STUDY ON LATERAL CEPHALOGRAM OF CHILDREN WITH NORMAL OCCLUSION IN THE PRIMARY DENTITION (유치열기 정상교합아동의 측모두부방사선 계측연구)

  • The present study was designed to formulate cephalometric norms of normal occlusion for usage in orthodontic diagnosis of malocclusion in Korean children. Thirty two children, aged 4 to 6, with normal occlusion were chosen for this study, Sagittal and vertical relations were analyzed using lateral cephalogram and clinical photos and the measurements were compared with those of adults. On skeletal sagittal analysis, the mean values of the SNA and SNB angles were $83^{\circ}\;and\;78.72^{\circ}$. It showed that the mandible was retrognathic and retropositioned in comparison to those of adults. On skeletal vertical analysis, the mean values of the genial angle was $127^{\circ}$. This showed high angle pattern in children and reduction of genial angle due to counterclockwise rotation of the mandible is expected with growth. On soft tissue analysis, children showed convex pronto, obtuse nasolabial angle. On dental analysis, the mean values of the U1 to SN and IMPA were $91.04^{\circ}\;and\;86.57^{\circ}$. This showed retroclined upper and lower deciduous teeth in comparison to adults. For skeletal values, the liner values were generally greater in males than females.

Development of the Measurement Method of Extremely Low Level Activity with Imaging Plate (Imaging Plate를 이용한 극저준위 방사능 측정에 관한 연구)

  • An imaging plate(IP) detector, a two-dimensional digital radiation detector that can acquire image of radioactivity distribution in a sample, has been applied in many fields; for industrial radiography, medical diagnosis, X-ray diffraction test, etc. In this study, the possibility of IP detector to be used lot measuring radioactivity of sample is explored using its high sensitivity, higher spatial resolution, wider dynamic range and screen uniformity for several kinds radiations. First, the IP detector is applied to measure the surface uniformity for area source. Surface uniformity is measured rapidly and nondestructively by measuring the radioactivity distribution of common standard area source$(^{241}Am)$. Next, the IP is employed to study the possibility of measuring an extremely low-level activity of environmental sample. For this study the screen uniformity, shield effect of background radiation, linear dynamic range and fading effect of the IP detector is investigated. The potato, banana, radish and carrot samples are chosen to measure ultra low-level activity of $^{40}K$ isotope. The efficiency calibration of IP detector is carried out using the standard source.

A Systematic Review on Sensory Integration Intervention in Korea: Focusing on Ayres Sensory Integration (ASI) (국내 감각통합치료 중재에 관한 체계적 고찰 - Ayres의 감각통합중재(ASI)를 중심으로)

  • Objective : The aim of this study was to investigate how many studies for sensory integration intervention in Korea reflect the principle of Ayers' sensory integration through a systemic review. Methods : The systemic review was executes using RISS, NDSL, KISS, DBpia. The key words for search were 'sensory integration therapy', 'sensory integration intervention', 'sensory integration program', 'sensory integration training' to search the articles, published in 2000-2012. To analyze ASI fidelity of 49 studies included in the present study, the numbers of items were calculated if the study describe the ASI principle. Results : The IV was the most levels of evidence, and the subjects for the studies were mental intellectual disability (15), developmental disability (14) and autism spectrum disorders (13) etc. It was described the most that presenting sensory opportunities (43), challenging postural, ocular, oral and/or bilateral motor control (37) of ASI fidelity. The studies using more than 4 ASI principle were 26.5%(13), and 7 studies using 8 or more than 8 were studied in OT. Conclusion : In OT, 34 of 49 studies performed, and they used several principles of ASI. To have unity for sensory integration principle and to improve therapy professionalism the study using various data and systematic review study will be needed in the future.

