The Mechanisms of Somatization (신체화의 기전)

  • It is important not only understanding the underlying psychodynamic mechanism of the somatization but also understanding the somatization as a process where biological and sociocultural factors are acting as maintaining and exaggerating the primary vague somatic symptoms. Recently, among mechanisms of the somatization biological and cognitive aspect became more important than psychodynamics. When the doctors see patients complain physical discomforts without organic foundation, they should give attention to the mechanism of symptom amplification, misinterpretation, individual cognitive characteristics and learned behavior. Psychiatric disorders which show somatic symptoms should be also evaluated. Autonomic dysfunctions linked with stress would give some clues of the mystery of the mind-body relationship.

The Study of the Initial Presentations of Wilson Disease at Diagonosis (Wilson 병의 진단시 임상 발현 양상에 대한 고찰)

  • Purpose: To review the clinical and laboratory features of patients with Wilson disease at diagnosis. Methods: In this retrospective study, records of all 20 patients, who were diagnosed as having Wilson disease at the Paik hospital in Busan from 1990 to 2000, were reviewed. Results: Out of 20 patients, 12 pateints (60%) have hepatic presentation alone, 2 patients (10%) have neurologic presentation, 4 patients (20%) have hepatic and neuropsychiatric presentation, and one patient (5%) has hematologic presentation at diagnosis. One patient (5%) has neither symptom nor laboratory finding of Wilson disease except very low serum ceruloplasmin level and positive family history. Family screening test revealed 3 cases of Wilson disease. 12 patients were revealed to be combined with liver cirrhosis at diagnosis. Conclusion: Early diagnosis and treatment is very important in patients with Wilson disease. Children or adolescents who manifest symptoms of hepatitis, who has prolonged elevation of liver enzymes, and has family history of hapatitis of unknown origin, with mild hematologic or urinary abnormalities must be suspected to have Wilson disease. Also, in adolescents with extrapyramidal symptoms or other neuropsychiatric symptoms, liver function test should be done.

Quality of Life and Influencing Factors related to the Health in Hemodialysis Patients (혈액투석환자의 건강 관련 삶의 질과 영향 요인)

  • The purpose of this study was to identify the experience symptoms and severity of hemodialysis patients and to determine the effects of each symptom on health related quality of life. This research involved 102 patients who were receiving hemodialysis at one university hospital and two Kidney Center in D city. Data collection were collected from April to May 2014. Data analysis was performed by SPSS/WIN 18.0 using ANOVA, t-test, Pearson correlation and stepwise multiple regression. Fatigue was the highest among symptoms on hemodialysis patients and dry skin, itching was in order. Factors affecting health related quality of life were income and symptoms level. These factors explain 42.2% of the variance in health related quality of life. Based on the results, it is necessary to develop various programs and apply for nursing interventions to alleviate the experience symptoms and severity and expand support in social service perspective in hemodialysis patients. Especially, we suggest that further research for developing nursing interventions for relieving fatigue, itching, and sleep disorders of hemodialysis patients.

An Analysis of the Factors Related to Musculoskeletal Symptoms among Teacher Librarians (사서교사의 근골격계질환 자각증상 관련 요인 분석)

  • This study aims to identify the symptoms and related factors of musculoskeletal disorders among teacher librarians, with the goal of raising awareness about their health and safety, and providing foundational data for the prevention of musculoskeletal disorders. An online survey was conducted with teacher librarians working in elementary, middle, and high school libraries, and data were analyzed based on responses from 204 participants. The survey provided by KOSHA was utilized to identify the musculoskeletal disorder symptoms and the collected data were analyzed using the SPSS 29.0 program. Of the 204 librarian teachers, 92.6% have experienced symptoms of musculoskeletal disorders such as pain, soreness, and tingling in relation to their work over the past year. Additionally, 20.6% were classified as normal, 44.1% as requiring management, and 35.3% as reporting pain. By body part, the highest incidence was reported in the hands·wrists·fingers, followed by the lower back, shoulders, neck, arms·elbows, and legs·feet.

