Many people in Korea suffer from a fear of dental treatments; however, only a few studies have focused on this population. This study assessed the effectiveness of cognitive behavioral therapy (CBT) and its interventions in reducing dental anxiety, especially when administered by trained dental staff. Using case studies, the authors analyzed the content of each session and examined the process of reducing dental anxiety. First, the authors observed that the results of both dental fear scale and subjective unit of disturbance decreased significantly with changes in cognitive distortions related to dental anxiety and that rapid improvements were noted after two therapy sessions. Second, the study reported that practicing relaxation (deep-breathing) and muscle relaxation techniques; using hand signals to indicate anxiety or discomfort; establishing an empathic relationship with the therapist; making an anxiety list; and graded insensitive and exposure along with rehearsal, were effective in coping with dental anxiety and in changing negative thoughts towards the dentist. Third, a simple cognitive behavioral intervention can be effective in reducing dental anxiety. In summary, a short-term, five-session psychological intervention with CBT produced a sustained decrease in the symptoms of dental anxiety, allowing the patients to receive the required procedures. The authors discussed the study limitations and the implication of their results on future research.
In Korea, the regional differences of medical facilities and man-powers are very serious recently. in order to solve rural medical problem, the comprehensive health care service is required earnestly in rural area. The present study was performed to provide the material for rural medical policy by analyzing the diseases occurring frequently in rural area and assuming the paramedical workers' abilities of medical treatment. The frequently by occurring diseases were classified by investigating. The chief complaints of 4559 subjects through home visiting for last weeks occurred in 1978. The paramedical workers' abilities of medical treatment were investigated by analyzing the clinical charts of patients treated by paramedical workers by systemic health care delivery system from, September 1977 to December 1977. The results obtained are summarized as fellows; 1. The rate of disease suffering recently for 2 weeks was 22.5% in Rural area. 2. The rate of respiratory disease was 36.%, gastrointestinal disease 18%, trauma 8% and neuromuscular disease 7.5%, respectively. 3. The coverage of treatment by health workers was 97.6% in general practitioner, 70% in community health practitioner and 42.1% in community health aid, respectively.
In this paper, we propose a home medical system that integrates a self-diagnosing disease classification system and a tele-consulting system by communication technology. The proposed disease classification system supports to self-diagnose the health condition based on oriental medical science using fuzzy neural network algorithm. The prepared database includes 72 different diseases and their associated symptoms based on a famous medical science book "Dong-eui-bo-gam". The proposed system extracts three most prospective diseases from user's symptoms by analyzing disease database with fuzzy neural network technology. Technically, user's symptoms are used as an input vector and the clustering algorithm based upon a fuzzy neural network is performed. The degree of fuzzy membership is computed for each probable cluster and the system infers the three most prospective diseases with their degree of membership. Such information should be sent to medical doctors via our tele-consulting system module. Finally a user can take an appropriate consultation via video images by a medical doctor. Oriental medical doctors verified the accuracy of disease diagnosing ability and the efficacy of overall system's plausibility in the real world.
The purpose of this study was identified about the self-perceived job stress and the relationship between turnover and job stress of dental hygienists'. This results were based for improved of job satisfaction of dental hygienists'. 214 dental hygienist in Ulsan and Kyung Nam areas were participated and this study was done from March to May in 2009. Results: In the job-related environmental characteristic, 57.5% of participant worked in dental clinic and 42.5% of participant worked in dental hospital. The majority of career year was 1~3 years, 42.5%. 38.3% of participant had turnover, and the majority of number of turnover was once, 45.1%. 33.6% of participant had 1,300,000~1,490,000 won/month, 49.5% of participant worked for 8~9hours/day and the major working service was assistant for general dental treatment, 70.1%. Younger dental hygienist had more stress about inappropriate compensation. Higher education and married dental hygienist had more stress about job demand. The number of turnover, workplace culture and experience of turnover were significantly related. In the job-related environmental characteristic, job demand, interpersonal conflict and a case of dental clinic were significantly related. Other effecting factors were interpersonal conflict in salary, organization system in working time, and working demand in conditions of 5day's working. Experience of turnover was influenced with interpersonal conflicts and the number of dental hygienist in the working place was related with job stress significantly.
Objective: In spite of increasing dental treatment for the disabled, there was a few collected data on dental treatment status and dentists' perception in Korea. The purpose of this' study is to research the current dental treatment status for the disabled in Korea and to suggest public health policies for the disabled. Material and methods: Total of 68 dentists who have treated the disabled regularly were asked to fill questionnaires regarding dental treatments for the disabled. Results: 1. It is necessary to set up public dental center for the disabled in rural area. 2. In case of dental treatments relating to cerebral palsy, ADHD, autism, the compensation for treatment time and extra investment is needed. 3. Only 39.7% of dentists charged treatment fees to the disabled as same as normal patients. Moreover 38.2% of dentists answered that they didn't charge treatment fees at all when their patient are disabled. 4.23.5% of dentists provided all of dental treatments including dental implant and orthodontic appliances in Korea. However, 54.3% of dentists reported the improvement of access to dental implant, orthodontic and esthetic treatment is needed. 5. It is imperative for the government to improve the support system for the disabled. Conclusions: Through this research only 26.5% of dentists answered that they had received the fund from the government and 11.1% of them were satisfied with the support for the disabled in Korea. This indicated the improvement of government financial support system for the disabled is needed.
