Status of the Development and Utilization of Critical Pathways in Medical Institutions in South Korea (국내 의료기관의 표준진료지침 개발 및 운영 현황)

  • Purpose: This study assessed the status of the Development and Utilization of critical pathways (CP) in South Korea. Methods: We surveyed 195 hospitals obtained on the Korean Hospital Association website. Data were collected using structured questionnaires for staff members in charge of CP management personnel in these hospitals. The questionnaire included CP developed by the institutions, the coverage rates and completion rates of CP in the current year, and management indicators related to CP. The questionnaire also included CP support systems and content within the institutions and questions regarding the advantages of CP utilization and obstacles associated with the CP development process. Results: Analysis of the responses from 70 hospitals (35.9% response rate) showed that a total of 1,370 CP sets were developed. The number of CP related to surgery departments was 365 (26.6%), and CP related to surgery and procedure was 1,093 (79.8%), respectively. The CP coverage rate was the most frequently used indicator to monitor the effect of CP (97.1%), followed by the completion rate (90.0%) and the length of stay in hospital (61.4%). CP managers reported that CP were highly useful for communication (3.39±0.493) and accurate information provision (3.39±0.491). The perception that CP violated doctors' autonomy in treatment was relatively low (2.87±0.645). Conclusion: It is necessary to establish an infrastructure in hospitals for CP. CP can facilitate communication and provide accurate information.

The Status Research on the Architectural Planning for Children's Specialized Public Medical Center of National University Affiliated Hospitals in Korea - Focused on the Independent Type with Ministry of Health and Welfare's Policy Support (한국 국립대학병원 내 어린이 공공전문진료센터의 건축계획을 위한 현황 연구 - 보건복지부 정책 지원 대상 독립형 어린이 공공전문진료센터 중심으로)

  • Purpose: The medical care for children in Korea is needed more than previous time as the situation changed. However, there is a few useful research for the planning of the children's specialized public medical center. This study has been started to provide basic information for the planning of Korean children's specialized public medical center. Methods: Research and architectural documentation with field surveys to 5 of 14 children's specialized public medical center, data have been analyzed for characteristics of children's medical care. Results: The result of this study can be summarized into three points. The first one is that children's medical care can be categorized independent and integrated type according to connection with main hospital. Independent type has vertical or horizontal form from spatial composition. The second one is that spatial composition are allocated by typical service, outpatient, central, and inpatient ward zones. The outpatient zone has horizontal/vertical type, central zone has concentrated/separated type by spatial and treatment situation of medical center. The third one is that children's hospital school is the specific program and healing environment for children. Implications: It is necessary to make typical model for national wide medical care for children in Korea.

An Increase of Patients Diagnosed as Precocious Puberty among Korean Children from 2010 to 2015 (통계자료를 통한 국내 성조숙증 진료현황 분석)

  • Objectives The purpose of this study is to emphasize the importance of preventing precocious puberty. This study assessed current number of the patients with early puberty and their medical expenses from 2010 to 2015. Methods Using the data from Korean Statistical Information Service and Heathcare Bigdata Hub, number of patients diagnosed with precocious puberty by gender, age, and year from 2010 to 2015 were reviewed. Also, annual medical insurance expenses and the incidence of leuprorelin use were reviewed. Results Number of the patients with precocious puberty has increased from 29,251 in 2010 to 75,945 in 2015. Total medical insurance expenses have increased from 25,716,431 won in 2010 to 56,367,981 won in 2015. The use of lueprorelin also has increased annually from 11,097,590,000 won in 2010 to 21,617,585,000 won in 2015. Conclusions As a result, the patients diagnosed with precocious puberty are increasing in number, and their medical costs have been rising as well. It may be necessary to control the environmental causes of precocious puberty to reduce not only the physical and psychosocial health problems, but also the social costs.

The prevalence and association factors of unmet dental care needs in Korean adults: The 5th Korea national health and nutritional examination survey (우리나라 성인의 미충족 치과진료 현황 및 관련요인: 제5기 국민건강영양조사 자료를 이용하여)

  • Objectives: The purpose of the study is to investigate the degree of disparity in unmet dental care needs in Korean adults. Methods: Using data from the 5th Korea National Health and Nutrition Examination Survey, 9,573 adults between 25-54 years old were selected. The reason for unmet dental care needs was analyzed by chi square test and logistic regression analysis. Results: Those having unmet oral health care needs accounted for 41.3%. The majority of the reason (1,036 persons) was "busy with school or work" and the second reason was financial burden (1,028 persons). Those who were female individuals (OR: 1.14, CI: 1.02-1.27), having higher income (OR: 0.85, CI: 0.72-0.99), and perceiving poor oral health status (OR: 5.68, CI: 4.64-6.95). Conclusions: It is necessary to extend and implement the nationwide public assistance of dental care services among the second-to-the bottom and low income people.

