산란계 질병명과 예방$\cdot$치료

  • B.S.Pomeroy 박사의 산란계 질병에 대한 예방과 치료약품을 소개함과 아울러 현재 국내에서 발생되는 질병명 및 시판되는 약약품등을 필자의 편의대로 설명하였다. 국내에서 발병되는 질병명은 한국가금협회 산란계 능력검정시(1971.8.1$\~$1972.1.31)안양 가축위생연구소에 의뢰한 가검물의 진단을 참고로 한 것인데 요약하면 CRD, 마렉병, 곰팡이성폐렴, 살모넬라 균증, 류코싸이토준증, 임파구성백혈병, 지방과자등, 대장균증, 뇨산침착증, 간파열증, 장염, 비특이성장염, 혈종, 탈홍등이다.

비타민(IV) 수용성비타민

  • 수용성 비타민은 지용성비타민과는 달리 체내에 축적이 많이 되지 않기 때문에 자주 계속적으로 공급해 주어야 한다. 수용성비타민은 주로 탄수화물, 지방, 단백질의 대사작용에 작용하며 조효소의 구성성분이다. 수용성 비타민에는 비타민 B군(群)과 비타민 C가 있다. 비타민 B군에 속하는 비타민에는 비타민$B_1$(thiamin), 비타민 $B{_2}$(riboflavin), 나이아신(niacin), 비타민$B{_6}$(pyridoxine), 판토텐산(pantothenicaicd), 바이오틴(biotin), 콜린(choline), 엽산(folacin), 비타민$B_{12}$ 등이 있다. 이들 B군에 속하는 비타민들은 대체로 자연에 함께 존재한다. 즉, 어느 한가지가 풍부한 곳에는 B군에 속하는 다른 비타민들도 풍부하게 존재하고 있다. 따라서 인간이나 동물이 순수한 어느 한가지 비타민 B의 결핍증에 걸리기는 매우 어렵다.

  • Fatty pancreas is an abnormal process of lipid deposition in cells, resulting in increased fat tissue and obesity. The result is a risk factor for cardiovascular and metabolic diseases. The aim of this study was to evaluate the usefulness of pancreatic fat as a predictor of cardiovascular disease and metabolic syndrome in pancreatic ultrasonography. In 407 patients who underwent a comprehensive screening at the W Health Care Center in Busan from September 2. 2018 to December 31, 2018, the degree of fat deposition in the pancreas was evaluated as the degree of mild, moderate. Data on non-obstructive atherosclerosis, BMI, hyperlipidemia, hypertension, and diabetes were collected to assess the association of pancreatic fat deposition with cardiovascular disease and metabolic syndrome. In addition, we tried to analyze the correlation between liver dysfunction and thyroid dysfunction as the degree of fat pancreas increased. We examined the relationship between six parameters including atherosclerosis, BMI, hyperlipidemia, hypertension, diabetes, liver dysfunction, and thyroid dysfunction among patients with fatty pancreas. We concluded that the carotid intima-media thickness of atherosclerosis, which is a risk factor of cardiovascular disease, is most closely related to fatty pancreas.

Medium-chain Acyl-CoA Dehydrogenase Deficiency in an Asymptomatic Neonate (무증상 신생아에서 진단된 중쇄 acyl-CoA 탈수소효소 결핍증 1례)

  • Medium-chain acyl-CoA dehydrogenase (MCAD) deficiency is the most common mitochondrial fatty acid oxidation disorder which is inherited as an autosomal recessive pattern. MCAD deficiency is caused by mutations in the ACADM gene; medium-chain acyl-CoA dehydrogenase gene (ACADM; OMIM 607008) on chromosome 1p31 which encodes MCAD, the mitochondrial enzyme which catalyzes the first reaction in beta-oxidation of fatty acids with medium-chain length. Here, we describe one Korean pediatric case of MCAD deficiency, which was diagnosed during newborn screening by tandem mass spectrometry and confirmed by molecular analysis. The level of hexanoyl (C6), octanoyl (C8), decenoyl (C10:1) carnitine, and C8/C2 ratio was elevated. Homogenous c.1189T>A (p.Tyr397Asn) mutation of ACADM gene was identified by direct sequencing. He has been asymptomatic and has shown normal growth and development by 25 months of age without any intervention. There was no episode of metabolic acidosis during follow-up period.

