Two Cases of Bronchial Leiomyoma Initially Misdiagnosed as Bronchial Asthma (기관지 천식으로 오인된 기관지 평활근종 2예)

  • Leiomyoma of the bronchus is a very rare benign tumor of the lung. Leiomyoma is usually found in the young and the middle age. The symptom depends on the location of the tumor, it's size, and changes in the lung distal to the lesion. Obstructive symptoms due to leiomyoma could be similar to those of asthma and bronchitis, and therefore delayed diagnosis is common. The treatment of leiomyoma is conservative since there have been no reports of recurrence after limited resection. Recently bronchoscopic tumor resection has been applied to selected cases. We experienced two cases of bronchial leiomyoma initially misdiagnosed as bronchial asthma which were successfully excised by resection, end-to-end anastomosis and bronchoplasty. To prevent destructive changes of lung distal to obstruction and to preserve the pulmonary function, early diagnosis and appropriate treatment are important points of consideration.

The Effect of Hard Insole on Metatarsophalangeal Joint in Patients With Hallux Valgus (무지 외반증 환자의 hard insole 착용에 의한 중족지절관절 각도의 변화)

  • The purpose of this study was to investigate the effect of donning of a hard insole in patients with hallux valgus. Fourteen subjects were selected from patient with foot pain at Lee Chang-Heon Foot Clinic from August 4, 2000 to September 15, 2000. The hallux valgus angle and the first-second intermetatarsal angle were radiographically measured before and after donning the hard insole. Based on these two kinds of angles, a mild hallux valgus deformity group was characterized by the hallux valgus angle of less than 20 degrees, and a moderate hallux valgus deformity group was characterized by the hallux valgus angle of 20 to 40 degrees. After three weeks with the hard insole donned, the foot angles of the patients with hallux valgus were measured again. The data were analyzed by Wilcoxon signed ranks test, and the following results were obtained: 1) After the trial, both mild hallux valgus deformity group and moderate hallux valgus deformity group demonstrated that the hallux valgus angles were significantly decreased. 2) After the trial, mild hallux valgus deformity group demonstrated that the first-second intermetatarsal angle was significantly decreased. 3) After the trial, moderate hallux valgus deformity group demonstrated that the first-second intermetatarsal angle was not significantly decreased. The above findings revealed that according to donning hard insole, the hallux valgus angles of mild and moderate hallux valgus deformity groups and the first-second intermetatarsal angle of mild hallux valgus deformity group were significantly decreased. The results of this study have some limitation for generalization due to the limited number of subjects. Further studies are needed to evaluate the effect of hard insole on hallux valgus with more precise laboratory equipments and measurements in patients with hallux valgus.

  • Foreign body in the trachea & bronchus is considered as one of the frequently observed accidental cases in the field of E.N.T. and many cases & analysis of them are reported by many authors. We have, recently, experienced three cases of the foreign bodies in the trachea & bronchus and removed successfully through bronchoscopy under general anesthesia. Case A : a kind of plastic reed (2.2CM$\times$0.8CM$\times$0.8CM) was removed from the carina of trachea of a 7 year old female. Case B: a nail (4.9CM$\times$0.3CM$\times$0.3CM) was removed from the right main bronchus of a 2 year old male. Case C : a kind of plastic reed (2.0CM$\times$0.8CM$\times$0.8CM) was removed from the right main bronchus of a 7 year old female.

  • Rheumatoid arthritis(RA) is an of autoimmune inflammatory systemic disease. It is characterized by uncontrolled proliferation of synovial tissue and a wide array multisystem comorbidities. The disease may involve any joint of the body, but often statrs in the peripheral joints. It was reported that more than 50% of RA patients exhibit clinical involvement of TMJ. This report is a case report of dental management and progression for 16 months in patients who had severe bony change in TMJ involved rheumatoid arthritis Dental management was included palliative treatment such as interocclusal splints, physical therapy, mouth opening exercise. Although it was progressed rapidly osteolytic bone change during follow-up, no more advanced occulsal change and improved symptom and jaw motion. Further investigations about rule of dentistry in TMJ involvement in RA maybe needed.