Current Status of Clinical Practice for Gastric Cancer Patients in Korea -A Nationwide Survey- (전국 위암 환자 진료 현황에 관한 설문조사 결과)

  • This nationwide survey was conducted to evaluate the current status of clinical practice for gastric cancer patients in Korea. The Information Committee of the Korean Gastric Cancer Association (KGCA) sent questionnaires containing 45 items about the preoperative diagnosis, medical and surgical treatment, and postoperative follow-up for gastric cancer patients to all 298 KGCA members in 108 institutes. Response rates were $32.6\%$ (97/298) for individuals and $59.3\%$ (64/108) for institutes. Most university hospitals responded (response rate of university hospitals: $71.6\%$, 48/67). The preoperative staging work up was performed primarily by abdominal CT, followed by bone scans, abdominal ultrasound, endoscopic ultrasound, and so on. Gastric cancer patients with stages II, III, and IV usually received adjuvant chemotherapy after a curative operation. About half of the surgeons regarded 2 cm as a safe resection margin in early gastric cancer and 5 cm in advanced gastric cancer. More than half of surgeons usually performed a D2 lymph node dissection in early gastric cancer and D2+$\alpha$ lymph node dissection in advanced gastric cancer. About $20\%$ of surgeons performed less invasive surgery and/or function-preserving surgery, such as a pylorus-preserving gastrectomy, a laparoscopic wedge resection, or a laparoscopy-assisted distal gastrectomy.

Effects of Education Status to Respond to COVID-19 and Work Fatigue of Primary Health Care Practitioners (보건진료 전담공무원의 코로나 19 대응을 위한 교육 현황 및 업무피로도에 미치는 영향)

  • Purpose: This study is a descriptive research that analyzes the current status of education for COVID-19 response tasks and factors affecting work fatigue of primary health care practitioners who have experience in dispatching to respond to COVID-19. Methods: This study collected data through an online survey from September 21 to 29, 2022, targeting 193 primary health care practitioners. The data were analyzed by frequency, percentage, mean, standard deviation, and logistic regression analysis using the SPSS 25.0 program. Results: 74.1% of the study subjects were dispatched without receiving disaster medical training to respond to COVID-19. 59.6% of the study subjects' work fatigue was above the level of being very tired. The factors that affect the work fatigue of the subjects were disaster participation experience, work intensity, compensation regulations, compensation satisfaction, and understanding of COVID-19 guidelines. Conclusion: Based on the results of this study, an educational program for systematic disaster response and preparation for primary health care practitioners in charge of public health care should be developed to efficiently cope with the occurrence of new infectious diseases in the future.

A Study on Actual Conditions for Prevention of Infections by Dental Hygienists (치과위생사의 감염 예방 실태 조사)

  • This study aimed to provide basic information on dental hygienists' practicing the prevention of infections by figuring out their actual conditions in dental clinics. The subjects of the study were the dental hygienists who participated in the continuing medical education of Incheon & Gyeonggi-do association and Seoul city association in October and November 2005 and the self-administered surveys were used for the prevention of infections. The results were as below. 1. In terms of education experiences of infection prevention, those who answered "there were" were 72 persons (42.9%) and those who followed the educational route for infection prevention were "through the in-house education from the hospital" and they were 42 persons (58%), which were highest. 2. In terms of the injury experiences, those who answered "there were" were 147 persons (87.5%) and the number of annual injury out of 147 persons with injury experiences was 7.7 time. For the tools that were damaged, 125 persons (75%) damaged the "explorer," which was highest. 3. For the experiences of being infected with contagious diseases, those who answered "there were" were 6 persons (3.6%) and there were four persons for "hepatitis B", one person for "rubella" and one person for "TB." 4. The questions with high practice scores were as in the following: "2. I wash my hands after conducting medical examinations (1.86 points)," "7. I always close the lid of a shot of Novocain after doing local anesthesia (1.86 points)" and "20. I separate and collect the wastes and give them to those who treat accumulated materials (1.85 points)". Meanwhile, the questions with low practice scores were as below: "16. I change my medical gowns (doctor wears) once a day (0.24 point)" and "I wash my medical gowns every time after examining patients with contagious diseases (0.52 points)." 5. The question with high knowledge was as below: "1. The contagion during the dental treatment is determined by source of infection, infection methods, infection routes and the host that is prone to infection (0.95 point)" and the question with the lowest knowledge was "5. HBV(hepatitis B) is destroyed after adding 95oC of heat for more than 5 minutes (0.27 points)." 6. The question with the highest organization-related factors was "I am always ready to use a mask, gloves, etc. if necessary" (0.89 points)" and the question with the lowest score was "There is a guideline that I can refer when I am exposed to dangerous situations related to the contagion in my workplace (0.33 point)." 7. In terms of the equipment conditions of protectors in medical environments, 168 persons for (disposable) mask (100%), 167 persons for disposable gloves (Latex) (99.4%), which meant that most of them were equipped with them. On the contrary, 108 persons (64.3%) are equipped with the protectors for frontal faces, which is the lowest and 165 persons (98.2%) said that they had autoclave in their disinfecting and sterilizing devices.

