MILD HEMOPHILIA A DETECTED BY DELAYED BLEEDING: A CASE REPORT (지연된 출혈로 밝혀진 경증 혈우병 A에 대한 증례 보고)

  • Hemophilia A is an X-linked disorder resulting from a deficiency in factor VIII. Hemophilia A is classified into severe, moderate, and mild forms. A 12-month old boy visited for uncontrolled bleeding on the upper labial frenum area. Spontaneous bleeding occurred about 13 days ago. We sutured the wound. However, the patient returned after 6 days with large hematoma formation. Consultation to the hematologist and laboratory examination were carried out. Activated PTT was elongated and factor VIII was only 6%. He was diagnosed as mild hemophilia A. Fresh frozen plasma (FFP) was provided and hemostasis was achieved. At 5-day check-up, no more bleeding was observed. For the management of prolonged bleeding in hemophilia A patient, not only local hemostatic measures, but factor VIII replacement therapy, antifibrinolytics, and Desmopressin are also available. Mild hemophilia A often occurs without either familial or medical history. It is often first detected by dental trauma.

Analysis of cognitive factors affecting stroke patient's activity of daily living performance -Using the computerized neurocognitive function test- (뇌졸중 환자의 일상 활동 수행에 영향을 미치는 인지요인 분석 -전산화 신경인지기능검사 중심으로-)

  • This research analyzed the cognitive factors affecting stroke patient's activity of daily living performance and suggests the approaches which can contribute to the effective activity of daily living performance in the rehabilitation treatment of stroke patients. In this study, Seoul Computerized NeuroCognitive Function Test (SCNT) and MBI have been performed for 21 patients under extensive rehabilitation treatments, hospitalized in the rehabilitation clinic of A hospital after being diagnosed as a stroke caused by cerebrovascular disorders. To assess the effectiveness of the cognitive factors which affect the stroke patient's activity of daily living performance, activity of daily living performance values were set as dependent variables and 10 cognitive factors were included in the model to carry out analysis of the multiple regression analysis. The results show that stroke patient's activity of daily living performance have statistically significant correlations with divided attention, motor control and selectivir attention. In addition, cognitive factors explained 69.8% of the stroke patient's activity of daily living performance. Consequently, if divided attention and motor control are considered as a focal point of training in the rehabilitation treatment of stroke patients, we can effectively promote the improvement of the activity of daily living performanceroutine activities.

A Study on the Interface between DAS and SCADA by using Direct Method (직접연계방법에 의한 DAS-SCADA 연계 연구)

  • DAS and SCADA system are main systems, which operate and manage the power system by using computer and communication technology. But DAS and SCADA have been operated independently in Korea, so there was some time delay in the fault handling and live-line work. To improve that problems, studies on the interface between DAS and SCADA have been needed. From November 2005 to December 2007, the pilot project has been performed in KEPCO's branch offices, as a result, direct method will be applied widely all over the country until 2010 because the operation information of substation users want can be transmitted fast and it is possible to control 43RC directly although the expense for direct method is about 1.3 times larger than that for web method. To improve problems revealed during the pilot project, substation object to object communication method is proposed and interface database automatic generation and interface system diagnosis and logging functions are developed. Interface data point and communication data type are defined, substation single line diagram automatic generation or manual editing function, SCADA monitoring and controlling function, log data managing function are developed in SCADA HMI program. It is expecting to reduce the average time for each outage about 33% and to improve the reliability, efficiency of operation, execution of DAS from automatic load transfer under a fault, and to contribute to export project.

Clinical manifestation and outcome of alagille syndrome in Korea: a study of 30 patients (Alagille 증후군 환아의 임상 양상과 장기 예후에 대한 연구)

  • Purpose : The purpose of this study was to examine the clinical courses and long-term outcomes of children with Allagille syndrome in Korea, and to evaluate the prognostic potentials of identified variables. Methods : We reviewed the clinical manifestations and outcomes of 30 children with Alagille syndrome, investigated from 1984 to 2006 until the end of this study (defined as death or last visit; mean follow-up : 5 years). Results : Cholestasis occurred in 100 percent, cardiovascular abnormalities in 83.3 percent, butterfly vertebrae in 30.0 percent, posterior embryotoxon in 43.3 percent, and a characteristic facial appearance in 100 percent. At study conclusion, of these 30 patients, eight had died (26.7 percent); six related to Alagille syndrome. Five patients died of a liver disease complication. Liver transplantation was carried out in five of the 30 patients (16.7 percent) and one of these died due to hyperacute rejection. At age two, cholestasis improved in 17 of the 30 patients. Those who had severe cholestasis at 2 years of age tended to have a complication, such as liver cirrhosis or liver transplantation, or to have died. Conclusion : Hepatic complications account for the most mortalities in patients with Alagille syndrome. Careful and complete assessments should be made in children who have cholestasis at 2 years of age. Further investigations of more cases are required.

Effect of Delayed Intensification on Survival of Childhood Acute Lymphoblastic Leukemia (소아기 급성 림프구성 백혈병에서 후기강화요법의 효과)

  • Purpose : In 1970, the Berlin-Frankfurt-Munster(BFM) group introduced an intensification therapy after remission induction to reduce relapse of acute lymphoblastic leukemia(ALL) in childhood. Delayed intensification(DI) phase has been included for treatment of ALL in our hospital since the mid-1990s. The purpose of this study is to evaluate the outcome with vs. without DI phase and the outcome with two vs. one DI phase for intermediate risk patients. Methods : One hundred and thirty nine children with ALL who were treated at the Department of Pediatrics of Wonju Christian Hospital and Yonsei University Medical Center between March, 1990 and July, 2002 were analysed retrospectively. Results : Thirty-eight patients were treated with a DI phase, and 101 patients were treated without a DI phase. Among the DI patients, seven patients were treated with a double DI phase. Five-year overall survival(OS) in the low, intermediate, and high risk groups were 68%, 66% and 58%, respectively. 5-year OS in DDI, DI, and control were 95%, 86% and 40%, respectively. In the low risk group, 5-year event free survival(EFS) in DI, and control were 94% and 58%, respectively. Conclusion : Delayed intensification improved EFS on childhood ALL in all risk groups.