신문지 재활용 공정의 일차 점착성 이물질 실시간 정략을 위한 새로운 방법

  • 본 연구는 신문지 고지의 점착성 이물질 중 Primary Stickies의 정 량에 관한 것으로 서, 실제 신문지료를 화상분석하여 실시간으로 매크로크기의 점착성 이물질올 측정할 때 효율적인 측정조건을 탐색하여 제지공정의 l 현장에서 일차 점착성 이물질을 정량 하는 새로운 측정기의 측정 기준을 제시하고자 하였다. 이물질이란 제지공정에 의도적으로 첨가되지 않은 불질의 총칭으로 dirt 및 각종 점착 성 이물질 즉, stickies를 들 수 있다. 이 가운데 스틱키{stickies}란 부드럽고, 점착성을 나타내는 이물질의 총청으로 주로 점착제와 확스에 의해 형성된다. 스틱키는 고지 재활 용의 효율성을 가장 크게 저해하는 이물질로 와이어, 펠트 및 기타 공정요소에 부착되 어 초지 시 지절을 발생시킴으로써 생산성을 저하시킬 뿐 아니라, 외관상 상품가치와 인쇄적성올 저하시키며, 최종 제품의 강도적 물성 및 가공적성에도 영향을 미친다. 특 히 국내에서는 원가절감을 위해 고지의 재활용율올 증대시키고자 전력을 다하고 있으 며 환경보호 및 용수 절감올 위해 공정수를 절감하고 궁극적으로는 무방류화를 목표로 하고 있어 토지 계 내의 점착성 이물질의 투입과 농축 현상이 심각하게 진행되고 있는 실정이다. 초지공정에서 발생하는 스틱키 즉 점착성 이물질은 미세한 스크린의 슬롯 폭인 0.15 - -0.3mm 이상의 크기를 가져 스크린에 의해 분리될 수 있는 매크로 스틱키{macro s stickies}와 이보다 작은 마이크로 스틱키(micro stickies)로 크게 분류된다. 즉 매크로 스틱키는 스크린에 의해 분리될 수 있으나 마이크로 스틱키는 스크린을 통하여 기계적 으로 분리할 수 없다는 문제점을 지니고 있다. 스틱키는 또 거동 특성에 따라 primary s stickies와 secondary stickies로 구분할 수 있다. primary stickies는 펄핑과정에서 점착 성 이물질이 파괴됨에 따라 나타나지만, secondary stickies는 지류 제지공정의 백수에 용해 되거나 분산된 상태인 1때1 이하의 콜로이 드로 폰재 한다. 이 러 한 secondary stickies 는 pH, 온도의 변화나 각종의 첨가제에 의해 웅집이나 홉착 등을 발생할 수 있는 잠재 적 문제점 등을 가지고 있다. 예를 들어 양성 고분자의 첨가는 펄프 섬유를 웅집시킬 뿐 아니라 지료에 함유된 secondary dtickies의 안정성을 저하시켜 응집, 침적시키는 작용을 한다. 따라서 분리가 어렵고, 제지공정에서 문제를 일으키기 쉬운 2차 스퇴키를 적절히 제어하기 위한 기술 개발은 용수절감 뿐 아니라, 초지공정의 침적물 절감을 통 한 공정개선에도 매우 중요한 요소기술이라 할 수 있다.

A Study on the Contamination of Solution with Suction used in Tracheostomy Patients (기관지절개술 환자의 흡인시 사용하는 용액의 오염수준 변화 연구)