Three Cases of Menopausal Hot Flush and Sweating Treated by Ascending Kidney Water and Descending Heart Fire (AKDH) Pharmacopuncture Treatment (약침을 이용한 수화조절법으로 호전된 갱년기 상열감 및 발한과다 환자 치험 3례)

  • Objectives : The purpose of this study is to evaluate the clinical effects of ascending kidney water and descending heart fire pharmacopuncture treatment for hot flush and sweating. Methods : Ascending kidney water and descending heart fire pharmacopuncture is achieved by injecting pharmacopuncture on a specific acupoint to change from the state of water-fire disharmony to harmonious state. Hwangryunhaedok-tang (黃連解毒湯) pharmacopuncture is injected on both side Gyeonjeong (GB 21 ), Pungji (GB 20 ). Total 0.4 cc was injected by 0.1 cc each point. BUM (Bear’s gall bladder, ox bezoar and musk) pharmacopuncture is injected on both side Jeonjung (CV 17 ), Jungwan (CV 12 ), Gihae (CV 6 ). Total 0.15 cc was injected by 0.05 cc each point. Treatment was done daily. The method of evaluation are Hot Flush Score (HFS), VAS and sweating areas. Results : In case 1, after treatment hot flush score was reduced from 24 to 4 points. VAS scale was reduced from 7 to 2. It took about two weeks for the symptoms to decrease by half. Associated symptoms almost did not stay at discharge. In case 2, after treatment hot flush score was reduced from 28 to 2 points. VAS scale was reduced from 10 to 3. And emotional symptoms were reduced about 70%. In case 3, after treatment hot flush score was reduced from 8 to 1 points. VAS scale was reduced from 6 to 1. And headache and chest discomfort symptoms have disappeared. Conclusions : Ascending kidney water and descending heart fire pharmacopuncture treatment is effective for improve ascending kidney water and descending heart fire energy. Therefore, it will be used to alleviate hot flush and sweating.

Response Characteristics of Perceived Stress Response Inventory in Patients with Upper Gastrointestinal Disorder (상부 위장관 장애 환자에서의 스트레스반응 지각척도의 반응특성)

  • Objectives : We investigated the characteristics of perceived stress response and relationship between some variables of gastrointestinal symptoms(esp., dyspepsia) and subscales of perceived stress response inventory(PSRI) in patients with upper gastointestinal disorder when they perceived stress. Methods : 84 patients with upper gastrointestinal disorder(gastritis, gastric ulcer, duodenal ulcer etc.) and 94 normal controls completed the PSRI developed by Korean psychiatrists. The patient group performed the questionnaire including some variables of gastrointestinal symptoms. Results : Internal consistency was statistically significant in all subscales of PSRI. The patient group was significantly higher at total score of PSRI, general somatic symptom subscale score, specific somatic symptom score than control group. As the result of stepwise regression analysis for relationship between some variables of gastrointestinal symptoms ans subscales of PSRI, specific somatic symptom subsclae closely related with illness duration, past illness history and severity of symptom, and the lowered cognitive function & general negative thinking subscale related with the existence of emotional distress. Conclusion : Patients with upper gastrointestinal disorder showed stronger perceived stress response than control group and they experiences somatic symptoms related to autonomic nervous system and/or gastrointestinal symtoms rather than emotional, cognitive, behavioral symtoms when they perceived stress. They also responded to stress as they expeirenced specific somatic symtom when they had long illness duration, past illness history, and high severity of symptom and the existence of emotional distress could develop lowered congnitive function and general negative thinking.