Dental Service System and Oral Health Providers in Correctional Institutions (구금시설의 치과진료체계 및 구강보건의료인력 현황 조사)

  • The purpose of this study is to analyze the dental service system in correctional institutions and 10 find the factors for improving inmates' oral health. This study is comprised of document review, telephone and questionnaire survey. The subjects of questionnaire survey are public health dentists and doctors in correctional institutions. They responded to questionnaire and the survey was collected from previous research and selected information about the dental service system. The findings of the study were as follow : Documentary survey 1. According to 2004's study, there are 42 dental offices in 46 all correctional institutions. 2. Criminals who took an health examination occupied 69.0% when committed to a jail in 2002's study. Majorities of them(81.5%) responded that they didn't take any oral examination. Telephone & Questionnaire survey 1. Full-time public health dentists are 26 in 2009. There is no correctional institution having oral health providers in 26 correctional institutions surveyed. 2 About 10 patients use the dental services in a day. Part-time dentists visit 4 times a month as average in 80% of institutions. 40% of institutions responded dental treatments can't be progressed conveniently because of the lack of oral health providers. 3. 80% of respondents answered that it is hard to cure prisoners, and that's because they are forbidden to get out of the institutions. 4. Only 20% of correctional institutions offered the oral hygiene instructions. There is no regular oral hygiene education for all inmates. 5. They need to increase the number of oral health providers.

산업보건관련 기록의 표준화를 위한 연구 - 산업장 건강관리실 기록지를 중심으로 -

  • 본 연구는 산업보건관련기록의 표준화를 위하여 산업장 건강관리실 기록지를 수집하여 기록의 종류, 내용과 양식을 비교 분석하므로써 간결하고 유용하고 필요한 정보를 효율적으로 제공할 수 있는 기록을 개발코자 조사연구한 것이다. 조사대상은 총 32개 산업체로써 조사기간은 1991년 2월 4일부터 4월 6일까지이며 자료 수집 방법은 우편을 이용하였다. 조사결과는 다음과 같다. 1. 근로자의 건강관련 정보기록으로 일일업무기록은 모두 쓰고 있었으며, 의약품 보호구 수불현황은 65.6%에서 사용하였고, 개인건강기록부, 매월업무보고서, 진료의뢰서는 각각 40.6%, 21.9%, 28.1%에서 사용하였다. 2. 간호사의 건강관리 업무관련정보기록은 일일간호 및 처치기록은 84.4%가 사용하였고, 현장순회일지, 환경위생점검일지 사용이 가장 낮았으며, 건강상담일지, 보건교육일지, 건강관리대상자 관리현황, 공상자 치료 및 관리현황은 제조업인 경우에는 21~26%가 사용하였고 기타 사업장인 경우에는 업거나 사용율이 매우 저조하였다. 산업안전보건법에 의한 사업장의 순회점검 지도 및 조치의 건의사항이나 작업환경개선 및 유지, 관리에 관한 사항을 수행하기 위해서는 현장순회일지나 환경위생점검일지에 대한 기록이 정확히, 철저히 이루어져야 될 것으로 생각된다. 또한 상담 및 보건교육일지 작성도 보건교육이 전반적인 산업보건 사업계획과 통합되고 지속적으로 이루어지기 위해서는 표준화된 기록양식을 개발해야 할 것으로 생각된다. 기록양식의 내용을 업종별 중요성에 따라 표준화된 기록지 개발을 제언한다.

Current Situation of Psychiatry in North Korean : From the Viewpoint of North Korean Medical Doctors (북한 의사들이 바라보는 북한의 정신의학 현황)

  • Objective : Psychiatry in North Korea is believed to seem very different from psychiatry in South Korea. However, there is nearly no information regarding psychiatry in North Korea until now. Our study aimed to get information about North Korean psychiatry. Methods : Three North Korean defectors in South Korea, whose clinical experience as medical doctors in North Korea was over 10 years, were recruited. They underwent the semi-structured interview, content of which included the clinical experience with psychiatric patients, the details of psychiatry, the treatment of psychiatric patients, the stigma of mental illness, and the suicide, in North Korea. Results : In North Korea, psychiatric department was called as 49th(pronounced as Sahsip-gu-ho in Korean). Only patients with vivid psychotic symptoms came to psychiatric department. Non-psychotic depression or anxiety disorders usually were not dealt in psychiatry. The etiology of mental illness seemed to be confined to biological factors including genetic predisposition. Psychosocial or psychodynamic factors as etiology of mental illness appeared to be ignored. Psychiatry was apparently separated from political or ideological issues. The mainstay of psychiatric treatment is the inpatient admission and out-of-date therapy such as insulin coma therapy. Stigma over mental illness was common in North Korea. Suicide is considered as a betrayal to his/her nation, and has been reported to be very rare. Conclusion : The situation of psychiatry in North Korea is largely different from that of South Korea. Although some aspects of North Korean psychiatry are similar to psychiatry in former socialist countries, North Korean psychiatry is considered to have also its unique characteristics.