  • It has been contributed to public health that the hospital has multiplied in the aspect of number and has been a large size with development of modern medical science, meanwhile the problem of hospital infection is coming out seriously. Respiratory hospital infection among hospital infections develops, very commonly from patients having taken the operation of intubation or tracheostomy, which results from a big factor that the infection developed from medical appliances used for respiration aids, contamination of solution and infection of medical staff. This study is separated into four steps-the time to use normal saline and distillation water for storaging catheter which are the cause of the infection of solution to store distillation water and catheter, not to say the catheter used when the patient who should get tracheostomy operation takes suction. The purpose of this study is to offer the basic data which are needed to check contamination degree as time goes by and nurse intervention and grope for a new nursing intervention. The target of this study is hospitalized 1D an intensive care unit having 700 sickbeds which is located in IKSAN city and it targeted patients before 7 days passed after an operaion of a tracheostormy. Materials collected were analyzed by SPSS PC+ figures program. The result of this study were as follows ; 1. The gradual contamination levels of the normal saline used In suction are showing that colony increase in proportion to the length of time. 2. while colony increases in normal saline with the lapse of time. distillation water mixed with 5cc of potadine did not show any sign of the formation of colony from its preparation until it was used for 8 hours. 3. Such variables as the period of intubation insertion. the length of hospitalization in I.C.U. the age and the level of contamination of normal saline have no inter-relationship. Therefore. as the length of normal saline used In suction. the contamination level increases with the excelleration of the contamination speed. 4. Regarding the number of suction and the contamination level of the normal saline. We can observe correlation contamination level in the 3 step of suction(mean value:13.4) and the saline which was used for one hours(r=0.702. P=0.00l). four hours(r=0.694. P=0.00l). eight hours(r=0.488. P=0.029). Further we can observe contamination in the 4 step of suction (mean value: 17 .8) well as saline used for eight hours; [for one hours (r=0.64l. P=0.002). four hours (r=0.670. P=0.00l). eight hours (r=0.57 4. P=0.008)]. Thesedays clinics use normal saline by changing it. three times a day. however. the timing of saline change and the current suction methoed should be changed given the one hour used normal saline contamination number 79.850. Regarding the number of suction and the contamination lend of the normal saline.

양성 고분자로 표면 사이징된 원지에 의한 도공층의 조기 구조화 및 도공지의 물성 개선

  • 도공지 제조 시 적절한 표면특성을 지닌 원지를 사용하는 것은 최종 도공지 품질향상은 물론 조업성 개선에도 필수적인 요소이다. 이러한 목적을 달성하기 위해 도공원지의 표면 사이징이 실시되고 있다. 도공원지의 표면 사이정 시 양성전분을 이용할 경우 서로 반대로 하전된 전분 과 섬유 사이에 발현되는 정전기적 인력에 의해서 표면 사이정 전분의 원지로의 전분 침투가 억제되어 도공원지의 불투명도, 광택도 인쇄적성 둥의 물성 향상을 꾀할 수 있다. 또 이러한 원지를 이용하여 도공을 할 경우 음전하를 띤 도공액 구성성분과 양전하를 띤 원지 표면의 정 전기적 작용에 의해 도공액의 부동화가 촉진될 수 있으므로 도공액의 표면 잔류성이 향상되 며, 도공층 공극 구조 개선에 따른 광학적인 성질의 개선도 기대할 수 있을 것으로 기대된다. 본 연구에서는 양성전분으로 표면 사이정 된 원지를 사용한 도공지와 기존의 산화전분을 이용하여 표면 사이정한 원지를 이용하여 제조된 도공지의 특성을 비교하였다. 또 양성전분에 의한 표면 사이징 효과를 보다 극대화하고 도공안료와 원지 표면과의 반응성을 증가시키기 위 해 양이온성 폴리머를 표면 사이정 시 첨가하여 표면 사이칭을 하는 방법을 평가하였다. 아울 러 양성전분과 도공안료와의 반응에 따라 도공층의 부동화가 촉진될 수 있다는 근거를 구명하 기 위해 레올로지적인 접근을 시도하였으며, 도공층의 공극 특성 관찰, 광산란 및 광홉수 계수 측정 등을 통해 양성전분으로 표면 사이정된 도공층의 구조적 특성을 분석하였다. 그 결과 산화전분에 비해 양성전분으로 표면 사이징한 원지의 경우 도공지의 불투명도가 높게 나타났다. 이는 양성전분으로 표면 사이정한 원지를 사용한 경우 도공층의 구조가 광 산란계수를 향상시킬 수 있도록 변화되었기 때문임을 확인하였다. 표면 사이징시 양이온성 폴리머를 첨가할 경우 불투명도 개선에 상당한 효과가 있었으며, 전분 대비 1% 이하의 첨 가량에서도 어느 정도의 개선이 가능하였다. 양성전분에 의한 도공액의 조기 부동화가 발현 된다는 것을 구명하기 위해서 도공액의 점탄성적 평가를 실시한 결과 산화전분에 비해 양성 전분 상에서 도공액의 storage modulus가 높고 critical strain point 역시 높은 값을 나타낸 다는 것을 확인하였다. 이는 양성전분과 도공액, 특히 클레이와의 정전기적 인력에 의한 반 응 결과로 생각된다. 양성전분을 이용한 도공원지의 표면 사이정 기술은 특히 평량이 낮은 도공지의 경우 수분 의 침투를 억제함으로써 도공공정에서의 지절을 감소시킬 뿐 아니라 불투명도 등 광학적 성질 을 개선시키고, 표면 커버리지를 향상시키는 효과를 나타낼 것으로 기대된다.