Effects of Estrogen on Nitric Oxide and Aortic Atherosclerosis in Cholesterol-Fed Rabbits (고지방식 가토에서 nitric oxide와 대동맥 동맥경화증에 대한 에스트로겐의 영향)

  • 배경: 에스트로겐이 동맥경화증의 진행을 억제하고 예방하는데 중요한 역할을 하는 것으로 알려졌으나 그 기전은 아직 명확하지 않다. 본연구에서는 에스트로겐이 혈관의 확장에 중요한 역할을 하고 있는 nitric oxide와 동맥경화에 어떠한 영향을 미치는지를 알아보고자 하였다. 대상 및 방법: 뉴질랜드종 웅성 가토 40마리를 실험동물로 하여 정상식이군(ND), 정상식이와 0.02ig의 estradiol-2 patch 투여군(END), 0.5% 콜레스테롤 투여군(CD), 0.5% 콜레스테롤 투여와 0.02 $\mu\textrm{g}$의 estradiol-2 patch 투여군 (ECD)의 네군으로 나누어 12주간 실험을 하여 혈중 estradiol-2, nitric oxide, cholesterol 등을 측정하고 흉부동맥과 복부동맥의 동맥경화 병변을 관찰하였다. 결과: 혈중 estradiol-2의 농도는 ED, ECD 군에서 ND, CD 군보다 의의 있게 높았다. (p<0.0001). 혈중 nitric oxide 농도는 정상 대조군 (ND)을 제외한 나머지 세군(END, CD, ECD)에서 모두 의의 있게 감소하였다.(p<0.001). 혈중 콜레스테롤은 총콜레스테롤, LDL콜레스테롤, HDL 콜레스테롤은 CD, ECD의 두 군에서 모두 증가하였으나 의의는 없었다(p<0.05). 혈중 콜레스테롤과 중성지방의 농도는 에스트로겐에 의한 영향이 관찰되지 않았다. 흉부 및 복부동맥의 동맥경화 병변은 ND군과 END군에서는 발생하지 않았으나 콜레스테롤을 투여한 CD군과 ECD군의 두 군에서 현저하게 발생하였으며 혈관별 발생정도의 차이는 없었다. 에스트로겐에 의한 병변의 발생 차이는 관찰되지 않았다. 광학현미경상 동맥경화병변은 거의 지방을 탐식한 대식구의 증식으로 이루어졌었으며, 괴사나 섬유화 병변은 관찰되지 않았다. 전자현미경상에서는 내피세포의 광범위한 손상이 관찰되었다. 병리학적으로 에스트로겐에 의한 영향은 인정하기 어려웠다. 결론: 고지방식은 혈중 nitric oxide의 생산을 크게 저하 시키며, 그 원인으로는 지방의 과산화에 의한 내피세포의 손상때문인 것으로 추정된다. 또한 estradiol-2 patch 처치에 의해서 nitric oxide가 감소된 것은 그이유를 설명하기 곤란하나 성차이 및 에스트로겐의 투여방법에 의한 어떤 영향이 있었을 것으로 추측된다. 피부를 통한 에스트로겐의 투여는 혈중 콜레스테롤이나 중성지방의 농도에 영향을 주지 못했으며, 동맥경화의 진행에도 영향이 없었다. 에스트로겐의 동맥경화에 주는 영향에 대해서는 지속적이고 광범위한 연구가 더 필요할 것으로 사료된다.