Transient neonatal diabetes mellitus with macroglossia diagnosed by methylation specific PCR (MS-PCR) (메틸화 특이 PCR로 진단된 거설증을 동반한 일과성 신생아 당뇨병)

  • Transient neonatal diabetes mellitus (TNDM) has been associated with paternal uniparental isodisomy of chromosome 6, paternally inherited duplication of 6q24, or a methylation defect at a CpG island of the ZAC or HYMAI gene. We experienced a case of TNDM in which the patient presented with hyperglycemia, macroglossia, and intrauterine growth retardation, caused by a paternally derived HYMAI. An 18-day-old female infant was admitted to the hospital because of macroglossia and recurrent hyperglycemia. In addition to the macroglossia, she also presented with large fontanelles, micrognathia, and prominent eyes. Serum glucose levels were 200-00 mg/dL and they improved spontaneously 2 days after admission. To identify the presence of a maternal methylated allele, bisulfite-treated genomic DNA from peripheral blood was prepared and digested with BssHII after polymerase chain reaction (PCR) amplification with methylation-specific HYMAI primers. PCR and restriction fragment length polymorphism analysis showed that the patient had only the paternal origin of the HYMA1 gene. TNDM is associated with a methylation defect in chromosome 6, suggesting that an imprinted gene on chromosome 6 is responsible for this phenotype.

Unprovoked seizures in children with complex febrile convulsion; short-term outcome (복합 열성 경련 후 간질 발생 환아의 임상적 고찰; 단기 추적 관찰 결과)

  • Purpose : Febrile convulsions (FC) were considered to be a benign seizure syndrome that is distinct from epilepsy. But it is thought that children with complex features i.e., partial or prolonged seizures or multiple episodes of FC would bear a higher risk of developing unprovoked seizures. The aim of this study is to look into the relative significance of each criteria that define complex febrile convulsions (CFC) as a predictor of subsequent epilepsy. Methods : All children were retrospectively identified for a febrile seizure through pediatric departments of the Konyang University Hospital. Information was collected from medical records and interviews with parents. Patients with abnormal neurological examinations at presentation were excluded. Results : This study was performed from March 2000 to December 2003. Sixty-three out of 314 children (20.0%) with febrile convulsion fulfilled the criteria for CFC and forty-four children of them have been followed for 12 months or more. Ten of these (23.2%) had unprovoked seizures for 14-62 months (median $34.2{\pm}11.6$ months). The patients with partial FC showed a trend toward a higher risk (57.1%) of developing epilepsy than the patients with multiple or prolonged febrile convulsions (26.7%, 24.1% respectively). Conclusion : We found that the partial feature of febrile convulsion is associated with subsequent epilepsy.

A CLINICAL AND RADIOGRAPHIC STUDY OF DENTIGEROUS CYSTS (함치성 낭의 임상 방사선학적 연구)

  • The purpose of this study was to obtain information on the clinical and radiographic features of the dentigerous cysts in the jaws. For this study, the authors examined and analysed the clinical records and radiographs of 233 patients who had lesions of dentigerous cyst diagnosed by clinical and radiographic or histopathological examinations. And the obtained results were as follows: 1. Dentigerous cysts occurred the most frequently in the 2nd decade(38.2%) and occurred more frequently in males(67.4%) than in females(32.6%). 2. The most common clinical symptom was swelling of the jaw(33.9%), and the lesions were treated by the method of surgical removal. 3. The type of lesions was mainly observed as central type(72.5%), and size of the lesion was most frequently observed 2 - 2.9cm in the widest length. 4. The lesions were most frequently observed well-defined outline with hyperostotic border(49.8%), and smooth margin(73.4%), and homogeneous lesional radiolucency(79.4%). 5. Cortical thinning and expansion of the lesions(82.0%) were observed, and their direction were most frequently observed toward buccal side(64.0%). 6. The effect on the causative tooth were observed as tooth displacement(41.2%) and delayed root development(l9.3%), and the distance between cemento-enamel junction and lesional wall attachment of the causative tooth was mainly observed as below 2mm(79.6%). 7. The effect on the adjacent tooth were observed as loss of lamina dura(66.8%), root resorption(33.9%), and tooth displacement(31.5%). 8. The effects on the adjacent anatomic structures were observed as displacement of the mandibular canal(46.5%) and maxillary sinus or nasal cavity(72.2%).

