpatients with Dental Trauma (치과적 외상이 있는 소아청소년 입원환자의 치과 협진 의뢰)

  • The purpose of this research is analyzing present condition, and pursuing developmental direction of consultation to department of dentistry for medical inpatient with dental trauma. This research conducted a retrospective analysis of consultation to department of dentistry for medical inpatient at Wonju Severance Christian Hospital from March 2011 to February 2017. This research analyzed chief complaint and dental diagnosis, referring department, time of consultation after dental trauma, relation between hospitalization period and time of consultation after dental trauma, relation between dental trauma and admission in the medical department due to trauma, cause and age of dental trauma occurrence, treatment of dental trauma, and revisiting rate of inpatient with dental trauma after discharge. Among all the chief complaint and dental diagnosis, dental trauma was the highest. Among all the referring departments, departments of surgery were the highest. In relation to being delayed of consultation term after dental trauma, it turned out that there's pretty high mutual relation between the hospitalization period and the consultation term after dental trauma, and the hospitalization period had the quantitative influence on the consultation term. Statistically, dental trauma and admission in the medical department due to trauma had significant relations. In case of those patients due to car accidents, they had dental trauma without exception. Statistically, patient's group with dental trauma in hospital room was younger than patient's group with dental trauma in non-hospital room. Statistically, dental trauma and treatment in hospitalization had no significant relations, and dental trauma and revisiting department of dentistry after discharge had no significant relations. For the child and adolescent inpatients hospitalized for car accidents, there is high possibility of accompanying dental trauma, and dental consultations can be delayed. For the young child inpatients, the possibility of dental trauma occurrence in hospital room is high. Moreover, treatments during hospitalization and revisiting department of pediatric dentistry after discharge are not performed well for inpatients with dental trauma. Regarding these results, it can be an alternative that improving of consultation system, dedicated workforce placement and having dental facilities and equipment in the hospital ward, education to medical doctor and dentist, communication between medical doctor and dentist. This research can be used as a study material of medical and dental departments, and it's expected to be committed to developing of consultations to department of dentistry for child and adolescent inpatients with dental trauma.

Treatment of Patients with Cancer in a Secondary Hospital in Korea (국내 일개 2차 병원의 암환자 치료 실태)

  • Purpose: This study aims to investigate treatment of cancer patients at a secondary hospital. Methods: A retrospective analysis was performed with electronic medical records of cancer patients admitted to a secondary hospital from January 1, 2009 through September 31, 2017. Results: A total of 223 patients were studied. Sixty-nine patients were hospitalized for supportive care after receiving a surgery, chemotherapy, and radiotherapy at a tertiary hospital, 58 patients for other supportive care, 53 patients for symptom control, 16 patients with a decision not to take active cancer treatment, and 27 patients for treatment of cancer that was diagnosed during their hospital stay. Among 75 patients who were discharged to other institutions, 50 were transferred to tertiary hospitals, 10 to long-term care hospitals, eight to hospice hospitals, four to nursing homes and two to secondary hospitals. Comorbidities were found in 120 patients (53.8%). For patients who consulted with more than one department, more consultations were for non-cancer diseases than cancer. Seventy-three patients had a do-not-resuscitate order. Conclusion: For treatment of cancer patients, it is needed to establish a cooperation system among medical institutions and provide comprehensive management including treatment of comorbidities.