Suppressive Effects of By-Product Extracts from Soybean on Adipocyte Differentiation and Expression of Obesity-Related Genes in 3T3-L1 Adipocytes (대두부산물의 지방세포분화 유도유전자의 발현저해 및 전지방세포 분화 억제 효과)

  • Soybean is known to contain various phytochemicals that are related to anti-oxidant, anti-inflammatory and anti-obesity effects in mice and humans. The anti-obesity effect of by-product extracts from soybean on the differentiation of 3T3-L1 pre-adipocytes to adipocytes was investigated by suppressing adipocyte differentiation and lipid accumulation with Oil Red-O assay and quantitative PCR. In inducing differentiation of 3T3-L1 pre-adipocytes in the presence of an adipogenic cocktail, isobutylmethylanthine (IBMX), dexamathasone, and insulin, treatment with filtrated soybean soaked water, soybean milk, and soycurd residue from soybean curd processing significantly decreased mRNA expression of obesity-related gene such as PPAR${\gamma}$, Fabp4, and Scd1, adipsin, apolipoprotein (APOE) and adiponectin (ADIPOQ) without any significant cytotoxicity. We also determined the well-known isoflavones in soybean, such as daidzein and genistein, in the by-product extracts. Taken together, we suggest that soybean by-product extract showed anti-obesity effect by suppressing adipocyte related gene expression, and that by-products collected during soybean curd processing may be a good candidate as an ingredient in health care products.

Effect of aerobic exercise on peroxisome proliferator-activated receptor α anti-inflammatory in white adipose tissue (백색지방조직에서 peroxisome proliferator-activated receptor α 항염증에 대한 유산소 운동의 영향)

  • It was investigated whether a combination of PPARα activator fenofibrate and swimming exercise (H/F/S) would have a beneficial synergistic effect on improving inflammation of white adipose tissue compared to the single prescription of fenofibrate (H/F) and swimming exercise (H/S) in male mice that fed high fat diet. The body weight, weight of white adipose tissue and total cholesterol levels in the serum increased in mice-fed high fat diets (H) compared to mice-fed low fat diets (L). Compared to H, both H/F and H/S decreased these. These levels reduced by fenofibrate were more effectively reduced by the combination of fenofibrate and swimming exercise (H/F/S). As a result of examining the expression of inflammatory cytokines genes and fatty acid oxidation genes in white adipose tissue, H increased compared to L, both H/F and H/S decreased compared to H, and H/F/S decreased further compared to H/F. Thus, this study revealed that the combination of fenofibrate and swimming exercise in male mice fed high-fat diet suppresses inflammation of white adipose tissue caused by obesity through promoted fatty acid oxidation more effectively than the fenofibrate alone, and suggested a practical way to improve inflammation of adipose tissue caused by obesity.

  • The recent work on symptomatology and therapy of essential fatty acid (EFA) deficience has been reviewed. EFA deficiency is due to an absence of dietary linoleic acid which cannot be synthesized endogenously in man. The diagnosis of EFA deficiency is based on clinical features such as the appearance of scaly skin rash, sparse hair growth or failure to thrive, which occur late in the course of EFA deficiency and biochemical features occurring within two weeks of fat-free diet. The amount of linoleic acid required to prevent EFA deficiency varies with age of the patient and route of fat administration.

  • Short-chain acyl-CoA dehydrogenase (SCAD) deficiency is an autosomal recessive hereditary metabolic disorder of mitochondrial fatty acid beta-oxidation. Mutations in the ACADS gene cause short-chain acyl-CoA dehydrogenase deficiency, which is characterized by developmental delay, hypotonia, seizure, and hypoglycemia. Here, we describe one Korean pediatric case of SCAD deficiency, which was diagnosed during newborn screening by tandem mass spectrometry and confirmed by molecular analysis. The level of C4 was typically elevated 5.23 mg/dL (reference range <1.5 mg/dL). This patient had a homozygous mutation [c.1031A>G, p. E344G] in ACADS. Therefore, we present a case of SCAD deficiency in an otherwise healthy neonate and her subsequent development and growth over four years.
