Analysis of case reports based on dental hygiene process (치위생과정 기반의 임상치위생 증례보고서 분석)

  • Objectives : The purpose of this study was to analyse case reports performed through a dental hygiene process and provide basic data on clinical education of dental hygiene. Methods : 154 case reports which collected for six years were analysed. This study applied dental hygiene process model in dental hygiene diagnosis. Dental hygiene diagnosis was more cleared by dental a hygiene process model. Data analysis was performed by the Frequency statistics using SPSS 12.0 for Windows. Results : 1. The clients are mainly comprised 20's university student(91.9%). 2. In assessment phase, clients finished 100% test of subjective data. 3. When applied a dental hygiene process model in dental hygiene diagnosis, students have identified 23 type of dental hygiene problem and analysed dental hygiene problem frequently used as bleeding of gingiva, calculus and deposit of dental plaque. 4. In case of plan of dental hygiene intervention, Fluoride application showed the most high level(98.1%) in clinical intervention. 5. Results of intervention showed that performance rate(98.7%) of scaling is the most high level. Conclusions : Dental hygiene process model is more useful than other diagnostic models in clinical practice based on dental hygiene process.

Acute Gastric Dilatation in Rhesus (Macaca mulatta) and Cynomolgus (Macaca fascicularis) Monkeys (Rhesus 및 Cynomolgus 원숭이에서 급성위확장 증례)

  • Acute gastric dilatation (AGD), one of the common causes of emergency occurs in macaca monkeys that are accommodated as laboratory-housed nonhuman primates. This report introduces some cases of occurrence in raising primates. The primates revealed an acute gastric dilatation, including the histories that were trained by monkey chair, anesthetized for the study or intact case. The clinical signs were comatose condition with sever abdominal distension, dehydration, cyanosis and apnea. One case died by deterioration of systemic body condition and performed necropsy. The other cases recovered from the AGD by the emergency treatment using the gastric tube and fluid therapy. Necropsy revealed the huge stomach filled with water, gas and ingesta. This report suggests that etiologic factors of AGD may include non-specific factors like these cases, with special emphasis on the incidence and management of AGD in nonhuman primates.

  • 이 증례는 드물게 보는 경우로 simvastatin과 gemfibrozil을 오랫동안 함께 복용했음에도, 특이한 문제가 발현되지 않았지만, 이들을 warfarin와 함께 치료하는 경우, 아주 높은 alanine aminotransferase (ALT), aspartate aminotransferase(AST) 혈중 농도, rhabdomyolysis, 급성 신장 장애가 발생하였다. 그 후, Simvastatin와 gemfibrozil을 복용 중단시켰더니, ALT/AST는 빠르게 정상수치로 돌아온 경우이다. 이 증례 보고서는 의료인들에게 simvastatin과 gemfibrozil을 함께 혹은 따로 warfarin과 함께 복용시켜 치료할 경우, creatine phosphokinase (CPK) 와 creatinine 혈중 수치들을 포함하여 ALT/AST 농도들을 주의 깊게 모니터하도록 경각심을 주고자 한다.

  • Objectives: The modified fasting therapy(MFT) of traditional korean medicine uses very low calorie diet on a patient with chronic disease and obesity. During the fasting period of MFT, calorie intake is restricted under 600kcal. Adverse events such as hunger, heartburn, headache, and hair loss can occur because of fasting. In case of Gilbert's syndrome, Jaundice can arise without disorder of liver function and hemolysis because of increase of unconjugated bilirubin. We experienced a case of Gilbert's syndrome patient who appeared jaundice during the fasting period, so we report about that with literature review.

  • Clear cell sarcoma-like tumor of the gastrointestinal tract (CCSLTGT) is a rare malignant mesenchymal tumor of the gastrointestinal (GI) tract with a high probability of local recurrence and distant metastasis in young adults. To the best of our knowledge, only seven case reports have described the imaging findings of a CCSLTGT originating from in the small intestine in English literatures so far. Therefore, we report the imaging findings of a CCSLTGT that occurred in the small intestine of a 22-year-old female and summarize the imaging findings of the previous reports.

The Dental Biofilm Reduction Effect & Control Difficulty Level of University Students through Dental Biofilm Control Program (일부 대학생에서 치면세균막 관리 프로그램에 의한 치면세균막 감소 효과 및 관리 난이도)

  • For the purpose of examining the dental biofilm reduction effect and control difficulty level through a dental biofilm control program in this study, the total of 131 medical records (82.9%) were used for the final analysis upon excluding the records that were not suitable for this study among the records of 158 persons for the medical records of clinical hygiene case reports that were submitted from 2006 to 2011 by students at the Department of Dental Hygiene at N University located in Chungnam region. The result of examining the dental biofilm reduction effect according to the visit number when conducting a dental biofilm control program showed that the dental biofilm index reduced meaningfully as the visit number increased. However, in the case of those that visited for 7 sessions, the level of reduction was not statistically meaningful. For the purpose of comparing the dental biofilm index mean per area of teeth during the final session visit of dental biofilm control program, the area of teeth was classified into labial/buccal surface, lingual/palatal surface and proximal surface, and the dental biofilm index of lingual/palatal surface was the highest with 26.5%. The result of measuring the dental biofilm of maxilla/mandible revealed meaningful differences between the dental biofilm index of maxilla and mandible. The result of analyzing the dental biofilm index of labial/buccal surface, lingual/palatal surface and proximal surface revealed meaingful differences among the dental biofilm index of labial/ buccal surface, lingual/palatal surface and proximal surface.