Late Holocene Paleoceanography from Core Sediments in the Admiralty Bay and Maxwell Bay, King George Island, Antarctica (남극 킹 죠지 섬 에드미럴티 만과 멕스웰 만 시추 퇴적물의 홀로세 후기 고해양환 경 연구)

  • The geochemical properties, sedimentation rates, foraminiferal distributions, and oxygen and carbon isotope records of sediment from Cores S-2 and S-19 were studied to investigate late Holocene paleoceanographic and paleoclimatic changes of the admiralty and Maxwell Bay, King George Island, Antarctica. Total organic carbon contents increased from the lower part to the upper part of Cores S-2 and S-19, whereas calcium carbonate contents decreased from the lower part to the upper part of Cores S-2 and s-19,whereas calcium carbonate contents decreased from the lower part to the upper part of Cores S-2 and S-19. Twenty-seven foraminiferal species were identified, and Globocassidurina biora was mostly a bundant in sediment samples. The sedimentation rates ranged from 24 cm/kyr to 237 cm/kyr based on /SUP 14/C-age dating of G. biora. The sedimentation rates increased rapidly in the upper part of the Cores. б/SUP 18/O values ranged from 0.3% to 6.2% and б/SUP 13/C values ranged from -3.0% to 0.0% with several fluctuations of the values. The lowest part of Core S-2, at 128 cmbsf in depth, had a /SUP 14/C-age of 3,100${\pm}$60 yr B.P. and the lowest part of Core S-19, at 230 cmbsf in depth, of 7,400${\pm}$ yr B.P. The results of geochemical and sedimentological analyses of the core sediments suggested five stages of paleoceanographic and paleoclimatic changes as follows: war,-cold stage of 7,500∼6,500 yr B.P., cold stage of 6,500∼3,600 yr B.P., cold-warm stage of 3,600∼2,770 yr B.P., warm stage of 2,770∼2,380 yr B.P. and cold-warm stage of 2,380∼2,100 yr B.P.

Analysis of lipid composition and fatty acids in poultry eggs -cage system, open barn system's hen egg, moscovy duck's egg mallard's egg- (난류(卵類)의 지질성분 및 지방산 조성 분석 - 달걀, 기러기알, 청둥오리알 -)

  • The lipid and fatty acid composition of various poultry eggs were examined after extracting yolk oils from the eggs of caged hen, open barn-reared hen, mallard, and muscovy duck. Total lipid content in 100 g of each edible portion was the highest in muscovy duck egg followed by open barn-reared hen, mallard, caged hen, and natural hen eggs. The triglyceride contents in the egg were in the order of caged hen, open barn-reared hen, natural hen, mallard, and muscovy duck. The cholesterol contents in total lipid were the highest in caged hen and muscovy duck eggs(4.5%), and the lowest in mallard(3.3%). Caged hen eggs had higher neutral lipid ratio among total lipid, but had lower ratio of glycolipid and phospholipid compared with those of the open barn-reared hens. For fatty acid composition, linoleic acid was the highest in the muscovy and linolenic acid was the highest in open barn-reared hen eggs. The contents of arachidonic acid and Eicosapentaenoic acid(EPA) were the highest in muscovy eggs. On the other hand, the content of Docosahexaenoic acid(DHA) was the highest in muscovy duck eggs.

  • The freshwater shrimp, Macrobrachium nipponensis is one of the largest species as well as one of the Important types of food. It can be found widely in rivers and swamps from Che-ju island in the south to Chung-ju in the north. The larval development of these shrimps was studied by Yu (1966) and CHUN and YU (1967), but they didn't provide any other features. Shrimps for the present study were collected from the Ntk-Dong River, near Pusan, once each month from March to December 1963. The following is a summary of the results. 1. The relationship between the carapace length (X) and the body length (Y) is: Y=2.68996X+1.14784 in female. Y=2.73121X+1.10827 in male. 2. The relationship between the carapace length(X)_ and tile basipodite length of the 2nd pereiopode (Y) is: Y=0.16910X-0.06422 in female Y=0.19410X-0.06075 in male. 3.