As the result of a significant improvement in the economic situation and development of scientific techniques in Korea during the last 20 years, the life expectancy of the Korean people has lengthened considerably and as a result the number of old aged people has markedly increased. Such an increase of the number of aged people brought about many problems. Authors would like to take a medical look at the "Problem of old aged people" presently facing us in Korea. Currently the number of people over 65 has increased rapidly and is 1,620,000, 4% of total population. But it is still much lower than 8.9% in Japan, 10.7% in U.S.A., and 14.9% in the United Kingdom. Over 25% of these aged people were found to have at least more than one disease which requires medical care. Diseases occur in the circulatory system, 30.9%, respiratory system, 17.1%, digestive system 8.6%, mental disorders, 8.4%, malignant neoplasms, 7.0%. About 51% of the aged over 65 are under medical security benefit, mostly with partial coverage plan. Their clinic visit rate was very low (2.0% in 1981), which might be due to financial reasons. Since diseases affecting the aged progress chronically, early detection and long term care are utter most important. However there is almost no special facility, long term care center or geriatric specialist. For proper management of medical problems in the growing population of the Korean eldery expansion of medical security coverage, greater number of specialized facilities, education of geriatric special manpower and efficient operating system should be established.

Objectives: This study aimed to reveal the oral health conditions based on the dental health behaviors of a community-dwelling elderly. Methods: The subjects included 206 rural dwellers(100 men & 106 women) aged over 65 years old, who received dental health check-ups in the local public health center and its branches attached to 6 respective Myons of Chunchongnamdo Province, during the 2-month period from Jan. 1st to Feb. 28th, 2005. They were examined by dentists and given self-administered questionnaires asking about their dental heaith behaviors and subjective symptoms of gingival bleeding. Results: The oral health conditions based on dental health behavior showed that those who have not taken dental health service a year were found to have significantly greater number of missing teeth(p=0.002), DMFT(p=0.002) and CPITN(p=0.018), and those who have not observed intra-oral conditions a week to have significantly less number of filled teeth(p=0.002) and significantly greater number of missing teeth(p=0.000) and CPITN (p=0.000) than their respective counterparts. In terms of brushing, those who brushed their teeth below "3 times/day" were found to have significantly greater number of decayed teeth(p=0.000), missing teeth(p=0.000), DMFT(p=0.000) and CPITN(p=0.000) than their counterparts. In terms of time spent in brushing, those who spends "below 3 minutes" had significantly greater number of missing teeth(p=0.002) and DMFT(p=0.041), and significantly less number of filled teeth(p=0.036). According to the use of aid tools for cleaning teeth, the group who don't use them had significantly greater number of DMFT(p=0.041) and CPITN(p=0.018) than its counterpart. Classified by smoking habits, smoking groups had significantly greater number of decayed teeth(p=0.035) and CPITN(p=0.001) than non-smoking groups. Multiple regression analysis of the study data revealed that the significant factors influencing number of decayed teeth were number of brushing, sex and intra-oral observation (explanatory power of 14.2%). The significant factors for number of filled teeth were sex, intra-oral observation, use of aid tools, frequency of brushing, subjective health conditions and drinking of sweet beverages(explanatory power of 18.2%), those for number of missing teeth, number of brushing and age(explanatory power of 13.9%) those for DMFT, number of brushing, sex, use of dental service, age(explanatory power: 13.5%), and those for CPITN included smoking habits, use of dental service, use of aid tools (explanatory power: 10.8%). Conclusions: The study results revealed that the dental health behavior of the elderly population is in poor conditions and their consequent intra-oral health conditions are not good. To improve their oral conditions, public campaign and education will be needed to modify unhealthy dental health behaviors.
The purpose of this study was to investigate the image and the awareness of dental hygienists in dental hygiene and non-dental hygiene students. The subjects in this study were 414 students who were in the three-year-course department of dental hygiene and in another four-year-course non-dental hygiene department in G college in the city of Gwangju. After a survey was conducted, the collected data were analyzed. The findings of the study were as follows: 1. As a result of general awareness about dental hygienists, the dental hygiene students knew 97.5% and non-dental hygiene students knew 80.6% about dental hygientist as a job. There were significant differences(p<0.001). In regard of awareness about status in medical law, medical technician was the highest in the dental hygiene students(52.8%), and medical assistant was the highest in the non-dental hygiene students(48.8%) 2. As a result of awareness about the main tasks of dental hygienists X-ray and scaling were the highest in the dental hygiene students(42.3%), and treatment assistance was the highest in the non-dental hygiene students(32.9%). 3. In regard to awareness about image of dental hygienists, both groups were the highest in professionalism and were the lowest in negative image. All variable were differences statistically between dental hygiene and non-dental hygiene students(p<0.05).