The Objective Image Analysis for HCC and HH with a Axial Image of Liver CT Scan (Liver CT 단면영상에서 간세포암과 간혈관종의 객관적 영상분석)

  • To distinguish between HCC and HH is one of the important test methods in determining the treatment method by determining the treatment method by distinguishing malignant growth and benign tumors in liver CT scan. Currently, the specialist is reading CT images by their subjective judgment. So, the purpose of this study is to treat reading the CT images even more objective way. The test times after injection contrast medium in this study are the before injection phase(Pre.), artery phase(35sec), portal phase(70sec) and delay phase(180sec). The general pattern change of HCC in change of contrast enhancement pattern shows 26.6% matching. And the case of HH shows 16.6% matching. In order to observe the change of HU value between HCC and HH, each average values and standard deviation was confirm and as a result, it shows the lagre difference between artery and portal phase in lesion.(HCC$19.76{\pm}23.52$, HH$60.23{\pm}29.43$). And it shows the 76.6% matching in HCC and 80.0% matching in HH. Thorough this study, to suggest a HU value as objective analysis method and if the anlaysis method was used in clinical will assist in the diagnosis.

  • This study was conducted to identify the factors affecting the quality of life of cancer patients based on demographic characteristics, health status, and mental health using the results of the 6th National Health and Nutrition Survey. The research tool used the demographic characteristics, health status, mental health, and quality of life of the KNHANES VI-2. Data analysis was conducted using SAS 9.3 version. There were significant differences in the quality of life in the study results by age, gender, economic activity, individual income quartile, subjective health status, mental health status, activity limitation, stress perception, and experience of depression symptoms. Factors influencing the quality of life of cancer patients included age, sex, income quintiles(individual), subjective health status, activity limitation, and experience of depression symptoms, which together explained 39.1% of the quality of life. To improve the quality of life of cancer patients, comprehensive management is required from diagnosis to treatment to return to society. It is also necessary to develop and apply long-term programs including multidimensional approaches and improvement of quality of life, early screening of cancer and cancer prevention education, management of depression symptoms, and social support.

  • Bypass surgery is performed for insuring continuity of a blood supply that is restricted due to obstruction of the native blood supply. Two types of surgery are commonly carried out: one is anatomic bypass and the other is extraanatomic bypass. Especially, extraanatomic bypass surgery is performed in patients who are a high risk for performing anatomical bypass surgery. The risk factors for anatomical bypass surgery are old age, infection, previous surgery and trauma. A 96-year-old patient underwent extraanatomic bypass surgery from the right femoral artery to the left femoral artery due to total occlusion of the left iliac artery, and 7 years after that operation, a pseudoaneurysm abruptly and spontaneously appeared from the bypass suture site without any predisposing factor. The lesion was revealed to be an organizing thrombus.

Dental Caries Status of 14-16 Year Old Adolescents in Yangsan Area (양산시 거주 14-16세 청소년의 치아우식증 상태에 대한 조사연구)

  • Although adolescents usually show peculiar aspects in oral health status, there have been not so many studies about it, while there have been innumerable studies for children. This study aimed at evaluating the prevalence of dental caries of adolescents in Yangsan in Republic of Korea, detecting its contributing factors, and distinguishing the high-risk group by epidemiological mass survey. Besides, the significance including early caries lesion in diagnosis criteria was assessed with minimal invasive concept. This survey was performed on 1,371 adolescents aged 14 to 16 in Yangsan as subject examinees by a single trained examiner. The dental caries status was checked by oral examination at schools and questionnaire survey was accompanied for some relevant risk factors. The data were analyzed to yield the results as follows: Regarding the caries experience, the DMFT score by WHO criteria with ECL including or not was 4.79 and 3.97 respectively. In both the criteria, female showed higher values and that of high-risk group was almost twice compared with whole examinees by Sic Index. About pit & fissure sealed rate, it revealed the tendency that the more sealed teeth, the lower DMFT values. Among the studied risk factors, only the habit of brushing before going to bed showed positive relationship (p < 0.05).

Familial Occurrence of Dentin Dysplasia Type I: Case Report (가족 내에 발생한 제 1형 상아질 이형성증: 증례보고)

  • Dentin dysplasia is a rare hereditary disturbance characterized by a dental anomaly of the dentin layer. The etiology is unclear, and this rare hereditary disturbance affects approximately one person in every 100,000. Dentin dysplasia is classified into two types, radicular dentin dysplasia as type I and coronal dentin dysplasia as type II. The characteristic clinical findings of dentin dysplasia type I are normal appearance of the crown and hypermobility of teeth. The radiographic findings are obliteration of all pulp canals, short, blunted and malformed or absent roots. Dentin dysplasia type II as coronal dentin dysplasia shows similar clinical features with dentinogensis imperfecta. This report shows a case of dentin dysplasia type I affecting one family except the father. The clinical, radiographic and histopathologic findings of this family are presented. Dentin dysplasia type I is difficult to diagnose unless dentist performs radiographic examination. If the affecting patient does not get regular dental care, dental abscesses or cysts may form spontaneously without caries. In this regard, early diagnosis is important to prevent premature loss of dentition.