강원지역 보건진료원에 관한 업무 분석 연구

  • 본 연구는 전국의 보건진료원이 하는 업무를 분석하여 우리나라 보건진료원 제도 정착에 도움을 주는 기초자료를 제공하는 연구의 일환으로 강원도 지역에서 실시하였다. 연구대상은 강원도에 있는 보건진료원 전수로 하였고 이중 응답자수는 108명이었다. 연구도구는 문헌과 간호교육자들에 의해 작성 된 구조화된 설문지를 사용하였으며 수집된 자료는 SPSS를 이용하여 빈도, 백분율, 평균 및 피어슨 상관계수를 구하였고 유의성 검정을 위해서 t-test, ANOVA의 통계방법을 사용하였다. 본 연구결과를 통하여 얻은 결론은 다음과 같다. 1) 조사대상자인 보건진료원의 평균 연령이 31.5세이며 과반수의 보건진료원이 30세 미만 이었고 기혼자가 마혼자보다 약간 많았다. 보건진료원의 반수 이상이 현재 가족과 동거하고 있었고 학력은 3년제 간호전문대학 졸업자가, 경력은 3년 미만인 사람이 대부분이었으며 종교를 가진 사람이 대부분 이었으며 종교를 가진 사람이 안가진 사람보다 더 많았다. 또한 보건진료원의 근무지역 조건은 대부분이 을지에서 근무하고 있었고 대부분의 보건진료원이 신축된 보건진료소 시설에서 업무를 수행하고 보건진료소 내의 숙소에서 거주하는 것으로 냐타났다. 2) 보건진료원이 담당하는 평균 주민수는 1,660.8명 이었으며 과반수 정도의 보건진료원이 $501\sim1,000$명이 이상적인 적정 담당 주민수라고 생각하고 있었다. 강원도 주민의 연평균 보건진료소 이용자수는 4,099.3명 이었고 이용 주민수가 5,000명 이상인 보건진료소도 11개소 12.9 %나 되었다. 3) 보건진료소 사업대상지역 내에 있는 보건 의료기관은 약방 및 약종상이 62.1 %로 가장 많이 분포되어 있었고 보건지소도 16 %나 사업대상지역 내에 함께 있는 것으로 나타났다. 지역주민의 보건의료기관 이용은 보건진료소가 59.0 % 로 지역주민이 가장 많이 이용하고 있었고 보건 진료원이 가장 많이 이용하는 의뢰기관은 뱅 의원이 66 %, 보건소가 36.4 %로 나타났다. 또한 보건진료소의 보건의료기관과의 협조관계는 보건소와는 과반수 정도가 잘 협조하고 있다고 응답 한 반면 보건지소와 잘 협조하고 있다고 응답한 율은 37.6 % 밖에 안되었다. 4) 보건진료원이 업무영역 수행 정도를 살펴 보면 5점 만점에 통상질환관리가 3.69점, 사업 운영 관리 및 지도는 3.45점, 모자보건 및 가족계획은 3.28 점, 지역사회 조직 및 개발은 3.27 점, 보건정보체계 개발 및 수집은 3.17 점, 사업 계획 수립은 3.14 점, 지역사회 보건관리는 3.13 점의 순으로 나타났다. 보건진료원의 업무영역을 l 일 8 시간으로 하여 l 주 44 시간을 기준으로 측정하면 통상질환관리 18.56시간, 지역사회 보건관리 5.67 시간, 모자보건 및 가족계획 5.52 시간, 사업 운영관리 및 지도 4.10시간, 지역사회 조직 및 개발 3.05 시간, 보건정보체계 개발 및 수집 2.94 시간, 사업계획 수립 2.89시간의 순으로 나타났다. 5) 보건진료원의 업무영역별 수행 소요시간의 상판판계를 살펴보면 지역사펴 조직 및 개발을 위 해 소요한 시간은 사엽계획 수립 소요시간 및 보건정 보체계 관리 소요시간과 순상관관계를, 사업 계획 수립 소요시간은 지역사회 보건관리, 모자보건 및 가족계획 관리 소요시간 및 보건정보체제 관리 소요시간과 순상관관계를 나타냈다. 또한 통상질환관리 소요시간은 지역 사회 조직 및 개발, 사업계획 수립, 지역사회 보건관리와 모자보건 및 가족계획 관리, 사업운영 관리 및 지도, 보건정보체계 관리 소요시간과 역상관관계를 나타내었다. 6) 보건진료원의 총 업무수행 정도를 잘펴보면 업무수행 점수의 평균은 87.5점이었으며 보건진료원의 근무지가 병지이고 보건진료소의 시설상태가 나쁜 경우 업무수행 점수가 높은 것으로 나타났으며 업무수행 정도와는 별 차이가 없었다.

PSYCHIATRIC CHARACTERISTICS OF CHILD PATIENTS WITH INVERSION OF CHROMOSOME 9 - A PRELIMINARY STUDY - (9번 염색체 전위를 지닌 환아들의 정신과적 특성 - 예비적 연구 -)

  • Objectives:Few studies have examined the psychiatric properties or child developmental problems associated with inversion of chromosome 9. The purpose of this study is to examine the psychiatric properties of child patients who have inversion of chromosome 9, focused on behavioral problems and child developmental problems like motor or language developmental delay, intellectual impairment, and growth retardation. Methods:1) The authors examined the cases referred for cytogenetic examination from 1984 to 2000 at Seoul National University Hospital in Korea. The cases with the examination result of inversion of chromosome 9 were collected and informations about the departments which referred and the main reasons for referral were also checked. 2) 12 child subjects with inversion of chromosome 9 and their parents underwent psychiatric interview and parent questionnaire(child and adolescent past history questionnare, CBCL). 45 normal students whose sex and age were matched to patients were selected as a control group. Results:1) There were 165 cases of inversion of chromosome 9. The major departments which referred were Obstetrics and Gynecology(47.3%), Pediatrics(23.6%) and Child and Adolescent Psychiatry(17.0%). The major reasons for referral from the Pediatrics and the Child and Adolescent Psychiatry department (67 cases total) were intellectual impairment(35.8%), language or motor developmental delay(31.3%), suspected Fragile X syndrome(23.9%), and growth retardation(20.9%). 2) Compared to normal control group, the rate to be included in the clinical range with regard to the social problems profile was higher in patient group according to the CBCL results. The patient group had language and motor developmental delay. Conclusion:There is a possibility of inversion of chromosome 9 to be associated with child developmental problems or behavioral problems. This study is the first approach to evaluate the developmental aspects associated with inversion of chromosome 9.