A Successful Therapy of Mite Infection with Topical and Spray Application of Ivermectin in Four Pruritic Pigeons (네 마리의 소양감을 보이는 비둘기에서 발생한 진드기 감염(Mite Infection)의 성공적인 Ivermectin 치료 증례)

  • Four pigeons which lived together of same cage were referred due to excessive feather loss, severe itching, and erythema on the head, neck and flank regions. They had behavioral problem with feather bloating and plucking. Although they were mildly depressed, appetite was normal. On microscopic examination of feather picking, prominent external parasite infection was found. Mite infection was diagnosed with morphological confirmation. On analysis of complete blood count (CBC), eosinophilia was evident. The patients were treated with ivermectin (apply 200 mcg/kg topically two times per every other week and spray 200 mcg/ml solution every week). Clinical signs of four, pigeons were improved 45 days following first therapy. This case report indicates that mite infection is accompanied with severe feather loss, itching, and generalized erythema on the skin and behavioral problem with feather bloating and plucking. And this infection can be managed with topical and spray application of Ivermectin without injection.

Second primary cancer in reconstructed neopharynx: a case report (재건된 새 인두의 이차암에 대한 증례 보고서)

  • Background. Ever since the first report of deltopectoral flap in pharyngo-esophageal reconstruction in 1965, various methods of flap reconstruction have been introduced, allowing surgical eradication of tumors that were once thought to be inoperable. Despite these advancement, many literatures emphasize distant metastasis and second primary malignancies as the most important factors that contribute to the low 5-year survival rate of the patients. Specific consensus about defining second primary cancer is still debatable, due to small number of reports regarding second primary tumors arising in flaps used for reconstruction of defects in the head and neck region. Case. We report a case of a 72-year-old male patient who, under the diagnosis of hypopharyngeal cancer, underwent total laryngectomy with partial pharyngectomy, extended right radical neck dissection with extended left lateral neck dissection, right hemithyroidectomy and radial forearm free flap reconstruction on June 16, 2003. After 37 cycles of radiation therapy, the patient exhibited no sign of recurrence. The patient revisited our department on June 14, 2016 with chief complaint of dysphagia that started two months before the visit. Radiologic studies and histology revealed squamous cell carcinoma in neopharynx, one that had been reconstructed with forearm free flap. Conclusion. Until now, only a handful of reports regarding patients with second primary cancer in reconstructed flaps have been described. Despite its rarity, diagnostic criteria for second primary cancer should always kept in consideration for patients with recurred tumor.

Simultaneous Heat-Massage Therapy for Migraine Without Aura : A Case Report (무전조성 편두통 환자에서 온열과 마사지 동시 치료의 효과: 증례보고)

  • A migraine was a headache disorder characterized by recurrent moderate to severe headaches. The diagnosis was based on clinical signs and symptoms. Medication, physical therapy, nerve block, and nerve stimulation could be applied for treatment. This report described a case of severe migraine without aura that lasted several weeks periodically in a 59-year-old woman. Periodic headache had lasted for more than 14 years, and although she took medicines and nerve blocks, severe pain (VAS 7) was persisted. We recommended her to use the thermo-spinal massage device (CGM MB-1401, CERAGEM Inc., Cheonan, South Korea) continuously three times a week applying in semi-automatic mode around the neck for 40 minutes. There was no change in the pain scale in the automatic mode for the first 4 weeks. Subsequently, the semi-automatic mode of the cervical area was treated for 2 weeks to relieve the pain scale, and it was confirmed that the relieved state maintained for 2 months. This case highlighted the importance in considering thermo-spinal massage devices for managing migraine without aura.

THE CLINICO-STATISTICAL ANALYSIS OF THE TREATMENT OF THE AMELOBLASTOMA (법랑아세포종의 치료에 관한 임상통계학적 분석)

  • The author studied on the 109 cases of ameloblastoma which had been diagnosed with biopsy during the period of 1962 to 1992 at the Kyungpook National University Hospital, Kyemyung University Hospital, Youngnam University Hospital, Catholic Medical School Hospital, Pusan National University and Maryknoll Hospital. This study contains the statistical analysis of the treatment method according to the clinicopathological findings such as sex, age, location, chief complaints, duration, radiographic findings, size and recurrence. The results were as follows : 1. The incidence rate was 59.7% in male and 40.3% in female. 2. At the time of diagnosis, the age of the patients ranged from 9 to 69 years(average 31.4years). The cases of 72.4% were in the 2nd, 3rd, and 4th decades of life. 3. The majority of cases occurred in the mandible 88.9%, especially in the mandiblar angle area 57.8%. 4. The most prevalent chief complaints was swelling 58.7%. 5. As regards duration, the cases less than 1 year appeared 49.5% and average duration is 31.34months. 6. Unilocular type showed 43.1% and multilocular type appeared 56.9%. 7. Conservative treatment was performed 39.4% and radical treatment was employed 60.6%. 8. There were variations of lesional sizes between 2.0cm and 15cm(average 6.26cm). 9. The recurrence rate is 29.4%. 10. The frequency of treatment was not in association with sexes, locations, chief complaints and sizes. 11. Radical treatment was performed more frequently in cases of older age group(above 20 years old), multilocular cases, above average duration(31.34 months) group and recurrent cases (P<0.05).