  • 휴대폰 전자파로 인해 두통, 불면증, 단기간의 기억력 소실 등의 신경 생리학적 증상을 호소하는 휴대론 전자파 과민증 (EHS, Electromagnetic Hypersensitivity) 환자들이 늘어나고 있다. 그러나 휴대폰 전자파 과민반응의 원인이 막연한 불안감 때문인지 혹은 전자파 노출로 인한 증상 인지에 대하여 확실하게 규명이 되지 않고 있다. GSM 방식의 휴대폰의 경우 전자파 노출 시 EHS 군을 대상으로 혈압, 맥박수 변화 등의 과민 반응에 대한 연구들이 수행되었으나 일반인과 EHS 군의 실험-대조군 연구로 생체신호, 자각증상 및 전자파 인지 여부에 대한 복합석인 연구와 CDMA 휴대폰 EHS 연구는 거의 없는 실정이다. 본 연구예서는 일반인 6명과 EHS 6명을 대상으로 30분간 300mW의 CDMA 휴대폰 전자파에 노출시 교감신경의 흥분을 나타내는 맥박, 호흡수, HRV(heart rate variability의 변화를 측정하였다. 그 결과, 일반인과 EHS 군 모두 전자파 노출여부에 따라 맥박, 호흡수, HRV의 변화가 없었다.

An Analysis of the Relationship between Climacteric Symptoms and Depression of Middle-Aged Women (중년여성의 갱년기 증상호소와 우울과의 관계)

  • The purpose of this study was to investigate the relationship between climacteric symptoms and depression of middle-aged women. Data were collected from Jul. 1 to Jul. 31, 2000 by a structured questionnaire. The Subjects were 102 middle-aged women who non-hysterectomized and ranged in age from 40 to 59. The instruments were the climacteric symptoms scale developed by Neugarten and depression scale developed by Zung. The Data were analyzed by the SPSS/PC+ program using t-test, ANOVA and Pearson Correlation Coefficient. The results of the study were as follows ; 1. Mean score of climacteric symptoms was 0.72. 2. There were statistically significant differences in the score of middle-abed women's self reported climacteric symptoms according to the age(F=3.13, P<0.05), and times of pregnancy(F=3.24, P<0.05). 3. Mean score of depression scale was 49.2. About 39% of the women displayed a variety depression symptoms. 4. Women's degree of climacteric symptoms showed a positive correlation with the degree of depression(r= 0.5393, P<0.001). This study shows possible implication for nursing intervention of middle-aged women's health to prevent and relieve climacteric symptoms.

Risk Factors Affecting Severity of Menopausal Symptoms in Early and Late Postmenopasusal Woman (초기와 후기 폐경후기 여성에서의 폐경기 증상의 심각도와 위험 요인들)

  • Objectives : The aim of this study was to assess health-related quality of life and evaluate the risk factors affecting severity of menopausal symptoms in early and late postmenopausal women based on the stages of reproductive aging workshop(STRAW) paradigm. Methods : This cross-sectional study examined 497 Korean postmenopausal women aged 41-59 years in Seoul and Gyeonggi province. We divided subjects into early postmenopause group and late postmenopause group. Menopause Rating Scale(MRS) was used to measure the quality of life. MRS scores, sociodemographic variables, smoking, alcohol use, age at menopause, and risk factors such as attitude to menopause, depression, history of premenstrual dysphoric disorder were compared between early and late postmenopause groups. Multiple regression analysis was performed in each group to assess the independent contribution of several variables. Results : Early postmenopause group showed significantly higher MRS scores, more negative attitude toward menopause, higher scores of depressive symptoms than late postmenopause group. Moderate to very severe hot flush group showed significantly, more negative attitude toward menopause, higher score of depressive symptoms, and higher MRS scores than none to mild hot flush groups. Depressive symptoms and attitude toward menopause contributed to the severity of menopausal symptom in both early and late postmenopause groups. Chronological age, age at menopause, history of PMDD contributed to severity of menopausal symptoms in early postmenopause group while marital status and occupation contributed in late postmenopause group. Conclusion : Health-related quality of life in postmenopause women was significantly lower in early postmenopause group than the late. Attitude toward menopause and depressive symptoms contributed significantly to quality of life in both early and late postmenopause groups but other variables contributed differently in each group. Further studies on clinical samples of postmenopausal women in order to confirm quality of life and its risk factor are needed to be done.