A Glomus Tumor of the Trachea (상부 기관에 발생한 사구 종양 1예)

  • A glomus tumor is composed of modified smooth muscle cells that are similar to those of the glomus body. Its occurrence in the trachea is quite rare. To our knowledge, only 20 cases of glomus tumor of the trachea have been reported worldwide and there has been only 1 case in Korea. We report one case of a 58-year-old man with dyspnea who had a glomus tumor in the upper trachea with a review of literatures. The chest CT scan and bronchoscopy demonstrated a 2.5 cm sized lobulated tumor at the posterior wall of the upper trachea. It had an elongated shape with a broad base and with a highly vascularity. A simple resection of 3 levels of the trachearing was with a tumor and end-to-end anastomosis performed. Microscopic and immunohistochemical staining of the tumor revealed the characteristics of a glomus tumor.

Comparing and Analyzing Construction Textbook by Elaboration Theory - Centered with National Textbook of 7th curriculum and the new revised curriculum (정교화이론에 의한 건설교과서의 교과내용 비교.분석 - 7차 교육과정, 2007 개정 교육과정의 국정교과서 중심으로)

  • The objectives of this study was to apprehend the displaying components by comparing and analyzing the ten types of construction textbooks used in Technical high schools by elaboration theory by Ferigeluth, and to supply preliminary data for the following revising textbooks. To achieve them, first, comparable units were excerpted by analyzing between the national textbooks of 7th curriculum and the new revised curriculum used in Technical high schools. Second, each five types of 7th and new curriculum textbooks were compared and analyzed by seven requisites in the Elaboration theory by Reigeluth. As the results of the study, first, elaborating process like conceptual, procedural, and theoretical elaboration has been applied to the different subjects according to their properties. Second, cognitive strategy activator among the seven strategies was widely used. Whereas, analogy was not applied to all the ten types of the text books. In addition, Learning prerequisite sequence was presented with only simple picture and explanation, and it was inadequately used in practice. Third, summary was more used in 7th curriculum text than the new revised text. Fourth, six strategy components were widely used to all texts except construction and materials in the five types of texts. Lastly synthesis of theory and practice was more used than summary in theory subjects and learner control was more used in the subject of the synthesis of theory and practice.

  • Acute respiratory distress syndrome(ARDS) is severe acute hypoxic respiratory failure state with dynamic impairment in oxygen and carbon dioxide transfer with the need for high levels of supplementary oxygen and a high minute ventilation. This syndrome is caused by pulmonary edema due to increased permeability of the alveolar capillary barrier by various factors. ARDS is an uncommon, but a potentially life-threatening complication. Therefore, immediate diagnosis and appropriate therapy must be performed. The present case is post-extubation ARDS immediately occurred in an orthognathic surgery patient who are healthy 19-year-old man. He rapidly recovered from ARDS without complication by early diagnosis and proper treatment. This case report was aimed to describe the process of the development, possible causes and the management of ARDS occurred in an orthognathic surgery patient

DENTAL TREATMENT UNDER GENERAL ANESTHESIA IN A PATIENT WITH LOWE SYNDROME : A CASE REPORT (로웨 증후군 환아의 치아우식 치료 증례 보고)

  • Lowe syndrome is X-linked gene disorder, characterized by cataracts, renal dysfunction and brain abnormalities. Oral healthcare of young patients with Lowe syndrome could be easily neglected due to the uncooperative behavior or other systemic condition of the child. We are presenting a case of successful treatment under general anesthesia with uncooperative child with Lowe syndrome. A 3-year old boy with Lowe syndrome visited Seoul National University Dental Hospital for multiple caries. He had been suffering from congenital cataract, medullary nephrocalcinosis and hypotonia. Multiple caries were observed from clinical and radiographic examination. Concerning behavior management problem and possibility of aspiration due to hypotonia, dental treatment under general anesthesia was planned. Left upper primary first molar was extracted to prevent further infection. Other teeth were treated with Stainless steel crown or composite resin restoration based upon the extent and the severity of dental caries. Under general anesthesia, dental procedure was carried out successfully and safely. Considering uncooperative behavior and other medical conditions of the patient, general anesthesia could be effective. Home oral care and periodic visit to dental clinic should be emphasized to the caregiver of patient with Lowe syndrome, considering the susceptibility of dental caries and other oral manifestation.