Development and Analysis of System Dynamics Model for Predicting on the Effect of Patient Transfer Counseling with Nurses (간호사의 전원상담 효과 예측을 위한 시스템다이내믹스 모델 개발 및 분석)

  • Purpose: This study aimed to construct a management model for patient transfer in a multilevel healthcare system and to predict the effect of counseling with nurses on the patient transfer process. Methods: Data were collected from the electronic medical records of 20,400 patients using the referral system in a tertiary hospital in Seoul from May 2015 to April 2017. The data were analyzed using system dynamics methodology. Results: The rates of patients who were referred to a tertiary hospital, continued treatment, and were terminated treatment at a tertiary hospital were affected by the management fee and nursing staffing in a referral center that provided patient transfer counseling. Nursing staffing in a referral center had direct influence on the range of increase or decrease in the rates, whereas the management fee had direct influence on time. They were nonlinear relations that converged the value within a certain period. Conclusion: The management fee and nursing staffing in a referral center affect patient transfer counseling, and can improve the patient transfer process. Our findings suggest that nurses play an important role in ensuring smooth transitions between clinics and hospitals.

  • In the Soviet Union School health services are provided as an integral part of the health care delivery system, which is under the Ministry of Health. This paper presents an overview of the Soviet Union's health care delivery system, the model for the delivery of school health service, the role and training of school personnel involved in school health services and implications the Soviet model may have for the countries. 1. School health services are a part overall Soviet health system under the Ministry of Health. 2. Municipal and rural health departments implement programs at the local level. Diagnosis and treatment are conducted through "polyclinics" that are outreach divisions of a district hospital. 3. Education institutions for the development of health manpower, including medical schools and nursing schools, are under the Ministry of Health, as are medical and scientific search institutes.

A Study on Hypertension Management of Community Health Practitioner Posts (보건진료소 고혈압 관리사업의 실태)

  • Objectives: The purpose of this study is to provide data for the improvement of hypertension management of community health practitioner posts through the study on hypertension management in community health practitioner posts. Methods: A questionnaire was mailed to 700 community health practitioners and 205 of them responded during the period from March 13, 2003 to May 13. The survey results were analyzed using SPSS program, version 11. Results: The results are as follows; 1. There are two major activities in a hypertension prevention project for community: health education and early detection. About 57% of community health care practitioners perform a health education for community people four times a year. The 64.5% of them used the materials for health education provided from a community health center and 22.1% of them performed a post-evaluation. The main method of early detection of hypertension was measurement of blood pressure of person to visit, which was 96.1%. Other methods included home visiting(89.3%), a referral from community hospitals and other resources(49.1%), health promotion events(39.5%), and a review of medical records(35.7%). 2. For the registration and management of patients with hypertension, about 36% of community health centers used a special form and more than 50% of them have registered patients who were managed by other health care institutions in the community. A computerized program was used for the management of patients with hypertension in 68.5% of them. More than 60% of them responded that it was used for report, treatment, and follow-up of patients with hypertension.

  • This study aims to further strengthen the medical expertise to offer specialized medical care specialization strategies to gain a competitive edge through the customer segmentation of the Regional Public Hospital. Investigation period was selected to study the inpatients 26,658 people January to December 2013. The method of analysis are Cluster analysis and Decision Tree Analysis. In conclusion, female, age over 60, and diseases in musculoskeletal system and connective tissue were commonly selected as identifiers of the target market of Regional Public Hospital. Customers in this target market are loyal to specialized medical service and keeping continuous relationship with these customers through communication and monitoring of results of provided medical service would be important because the effect of word of mouth propagated to other group of customers having equivalent scale of consumption is expected. And the concentration of the scope of medical service of Regional Public Hospital and the collaboration and mutual reliance of medical service under the strategic alliance with other institutions and private hospitals are also needed.
